Summary
Almost 40 years ago David Barker made his observation that poor in utero growth increased cardiovascular disease risk in the offspring. A few years prior to this, the first baby to be conceived through IVF was born. Since then, an estimated 8 million babies worldwide have been born via one form or another of Assisted Reproductive Technology (ART). However, data from experimental animal and human clinical studies have highlighted the period around conception as being particularly sensitive to sub-optimal environmental conditions. Furthermore, there is growing concern that aspects of the ART procedures themselves may alter fetal and neonatal growth and increase the incidence of cardiovascular and metabolic diseases, cancer, asthma and neurodevelopmental issues in the children. However, a large degree of confounding factors including parental infertility, disparity in ART culture media and methods, and even the design of the follow-up studies, mean that further investigations are required in the definition of causal relationships between ART and child health.
Antibiotic prescribing for acute gastroenteritis during ambulatory care visits—United States, 2006–2015

Objective:
To describe national antibiotic prescribing for acute gastroenteritis (AGE).
Setting:Ambulatory care.
Methods:We included visits with diagnoses for bacterial and viral gastrointestinal infections from the National Ambulatory Medical Care Survey and National Hospital Ambulatory Medical Care Survey (NAMCS/NHAMCS; 2006–2015) and the IBM Watson 2014 MarketScan Commercial Claims and Encounters Database. For NAMCS/NHAMCS, we calculated annual percentage estimates and 99% confidence intervals (CIs) of visits with antibiotics prescribed; sample sizes were too small to calculate estimates by pathogen. For MarketScan, we used Poisson regression to calculate the percentage of visits with antibiotics prescribed and 95% CIs, including by pathogen.
Results:We included 10,210 NAMCS/NHAMCS AGE visits; an estimated 13.3% (99% CI, 11.2%–15.4%) resulted in antibiotic prescriptions, most frequently fluoroquinolones (28.7%; 99% CI, 21.1%–36.3%), nitroimidazoles (20.2%; 99% CI, 14.0%–26.4%), and penicillins (18.9%; 99% CI, 11.6%–26.2%). In NAMCS/NHAMCS, antibiotic prescribing was least frequent in emergency departments (10.8%; 99% CI, 9.5%–12.1%). Among 1,868,465 MarketScan AGE visits, antibiotics were prescribed for 13.8% (95% CI, 13.7%−13.8%), most commonly for Yersinia (46.7%; 95% CI, 21.4%–71.9%), Campylobacter (44.8%; 95% CI, 41.5%–48.1%), Shigella (39.7%; 95% CI, 35.9%–43.6%), typhoid or paratyphoid fever (32.7%; (95% CI, 27.2%–38.3%), and nontyphoidal Salmonella (31.7%; 95% CI, 29.5%–33.9%). Antibiotics were prescribed for 12.3% (95% CI, 11.7%–13.0%) of visits for viral gastroenteritis.
Conclusions:Overall, ∼13% of AGE visits resulted in antibiotic prescriptions. Antibiotics were unnecessarily prescribed for viral gastroenteritis and some bacterial infections for which antibiotics are not recommended. Antibiotic stewardship assessments and interventions for AGE are needed in ambulatory settings.

Background
In the UK, 11.8% of expectant mothers undergo an elective caesarean section (ELCS) representing 92 000 births per annum. It is not known to what extent this procedure has an impact on mental well-being in the longer term.
AimsTo determine the prevalence and postpartum progression of anxiety and depression symptoms in women undergoing ELCS in Wales.
MethodPrevalence of depression and anxiety were determined in women at University Hospital Wales (2015–16; n = 308) through completion of the Edinburgh Postnatal Depression Scale (EPDS; ≥13) and State-Trait Anxiety Inventory (STAI; ≥40) questionnaires 1 day prior to ELCS, and three postpartum time points for 1 year. Maternal characteristics were determined from questionnaires and, where possible, confirmed from National Health Service maternity records.
ResultsUsing these criteria the prevalence of reported depression symptoms was 14.3% (95% CI 10.9–18.3) 1 day prior to ELCS, 8.0% (95% CI 4.2–12.5) within 1 week, 8.7% (95% CI 4.2–13.8) at 10 weeks and 12.4% (95% CI 6.4–18.4) 1 year postpartum. Prevalence of reported anxiety symptoms was 27.3% (95% CI 22.5–32.4), 21.7% (95% CI 15.8–28.0), 25.3% (95% CI 18.5–32.7) and 35.1% (95% CI 26.3–44.2) at these same stages. Prenatal anxiety was not resolved after ELCS more than 1 year after delivery.
ConclusionsWomen undergoing ELCS experience prolonged anxiety postpartum that merits focused clinical attention.

Silurian crinoids reached maximum abundance on shallow shelf bottoms of skeletal-rich carbonate. In-place crinoid preservation in the Wenlock Limestone of Dudley, England, provides a community model for these occurrences, showing a pattern among crinoids of high taxonomic diversity, conspecific clustering, relatively robust morphology, and numerical dominance of other invertebrates. Densities of about 40 crinoid individuals per m2 occupied a feeding level between 10 cm and 1 m above the bottom and may have effected trophic control over other fauna. Beneath the crinoids lived a second level of bryozoa, a third level of tabulates, and a sparse fauna of brachiopods and other solitary organisms. Crinoids also exercised significant biologic control of substrate. In contrast to the Dudley assemblage, Silurian communities of terrigenous, soft-bottom environments contained crinoids of low abundance, low taxonomic diversity, and small size. In the terrigenous communities, crinoids appear to have had no major effect on either trophic structure or substrate. High sedimentation rates limited the role of crinoids in such communities, and the dominant organisms were deposit feeders and solitary, low-level suspension feeders.

Controlling neuropathic pain is an unmet medical need and we set out to identify new therapeutic candidates. AV411 (ibudilast) is a relatively nonselective phosphodiesterase inhibitor that also suppresses glial-cell activation and can partition into the CNS. Recent data strongly implicate activated glial cells in the spinal cord in the development and maintenance of neuropathic pain. We hypothesized that AV411 might be effective in the treatment of neuropathic pain and, hence, tested whether it attenuates the mechanical allodynia induced in rats by chronic constriction injury (CCI) of the sciatic nerve, spinal nerve ligation (SNL) and the chemotherapeutic paclitaxel (Taxol¯). Twice-daily systemic administration of AV411 for multiple days resulted in a sustained attenuation of CCI-induced allodynia. Reversal of allodynia was of similar magnitude to that observed with gabapentin and enhanced efficacy was observed in combination. We further show that multi-day AV411 reduces SNL-induced allodynia, and reverses and prevents paclitaxel-induced allodynia. Also, AV411 cotreatment attenuates tolerance to morphine in nerve-injured rats. Safety pharmacology, pharmacokinetic and initial mechanistic analyses were also performed. Overall, the results indicate that AV411 is effective in diverse models of neuropathic pain and support further exploration of its potential as a therapeutic agent for the treatment of neuropathic pain.

Research on communication between glia and neurons has increased in the past decade. The onset of neuropathic pain, a major clinical problem that is not resolved by available therapeutics, involves activation of spinal cord glia through the release of proinflammatory cytokines in acute animal models of neuropathic pain. Here, we demonstrate for the first time that the spinal action of the proinflammatory cytokine, interleukin 1 (IL-1) is involved in maintaining persistent (2 months) allodynia induced by chronic-constriction injury (CCI). The anti-inflammatory cytokine IL-10 can suppress proinflammatory cytokines and spinal cord glial amplification of pain. Given that IL-1 is a key mediator of neuropathic pain, developing a clinically viable means of long-term delivery of IL-10 to the spinal cord is desirable. High doses of intrathecal IL-10-gene therapy using naked plasmid DNA (free pDNA-IL-10) is effective, but the dose required limits its potential clinical utility. Here we show that intrathecal gene therapy for neuropathic pain is improved sufficiently using two, distinct synthetic polymers, poly(lactic-co-glycolic) and polyethylenimine, that substantially lower doses of pDNA-IL-10 are effective. In conclusion, synthetic polymers used as i.t. gene-delivery systems are well-tolerated and improve the long-duration efficacy of pDNA-IL-10 gene therapy.
Efficacy of olanzapine combined with valproate or lithium in the treatment of dysphoric mania

Background
Few controlled studies examine the treatment of depressive features in mania.
AimsTo evaluate the efficacy of olanzapine, in combination with lithium or valproate, for treating depressive symptoms associated with mania.
MethodSecondary analysis of a 6-week, double-blind, randomised study of olanzapine (5–20 mg/day) or placebo combined with ongoing valproate or lithium open treatment for 344 patients in mixed or manic episodes. This analysis focused on a dysphoric subgroup with baseline Hamilton Rating Scale for Depression (HRSD) total scores of 20 or over contrasted with non-dysphoric patients.
ResultsIn the dysphoric subgroup (n=85) mean HRSD total score improvement was significantly greater in olanzapine co-therapy patients than in those receiving placebo plus lithium or valproate (P<0.001). Substantial contributors to this superiority included the HRSD Maier sub-scale (P=0.013) and the suicide item (P=0.001). Total Young Mania Rating Scale improvement was also superior with olanzapine co-therapy.
ConclusionsIn patients with acute dysphoric mania, addition of olanzapine to ongoing lithium or valproate monotherapy significantly improved depressive symptom, mania and suicidality ratings.
Attention in HIV-infected children: Results from the Hemophilia Growth and Development Study

Attentional functioning was examined in three groups of 7- to 19-year-old male participants with hemophilia: (1) HIV seronegative controls (HIV−, N = 66), (2) HIV seropositive participants with CD4+ lymphocyte counts greater than or equal to 200 (HIV+ CD4+ ≥200, N = 79), and (3) severely immune suppressed HIV seropositive participants (HIV+ CD4+ <200, N = 28). Two measures sensitive to attention deficits were used: the Continuous Performance Test (CPT) and the Span of Apprehension (Span). On the CPT, there was a decrement in attention in both HIV+ groups, as indexed by an increase in false alarm rate from Block 1 to Block 3, that was not present in the HIV− group. The longer the HIV+ children were required to sustain attention to the CPT, the more they responded to the incorrect stimulus. This effect decreased as age increased. Span percent correct and latency to correct were associated with the presence of a premorbid history of intracerebral hemorrhage, but were not sensitive to HIV status or degree of immune suppression in the HIV+ children, suggesting morbidity related to hemophilia. The remaining CPT and Span variables—hit rate, sensitivity, latency, percent correct, and latency to correct—showed the expected associations with age, but none showed conclusive associations with HIV status or immune suppression in the HIV+ participants. Electrically polarizable materials in an inhomogeneous electric field experience a dielectrophoretic force which is proportional to the gradient of the square of the electric field. For high frequency radiation, the time averaged electromagnetic field provides a dc force. The force exerted on a sapphire and a metal sphere by the microwave field in a single mode resonant cavity has been measured. The force measurements have been made at equally spaced points along three orthogonal axes centered in a cylindrical cavity operating in the TE111 mode using 20 watts of microwave power at about 4.84 GHz. These data are compared to the simple theory of dielectrophoresis on a small sphere and verify that, for this mode and in the absence of other forces, there is a unique and stable equilibrium position at the center of the cavity. Measurements of the quality factor of the cavity provide a means of making an absolute check to the theory. The TE111 mode is one of an infinite set of modes predicted to have stable equilibrium positions that exist away from the cavity walls. It is proposed that dielectrophoresis in a microwave resonant cavity can be used to position samples for material processing applications in a microgravity environment. Knowledge of these forces may also be important for interpreting thermogravimetric measurements made in a microwave oven.
