Multipronged infection control strategy to achieve zero nosocomial coronavirus disease 2019 (COVID-19) cases among Hong Kong healthcare workers in the first 300 days of the pandemic

Background:
Nosocomial outbreaks leading to healthcare worker (HCW) infection and death have been increasingly reported during the coronavirus disease 2019 (COVID-19) pandemic.
Objective:We implemented a strategy to reduce nosocomial acquisition.
Methods:We summarized our experience in implementing a multipronged infection control strategy in the first 300 days (December 31, 2019, to October 25, 2020) of the COVID-19 pandemic under the governance of Hospital Authority in Hong Kong.
Results:Of 5,296 COVID-19 patients, 4,808 (90.8%) were diagnosed in the first pandemic wave (142 cases), second wave (896 cases), and third wave (3,770 cases) in Hong Kong. With the exception of 1 patient who died before admission, all COVID-19 patients were admitted to the public healthcare system for a total of 78,834 COVID-19 patient days. The median length of stay was 13 days (range, 1–128). Of 81,955 HCWs, 38 HCWs (0.05%; 2 doctors and 11 nurses and 25 nonprofessional staff) acquired COVID-19. With the exception of 5 of 38 HCWs (13.2%) infected by HCW-to-HCW transmission in the nonclinical settings, no HCW had documented transmission from COVID-19 patients in the hospitals. The incidence of COVID-19 among HCWs was significantly lower than that of our general population (0.46 per 1,000 HCWs vs 0.71 per 1,000 population; P = .008). The incidence of COVID-19 among professional staff was significantly lower than that of nonprofessional staff (0.30 vs 0.66 per 1,000 full-time equivalent; P = .022).
Conclusions:A hospital-based approach spared our healthcare service from being overloaded. With our multipronged infection control strategy, no nosocomial COVID-19 in was identified among HCWs in the first 300 days of the COVID-19 pandemic in Hong Kong.

Background:
Extensive environmental contamination by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has been reported in hospitals during the coronavirus disease 2019 (COVID-19) pandemic. We report our experience with the practice of directly observed environmental disinfection (DOED) in a community isolation facility (CIF) and a community treatment facility (CTF) in Hong Kong.
Methods:The CIF, with 250 single-room bungalows in a holiday camp, opened on July 24, 2020, to receive step-down patients from hospitals. The CTF, with 500 beds in open cubicles inside a convention hall, was activated on August 1, 2020, to admit newly diagnosed COVID-19 patients from the community. Healthcare workers (HCWs) and cleaning staff received infection control training to reinforce donning and doffing of personal protective equipment and to understand the practice of DOED, in which the cleaning staff observed patient and staff activities and then performed environmental disinfection immediately thereafter. Supervisors also observed cleaning staff to ensure the quality of work. In the CTF, air and environmental samples were collected on days 7, 14, 21, and 28 for SARS-CoV-2 detection by RT-PCR. Patient compliance with mask wearing was also recorded.
Results:Of 291 HCWs and 54 cleaning staff who managed 243 patients in the CIF and 674 patients in the CTF from July 24 to August 29, 2020, no one acquired COVID-19. All 24 air samples and 520 environmental samples collected in the patient area of the CTF were negative for SARS-CoV-2. Patient compliance with mask wearing was 100%.
Conclusion:With appropriate infection control measures, zero environmental contamination and nosocomial transmission of SARS-CoV-2 to HCWs and cleaning staff was achieved.

Objective:
To develop a pediatric research agenda focused on pediatric healthcare-associated infections and antimicrobial stewardship topics that will yield the highest impact on child health.
Participants:The study included 26 geographically diverse adult and pediatric infectious diseases clinicians with expertise in healthcare-associated infection prevention and/or antimicrobial stewardship (topic identification and ranking of priorities), as well as members of the Division of Healthcare Quality and Promotion at the Centers for Disease Control and Prevention (topic identification).
Methods:Using a modified Delphi approach, expert recommendations were generated through an iterative process for identifying pediatric research priorities in healthcare associated infection prevention and antimicrobial stewardship. The multistep, 7-month process included a literature review, interactive teleconferences, web-based surveys, and 2 in-person meetings.
Results:A final list of 12 high-priority research topics were generated in the 2 domains. High-priority healthcare-associated infection topics included judicious testing for Clostridioides difficile infection, chlorhexidine (CHG) bathing, measuring and preventing hospital-onset bloodstream infection rates, surgical site infection prevention, surveillance and prevention of multidrug resistant gram-negative rod infections. Antimicrobial stewardship topics included β-lactam allergy de-labeling, judicious use of perioperative antibiotics, intravenous to oral conversion of antimicrobial therapy, developing a patient-level “harm index” for antibiotic exposure, and benchmarking and or peer comparison of antibiotic use for common inpatient conditions.
Conclusions:We identified 6 healthcare-associated infection topics and 6 antimicrobial stewardship topics as potentially high-impact targets for pediatric research.

Introduction
Little is known about medication adherence among Asians and Asian Americans in psychiatric treatment.
ObjectiveWe conducted a systematic review of studies of Asian American and Asian patients with depression or schizophrenia to understand adherence rates and tools used to measure adherence.
MethodsA key word search of PubMED and PsycINFO, restricted to journal articles available in English or Chinese and published between 1960 and March 2010 was performed. Reference lists of studies meeting inclusion criteria were manually reviewed and content experts were consulted. Two investigators independently reviewed all identified publications for inclusion using predetermined criteria and a pilot tested data-abstraction form.
ResultsOf the 1520 journal articles retrieved, 10 met criteria for inclusion. Adherence rates among patients with schizophrenia ranged from 5–71%; adherence rates among patients with depression ranged from 16–67%. Adherence rates varied among Asian sub-populations: Chinese patients’ rates ranged from 6–56%; Taiwanese patients’ rate was 46–61%; Asian American patients’ rate was 16%; Japanese patients’ rate was 56–71%; and Singaporean patients’ rate was 4.3%. Adherence was measured by: self-report; blood levels; refill rates; chart review; or physician/nurse or family caregiver report.
ConclusionMedication adherence rates varied across clinical populations and country of origin. Nearly all of the rates are lower than many clinicians would consider acceptable. A critical step to research on improving adherence will involve reaching consensus on how to measure rates.

Low-density jets are central to many natural and industrial processes. Under certain conditions, they can develop global oscillations at a limit cycle, behaving as a prototypical example of a self-excited hydrodynamic oscillator. In this study, we perform system identification of a low-density jet using measurements of its noise-induced dynamics in the unconditionally stable regime, prior to both the Hopf and saddle-node points. We show that this approach can enable prediction of (i) the order of nonlinearity, (ii) the locations and types of the bifurcation points (and hence the stability boundaries) and (iii) the resulting limit-cycle oscillations. The only assumption made about the system is that it obeys a Stuart–Landau equation in the vicinity of the Hopf point, thus making the method applicable to a variety of hydrodynamic systems. This study constitutes the first experimental demonstration of system identification using the noise-induced dynamics in only the unconditionally stable regime, i.e. away from the regimes where limit-cycle oscillations may occur. This opens up new possibilities for the prediction and analysis of the stability and nonlinear behaviour of hydrodynamic systems.

Mechanisms of dislocation generation and methods of crystal growth are two historically rich areas of scientific study. These two fields converge in the area of metamorphic epitaxial materials, where the goal is to produce high-performance devices that contain high densities of crystal defects in regions of the engineered material away from the active areas. Metamorphic epitaxy is a form of thin-film growth, where the lattice structure of the layer and substrate are mismatched, and its defining characteristic is that any elastic strain in the overlayer has been relaxed by the deliberate introduction of dislocations at the film–substrate interface. Metamorphic growth enables novel combinations of relaxed single-crystal materials to realize novel functionality and performance in numerous technological areas, including lasers, photovoltaics, transistors, and quantum computing. Many of the devices described in this issue are impossible to realize using the traditional approach of avoiding dislocation generation; instead, they rely on metamorphic epitaxy to attain high performance.

Patients with psychiatric disorders have an increased rate of cardiovascular morbidity and mortality compared with the general population. Metabolic issues such as weight gain, dyslipidemia, diabetes mellitus, diabetic ketoacidosis, and pancreatitis have been reported with the use of antipsychotic agents. Although atypical antipsychotics have not been linked directly to the development of metabolic syndrome, these medications have been shown to increase risk factors that can lead to metabolic and endocrine disturbances. Therefore, clinicians should provide ongoing monitoring for patients who are being treated for psychiatric disorders with these agents. According to the 2004 Consensus Report on Antipsychotics, screening measures should include baseline and follow-up monitoring of personal/family histories, weight (body mass index), waist circumference, blood pressure, fasting plasma glucose, and fasting lipid profile.

Growth of Metamorphic InGaP for Wide-Bandgap Photovoltaic Junction by MBE

Metamorphic triple-junction solar cells can currently attain efficiencies as high as 41.1%. Using additional junctions could lead to efficiencies above 50%, but require the development of a wide bandgap (2.0-2.2eV) material to act as the top layer. In this work we demonstrate wide bandgap InyGa1-yP grown on GaAsxP1-x via solid source molecular beam epitaxy. Unoptimized tensile GaAsxP1-x buffers grown on GaAs exhibit asymmetric strain relaxation, along with formation of faceted trenches 100-300 nm deep in the [01-1] direction. Smaller grading step size and higher substrate temperatures minimizes the facet trench density and results in symmetric strain relaxation. In comparison, compressively-strained graded GaAsxP1-x buffers on GaP show nearly-complete strain relaxation of the top layers and no evidence of trenches. We subsequently grew InyGa1-yP layers on the GaAsxP1-x buffers. Photoluminescence and transmission electron microscopy measurements show no indication of phase separation or CuPt ordering. Taken in combination with the low threading dislocation densities obtained, MBE-grown InyGa1-yP layers are promising candidates for future use as the top junction of a multi-junction solar cell.

A simple and efficient approach for fabricating silicon nanopillar arrays with a high aspect ratio and controllable sidewall profiles has been developed by using holographic lithography and a novel single-step deep reactive ion etching. During the etching process, scalloping of the sidewalls can be avoided while reserving the high mask selectivity and high etching rate. Besides, the sidewall angle of resultant patterns can be adjusted by tuning the composition of the gas mixture of single-step DRIE process. We further fabricate a tapered silicon nanopillar array and observe its photonic bandgap property. We believe that the good optical performance of this tapered silicon nanopillar array realized by the proposed approach shows the promising of this process for various applications.

Two hundred and eighty-four male college volunteers were screened for smooth pursuit eyetracking deficiencies, a commonly reported concomitant of schizophrenia. A sample of 36 subjects, weighted with poor eyetrackers, was brought into the National Institute of Mental Health laboratory and retested on electro-oculogram and infrared tracking procedures. They were also administered psychological tests which assessed nine dimensions relevant to schizophrenic, neuropsychological, and affective disorders. In the area of schizophrenia-like symptoms, measures of attention deficits, stimulation avoidance, and identity problems predicted poor eyetracking for the whole sample. Using the poor eyetracking subjects alone (N = 24), interpersonal withdrawal was also significantly related to poor performance but not stimulation avoidance. In the neuropsychological area, measures of attention control and perceptual-motor dysfunction for the total sample, and perceptual problems and general intellectual decrements for the poor eyetrackers were significantly related to poor performance. There was no relationship between measures of affective dysfunction and poor eyetracking.

Simultaneous measurements of the longitudinal (sx) and transverse (sz) components of the turbulent fluctuating velocity gradient at the wall of a pipe are presented for ten locations in the transverse direction. Separate measurements of sx and of sz are also presented for twenty locations. The sz pattern reveals alternating positive and negative values over lengths Λ of the magnitude suggested by the spacing of the streaky wall structure observed in a number of laboratories. Of particular interest is the observation of regular eddies for which the sz pattern is accompanied by a spatial variation of sx out of phase by about 1/4Λ. These regular eddies are associated with intermittent production of Reynolds stress and with the occurrence of large negative values of sx which could be identified with wall bursts previously reported. The time response of the axial flow appears to influence the development of these regular eddies in that the development of an sx pattern lags behind that of an sz pattern. Measurements of spatial correlations of sx and sz with different time delays, as well as direct observations of the temporal variation of sx and sz patterns, support these notions.

We have fabricated micro-probes consisting of gold microelectrode sites (500 μm long and 12 μm wide) modified with conductive polymers and carbon nanotubes to achieve intimate contact with the nervous system. The fabrication process includes photolithography, electroplating and micromachining techniques. In order to obtain a high quality neural contact, we have investigated the preparation and characterization of neural interface materials. Electrochemical polymerization using potentiostatic and galvanostatic methods was used to optimize the surface of the metal electrode sites. Scanning electron microscopy (SEM), cyclic voltammetry (CV), and electrochemical impedance spectroscopy (EIS) were used to study the surface morphology, electrochemical properties, and stability of electrodeposited polymers. Cytotoxicity tests using fibroblasts and Schwann cells were performed to evaluate the biocompatibility of the micro-probes and neural interface materials. Dorsal root ganglion (DRG) in vitro preparation was used to evaluate neuronal cell cell adhesion to the electrode. Polypyrrole (PPy) and poly(3,4-ethylendioxythiophene) (EDOT) with various thicknesses and dopants were deposited onto microelectrode sites from aqueous solution. Our results demonstrate that we can control the morphology, size and electrical properties of PPy and PEDOT by changing the polymerization conditions and adding dopant structures, such as chloride and carbon nanotubes (CNT). It was observed that the addition of carbon nanotubes favors the formation of nodules and increases the surface roughness. Also, electrochemical impedance spectroscopy revealed that conductive polymer composites lower the impedance of gold microelectrodes by three orders of magnitude. We found that PPy and PEDOT carbon nanotubes composite coated electrodes maintain intimate contact with axons. Using these conductive polymer composites, high quality nerve spike signals can be detected and electrical stimulation of axons can be achieved.
