Background:
Spinal muscular atrophy (SMA) is a devastating rare disease that affects individuals regardless of ethnicity, gender, and age. The first-approved disease-modifying therapy for SMA, nusinursen, was approved by Health Canada, as well as by American and European regulatory agencies following positive clinical trial outcomes. The trials were conducted in a narrow pediatric population defined by age, severity, and genotype. Broad approval of therapy necessitates close follow-up of potential rare adverse events and effectiveness in the larger real-world population.
Methods:The Canadian Neuromuscular Disease Registry (CNDR) undertook an iterative multi-stakeholder process to expand the existing SMA dataset to capture items relevant to patient outcomes in a post-marketing environment. The CNDR SMA expanded registry is a longitudinal, prospective, observational study of patients with SMA in Canada designed to evaluate the safety and effectiveness of novel therapies and provide practical information unattainable in trials.
Results:The consensus expanded dataset includes items that address therapy effectiveness and safety and is collected in a multicenter, prospective, observational study, including SMA patients regardless of therapeutic status. The expanded dataset is aligned with global datasets to facilitate collaboration. Additionally, consensus dataset development aimed to standardize appropriate outcome measures across the network and broader Canadian community. Prospective outcome studies, data use, and analyses are independent of the funding partner.
Conclusion:Prospective outcome data collected will provide results on safety and effectiveness in a post-therapy approval era. These data are essential to inform improvements in care and access to therapy for all SMA patients.
A review of guidance on fish consumption in pregnancy: is it fit for purpose?

Objective
Public health messages to reduce Hg exposure for pregnant women have focused exclusively on advice on fish consumption to limit Hg exposure, with little account being taken of the positive contribution of fish to nutritional quality. The aim of the present review was to compare and contrast the content and presentation of national guidelines on fish consumption in pregnancy, and comment on their evidence base and impact on consumption.
DesignWe searched for national and international guidelines on fish consumption in pregnancy using Internet search strategies. The detailed content and style of presentation of the guidelines were compared. The evidence base for the guidelines, and evidence for the impact of the guidelines on fish consumption levels, were assessed.
ResultsWe identified nineteen national guidelines and three international guidelines. There was great variation in the content, complexity and presentation style. The guidelines were based largely on the Hg content of fish with far less consideration being given to the positive beneficial effects of nutrients provided by fish. The complexity of the guidelines may lead to pregnant women reducing their fish intake, or not eating fish at all.
ConclusionsGuidelines on fish consumption in pregnancy should take the beneficial effects of fish into account. Guidelines need to be clear and memorable, and appropriately disseminated, to achieve impact. Guidelines could include visual rather than narrative content. Use of technology, for example apps, could enable women to record their fish consumption in real time and log compliance with guidance over a week or other time period.

Objective
Concern has recently been expressed about Pb levels in Pb-shot game meat. Our aim was to determine the consumption of game birds in a representative sample population in the UK, and in children and women of childbearing age in particular.
DesignPopulation-based cross-sectional cohort study. Data from 4 d diet diaries from the UK National Diet and Nutrition Survey (NDNS; 2008–2010) were extracted to analyse data on game bird consumption in the sample population, in women of childbearing age (15–45 years old) and in children ≤6 years old.
SettingHome-based study in representative areas of the UK.
SubjectsParticipants in the NDNS (2008–2010; n 2126, age 1·5 to >65 years).
ResultsFifty-eight participants (2·7 %) reported eating game birds. The mean intake was 19·5 (sd 18·1) g/d (median 15·6, range 1·3–92·9 g/d). In women of childbearing age (15–45 years), 11/383 (2·9 %) reported eating game birds, with a mean intake of 22·4 (sd 25·8) g/d (median 15·6, range 2·0–92·9 g/d). In children aged ≤6 years old, 3/342 (0·9 %) were reported as eating game birds, with a mean intake of 6·8 (sd 9·7) g/d (median 2·4, range 1·3–23·2 g/d).
ConclusionsThe prevalence of consumption of game birds by women of childbearing age and children ≤6 years old was relatively low and intakes were small. However, any exposure to Pb in these two groups is undesirable. As are uncertainties about the ability of the diet diary method to capture the consumption of food items that are infrequently consumed, alternative methods of capturing these data should be used in future studies.

The Dominion Radio Astrophysical Observatory (DRAO) is carrying out a survey as part of an international collaboration to image the northe, at a common resolution, in emission from all major constituents of the interstellar medium; the neutral atomic gas, the molecular gas, the ionised gas, dust and relativistic plasma. For many of these constituents the angular resolution of the images (1 arcmin) will be more than a factor of 10 better than any previous studies. The aim is to produce a publicly-available database of high resolution, high-dynamic range images of the Galaxy for multi-phase studies of the physical states and processes in the interstellar medium. We will sketch the main scientific motivations as well as describe some preliminary results from the Canadian Galactic Plane Survey/Releve Canadien du Plan Galactique (CGPS/RCPG).

A description is given of the life-cycle in Britain of Theocolax formiciformis Westw. (Pteromalidae), a parasite of Anobium punctatum (Deg.) (Anobiidae). Adults emerge from infested wood in large numbers from April to June. Eggs are laid through the wood surface and use was made of this habit as a means of rearing this parasite under observation in the laboratory. Anobium larvae were placed individually in channels gouged in the surface of 3-inch squares of plywood. They were covered with tracing paper and the parasites were caged on this by means of a glass ring covered with a glass plate. Development from egg to adult at 22 and 25°C., and 75 per cent, relative humidity, averaged about five and six weeks, respectively, compared with 12 weeks outdoors in summer. The number of progeny per female was highest (approximately five) at 22°C., at which the ratio of males to females was 1:3. Only about one per cent, of the adults reared were winged. The use of Theocolax as a biological-control agent is not considered practical.

Division X provides a common theme for astronomers using radio techniques to study a vast range of phenomena in the Universe, from exploring the Earth's ionosphere or making radar measurements in the Solar System, via mapping the distribution of gas and molecules in our own Galaxy and in other galaxies, to study the vast explosive processes in radio galaxies and QSOs and the faint afterglow of the Big Bang itself.

There have been important advances in radio astronomy in the last three years. New discoveries both at the galactic and extragalactic scale have been reported over this period and we highlight here several of them. The outstanding results of the Wilkinson Microwave Anisotropy Probe satellite, allowing an accurate determination of the main cosmological constants, are certainly among the most important. At the international level, the consolidation of the Atacama Large Millimeter Array project, with participation of the USA, Europe, and Japan and an estimated cost of around one billion US dollars, takes the construction of radio telescopes to a new level of complexity and potential. We also include the Progress Report of the Working Group on Historic Radio Astronomy, that includes a description of the duties and activities of this recently created working group.
Nursing Home Outbreak of Influenza A (H3N2): Evaluation of Vaccine Efficacy and Influenza Case Definitions

Objectives:
Describe an outbreak of influenza A (H3N2); provide an analysis of vaccine efficacy; measure the sensitivity specificity, and positive predictive value of 3 clinical case definitions of influenza.
Setting:A nursing home in Washington County, Maryland. The outbreak involved 52 residents (attack rate = 47.7%) and at least 10 of 140 employees (minimum attack rate = 7.1%).
Results:Twenty-five residents exhibited a 4-fold or greater increase in titer to influenza A/Sichuan/2/87. Vaccine efficacy was measured at -7.1%, suggesting that the influenza vaccine in 1988/1989 did not offer optimal protection against influenza A infection for the institutionalized elderly.
Conclusions:The outbreak was a clear indicator of the need for rapid diagnosis. With the use of rapid diagnostic tests, influenza A could have been detected in time to use amantadine.
Effects of dietary excesses of vitamins A and D on some constituents of the blood of chicks

1. Groups of chicks were given diets containing four levels of vitamins A and D, 1, 10, 100 and 1000 times the basal level, in all sixteen combinations, with the object of investigating a possible antagonism between the two vitamins.
2. Only diets containing 1000 times the basal level (approx. 1700 times the dietary requirements) of one or both vitamins depressed growth and induced changes in the blood.
3. The packed cell volume was substantially reduced from 4 weeks of age in the chicks given the highest level of vitamin A. It is suggested that this was due to an effect of the vitamin on the fragility of the red cells and thus on their life span.
4. Chicks given the toxic level of vitamin D showed an increase in plasma calcium and a decrease in plasma inorganic phosphorus. The highest level of vitamin A depressed plasma Ca without influencing plasma inorganic P. Increasing amounts of vitamin A given in combination with the highest level of vitamin D caused a progressive increase in the plasma inorganic P.
5. The highest level of dietary vitamin A significantly increased the activity in the plasma of three lysosomal enzymes: acid phosphatase, β-glucuronidase and arylsulphatase. Excess vitamin D given in conjunction with the basal level of vitamin A significantly depressed the plasma acid phosphatase and the activity of this enzyme increased with increasing amounts of vitamin A. Excess vitamin D had no influence on the other hydrolases studied.
6. A marked antagonism between the effects of excessive amounts of the two vitamins occurred only in respect of their actions on the plasma levels of Ca, inorganic P and acid phosphatase, all of which are involved in bone metabolism.
