Background: Currently there are no disease modifying treatment for Synucleinopathies including Parkinson’s disease Dementia (PDD). Carrying a mutation in the GBA gene (beta-glucocerebrosidase/ GCAse) is a leading risk factor for synucleinopathies. Raising activity GCAse lowers α-synuclein levels in cells and animal models. Ambroxol is a pharmacological chaperone for GCAse and can raise GCAse levels. Our goal is to test Ambroxol as a disease-modifying treatment in PDD. Methods: We randomized fifty-five individuals with PDD to Ambroxol 1050mg/day, 525mg/day, or placebo for 52 weeks. Primary outcome measures included safety, Alzheimer’s disease Assessment Scale-cognitive (ADAS-Cog) subscale and the Clinician’s Global Impression of Change (CGIC). Secondary outcomes included pharmacokinetics, cognitive and motor outcomes and and plasma and CSF biomarkers. Results: Ambroxol was well tolerated. There were 7 serious adverse events (SAEs) none deemed related to Ambroxol. GCase activity was increased in white blood cells by ~1.5 fold. There were no differences between groups on primary outcome measures. Patients receiving high dose Ambroxol appeared better on the Neuropsychiatric Inventory. GBA carriers appeared to improve on some cognitive tests. pTau 181 was reduced in CSF. Conclusions: Ambroxol was safe and well-tolerated in PDD. Hydroxy-interlayered vermiculite (HIV) is a ubiquitous phyllosilicate in the <0.05-mm fraction of sandy soils on the U.S. southeastern coastal plain. Extensive areas of soils with abundant HIV (i.e., peninsular Florida) have no detectable mica; yet the coarseness, platy habit, and nonexchangeable K associated with HIV grains suggest a mica precursor. The objectives of this study were: (1) to probe for mica zones (1.0-nm) within HIV grains, using high-resolution transmission electron microscopy (HRTEM), and (2) to determine intragrain elemental distributions via electron microprobe analysis (EMA). HIV grains from a Quartzipsamment medium-silt fraction, which contained no detectable mica by X-ray diffraction (XRD), were concentrated via high-density liquid separation. EMA transects and X-ray dot maps showed zonation or trends of K depletion near edges of some grains, with K2O contents ranging from trace levels to >40 g kg-1. Elemental oxide data indicated a dioctahedral phyllosilicate structure, with some octahedral substitution of Fe and Mg for Al. Intermittent 1.0-nm lattice-fringe images obtained by HRTEM supported the presence of mica zones within grains. There were no detectable 1.4-nm fringes, despite the dominance of a 1.4-nm XRD peak, indicating the instability of the HIV specimen under the electron beam. Results support a transformational link between mica and HIV in these soils. Rapid incursion and polymerization of Al following loss of K from mica may limit the extent of the vermiculite intermediate. The latter idea is consistent with the paucity of vermiculite in Florida soils. Traces of occluded mica may be the last remnants of the precursor grain. A sand-sized mica precursor would likely have weathered in place during the period when colloidal components such as kaolinite illuviated to deeper zones. Thus, the transformation product (HIV) would comprise a significant proportion of the <0.05-mm fraction persisting in sandy eluvial horizons.
Development of a comprehensive, sustained community mental health system in post-earthquake Haiti, 2010–2019

Between 2010 and 2019 the international health care organization Partners In Health (PIH) and its sister organization Zanmi Lasante (ZL) mounted a long-term response to the 2010 Haiti earthquake, focused on mental health. Over that time, implementing a Theory of Change developed in 2012, the organization successfully developed a comprehensive, sustained community mental health system in Haiti's Central Plateau and Artibonite departments, directly serving a catchment area of 1.5 million people through multiple diagnosis-specific care pathways. The resulting ZL mental health system delivered 28 184 patient visits and served 6305 discrete patients at ZL facilities between January 2016 and September 2019. The experience of developing a system of mental health services in Haiti that currently provides ongoing care to thousands of people serves as a case study in major challenges involved in global mental health delivery. The essential components of the effort to develop and sustain this community mental health system are summarized.

Summary
The rocky shores of the north-east Atlantic have been long studied. Our focus is from Gibraltar to Norway plus the Azores and Iceland. Phylogeographic processes shape biogeographic patterns of biodiversity. Long-term and broadscale studies have shown the responses of biota to past climate fluctuations and more recent anthropogenic climate change. Inter- and intra-specific species interactions along sharp local environmental gradients shape distributions and community structure and hence ecosystem functioning. Shifts in domination by fucoids in shelter to barnacles/mussels in exposure are mediated by grazing by patellid limpets. Further south fucoids become increasingly rare, with species disappearing or restricted to estuarine refuges, caused by greater desiccation and grazing pressure. Mesoscale processes influence bottom-up nutrient forcing and larval supply, hence affecting species abundance and distribution, and can be proximate factors setting range edges (e.g., the English Channel, the Iberian Peninsula). Impacts of invasive non-native species are reviewed. Knowledge gaps such as the work on rockpools and host–parasite dynamics are also outlined.

There is community concern about the treatment of farm animals post-farm gate, particularly animal transport and slaughter. Relationships between lamb behavioural and physiological variables on farm, stockperson, dog and lamb behavioural variables pre-slaughter and plasma cortisol, glucose and lactate in lambs post-slaughter were studied in 400 lambs. The lambs were observed in three behavioural tests, novel arena, flight distance to a human and temperament tests, before transport for slaughter. Closed-circuit television video footage was used to record stockperson, dog and lamb behaviour immediately before slaughter. Blood samples for cortisol, glucose and lactate analyses were collected on farm following the three behavioural tests and immediately post-slaughter. The regression models that best predicted plasma cortisol, glucose and lactate concentrations post-slaughter included a mixture of stockperson and dog behavioural variables as well as lamb variables both on-farm and pre-slaughter. These regression models accounted for 33%, 34% and 44% of the variance in plasma cortisol, glucose and lactate concentrations post-slaughter, respectively. Some of the stockperson and dog behaviours pre-slaughter that were predictive of the stress and metabolic variables post-slaughter included the duration of negative stockperson behaviours such as fast locomotion and lifting/pulling lambs, and the duration of dog behaviours such as lunging and barking at the lamb, while some of the predictive lamb behaviour variables included the durations of jumping and fleeing. Some of the physiological and behavioural responses to the behavioural tests on farm were also predictive of the stress and metabolic variables post-slaughter. These relationships support the well-demonstrated effect of handling on fear and stress responses in livestock, and although not direct evidence of causal relationships, highlight the potential benefits of training stockpeople to reduce fear and stress in sheep at abattoirs.
A numerical study of a weakly stratified turbulent wake

Direct numerical simulation (DNS) of a time-developing turbulent wake evolving in a stably stratified background is presented. A large initial Froude number is chosen to allow the wake to become fully turbulent and axisymmetric before stratification affects the spreading rate of the mean defect. Turbulence statistics are formed by averaging over the homogeneous streamwise direction of a domain that is larger than earlier stratified-wake simulations in order to reduce the statistical uncertainty. The DNS results are used to cast light on the mechanisms that lead to the various states of this flow – namely the three-dimensional (essentially unstratified), non-equilibrium (or ‘wake-collapse’) and quasi-two-dimensional (or ‘two-component’) regimes, previously observed for wakes embedded in both weakly and strongly stratified backgrounds. For this relatively high-initial-Reynolds- and Froude-number simulation, we find that the signature reduction in the rate of decay of the maximum mean defect velocity during the wake-collapse regime is due to buoyancy-induced alterations of the turbulence structure, which weaken and redistribute the Reynolds shear stresses whose gradients appear in the streamwise mean momentum equation. The change in the rate of decay of the turbulence kinetic energy (TKE) observed during the wake-collapse regime (which occurs well after the mean velocity decay reduction begins) is not caused by transfer of turbulent gravitational potential energy to TKE, as has been previously suggested. The results instead reveal that the reduction in TKE decay – which for this flow, with its relatively weak internal waves, eventually leads to TKE growth, heralding the arrival of the two-component regime – is caused by an increase in the rate of TKE production associated with the wake structure becoming increasingly two-dimensional, such that the lateral Reynolds shear stress, $-\overline{u^{\prime }v^{\prime }}$, becomes dominant. The present results are also compared with those of previous simulations at different Froude and Reynolds numbers, and whose initial conditions contain different turbulence structures. This comparison confirms a strong degree of commonality in the late-wake behaviour, which lends support to the hypothesis that all wakes in stably stratified environments achieve a universal state in the final stages of decay.

In the search for tidal structure in Galactic satellite systems, we have conducted a photometric survey over a 10 deg2 area centred on the Fornax dSph galaxy. The survey was made in two colours, and the resulting colour–magnitude data were used as a mask to select candidate Fornax RGB stars, thereby increasing the contrast of Fornax stars to background sources in the outer regions. Previously, we reported the presence of a shell (age 2 Gyr) located towards the centre of Fornax. In this contribution we reveal a second shell, significantly larger than the first, located 1.3° north-west from the centre of Fornax, outside the nominal tidal radius. Moreover, the distribution of Fornax RGB stars reveals two lobes extending to the spatial limit of our survey, and aligned with the minor axis and with the two shells. These results support the hypothesis of a merger between Fornax and a gas-rich companion approximately 2 Gyr ago.

This paper was written by the Derivatives Working Party, a permanent working party of the Life Research Committee of the Institute and Faculty of Actuaries. Our aim is to consider how life assurers may use, or may wish to use, derivatives, and if their use is unduly constrained, e.g. by regulation. This paper focuses on credit derivatives. We provide an overview of the credit derivatives market, and the strong growth in this market over recent years. We then focus on the two main traded credit derivative instruments — Credit Default Swaps (CDSs) and Collateralised Debt Obligations (CDOs). We explain how these instruments work and are priced, and clarify some of the more complex topics involved, such as the settlement of CDSs, basis risk and the relevance of implied correlation in pricing CDOs. We then consider how life insurers could make use of credit derivatives, for example to provide more efficient investment management in taking exposure to credit risk, or to hedge credit exposures, and consider the regulatory implications of so doing. Finally, in the Appendix, we discuss the credit spread puzzle, and the existence or otherwise of a liquidity premium in corporate bond spreads, with implications for the valuation of illiquid liabilities.

Serum and cervical secretions were collected from patients with cervical dysplasia, carcinoma-in-situ (CIS), squamous cell carcinoma (cervical SCC), and controls with normal cervices, attending clinics within the West Lambeth Health District, London. Enzyme-linked immunosorbent assays were used to examine cervical secretory IgA (sIgA) and serum IgG and IgA antibodies to herpes simplex virus (HSV). Sexual and demographic factors were considered during data analysis, which involved fitting multiple linear or multiple logistic regressions to HSV antibody levels. Prevalence of sIgA-HSV and levels of serum antibodies to HSV in all groups were compared with those of gynaecology controls. Caucasian women with mild dysplasia had a significantly higher prevalence of sIgA-HSV. Serum IgG levels to HSV (IgG-HSV) were significantly elevated in women with mild dysplasia and severe dysplasia/ClS. Serum IgA levels to HSV1 (IgG-HSV1) were significantly higher in women with cervical SCC (after adjusting for smoking habits) and other genital tumours. Significantly higher levels of serum IgA to HSV2 (IgA-HSV2) were also found among Caucasian women with cervical SCC. The possible role of HSV as a co-factor in cervical carcinogenesis is discussed.

A point source outbreak of haemorrhagic colitis due to Escherichia coli O 157. H7 producing verocytotoxin (VT), took place following a christening party in Birmingham in June 1987. Twenty-six people were affected, six were admitted to hospital and one developed haemolytic uraemic syndrome: there were no deaths. VT + E. coli O 157. H7 was isolated from 13 (57%) of 23 faecal specimens from affected people and from 3 (9%) of 33 specimens from asymptomatic people. Free VT was detected in the faeces of one further asymptomatic person. Illness was associated with eating turkey-roll sandwiches (P < 0·001) suggesting that cold meats might be an important source of infection.

The distribution of carboxymethylcellulase (CMCase) between various rumen fractions has been determined at three times after feeding in sheep containing no ciliate protozoa or five different protozoal populations. The total rumen CMCase was lowest in defaunated animals and tended to be higher in animals containing the amylolytic Entodinium caudatum or a natural mixed protozoal population and highest in sheep containing single cellulolytic species. In animals containing the cellulolytic species, Epidinium ecaudatum caudatum or Eudiplodinium maggii, 70% of the CMCase was associated with the protozoal fraction whereas less than 15% was present in a fraction prepared in the same way from sheep containing no protozoa or only Entodinium caudatum. The activity associated with the free bacteria was higher in sheep containing only Entodinium caudatum than in defaunated animals or those containing the cellulolytic species. Similar results were obtained 1·5, 6·5 or 24 h after feeding. Almost no activity was present in the cell-free rumen liquor under any condition. A variable amount of CMCase (up to 25% of the total) was associated with the plant debris in the rumen and the specific activity of the enzyme released by sonication was often higherthan that released from the free bacteria under the same conditions.

The protozoa in washed suspensions of rumen ciliates were killed by freezing or heating at 49 °C for 5 min before incubation at 39 °C, aeration during incubation or incubation in hypotonic salt solution (30% of ncrmal) and released their intracellular cellulose and amylase during 4–6 h. With the last two treatments it was only necessary to expose the protozoa to the conditions for 2 min to obtain over 50% of maximal enzyme release 4–6 h later. It is suggested that death of the protozoa during exposure to air and/or hypotonic salt solution for short times during drinking and rumination by the host animal could be the explanation for the high postulated protozoal death rate in the rumen. The amylase and cellulase released from the protozoa tested were stable in the presence of mixed rumen bacteria for 1·5 h and could be of importance in the degradation of plant constituents in the rumen.

The cellulolytic activity of cell-free extracts of 15 species of entodiniomorphid protozoa grown in vitro and under two conditions in vitrohas been measured using six different assays with substrates varying in complexity from carboxymethylcellulose to microcrystalline cellulose. Although there were differences between the assays, the highest activities were found in Eudiplodinium maggii, Epidinium ecaudatum caudatum and Ostracodinium obtusum bilobum with little or no activity in five Entodiniumspp. There was no consistent effect of growth conditions on cellulase content although with protozoa grown in vitro those grown on grass alone tended to contain more cellulase than those grown on starch and grass.
A survey of methods for the release of cellulase from other rumen fractions was made and mild treatment with ultrasound of the fraction suspended in water gave the maximal release of enzyme with the highest specific activity. Using this method 62% of the total rumen cellulase was found in the protozoal fraction and 27% was associated with the plant debris in a normal sheep, fed on hay and oats, with an A-type protozoal population.

Five species of rumen ciliate protozoa, isolated from sheep in which they were the only ciliate protozoon, and mixed rumen ciliates were incubated with labelled bacteria and the rate of uptake and digestion of the bacteria was followed. Evidence is presented that mixed rumen bacteria labelled by incubation with 14C-isoleucine, 14C-guanine or 35S-sulphate were engulfed more rapidly by the protozoa than those labelled with 14C-glucose but that, with Entodinium spp. at least, this apparent lower rate of uptake was associated with an increased rate of digestion. The rate of uptake of mixed rumen bacteria (at 109/ml) varied from 230 to 2670 bacteria/protozoon per hour, depending on the protozoal species, and increased with increasing bacterial population density. Starvation of the protozoa decreased the initial rate of uptake of bacteria by up to 60% and the length of time over which the protozoa would engulf bacteria. Entodinium spp. and Epidinium ecaudatum caudatum engulfed fewer bacteria when these bacteria were prepared from the same sheep as the protozoa than when they were prepared from sheep with no or other protozoal populations. Of the protozoa tested, only Entodinium spp. and the mixed large rumen protozoa digested mixed rumen bacteria, Eremoplastron bovisand Epidinium ecaudatum caudatum being inactive. It is suggested that previous estimates of the rate of uptake and digestion of bacteria based on studies in vitro were too high.
The non-rumen bacteria Eschericfoia coli and Klebsiella aerogenes and the rumen bacterium Megasphaera elsdenii were not or only poorly taken up by the protozoa. Entodinium spp. selectively engulfed bacteria of rumen origin and digested two cellulolytic bacteria rapidly. In contrast, Eremplastron bovis engulfed these cellulolytio bacteria more slowly than other bacteria. Entodinium spp. and the large rumen protozoa digested more bacterial species than the other protozoa but Butyrivibrio fibrisolvens was digested more rapidly than other bacteria by all the protozoa.

The amylolytic activity of the cytoplasmic fraction prepared from 14 single species of rumen ciliate protozoa and two natural mixed populations has been determined using five different assays. All the protozoa contained an a-amylase and the major product of amylose digestion was maltose. The highest activities were found in Eremoplastron bovis, Diploplastron affine, Ophryoscolex caudatus and Polyplastron multivesiculatum grown in vitro on grass and wholemeal flour and the lowest in Ostracodinium obtusum bilobum and Diplodinium pentacanthum under all growth conditions and in cultured Entodinium bursa and E. caudatum. The maximum activities depended on the substrate and with Eremoplastron bovis varied from 2·3 (with starch grains) to 48·8 (with amylose) μtmol maltose produced/mg protein per h.
The distribution of amylase between various rumen fractions has been determined at three times after feeding in sheep containing no ciliate protozoa or seven different protozoal populations. A method to give maximal release of the enzyme from mixed rumen bacteria was devised and three different assay methods were compared. The total rumen amylase varied by up to 2·6 times and was highest in animals that contained only Entodinium caudatum or natural A- or B-type populations. The distribution of amylase activity between rumen fractions varied considerably with the various protozoal populations, that in the protozoal cytoplasm being highest in animals containing Epidinium ecaudatum caudatum (46–53%) and the natural A- and B-type populations (35–57 %) and lowest in the animals containing no protozoa (3–11 %) or only Ostra-codinium obtusum bilobum (6–21%). The reverse was true for the activity in the bacterial fraction which contained little activity in animals with natural A- and B-type populations and over 58% of the total activity in an animal containing only Entodinium caudatum. The lowest activities in all fractions were obtained 1·5 h after feeding. Comparatively little activity was found in the plant debris or clarified rumen fluid fractions from any animal. Using results obtained previously the ratios of carboxymethylcellulase to amylase activities have been calculated and shown to be highest in the protozoal cyto-plasmic fraction of Diplodinium pentacanthum and Ostracodinium obtusum bilobum and lowest in Entodinium spp., Diploplastron affine and Eremoplastron bovis.
Efficacy of the EG95 hydatid vaccine in a macropodid host, the tammar wallaby

In Australia, macropodids are common intermediate hosts for the cestode Echinococcus granulosus, and sylvatic transmission is maintained via wild dogs. The parasite causes mortality in a number of macropodid species and the sylvatic cycle provides a source of infection to domestic livestock and humans. We determined the efficacy of the hydatid vaccine, EG95 in the tammar wallaby, Macropus eugenii, challenging either 1 or 9 months post-vaccination. EG95 provides similar protection to that seen in sheep (96–100%). Control tammars were significantly more likely to become infected (odds ratio 29·44; CI 4·13, 209·97; P=0·001) and to develop more cysts (count ratio 26·69; CI 5·83, 122·19; P<0·001). The vaccination may be beneficial if administered pre-release in captive breeding programmes for endangered macropodids. Further work to develop oral delivery methods may enable vaccine administration of wild animals and thereby a reduction in sylvatic transmission.

Infection of small macropodids with the larval stage of Echinococcus granulosus can cause fatalities as well as significant pulmonary impairment and other adverse sequelae. The brush-tailed rock-wallaby (Petrogale penicillata) is a small macropodid listed as vulnerable on the IUCN's Red List of Threatened Species. This study used radiographic techniques to determine the prevalence and severity of pulmonary hydatid infection and growth rates of hydatid cysts in a wild population of this macropodid. The overall prevalence was 15·3% (9/59 animals) with 20·0% (8/40 animals) of adults infected. During the study period, the death of at least 1 infected animal was directly attributed to pulmonary hydatidosis. Rapid cyst growth occurred in some animals (up to 43% increase in cyst volume in 3 months). Cyst volume reduced lung capacity by up to 17%. Secondary pulmonary changes were uncommon but, in 1 animal, resulted in reduction in lung capacity by approximately 50%. Infection was associated with a higher blood urea concentration, but no significant differences in other blood variables were detected. These results indicate that hydatid infection may be a significant risk to threatened populations of small macropodids and should be addressed in conservation management plans for these animals.
Impact of Interfacial Nitridation of HfO2 High-k Gate Dielectric Stack on 4H-SiC

HfO2 films were grown on SiO2/4H-SiC and SiON/4H-SiC layers by evaporation of metallic Hf in an electron beam deposition system followed by thermal oxidation. X-ray photoelectron spectroscopy confirmed the formation of HfO2 films. There is no evidence of formation of hafnium silicide or carbon pile up at the surface as well as at the interfacial layer. Electrical measurements show the presence of fewer slow traps in the HfO2/SiON gate dielectric stack on 4H-SiC and comparable values of interface state density. The HfO2/SiON stack layer improves leakage current characteristics with a higher breakdown field and has better reliability under electrical stress.
