There are numerous challenges pertaining to epilepsy care across Ontario, including Epilepsy Monitoring Unit (EMU) bed pressures, surgical access and community supports. We sampled the current clinical, community and operational state of Ontario epilepsy centres and community epilepsy agencies post COVID-19 pandemic. A 44-item survey was distributed to all 11 district and regional adult and paediatric Ontario epilepsy centres. Qualitative responses were collected from community epilepsy agencies. Results revealed ongoing gaps in epilepsy care across Ontario, with EMU bed pressures and labour shortages being limiting factors. A clinical network advising the Ontario Ministry of Health will improve access to epilepsy care.

The coronavirus disease 2019 (COVID-19) pandemic continues to present unique public health challenges both within the United States and across the globe. Institutions of higher learning are tasked with preventing and responding to COVID-19 on campus while also considering implications for the surrounding communities. The process of re-opening campus, whether at full or partial capacity, has tasked these institutions with overcoming complex challenges associated with balancing the resumption of campus operations while simultaneously protecting university affiliates and surrounding community members from COVID-19 through robust surveillance, contact tracing, and testing efforts. Here, we provide a concise outline related to the development and implementation of the comprehensive and sustainable COVID-19 surveillance program at the University of Florida. We also critically discuss the successes and pitfalls of this program while also providing recommendations for the development of similar programs in the future.

Models of products and design processes are key to interacting with engineering designs and managing the processes by which they are developed. In practice, companies maintain networks of many interrelated models which need to be synthesised in the minds of their users when considering issues that cut across them. This article considers how information from product and design process models can be integrated with a view to help manage these complex interrelationships. A framework highlighting key issues surrounding model integration is introduced and terminology for describing these issues is developed. To illustrate the framework and terminology, selected modelling approaches that integrate product and process information are discussed and organised according to their levels and forms of integration. Opportunities for further work to advance integrated modelling in engineering design research and practice are discussed.
Concentration and retention of Toxoplasma gondii surrogates from seawater by red abalone (Haliotis rufescens)

Small marine snails and abalone have been identified as high- and low-risk prey items, respectively, for exposure of threatened southern sea otters to Toxoplasma gondii, a zoonotic parasite that can cause fatal encephalitis in animals and humans. While recent work has characterized snails as paratenic hosts for T. gondii, the ability of abalone to vector the parasite has not been evaluated. To further elucidate why abalone predation may be protective against T. gondii exposure, this study aimed to determine whether: (1) abalone are physiologically capable of acquiring T. gondii; and (2) abalone and snails differ in their ability to concentrate and retain the parasite. Abalone were exposed to T. gondii surrogate microspheres for 24 h, and fecal samples were examined for 2 weeks following exposure. Concentration of surrogates was 2–3 orders of magnitude greater in abalone feces than in the spiked seawater, and excretion of surrogates continued for 14 days post-exposure. These results indicate that, physiologically, abalone and snails can equally vector T. gondii as paratenic hosts. Reduced risk of T. gondii infection in abalone-specializing otters may therefore result from abalone's high nutritional value, which implies otters must consume fewer animals to meet their caloric needs.
Patient-level analysis of incident vancomycin-resistant enterococci colonization and antibiotic days of therapy

Vancomycin-resistant enterococci (VRE) infections are a public health threat associated with increased patient mortality and healthcare costs. Antibiotic usage, particularly cephalosporins, has been associated with VRE colonization and VRE bloodstream infections (VRE BSI). We examined the relationship between antimicrobial usage and incident VRE colonization at the individual patient level. Prospective, weekly surveillance was undertaken for incident VRE colonization defined by negative admission but positive surveillance swab in a medical intensive care unit over a 17-month period. Antimicrobial exposure was quantified as days of therapy (DOT)/1000 patient-days. Multiple logistic regression was used to analyse incident VRE colonization and antibiotic DOT, controlling for demographic and clinical covariates. Ninety-six percent (1398/1454) of admissions were swabbed within 24 h of intensive care unit (ICU) arrival and of the 380 patients in the ICU long enough for weekly surveillance, 83 (22%) developed incident VRE colonization. Incident colonization was associated in bivariate analysis with male gender, more previous hospital admissions, longer previous hospital stay, and use of cefepime/ceftazidime, fluconazole, azithromycin, and metronidazole (P < 0·05). After controlling for demographic and clinical covariates, metronidazole was the only antibiotic independently associated with incident VRE colonization (odds ratio 2·0, 95% confidence interval 1·2–3·3, P < 0·009). Our findings suggest that risk of incident VRE colonization differs between individual antibiotic agents and support the possibility that antimicrobial stewardship may impact VRE colonization and infection.

The Research Center for Surgery (RCS) in Moscow is recognized as one of the largest and most prestigious surgical institutions in Russia. In this 400-bed facility more than 3,000 surgical procedures are performed annually, including heart, liver, and pancreas interventions and the reimplantation of limbs. The main focus of the research program at the RCS is on the transplantation of organs and reconstructive surgery. All procedures are free of charge to the patient.

The secretion of prolactin and growth hormone (GH), together with subjective ratings of sedation and hunger, were determined in 13 in-patients with anorexia nervosa and 15 controls during the intravenous infusion of L-tryptophan (100 mg/kg). Prolactin responses were not different between groups but GH responses were markedly blunted in patients. In addition sedation responses in patients were attenuated compared with controls. Hunger ratings were reduced by the infusion in controls but were too variable to be interpreted in the patients. Plasma amino acid levels were also determined before and after infusion of L-tryptophan. Tryptophan levels were comparable in the two groups as were the levels of tyrosine, phenyl alanine, valine, leucine and iso-leucine. The results suggest that some aspects of 5-hydroxytryptamine function may be attenuated in anorexia nervosa. However, they undoubtedly contrast with the finding of enhanced hormonal responses in acute dieting and may be relevant to the interpretation of similar experiments in depressive illness.

This paper reports the development of an experiment (TEPEE/GReAT) to test the Equivalence Principle (EP) at a level of accuracy equal to 5 × 10-15, by means of a differential accelerometer free falling in a cryogenic vacuum capsule released from a stratospheric balloon. Such an accuracy requires resolving a very small signal out of the instrument's intrinsic noise and the noise associated with the instrument's motion. Imperfections in the construction of the detector introduce gravity gradient noise that it is possible to separate from the violation signal spinning the detector around an horizontal axis in order to have the EP violation signal and the gravity gradients one modulated at two different frequencies. Experimental results on prototype instruments showing high sensitivity and common mode rejection factor are shown.

Shock wave propagation arising from steady one-dimensional motion of a piston in a granular gas composed of inelastically colliding particles is treated theoretically. A self-similar long-time solution is obtained in the strong shock wave approximation for all values of the upstream gas volumetric concentration v0. Closed form expressions for the long-time shock wave speed and the granular pressure on the piston are obtained. These quantities are shown to be independent of the particle collisional properties, provided their impacts are accompanied by kinetic energy losses. The shock wave speed of such non-conservative gases is shown to be less than that for molecular gases by a factor of about 2.
The effect of particle kinetic energy dissipation is to form a stagnant layer (solid block), on the surface of the moving piston, with density equal to the maximal packing density, vM. The thickness of this densely packed layer increases indefinitely with time. The layer is separated from the shock front by a fluidized region of agitated (chaotically moving) particles. The (long-time, constant) thickness of this layer, as well as the kinetic energy (granular temperature) distribution within it are calculated for various values of particle restitution and surface roughness coefficients. The asymptotic cases of dilute (v0 [Lt ] 1) and dense (v0 ∼ vM) granular gases are treated analytically, using the corresponding expressions for the equilibrium radial distribution functions and the pertinent equations of state. The thickness of the fluidized region is shown to be independent of the piston velocity.
The calculated results are discussed in relation to the problem of vibrofluidized granular layers, wherein shock and expansion waves were registered. The average granular kinetic energy in the fluidized region behind the shock front calculated here compared favourably with that measured and calculated (Goldshtein et al. 1995) for vibrofluidized layers of spherical granules.

The problem of expansion into a vacuum of a semi-infinite layer composed of chaotically moving inelastic rough spherical particles is solved analytically. A variant of the matched asymptotic expansion scheme is used to obtain a matched composite solution, which is valid for both small and large times in the wave head, wave tail and intermediate domains of the disturbed part of the layer.
The effects of granular initial energy and particle collisonal properties on their hydrodynamic velocity, temperature, density and pressure are studied in the limit of low initial density of the granular gas. The total granular mass, M, within the disturbed region was found to change with time as log t. This is in contrast with the comparable classical result M ∼ t obtained for conservative (molecular) gases. This logarithmic dependence stems from the influence of kinetic energy losses, which reduce the granular temperature and speed of sound in the wave head region.
The ultimate escape energy and momentum (i.e. those achieved for long times by the expanding part of the layer) are shown to be finite quantities, dependent on the particle restitution coefficient, roughness and the initial granular temperature. The estimated mass of the escaping part of the layer, calculated here for dilute gases, serves as an upper bound on this quantity for all (also dense) comparable granular gases. This mass is determined by the collisional losses, as embodied within the particle restitution and roughness coefficients.

N-body simulations find a universal structure for the halos which result from the nonlinear growth of Gaussian-random-noise density fluctuations in the CDM universe. This talk summarized our attempts to derive and explain this universal structure by analytical approximation and simplified models. As an example, we show here that a 1D spherical infall model involving a fluid approximation derived from the Boltzmann equation can explain not only the halo density profile but its phase-space density profile, as well.
