27 Assessing Differences in Academic Achievement Among a National Sample of Children with Epilepsy Before and During the COVID-19 Pandemic

Objective:
The COVID-19 pandemic significantly disrupted schools and learning formats. Children with epilepsy are at-risk for generalized academic difficulties. We investigated the potential impact of COVID-19 on learning in those with epilepsy by comparing achievement on well-established academic measures among school-age children with epilepsy referred prior to the COVID-19 pandemic and those referred during the COVID-19 pandemic.
Participants and Methods:This study included 466 children [52% male, predominately White (76%), MAge=10.75 years] enrolled in the Pediatric Epilepsy Research Consortium Epilepsy (PERC) Surgery database project who were referred for surgery and seen for neuropsychological testing. Patients were divided into two groups based on a proxy measure of pandemic timing completed by PERC research staff at each site (i.e., “were there any changes to typical in-person administration [of the evaluation] due to COVID?”). 31% of the sample (N = 144) were identified as having testing during the pandemic (i.e., “yes” response), while 69% were identified as having testing done pre-pandemic (i.e., “no” response). Of the 31% who answered yes, 99% of administration changes pertained to in-person testing or other changes, with 1% indicating remote testing. Academic achievement was assessed by performance measures (i.e., word reading, reading comprehension, spelling, math calculations, and math word problems) across several different tests. T-tests compared the two groups on each academic domain. Subsequent analyses examined potential differences in academic achievement among age cohorts that approximately matched grade level [i.e., grade school (ages 5-10), middle school (ages 11-14), and high school (ages 15-18)].
Results:No significant differences were found between children who underwent an evaluation before the pandemic compared to those assessed during the pandemic based on age norms across academic achievement subtests (all p’s > .34). Similarly, there were no significant differences among age cohorts. The average performance for each age cohort generally fell in the low average range across academic skills. Performance inconsistently varied between age cohorts. The youngest cohort (ages 5-10) scored lower than the other cohorts for sight-word reading, whereas this cohort scored higher than the middle cohort (ages 11-14) for math word problems and reading comprehension. There were no significant differences between the two pandemic groups on demographic variables, intellectual functioning, or epilepsy variables (i.e., age of onset, number of seizure medications, seizure frequency).
Conclusions:Academic functioning was generally equivalent between children with epilepsy who underwent academic testing as part of a pre-surgical evaluation prior to the pandemic compared to those who received testing during the pandemic. Additionally, academic functioning did not significantly differ between age cohorts. Children with epilepsy may have entered the pandemic with effective academic supports and/or were accustomed to school disruptions given their seizure history. Replication is needed as findings are based on a proxy measure of pandemic timing and the extent to which children experienced in-person, remote, and hybrid learning is unknown. Children tested a year into the pandemic, after receiving instruction through varying educational methods, may score differently than those tested earlier. Future research can address these gaps. Although it is encouraging that academic functioning was not disproportionately impacted during the pandemic in this sample, children with epilepsy are at-risk for generalized academic difficulties and continued monitoring of academic functioning is necessary.

Objective:
Children with epilepsy are at greater risk of lower academic achievement than their typically developing peers (Reilly and Neville, 2015). Demographic, social, and neuropsychological factors, such as executive functioning (EF), mediate this relation. While research emphasizes the importance of EF skills for academic achievement among typically developing children (e.g., Best et al., 2011; Spiegel et al., 2021) less is known among children with epilepsy (Ng et al., 2020). The purpose of this study is to examine the influence of EF skills on academic achievement in a nationwide sample of children with epilepsy.
Participants and Methods:Participants included 427 children with epilepsy (52% male; MAge= 10.71), enrolled in the Pediatric Epilepsy Research Consortium (PERC) Epilepsy Surgery Database who had been referred for surgery and underwent neuropsychological testing. Academic achievement was assessed by performance measures (word reading, reading comprehension, spelling, and calculation and word-based mathematics) and parent-rating measures (Adaptive Behavior Assessment System (ABAS) Functional Academics and Child Behavior Checklist (CBCL) School Performance). EF was assessed by verbal fluency measures, sequencing, and planning measures from the Delis Kaplan Executive Function System (DKEFS), NEPSY, and Tower of London test. Rating-based measures of EF included the 'Attention Problems’ subscale from the CBCL and 'Cognitive Regulation’ index from the Behavior Rating Inventory of Executive Function (BRIEF-2). Partial correlations assessed associations between EF predictors and academic achievement, controlling for fullscale IQ (FSIQ; A composite across intelligence tests). Significant predictors of each academic skill or rating were entered into a two-step regression that included FSIQ, demographics, and seizure variables (age of onset, current medications) in the first step with EF predictors in the second step.
Results:Although zero-order correlations were significant between EF predictors and academic achievement (.29 < r’s < .63 for performance; -.63 < r’s < -.50 for rating measures), partial correlations controlling for FSIQ showed fewer significant relations. For performance-based EF, only letter fluency (DKEFS Letter Fluency) and cognitive flexibility (DKEFS Trails Condition 4) demonstrated significant associations with performance-based academic achievement (r’s > .29). Regression models for performance-based academic achievement indicated that letter fluency (ß = .22, p = .017) and CBCL attention problems (ß = -.21, p =.002) were significant predictors of sight-word reading. Only letter fluency (ß = .23, p =.006) was significant for math calculation. CBCL Attention Problems were a significant predictor of spelling performance (ß = -.21, p = .009) and reading comprehension (ß = -.18, p =.039). CBCL Attention Problems (ß = -.38, p <.001 for ABAS; ß = -.34, p =.002 for CBCL School) and BRIEF-2 Cognitive Regulation difficulties (ß = -.46, p < .001 for ABAS; ß = -.46, p =.013 for CBCL School) were significant predictors of parent-rated ABAS Functional Academics and CBCL School Performance.
Conclusions:Among a national pediatric epilepsy dataset, performance-based and ratings-based measures of EF predicted performance academic achievement, whereas only ratings-based EF predicted parent-rated academic achievement, due at least in part to shared method variance. These findings suggest that interventions that increase cognitive regulation, reduce symptoms of attention dysfunction, and promote self-generative, flexible thinking, may promote academic achievement among children with epilepsy.

Life course research embraces the complexity of health and disease development, tackling the extensive interactions between genetics and environment. This interdisciplinary blueprint, or theoretical framework, offers a structure for research ideas and specifies relationships between related factors. Traditionally, methodological approaches attempt to reduce the complexity of these dynamic interactions and decompose health into component parts, ignoring the complex reciprocal interaction of factors that shape health over time. New methods that match the epistemological foundation of the life course framework are needed to fully explore adaptive, multilevel, and reciprocal interactions between individuals and their environment. The focus of this article is to (1) delineate the differences between lifespan and life course research, (2) articulate the importance of complex systems science as a methodological framework in the life course research toolbox to guide our research questions, (3) raise key questions that can be asked within the clinical and translational science domain utilizing this framework, and (4) provide recommendations for life course research implementation, charting the way forward. Recent advances in computational analytics, computer science, and data collection could be used to approximate, measure, and analyze the intertwining and dynamic nature of genetic and environmental factors involved in health development.
Placental amino acid transport may be regulated by maternal vitamin D and vitamin D-binding protein: results from the Southampton Women's Survey

Both maternal 25-hydroxyvitamin D (25(OH)D) concentrations during pregnancy and placental amino acid transporter gene expression have been associated with development of the offspring in terms of body composition and bone structure. Several amino acid transporter genes have vitamin D response elements in their promoters suggesting the possible linkage of these two mechanisms. We aimed to establish whether maternal 25(OH)D and vitamin D-binding protein (VDBP) levels relate to expression of placental amino acid transporters. RNA was extracted from 102 placental samples collected in the Southampton Women's Survey, and gene expression was analysed using quantitative real-time PCR. Gene expression data were normalised to the geometric mean of three housekeeping genes, and related to maternal factors and childhood body composition. Maternal serum 25(OH)D and VDBP levels were measured by radioimmunoassay. Maternal 25(OH)D and VDBP levels were positively associated with placental expression of specific genes involved in amino acid transport. Maternal 25(OH)D and VDBP concentrations were correlated with the expression of specific placental amino acid transporters, and thus may be involved in the regulation of amino acid transfer to the fetus. The positive correlation of VDBP levels and placental transporter expression suggests that delivery of vitamin D to the placenta may be important. This exploratory study identifies placental amino acid transporters which may be altered in response to modifiable maternal factors and provides a basis for further studies.

Background
Self-harm is a common reason for Emergency Department (ED) attendance. We aimed to develop a clinical tool to help identify patients at higher risk of repeat self-harm, or suicide, within 6 months of an ED self-harm presentation.
MethodThe tool, the ReACT Self-Harm Rule, was derived using multicentre data from a prospective cohort study. Binary recursive partitioning was applied to data from two centres, and data from a separate centre were used to test the tool. There were 29 571 self-harm presentations to five hospital EDs between January 2003 and June 2007, involving 18 680 adults aged ⩾16 years. We estimated sensitivity, specificity and positive and negative predictive values to measure the performance of the tool.
ResultsA self-harm presentation was classified as higher risk if at least one of the following factors was present: recent self-harm (in the past year), living alone or homelessness, cutting as a method of harm and treatment for a current psychiatric disorder. The rule performed with 95% sensitivity [95% confidence interval (CI) 94–95] and 21% specificity (95% CI 21–22), and had a positive predictive value of 30% (95% CI 30–31) and a negative predictive value of 91% (95% CI 90–92) in the derivation centres; it identified 83/92 of all subsequent suicides.
ConclusionsThe ReACT Self-Harm Rule might be used as a screening tool to inform the process of assessing self-harm presentations to ED. The four risk factors could also be used as an adjunct to in-depth psychosocial assessment to help guide risk formulation. The use of multicentre data helped to maximize the generalizability of the tool, but we need to further verify its external validity in other localities.

William Ringrose Gelston Atkins was appointed Head of the Department of General Physiology at the Plymouth Laboratory in February 1921 at the age of 36 having already at Trinity College, Dublin, applied widely the methods of physics and chemistry to the problems of biology, particularly botany. He was very well equipped to study the physics and chemistry of sea water as an environment for marine life. He served with great distinction at Plymouth, apart from the years of the Second World War, until he retired from the staff in March 1955 and enjoyed only four years of leisure before his death on 4 April 1959.

For a third of a century many and varied studies on sea water have been concentrated upon a single accessible shallow-water station in the English Channel, EI, supplemented by more extensive excursions usually designed to test working hypotheses. This concentration of effort has been richly rewarded. In deep-water oceanography a similar approach is appropriate; a standard station, ‘Cavall’ has therefore been selected in the north-eastern Bay of Biscay in 4700 m of water at lat. 46° 30' N., long. 8° 00' W., nominal, a position worked by the Danish Research Vessel ‘Dana’ in 1922 and 1930. It is not only the nearest deep water position to Plymouth but it has best possible coverage by the Decca Navigator system. For our experimental programme it is essential that the station positions be maintained and repeated precisely. To achieve this a working Decca chartlet (Fig. 1) on a scale of 1:20,000 has been prepared for each station from a portion of the Admiralty Decca Lattice chart no. L 1104. On this a circle of radius one mile is inscribed. Throughout operations the ship is maintained within this circle by stopping work when necessary and steaming back on Decca co-ordinates towards the centre, or beyond to pre-compensate expected drift.

Biological and physical evidence suggests that the area south-west of Ireland is one of loss from the Celtic Sea. In winter the shelf water there may cascade to 400 m. or even deeper (Cooper & Vaux, 1949). All the evidence suggests that new water enters the Celtic Sea and English Channel over the continental slope between Ushant and the Sole submarine promontory 250 miles to the west. No evidence exists that the Atlantic surface waters are ever rich enough to account for the enrichment of the English Channel observed in the nineteen-twenties. Although the existence then of sufficiently rich oceanic surface water cannot be excluded, it is much more probable that the enrichment was brought about by some form of upwelling. The cause of this is thought to be composite. Only a few times in a century may the component phenomena work together to produce the maximum effect. Each component needs separate study.

A theoretical study of the behaviour of ferric phosphate in sea water has been made with existing data. The conditions least favourable for the continued existence of ferric phosphate should occur in the upper layers of the sea during active plant growth. The formation and stability of ferric phosphate is likely to be most favoured under the more acid conditions found in the gut of many animals. Ferric phosphate is likely to be introduced into sea water in animal faeces. Experimental confirmation of this deduction is essential.
In all experimental work involving ferric phosphate in sea water, the need is indicated for very accurate control of hydroxyl-ion activity and probably pressure, and for attention to the time factor and to the physical condition of the precipitated phase. The topic is of geochemical as well as of biological interest.
Dipyridyl is a reagent for ferrous iron. Reasons are set out why in sea and lake water at high dilutions it may also determine ferric iron without presence of an added reducing agent.
Evidence is presented that ferrifluoride plays no part in the iron cycle in he sea.

The new species of Gammarus here described was first noted in 1931 by Prof. H. Munro Fox, to whom I am indebted for the opportunity of examining it. He found it in abundance in the Droitwich district, living in the brackish waters of the River Salwarpe near the town, in the Canal, and in Wyken Slough near Coventry.
“Organic” Phosphorus in Sea Water from the English Channel

Determinations of organic phosphorus (so called) have been made in waters from off Plymouth. The amount varies little with the time of year and averages 0·44 mg. -atom per cubic metre. This is of the same order as determinations in other parts of the North Atlantic. About one-half of this “organic phosphorus” is considered really to be arsenite.

The paper on the ratio of nitrogen to phosphorus in the sea (Cooper, 1937) was written before it was realized that in phosphate determinations, using two ml. of molybdate reagent per 100 ml. of water, the magnitude of the salt error correction factor is changed by the use of a spectral filter. Due to the resulting uncertainty it was felt better to use uncorrected figures for phosphate content rather than to risk a correction which might later prove to be wrong.

Monthly mean sea-water temperatures in Plymouth Sound, prepared by the Plymouth City Meteorologist, are presented as data for ecological studies in this laboratory. In the last 40 years there has been a rise in mean temperature of about 0·3°C.

Three methods of determining extinction coefficients of light in sea water have been compared: (a) on water samples contained in a Pulfrich photometer, (b) from brightness profiles determined by aerial photographic survey, and (c) by photoelectric underwater measurements. The ratios of extinction coefficients for green and red light as determined in 1945 by the Pulfrich photometer and by the brightness profile method are given in Table 3, columns 9 and 10. For green light the ratio is about i-8 and for red light 1-3.
The departure from unity is attributed to the geometry within a confined tube and, within the Pulfrich measuring assembly, of light rays scattered forward by particles present in sea-water samples. In the following year, 1946, comparisons were made between the Pulfrich method and the submarine photometric method of Atkins and Poole. The discrepancies were variable and even larger, the ratios averaging 3-8 for green light and 2-0 for red light.
The brightness profile and submarine photometric methods have not been directly compared, but, by comparing the two sets of determinations against the Pulfrich photometer, it would seem that extinction coefficients determined with the brightness profile method are about 2-2 times those determined by the submarine photometer in green light and about 15 times in red light.
