Introduction
Hospitals play a significant and important role in funding high-cost medicines so patients can access treatments they need. High-cost medicines are often specialty medicines, which contribute to a significant and increasing portion of the hospital budget. It is imperative that these expensive medicines are governed and managed with a fair, standardized evidence-based process. We aim to provide a framework for Drugs and Therapeutics Committees (DTCs).
MethodsDuring 2021, Guiding Principles were developed following a literature review and survey of current practices by DTCs in Australia. An Expert Advisory Group (EAG) was convened, comprising individuals with expertise in quality use of medicines, evidence-based medicine and medicines governance. The guiding principles were drafted by the EAG, in consultation with a range of stakeholders and relevant external organizations. All feedback was collated, reviewed and discussed to refine the content of the final Guiding Principles released in January 2022.
ResultsSeven overarching principles provide key recommendations for the governance of high-cost medicines:
(i) A definition of high‑cost medicines should be determined and clearly articulated for use by each medicines governance committee.
(ii) Review of high-cost medicines requires members with relevant expertize to facilitate good and effective decision-making.
(iii) The committee should engage directly with the applicant prior to review to ensure a full understanding of the rationale for the request.
(iv) consistent, robust and transparent procedure for the assessment of high-cost medicine applications should be defined and implemented for use by each medicines governance committee to ensure fair process.
(v) Ethical considerations fundamentally underpin deliberations around high-cost medicines.
(vi) The decisions and outcomes of the decision making should be transparent and appropriately communicated to the various audiences.
(vii) The high-quality assessment of high-cost medicines requires appropriate training and resourcing.
ConclusionsThese national Guiding Principles promote consistent, evidence-based use of high-cost medicines and provide a framework for DTCs to assess and achieve effective governance for the quality use of high‑cost medicines.
Improved resilience following digital cognitive behavioral therapy for insomnia protects against insomnia and depression one year later

Background
While the negative consequences of insomnia are well-documented, a strengths-based understanding of how sleep can increase health promotion is still emerging and much-needed. Correlational evidence has connected sleep and insomnia to resilience; however, this relationship has not yet been experimentally tested. This study examined resilience as a mediator of treatment outcomes in a randomized clinical trial with insomnia patients.
MethodsParticipants were randomized to either digital cognitive behavioral therapy for insomnia (dCBT-I; n = 358) or sleep education control (n = 300), and assessed at pre-treatment, post-treatment, and 1-year follow-up. A structural equation modeling framework was utilized to test resilience as a mediator of insomnia and depression. Risk for insomnia and depression was also tested in the model, operationalized as a latent factor with sleep reactivity, stress, and rumination as indicators (aligned with the 3-P model). Sensitivity analyses tested the impact of change in resilience on the insomnia relapse and incident depression at 1-year follow-up.
ResultsdCBT-I resulted in greater improvements in resilience compared to the sleep education control. Furthermore, improved resilience following dCBT-I lowered latent risk, which was further associated with reduced insomnia and depression at 1-year follow-up. Sensitivity analyses indicated that each point improvement in resilience following treatment reduced the odds of insomnia relapse and incident depression 1 year later by 76% and 65%, respectively.
ConclusionsImproved resilience is likely a contributing mechanism to treatment gains following insomnia therapy, which may then reduce longer-term risk for insomnia relapse and depression.

Development and validation of a non-remission risk prediction model in First Episode Psychosis: An analysis of two longitudinal studies

Aims
Psychosis is a major mental illness with first onset in young adults. The prognosis is poor in around half of the people affected, and difficult to predict. The few tools available to predict prognosis have major weaknesses which limit their use in clinical practice. We aimed to develop and validate a risk prediction model of symptom non-remission in first-episode psychosis.
MethodOur development cohort consisted of 1027 patients with first-episode psychosis recruited between 2005 to 2010 from 14 early intervention services across the National Health Service in England. Our validation cohort consisted of 399 patients with first-episode psychosis recruited between 2006 to 2009 from a further 11 English early intervention services. The one-year non-remission rate was 52% and 54% in the development and validation cohorts, respectively. Multivariable logistic regression was used to develop a risk prediction model for non-remission, which was externally validated.
ResultThe prediction model showed good discrimination (C-statistic of 0.74 (0.72, 0.76) and adequate calibration with intercept alpha of 0.13 (0.03, 0.23) and slope beta of 0.99 (0.87, 1.12). Our model improved the net-benefit by 16% at a risk threshold of 50%, equivalent to 16 more detected non-remitted first-episode psychosis individuals per 100 without incorrectly classifying remitted cases.
ConclusionOnce prospectively validated, our first episode psychosis prediction model could help identify patients at increased risk of non-remission at initial clinical contact.

Field surveys were conducted across the Blacklands region of Texas during 2016 and 2017 to document the distribution of herbicide-resistant Lolium spp. infesting winter wheat production fields in the region. A total of 68 populations (64 Italian ryegrass, four perennial ryegrass) were evaluated in a greenhouse for sensitivity to herbicides of three different modes of action: an acetolactate synthase (ALS) inhibitor (mesosulfuron-methyl), two acetyl-coenzyme-A carboxylase (ACCase) inhibitors (diclofop-methyl and pinoxaden), and a 5-enolpyruvylshikimate-3-phosphate synthase (EPSPS) inhibitor (glyphosate). Herbicides were applied at twice the label-recommended rates for mesosulfuron-methyl (29 g ai ha−1), diclofop-methyl (750 g ai ha−1), and pinoxaden (118 g ai ha−1); and at the recommended rate for glyphosate (868 g ae ha−1). The herbicide screenings were followed by dose-response assays of the most-resistant ryegrass population for each herbicide at eight rates (0.5, 1, 2, 4, 8, 16, 32, and 64×), compared with a susceptible population at six rates (0.0625, 0.125, 0.25, 0.5, 1, and 2×). The initial screening and dose-response experiments were conducted in a completely randomized design with three replications and two experimental runs. Survivors (<80% injury) were characterized as highly resistant (0% to 20% injury) or moderately resistant (21% to 79%). Results showed that 97%, 92%, 39%, and 3% of the Italian ryegrass populations had survivors to diclofop-methyl, mesosulfuron-methyl, pinoxaden, and glyphosate treatments, respectively. Of the four perennial ryegrass populations, three were resistant to diclofop-methyl and mesosulfuron-methyl, and one was resistant to pinoxaden as well. Perennial ryegrass populations did not exhibit any resistance to glyphosate. Dose-response assays revealed 37-, 196-, and 23-fold resistance in Italian ryegrass to mesosulfuron-methyl, diclofop-methyl, and pinoxaden, respectively, compared with a susceptible standard. One Italian ryegrass population exhibited three-way multiple resistance to ACCase-, ALS-, and EPSPS-inhibitors. The proliferation of multiple herbicide–resistant ryegrass is a challenge to sustainable wheat production in Texas Blacklands and warrants diversified management strategies.
Major stopover regions and migratory bottlenecks for Nearctic-Neotropical landbirds within the Neotropics: a review

Nearly 300 species of landbirds, whose populations total billions, migrate between the Neotropics and North America. Many migratory populations are in steep decline, and migration is often identified as the greatest source of annual mortality. Identifying birds’ needs on migration is therefore central to designing conservation actions for Nearctic-Neotropical migratory birds; yet migration through the Neotropics is a significant knowledge gap in our understanding of the full annual cycle. Here, we synthesise current knowledge of Neotropical stopover regions and migratory bottlenecks, focusing on long-distance, migratory landbirds that spend the boreal winter in South America. We make the important distinction between “true” stopover—involving multi-day refuelling stops—and rest-roost stops lasting < 24 hours, citing a growing number of studies that show individual landbirds making long stopovers in just a few strategic areas, to accumulate large energy reserves for long-distance flights. Based on an exhaustive literature search, we found few published stopover studies from the Neotropics, but combined with recent tracking studies, they describe prolonged stopovers for multiple species in the Orinoco grasslands (Llanos), the Sierra Nevada de Santa Marta (Colombia), and the Yucatan Peninsula. Bottlenecks for diurnal migrants are well described, with the narrowing Central American geography concentrating millions of migrating raptors at several points in SE Mexico, Costa Rica, Panama and the Darién. However, diurnally migrating aerial insectivores remain understudied, and determining stopover/roost sites for this steeply declining group is a priority. Despite advances in our knowledge of migration in the Neotropics, we conclude that major knowledge gaps persist. To identify stopover sites and habitats and the threats they face, we propose a targeted and collaborative research agenda at an expanded network of Neotropical sites, within the context of regional conservation planning strategies.
Acquirer Valuation and Acquisition Decisions: Identifying Mispricing Using Short Interest

We use short interest as an investor-based measure of over- or undervaluation that distinguishes between the misvaluation and Q-theories of mergers. Using this measure, we find that misvaluation is a strong determinant of merger decision-making. Firms in the top quintile of short interest are 54% more likely to engage in stock acquisitions and 22% less likely to engage in cash acquisitions. Stock (but not cash) acquirers have higher short interest than their targets. Overall, our results suggest that the previously documented underperformance of stock acquirers and the overperformance of cash acquirers can be explained by misvaluation, as captured by short interest.

The paper reports on the fourth (2010) season of fieldwork of the Cyrenaican Prehistory Project, and on further results of analyses of artefacts and organic materials collected in the 2009 season. Ground-based LiDar has provided both an accurate 3D scan of the Haua Fteah cave and information on the cave's morphometry or origins. The excavations in the cave focussed on Middle Palaeolithic or Middle Stone Age ‘Pre-Aurignacian’ layers below the base of the Middle Trench beside the McBurney Deep Sounding (Trench D) and on Final Palaeolithic ‘Oranian’ layers beside the upper part of the Middle Trench (Trench M). Although McBurney referred to the upper part of the Deep Sounding as more or less sterile, the 2010 excavations found evidence for small-scale but regular human presence in the form of stone artefacts and debitage, though given the sedimentary context the latter are unlikely to represent in situ knapping. The excavations of Trench M extended from the basal Capsian layers investigated in 2009 through Oranian layers to the transition with the Dabban Upper Palaeolithic. Some 17,000 lithic pieces have been studied from the Capsian and Oranian layers excavated in Trench M, in an area measuring less than 2 m by 1 m by 1.1 m deep, along with numerous animal bones, molluscs, and macrobotanical remains, as well as occasional shell beads. Preliminary studies of the lithics, bones, molluscs, and plant remains are revealing the changing character of late Pleistocene (Oranian) and early Holocene (Capsian) occupation in the Haua Fteah. Alongside the work in the Haua Fteah, the project continued its assessment of the Quaternary and archaeological sequences of the Cyrenaican coastland and completed a transect survey of surface lithic materials and their landform contexts from the pre-desert across the Gebel Akhdar to the coast, with a new focus on the al-Marj basin. Significant differences are emerging in patterns of Middle Palaeolithic and later hominin occupation and palaeodemography.

The paper reports on the third (2009) season of fieldwork of the Cyrenaican Prehistory Project, and on further results from the analysis of materials collected in the previous (2007 and 2008) fieldwork. Sediments in a 14 m-deep core drilled beside the McBurney trench provide an invaluable overview of the overall stratigraphic sequence, including at depths reached by the 1950s Deep Sounding but not yet investigated by the present project. Sampling of newly-exposed faces of the original excavation trench for dating (14C, ESR, OSL, U-series) and palaeoenvirommental indicators continued. Excavation was begun of sediments assigned to the early Holocene Libyco-Capsian (McBurney's Layer X), and of Pre-Aurignacian layers beside the top of the Deep Sounding. The Libyco-Capsian layers are particularly prolific in lithic debris, shells, and animal bones; preliminary analysis of the lithics suggests a development from Typical to Upper Capsian within the layers excavated in 2009. Geoarchaeological survey along the littoral to the west and east of the Haua Fteah identified complex sequences spanning most of the last interglacial-glacial cycle. Geoarchaeological survey south of the Haua Fteah characterized the major landforms of the Gebel Akhdar mountain and of the pre-desert and desert-edge zones further south, with Late Stone Age (Upper Palaeolithic and Epipalaeolithic) material being found especially on the southern side of the Gebel Akhdar, and Middle Stone Age (Middle Palaeolithic) material in the pre-desert and desert regions. The first suite of 14C dates (from charcoal samples taken in 2007) indicates the use of the Haua Fteah by Oranian hunter-gatherers during the Last Glacial Maximum and in the succeeding millennia, but not in the Younger Dryas cold/dry phase (c. 11,000–10,000 cal. BC), with Libyco-Capsian occupation resuming soon after the beginning of the Holocene c. 9000 cal. BC, suggesting that the cave, and perhaps the Gebel Akhdar in general, have a complex history as refugia for human settlement during the Pleistocene.

We present limits on transit timing variations and secondary eclipse depth variations at 8 microns with the Spitzer Space Telescope IRAC camera. Due to the weak limb darkening in the infrared and uninterrupted observing, Spitzer provides the highest accuracy transit times for this bright system, in principle providing sensitivity to secondary planets of Mars mass in resonant orbits. Finally, the transit data provides tighter constraints on the wavelength-dependent atmospheric absorption by the planet.

The NASA Discovery mission EPOXI, utilizing the Deep Impact flyby spacecraft, comprises two phases: EPOCh (Extrasolar Planet Observation and Characterization) and DIXI (Deep Impact eXtended Investigation). With EPOCh, we use the 30-cm high resolution visible imager to obtain ultraprecise photometric light curves of known transiting planet systems. We will analyze these data for evidence of additional planets, via transit timing variations or transits; for planetary moons or rings; for detection of secondary eclipses and the constraint of geometric planetary albedos; and for refinement of the system parameters. Over a period of four months, EPOCh observed four known transiting planet systems, with each system observed continuously for several weeks. Here we present an overview of EPOCh, including the spacecraft and science goals, and preliminary photometry results.

We present here results from observations of TrES-2 made using the Infrared Array Camera on the Spitzer Space Telescope. We monitored this transiting system during two secondary eclipses, when the planetary emission is blocked by the star. The resulting decrease in flux is 0.135 ± 0.036%, 0.245 ± 0.027%, 0.162 ± 0.064%, and 0.295 ± 0.066%, at 3.6-μm, 4.5-μm, 5.8-μm, and 8.0-μm, respectively. We find evidence for a temperature inversion in the atmosphere of TrES-2, which is predicted by Fortney and collaborators based on the proposed importance of TiO and VO opacities for this highly irradiated gas giant. We also find the time of the center of the eclipse to be consistent with predictions from transit timing observations of TrES-2. This implies that TrES-2 most likely has a circular orbit, and thus does not obtain additional thermal energy from tidal dissipation of a non-zero orbital eccentricity, a proposed explanation of the large planetary radius.

EPOXI (EPOCh + DIXI) is a NASA Discovery Program Mission of Opportunity using the Deep Impact flyby spacecraft. The EPOCh (Extrasolar Planet Observation and Characterization) Science Investigation will gather photometric time series of known transiting exoplanet systems from January through August 2008. Here we describe the steps in the photometric extraction of the time series and present preliminary results of the first four EPOCh targets.

The palaeoanthropological and geomorphological sub-projects of the Desert Migrations Project (DMP) focus on the Pleistocene and early Holocene environment and prehistory of Fazzan so as to assess the timing and extent of hominin and human movement across the Sahara through time. This paper reports on the findings of the 2008 field season, with a focus on the prehistoric evidence along the northern margin of the Ubari sand sea.
The geomorphological record of the area preserves evidence of at least five past episodes of lake formation. The exact chronology of these, as well as the spatial extent of these lakes, remains the focus of further study.
The archaeological record of hominin and human occupation of Fazzan prior to the establishment of the Garamantian civilisation is extraordinarily rich. Between 2007 and 2008, the DMP palaeoanthropological project surveyed thirty-five localities along the northern margin of the Ubari sand sea, recording a range of assemblages spanning all Palaeolithic industries. Most of the archaeological remains found consisted of stone-tools, while grinding stones were comparatively restricted geographically. Mode 1/Oldowan tools were found at two localities, contrasting with the widespread presence of Mode 2/Acheulean, Mode 3/Middle Stone Age and Mode 5/microlithic artefacts. This indicates that, although hominin presence in the area is probably earlier than previously thought, populations were comparatively sparse until the Middle Pleistocene. Twenty-one localities within the Ubari sand sea, as well as seven south of the Messak Settafet were also surveyed between 2007 and 2008. The detailed study of the lithics from these areas will be carried out next year, but preliminary results stress the different nature of the assemblages found within interdune corridors — very low frequency of cores, no Mode 1 and extremely rare Mode 2 lithics (found at a single locality).
The 2009 field season will focus on obtaining further samples of palaeolake sediments for dating, on the evidence of Mode 1 assemblages south of the Messak, as well as on the refining of the archaeological indicators that may distinguish the different phases of hominin and human occupation of Fazzan during the Later Pleistocene and Holocene.

The Desert Migrations Project is a new interdisciplinary and multi-dimensional collaborative project between the Society for Libyan Studies and the Department of Antiquities. The geographical focus of the study is the Fazzan region of southwest Libya and in thematic terms we aim to address the theme of migration in the broadest sense, encompassing the movement of people, ideas/knowledge and material culture into and out of Fazzan, along with evidence of shifting climatic and ecological boundaries over time. The report describes the principal sub-strands of the project's first season in January 2007, with some account of research questions, methods employed and some preliminary results. Three main sub-projects are reported on. The first concerns the improved understanding of long-term climatic and environmental changes derived from a detailed palaeoenvironmental study of palaeolake sediments. This geo-science work runs alongside and feeds directly into both archaeological sub-projects, the first relating to prehistoric activity and mobility around and between a series of palaeolakes during wetter climatic cycles; the second to the excavation of burials in the Wadi al-Ajal, exploring the changing relationship between material culture, identity and ethnicity across time, from prehistory to the early Islamic period (the span of the main cemetery zones). In addition, some rock art research and a survey of historic period sites was undertaken in the Wadi ash-Shati and Ubari sand sea.
Cognitive–behavioural therapy in first-episode and early schizophrenia: 18-month follow-up of a randomised controlled trial

Background
The initial phase of a trial of cognitive–behavioural therapy (CBT) for acutely ill patients with schizophrenia of recent onset showed that it speeded recovery.
AimsTo test the hypothesis that CBT in addition to treatment as usual (TAU) during the first or second acute episode of schizophrenia will confer clinical benefit over a follow-up period.
MethodThis was an 18-month follow-up of a multicentre prospective trial of CBT or supportive counselling administered as an adjunct to TAU, compared with TAU alone, for patients hospitalised for an acute episode of schizophrenia of recent onset. Primary outcomes were total and positive symptom scales, time to relapse and re-hospitalisation.
ResultsThere were significant advantages for CBT and supportive counselling over TAU alone on symptom measures at 18 months but no group difference was seen for relapse or re-hospitalisation. There was a significant centre–treatment interaction, reflecting centre differences in the effect of introducing either treatment, but not in the comparison of CBT and supportive counselling. Medication dosage and compliance did not explain group differences.
ConclusionsAdjunctive psychological treatments can have a beneficial longterm effect on symptom reduction.
Water table decline, springline desiccation and the early development of irrigated agriculture in the Wādi al-Ajāl, Libyan Fazzān

Uranium/Thorium (U/Th) and 226Ra dating has been applied to spring deposits in order to determine the timing of the drying up of springs in the Wādī al-Ajāl, Fazzān, Libya. These dates are compared with results of archaeological surveys of irrigation structures and archaeobotanical studies of plant remains to further our understanding of the factors that controlled the introduction of irrigated agriculture, its nature and timing.
Though there are still important gaps in our understanding, the results suggest that by 3,100 ±125 BP surface water was either very scarce or nonexistent at the base of the escarpment, although the water table was probably very near the surface at the base of the Wādī in the vicinity of Jarma. Archaeobotanical evidence indicates that irrigated agriculture was introduced in the early part of the first millennium BC, soon after the desiccation of the springs and presumably in response to the fall in the water table and the increasingly scarce availability of surface water. No irrigation structures have been found that relate to this period, but the most likely scenario involves wells tapping the near surface water in the vicinity of Jarma. The earliest irrigation systems are foggaras which archaeological evidence suggests were probably introduced by the final few centuries BC, and definitely before the fourth century AD. The extensification of agriculture associated with extensive foggara development broadly corresponds with archaeobotanical evidence for an intensification and diversification of agriculture involving the introduction of a farming system utilising both winter and summer crops. There are likely to be strong connections between these two developments and that of the rise of the Garamantes.
