Complementarity between the updated version of the front-of-pack nutrition label Nutri-Score and the food-processing NOVA classification

Objective:
To compare the initial and the updated versions of the front-of-pack label Nutri-Score (related to the nutritional content) with the NOVA classification (related to the degree of food processing) at the food level.
Design:Using the OpenFoodFacts database – 129,950 food products – we assessed the complementarity between the Nutri-Score (initial and updated) with the NOVA classification through a correspondence analysis. Contingency tables between the two classification systems were used.
Settings:The food offer in France.
Participants:Not applicable.
Results:With both versions (i.e. initial and updated) of the Nutri-Score, the majority of ultra-processed products received medium to poor Nutri-Score ratings (between 77·9 % and 87·5 % of ultra-processed products depending on the version of the algorithm). Overall, the update of the Nutri-Score algorithm led to a reduction in the number of products rated A and B and an increase in the number of products rated D or E for all NOVA categories, with unprocessed foods being the least impacted (–3·8 percentage points (–5·2 %) rated A or B and +1·3 percentage points (+12·9 %) rated D or E) and ultra-processed foods the most impacted (–9·8 percentage points (–43·4 %) rated A or B and +7·8 percentage points (+14·1 %) rated D or E). Among ultra-processed foods rated favourably with the initial Nutri-Score, artificially sweetened beverages, sweetened plant-based drinks and bread products were the most penalised categories by the revision of Nutri-Score while low-sugar flavoured waters, fruit and legume preparations were the least affected.
Conclusion:These results indicate that the update of the Nutri-Score reinforces its coherence with the NOVA classification, even though both systems measure two distinct health dimensions at the food level.
Body composition and anthropometric indicators as predictors of blood pressure: a cross-sectional study conducted in young Algerian adults

Various body indicators are used to predict health risks. However, controversies still exist regarding the best indicators to predict CVD. Using a large number of measurements, our aim was to assess their associations with blood pressure (BP) and to identify the most relevant parameters to be used in health surveillance studies. The population included 589 students (67·2 % women) aged 20–25 years from Constantine (Algeria). Sixteen parameters were considered, including crude body measurements, ratios and body fat indicators based on bioelectrical impedance analysis (BIA). We used multi-adjusted linear regression models to assess the associations between body measurements and BP. According to WHO definitions, underweight, overweight-without obesity, obesity and hypertension (HT) were identified in 6·1, 18·0, 2·4 and 5·1 % of the subjects, respectively. Prevalence of HT was higher in men than in women (11·9 % v. 1·8 %; P < 0·001). In the whole sample, almost all indicators were positively associated with systolic and diastolic BP. The suprailiac skinfold had the strongest associations with systolic (β = 3·498; P < 0·001) and diastolic (β = 2·436; P < 0·001) BP, and as a whole, arm circumferences and weight were also good candidates. The currently used BMI, waist-to-hip, waist-to-height ratio and BIA indictors also predicted BP, but they did not seem to be better determinants of BP than crude anthropometric measurements. This study showed that overweight and HT were already found in the present population of young Algerian adults. Most body indicators were highly associated with BP, but simple anthropometric measurements appeared to be particularly useful to predict BP.
Successful management of invasive rats across a fragmented landscape

Introduced mammalian predators are responsible for the decline and extinction of many native species, with rats (genus Rattus) being among the most widespread and damaging invaders worldwide. In a naturally fragmented landscape, we demonstrate the multi-year effectiveness of snap traps in the removal of Rattus rattus and Rattus exulans from lava-surrounded forest fragments ranging in size from <0.1 to >10 ha. Relative to other studies, we observed low levels of fragment recolonization. Larger rats were the first to be trapped, with the average size of trapped rats decreasing over time. Rat removal led to distinct shifts in the foraging height and location of mongooses and mice, emphasizing the need to focus control efforts on multiple invasive species at once. Furthermore, because of a specially designed trap casing, we observed low non-target capture rates, suggesting that on Hawai‘i and similar islands lacking native rodents the risk of killing non-target species in snap traps may be lower than the application of rodenticides, which have the potential to contaminate food webs. These efforts demonstrate that targeted snap-trapping is an effective removal method for invasive rats in fragmented habitats and that, where used, monitoring of recolonization should be included as part of a comprehensive biodiversity management strategy.
The Complex Cancer Care Coverage Environment — What is the Role of Legislation?: A Case Study from Massachusetts

Over the past decades, anti-cancer treatments have evolved rapidly from cytotoxic chemotherapies to targeted therapies including oral targeted medications and injectable immunooncology and cell therapies. New anti-cancer medications come to markets at increasingly high prices, and health insurance coverage is crucial for patient access to these therapies. State laws are intended to facilitate insurance coverage of anti-cancer therapies.
Using Massachusetts as a case study, we identified five current cancer coverage state laws and interviewed experts on their perceptions of the relevance of the laws and how well they meet the current needs of cancer care given rapid changes in therapies. Interviewees emphasized that cancer therapies, as compared to many other therapeutic areas, are unique because insurance legislation targets their coverage. They identified the oral chemotherapy parity law as contributing to increasing treatment costs in commercial insurance. For commercial insurers, coverage mandates combined with the realities of new cancer medications — including high prices and often limited evidence of efficacy at approval — compound a difficult situation. Respondents recommended policy approaches to address this challenging coverage environment, including the implementation of closed formularies, the use of cost-effectiveness studies to guide coverage decisions, and the application of value-based pricing concepts. Given the evolution of cancer therapeutics, it may be time to evaluate the benefits and challenges of cancer coverage mandates.
Ultra-processed food intake and risk of type 2 diabetes in a French cohort of middle-aged adults

Introduction:
The consumption of ultra-processed foods has been increasing during the last decades, and has been previously associated with increased risks of mortality and several chronic diseases such as cancer, cardiovascular diseases, hypertension, and dyslipidemia. The objective of this study is to assess the prospective associations between consumption of ultra-processed foods and the risk of type 2 diabetes (T2D).
Methods:Overall, 104707 participants aged at least 18 years (median age 41.5 years) from the French NutriNet-Santé cohort (2009–2019) were included. Dietary intakes were collected using repeated 24 hour dietary records (5.7/subject in average), designed to register participants' usual consumption for 3300 different food items. These were categorized according to their degree of processing by the NOVA classification. Associations between ultra-processed food intake and risk of type 2 diabetes were assessed using multivariable Cox proportional hazard models adjusted for known risk factors (sociodemographic, anthropometric, lifestyle, medical history and nutritional factors).
Results:Ultra-processed sugary products, fruits and vegetables, and beverages were the highest contributors of the ultra-processed category (respectively 27.9, 18.5 and 15.6%). Ultra-processed food intake was associated with a higher risk of T2D (n = 821 incident cases; hazard ratio for an absolute increment of 10 in the percentage of ultra-processed foods in the diet = 1.15 (95% confidence interval 1.06 to 1.25); P = 0.0009, median follow-up: 6.0y, 582252 person-years). These results remained statistically significant after adjustment for other metabolic comorbidities, for several markers of the nutritional quality of the diet (red meat and sugary drinks consumption, intakes of saturated fatty acids, sodium, sugar, dietary fiber or Healthy/Western patterns derived by principal component analysis) and after a large range of sensitivity analyses.
Conclusions:In this large observational prospective study, higher consumption of ultra-processed foods in the diet was associated with a higher risk of type 2 diabetes. These results need to be confirmed in other populations and settings, and causality remains to be established. Various dimensions of processing such as nutritional composition of the final product, food additives, contact materials, and neoformed contaminants might play a role in these associations and further studies are needed to better understand their relative contribution. Meanwhile, public health authorities in several countries recently started to recommend privileging unprocessed/minimally processed foods and limiting ultra-processed food consumption.

Introduction
Despite improvements in treatment for patients with severe mental disorders, such as bipolar disorder and schizophrenia, there is still a long delay from first symptoms to correct diagnosis and treatment. Since these illnesses usually manifest themselves within adolescence and young adulthood, this time lag impacts chances for an adequate individual development.
ObjectivesWith rather unspecific symptomatology in early stages of the development of the disorders and the still existing stigmatisation, a broad and low-threshold strategy is needed. In November 2008 an early recognition and prevention centre for mental disorders was opened at the University Hospital in Dresden.
AimsThe aim of this service is to shorten the delay to correct diagnosis and treatment in severe mental disorders. In parallel, early detection instruments are developed and early intervention strategies studied in research projects.
MethodsIn this specialized centre diagnostics, counselling and treatment are provided for subjects aged 12 to 40 by a multidisciplinary team in a low-threshold setting.
Results250 youth and young adults were offered diagnostics and, if appropriate, treatment. In about 2/3 of clients, mental disorders had already manifested themselves, in some cases an additional risk for even more severe developments was ascertained. Counselling and transfer to appropriate treatment is offered to all clients, all are followed up on a regular basis to validate the process and assess the prognostic value of our service.
ConclusionsA broad, low-threshold and multidisciplinary approach for early recognition and treatment of severe mental disorders holds the chance of earlier appropriate care.
Effects of a riboflavin source suitable for use in organic broiler diets on performance traits and health indicators

Riboflavin (vitamin B2) is essential for monogastric animals. It is mainly produced by recombinant microorganisms (Candida famata, Bacillus subtilis and Ashbya gossypii). The availability of genetically modified organism (GMO)-free riboflavin, obligatory in European organic agriculture, is a major issue. Besides, requirements for organic livestock might differ from conventional production because other genotypes and feed formulations are used. The effects of a fermentation suspension with a high native content of riboflavin produced with unmodified A. gossypii by fermentation were investigated at graded dosages as an alternative to conventional (GMO-based) riboflavin in slow-growing broilers on performance traits and health and welfare indicators. In 2 runs with 800 animals each, Ranger Gold™ broilers were fed with 4 dietary treatments. For starter diets (day 1 to 18), treatments included a basal diet (1) without any riboflavin supplementation (negative control, N-C), (2) with conventional riboflavin supplementation (Cuxavit B2 80% riboflavin) at 9.6 mg/kg (positive control, P-C), (3) with riboflavin supplementation from the alternative source at 3.5 mg/kg (A-low) and (4) with riboflavin supplementation from the alternative source at 9.6 mg/kg (A-high). For the finisher diet (day 29 until slaughtering), P-C and A-high were supplemented with 8.0 mg/kg and A-low with 3.5 mg/kg. Diets were formulated according to organic regulations. Animals were kept in floor pens with 20 chickens per pen. Weekly, BW, feed and water consumption were recorded. Every second week, animal-based health and welfare indicators (feather score and footpad dermatitis) were scored. Slaughter traits were assessed for five males and females per pen at 62/63 days of age. Final body weight of A-high differed from N-C and A-low, but not from P-C. From week 2 until six years of age, A-high had a higher daily weight gain when compared to all other groups. With 74.4%, dressing percentage was higher in A-high compared with all other groups (73.3%). Breast percentage of A-low was lower than that of both control groups but did not differ from A-high. The highest frequency of liver scores indicating fatty liver syndrome was found in P-C, followed by N-C and A-low. Feather scores did not respond to treatment; the highest frequency of mild footpad dermatitis was observed in A-high, however at a low prevalence. In conclusion, the tested fermentation suspension with a high native content of riboflavin derived from fermentation of A. gossypii can be used at levels of commercial recommendations as alternative to riboflavin produced from GMO in broiler feeding. Further studies must verify whether riboflavin can be reduced without inducing riboflavin deficiency in slow-growing broilers.

The cytoplasm of soybean seed axes is known to undergo a glass transition as moisture content decreases. In this study we have investigated whether the glass transition is reflected in the macroscopic mechanical properties of the seeds. Constant-deformation-rate compression tests and constant-deformation relaxation tests were performed with a cylindrical indenter on soybean cotyledons equilibrated to moisture contents ranging from 2% to 16% (dry basis). The apparent modulus of elasticity was found to rise sharply as moisture content declined below 10%, showing an inflection point at 9.8%, and a relatively constant elasticity below 7%. Other measures of mechanical behaviour were also constant in the drier ranges, including linear deformation, and plastic deformation. Each of these viscoelastic characteristics showed an inflection point or a departure from a linear function in the region of 7.5–9.8% moisture content, the hydration region below which the tissue is in a glassy state. It is suggested that the viscoelastic qualities of dry soybeans are principally a consequence of the tissues being in the glassy state.

Primary branchial cleft carcinoma is a diagnostic challenge. The majority of cases have been shown, following the application of rigid criteria, to have developed from an occult metastasis elsewhere.
We describe what we believe to be only the second reported case in a patient who fulfils all of the established criteria.

Ceramic remains of fragmentary vessels, believed to be smelting crucibles based on microscopic field investigation, were analyzed using a variety of techniques in order identify the metal being processed and to reconstruct the ancient processing, especially ranges of composition, temperature, atmosphere, and the constraints on processing which each of these processing variables would have imposed. The identification of the metal being processed in the crucible surface residues and the slags provide data to resolve a dispute over the nature of the ancient metallurgical processing at Goltepe; gold and iron have been suggested as alternate metals but were not found. Intensive study of the ceramic and slag microstructure, composition and thermal history was required to establish the processing constraints which will be used to aid future replicative smelting experiments at the site.

Deuteron magnetic resonance line shapes and spin lattice relaxation times are presented for a-Si:D, F and a-SiGe:D, F. These parameters differ from those for typical a-Si:D, H samples, but in some respects are similar to those for an annealed a-Si:D, H sample. The a-SiGe:D, F spectra display an unusually large broad central weakly bound D resonance component and a barely-resolved Ge-D quadrupolar doublet. Comparisons indicate substantial differences in void morphology between the a-Si:D, F and a-SiGe:D, F.
