Direction of the relationship between methamphetamine use and positive psychotic symptoms in regular methamphetamine users: evidence from a prospective cohort study

Background
Methamphetamine has been consistently associated with positive psychotic symptoms, but little is known about whether the reverse also occurs.
AimsThis study determined whether the relationship between methamphetamine use and positive psychotic symptoms is bidirectional over 12 months. The impact of lifetime psychotic disorders and methamphetamine dependence on these relationships was also examined.
MethodA total of 201 regular (at least monthly) primary methamphetamine users were recruited from free needle and syringe programmes in three Australian cities. Data on the frequency of methamphetamine and other drug use (from Timeline Followback inteviews) and the severity of positive psychotic symptoms (using the Brief Psychiatric Rating Scale) in the past 2 weeks were collected in 12 contiguous monthly face-to-face interviews (mean of 9.14/11 (s.d. = 3.16) follow-ups completed). Diagnoses were derived using the Psychiatric Research Interview for DSM-IV Substance and Mental Disorders.
ResultsThe mean age of participants was 31.71 years (s.d. = 8.19) and 39% (n = 77) were women. At baseline 55% (n = 110) were dependent on methamphetamine and 51% (n = 102) had a lifetime psychotic disorder. Cross-lagged dynamic panel models found a significant bidirectional relationship between psychotic symptoms and methamphetamine use (Comparative Fit Index (CFI) = 0.94, standardised root mean square residual (SRMR) = 0.05, root mean square error of approximation (RMSEA) = 0.05, 95% CI 0.04–0.06). The magnitude of the relationship in each direction was similar, and the presence of methamphetamine dependence or a lifetime psychotic disorder did not have an impact on results.
ConclusionsA dynamic, bidirectional relationship between methamphetamine and psychotic symptoms of similar magnitude in each direction was found over 1 year. This suggests integrated treatments that target methamphetamine, psychotic symptoms and their interrelationship may be of most benefit.

Is capitalist production unjust? It is easy to think, upon first reading Marx, that he answers this question in the affirmative. And I shall argue that this naive reading is correct. This needs to be argued, however, for a more careful scrutiny of Marx's writings reveals passages in which he seems to call capitalist production just or fair. Relying upon these passages, Robert Tucker and Allen W. Wood have urged that, in Wood's words,
it is simply not the case that Marx's condemnation of capitalism rests on some conception of justice (whether explicit or implicit), and those who attempt to reconstruct a “Marxian idea of justice” from Marx's manifold charges against capitalism are at best only translating Marx's critique of capitalism, or some aspect of it, into what Marx himself would have consistently regarded as a false, ideological, or “mystified” form.
What Marx regarded as false and mystified, however, is not the practice of assessing social institutions as just or unjust, but rather the picture of those institutions, and especially of capitalist production, to be found in bourgeois ideology.

Philosophers often contrast personal authority to authority vested in offices. Some such distinction is traditional and sometimes useful. But it does not provide us with an exhaustive classification of the types of authority, for there is a third type of authority that I shall argue is more fundamental than these two. Let us start with the types marked out by the usual distinction.
Consider first the sort of authority illustrated by the following sentences:
(1) Smith is an authority on physics.
(2) Smith has (some) authority as a physicist.
(3) Smith's views (utterances) on physics have (some) authority.
(4) O.K., I believe you—after all, you're the authority on physics!
A person is an authority in virtue of possessing extensive knowledge of a field or subject-matter. There seem to be no limits on what the field or subject-matter can be—for instance, one can be an authority on trivia.

Background
Previous studies using resting-state functional neuroimaging have revealed alterations in whole-brain images, connectome-wide functional connectivity and graph-based metrics in groups of patients with schizophrenia relative to groups of healthy controls. However, it is unclear which of these measures best captures the neural correlates of this disorder at the level of the individual patient.
MethodsHere we investigated the relative diagnostic value of these measures. A total of 295 patients with schizophrenia and 452 healthy controls were investigated using resting-state functional Magnetic Resonance Imaging at five research centres. Connectome-wide functional networks were constructed by thresholding correlation matrices of 90 brain regions, and their topological properties were analyzed using graph theory-based methods. Single-subject classification was performed using three machine learning (ML) approaches associated with varying degrees of complexity and abstraction, namely logistic regression, support vector machine and deep learning technology.
ResultsConnectome-wide functional connectivity allowed single-subject classification of patients and controls with higher accuracy (average: 81%) than both whole-brain images (average: 53%) and graph-based metrics (average: 69%). Classification based on connectome-wide functional connectivity was driven by a distributed bilateral network including the thalamus and temporal regions.
ConclusionThese results were replicated across the three employed ML approaches. Connectome-wide functional connectivity permits differentiation of patients with schizophrenia from healthy controls at single-subject level with greater accuracy; this pattern of results is consistent with the ‘dysconnectivity hypothesis’ of schizophrenia, which states that the neural basis of the disorder is best understood in terms of system-level functional connectivity alterations.

Status epilepticus is among the most dramatic of clinical presentations encountered by emergency room physicians, neurologists, neurosurgeons and intensivists. While progress in its management has been aided significantly with an increasing number of effective treatment options, improved diagnostic methods and more effective monitoring, poor outcomes and diagnostic failures are still frequently encountered. Refractory cases still carry significant morbidity and mortality rates, including poor cognitive outcomes. This review discusses basic pathophysiology and management of status epilepticus, neuroimaging findings, the role of continuous electroencephalogram monitoring and nonconvulsive status epilepticusas well as recent developments in treatment options for refractory cases.

Stocking characteristics and perceived increases in sales among small food store managers/owners associated with the introduction of new food products approved by the Special Supplemental Nutrition Program for Women, Infants, and Children

Objective
The present study assessed the impact of the 2009 food packages mandated by the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) on perceived sales, product selection and stocking habits of small, WIC-authorized food stores.
DesignA cross-sectional study involving in-depth interviews with store managers/owners.
SettingSmall, WIC-authorized food stores in eight major cities in the USA.
SubjectsFifty-two store managers/owners who had at least 1 year of experience in the store prior to study participation.
ResultsThe WIC-approved food products (fresh, canned and frozen fruits; fresh, canned and frozen vegetables; wholegrain/whole-wheat bread; white corn/whole-wheat tortillas; brown rice; lower-fat milk (<2 %)) were acquired in multiple ways, although acquisition generally occurred 1–2 times/week. Factors such as customer requests (87 %), refrigerator/freezer availability (65 %) and profitability (71 %) were rated as very important when making stocking decisions. Most managers/owners perceived increases in sales of new WIC-approved foods including those considered most profitable (wholegrain/whole-wheat bread (89 %), lower-fat milk (89 %), white corn/whole wheat tortillas (54 %)), but perceived no changes in sales of processed fruits and vegetables. Supply mechanisms and frequency of supply acquisition were only moderately associated with perceived sales increases.
ConclusionsRegardless of type or frequency of supply acquisition, perceived increases in sales provided some evidence for the potential sustainability of these WIC policy efforts and translation of this policy-based strategy to other health promotion efforts aimed at improving healthy food access in underserved communities.

The business session for Commission 37 was held on 11 August 2009 at the IAU General Assembly in Rio de Janeiro. The meeting was attended by about a dozen members of our Comission, including President Elmegreen, VP Carraro and several committee members. We introduced ourselves and then went through a powerpoint presentation first prepared by outgoing President Hatzidimitriou and revised by incoming President Elmegreen. The contents of the powerpoint presentation are given in this summary.

Star clusters are valuable tools for theoretical and observational astronomy across a wide range of disciplines from cosmology to stellar spectroscopy. For example, properties of globular clusters are used to constrain stellar evolutionary models, nucleosynthesis and chemical evolution, as well as the star formation and assembly histories of galaxies and the distribution of dark matter in present-day galaxies. Open clusters are widely used as stellar laboratories for the study of specific stellar phenomena (e.g., various emission-line stars, pulsating pre-MS stars, magnetic massive stars, binarity, stellar rotation, etc.). They also provide observational constraints on models of massive star evolution and of Galactic disk formation and chemical evolution.

A moratorium in the US on referrals of Medicare and Medicaid patients to new cardiac, orthopedic, or surgical specialty hospitals by physician-investors was recently lifted, yet the considerable controversy stirred by this growing hospital organizational form continues. This paper calls attention to the peculiar trade-offs introduced by entry of these specialty hospitals, and highlights distinctions among their different types. New policy approaches should be open to the notion that all physician-owned specialty hospitals need not necessarily be regulated in the same way. And caution will be required not to compromise the capacity of community general hospitals to service more complex patients and to maintain the social safety net.

Summary
INTRODUCTION
The tag-line that “people are our greatest assets” has been heard time and time again in the popular media. Peter Drucker, often regarded as the father of modern management, reinforced the importance of human assets when he said that “ the single most important competitive advantage will be…the ability of a company to attract, hold and motivate workers…”.
Nevertheless, retrenchment, redundancies and job cuts are equally everyday news. In this situation, do workers actually believe that they are treated as if they were the firm'sgreatest assets? Is it all lip service?
This paper looks at the experience of Brunei Shell Petroleum Company (BSP) in attracting and developing its professional workforce. It is hoped that our experience goes some way towards at least giving our employees, Bruneians and expatriates alike, a real sense that they “make a difference” in building a competitive edge for BSP.
BSP PEOPLE STRATEGY
The current BSP People Strategy was crafted in 2001. This strategy puts the balance between local and global needs at its centre by putting emphasis on the following:
• The aspirations of the nation will drive our Bruneianization policies, while we at the same time wish to be an employer of choice for both locals and foreigners.
• The use of global competence-based development (CBD) tools and processes to ensure a level playing field.
• The recognition of professional qualifications that meet international standards.
• The importance of maintaining an international flavour in our workforce.
• The increasing use of international assignments for Bruneians to develop their skills and leadership.
An Early to Middle Saxon Settlement at Quarrington, Lincolnshire

Excavations undertaken at Quarrington, near Sleaford, Lincolnshire, revealed part of an Early and Middle Saxon settlement with associated fields or enclosures. Several timber buildings were identified, both round and rectangular, and there was evidence of ferrous and non-ferrous metalworking at the site. Finds included a large and regionally significant pottery assemblage, showing that Quarrington had an extensive trading network and obtained pottery from Lincolnshire and Leicestershire in the Early Saxon period. Pottery from the Northamptonshire area dominated the assemblage in the Middle Saxon period, with a smaller component from Lincolnshire and a limited quantity from Ipswich. Spatial variations in the Saxon ceramic assemblage suggest that the focus of settlement shifted towards the west or north west in the Middle Saxon period.
Few rural sites of Early and Middle Saxon date have produced sufficient faunal material to permit an analysis of changes during the fifth to eighth centuries until now; the large bone assemblage from Quarrington indicates changes in diet and the husbandry of domestic animals through time, with sheep becoming more important as pig diminished. Alterations to the slaughter profiles of cattle and sheep were also evident and may indicate changes from subsistence to surplus production. An apparently isolated Neolithic cremation and a post-medieval stone building were also revealed.
