Analysis of a beta-lactam allergy assessment protocol challenging diverse reported allergies managed by an antimicrobial stewardship program

Objective:
To assess the safety and efficacy of a novel beta-lactam allergy assessment algorithm managed by an antimicrobial stewardship program (ASP) team.
Design:Retrospective analysis.
Setting:One quaternary referral teaching hospital and one tertiary care teaching hospital in a large western Pennsylvania health network.
Patients or participants:Patients who received a beta-lactam challenge dose under the beta-lactam allergy assessment algorithm.
Interventions:A beta-lactam allergy assessment protocol was designed and implemented by an ASP team. The protocol risk stratified patients’ reported allergies to identify patients appropriate for a challenge with a beta-lactam antibiotic. This retrospective analysis assessed the safety and efficacy of this protocol among patients receiving a challenge dose from November 2017 to July 2021.
Results:Over a 45-month period, 119 total patients with either penicillin or cephalosporin allergies entered the protocol. Following a challenge dose, 106 (89.1%) patients were treated with a beta-lactam. Eleven patients had adverse reactions to a challenge dose, one of which required escalation of care to the intensive care unit. Of the patients with an unknown or low-risk reported allergy, 7/66 (10.6%) had an observed adverse reaction compared to 3/42 (7.1%) who had an observed reaction with a reported high-risk or anaphylactic allergy.
Conclusions:Our implemented protocol was safe and effective, with over 90% of patients tolerating the challenge without incident and many going on to receive indicated beta-lactam therapy. This protocol may serve as a framework for other inpatient ASP teams to implement a low-barrier allergy assessment led by ASP teams.

The Spoon-billed Sandpiper Calidris pygmaea is a ‘Critically Endangered’ migratory shorebird. The species faces an array of threats in its non-breeding range, making conservation intervention essential. However, conservation efforts are reliant on identifying the species’ key stopover and wintering sites. Using Maximum Entropy models, we predicted Spoon-billed Sandpiper distribution across the non-breeding range, using data from recent field surveys and satellite tracking. Model outputs suggest only a limited number of stopover sites are suitable for migrating birds, with sites in the Yellow Sea and on the Jiangsu coast in China highlighted as particularly important. All the previously known core wintering sites were identified by the model including the Ganges-Brahmaputra Delta, Nan Thar Island and the Gulf of Mottama. In addition, the model highlighted sites subsequently found to be occupied, and pinpointed potential new sites meriting investigation, notably on Borneo and Sulawesi, and in parts of India and the Philippines. A comparison between the areas identified as most likely to be occupied and protected areas showed that very few locations are covered by conservation designations. Known sites must be managed for conservation as a priority, and potential new sites should be surveyed as soon as is feasible to assess occupancy status. Site protection should take place in concert with conservation interventions including habitat management, discouraging hunting, and fostering alternative livelihoods.

When planted with narrow (53 cm) row spacing, cotton (Gossypium hirsutum L.) ‘McNair 210’ produced maximum yields with as little as 6 weeks of weed-free maintenance. With wider row spacing (79 and 106 cm), however, 10 and 14 weeks of weed-free maintenance were required for maximum yield. When weeds were allowed to grow in cotton for more than 2 to 4 weeks after planting, yield reductions occurred. This tolerance period did not differ with row widths of 53, 79, or 106 cm.

The Numeniini is a tribe of 13 wader species (Scolopacidae, Charadriiformes) of which seven are Near Threatened or globally threatened, including two Critically Endangered. To help inform conservation management and policy responses, we present the results of an expert assessment of the threats that members of this taxonomic group face across migratory flyways. Most threats are increasing in intensity, particularly in non-breeding areas, where habitat loss resulting from residential and commercial development, aquaculture, mining, transport, disturbance, problematic invasive species, pollution and climate change were regarded as having the greatest detrimental impact. Fewer threats (mining, disturbance, problematic native species and climate change) were identified as widely affecting breeding areas. Numeniini populations face the greatest number of non-breeding threats in the East Asian-Australasian Flyway, especially those associated with coastal reclamation; related threats were also identified across the Central and Atlantic Americas, and East Atlantic flyways. Threats on the breeding grounds were greatest in Central and Atlantic Americas, East Atlantic and West Asian flyways. Three priority actions were associated with monitoring and research: to monitor breeding population trends (which for species breeding in remote areas may best be achieved through surveys at key non-breeding sites), to deploy tracking technologies to identify migratory connectivity, and to monitor land-cover change across breeding and non-breeding areas. Two priority actions were focused on conservation and policy responses: to identify and effectively protect key non-breeding sites across all flyways (particularly in the East Asian- Australasian Flyway), and to implement successful conservation interventions at a sufficient scale across human-dominated landscapes for species’ recovery to be achieved. If implemented urgently, these measures in combination have the potential to alter the current population declines of many Numeniini species and provide a template for the conservation of other groups of threatened species.

BOUT++ is a 3D nonlinear finite-difference plasma simulation code, capable of solving quite general systems of Partial Differential Equations (PDEs), but targeted particularly on studies of the edge region of tokamak plasmas. BOUT++ is publicly available, and has been adopted by a growing number of researchers worldwide. Here we present improvements which have been made to the code since its original release, both in terms of structure and its capabilities. Some recent applications of these methods are reviewed, and areas of active development are discussed. We also present algorithms and tools which have been developed to enable creation of inputs from analytic expressions and experimental data, and for processing and visualisation of output results. This includes a new tool Hypnotoad for the creation of meshes from experimental equilibria. Algorithms have been implemented in BOUT++ to solve a range of linear algebraic problems encountered in the simulation of reduced Magnetohydrodynamics (MHD) and gyro-fluid models: A preconditioning scheme is presented which enables the plasma potential to be calculated efficiently using iterative methods supplied by the PETSc library (the Portable, Extensible Toolkit for Scientific Computation) (Balay et al. 2014), without invoking the Boussinesq approximation. Scaling studies are also performed of a linear solver used as part of physics-based preconditioning to accelerate the convergence of implicit time-integration schemes.

Grazing Incidence Small-Angle X-ray Scattering (GISAXS) is a versatile technique for the analysis of nano and micro thin films surfaces. The scattering data depend strongly on the form and distribution of the scattering objects. In the present work GISAXS is used to study hafnium dioxide (HfO2) thin films deposited by magnetron sputtering using different deposition processes and post-deposition annealing conditions. Two distinct types of 15 nm thick samples were produced using different sputtering targets and different gas mixtures. The GISAXS results show that the ellipsoids that compose the thin films present a reduction in their size for both samples sets. For the sputtered Hf metal target samples, the ellipsoid diameter value shifted from 9 nm (as-deposited) to 6 nm following a 800 °C thermal treatment. For the sputtered HfO2 target samples the diameter value shifts from 19 nm (as-deposited) to 3 nm after a 800 °C anneal in oxygen. The size distribution, for both sets of samples, follows a Gaussian distribution function.

The Critically Endangered Liben Lark (formerly Sidamo Lark) is known only from the Liben Plain of southern Ethiopia, where rapid grassland deterioration is driving the species towards extinction. Fieldwork on the Liben Plain in May 2009 to assess changes in habitat and population since June 2007 recorded a significant deterioration in habitat and decline in numbers. In both 2007 and 2009, birds were associated with areas with greater than average grass cover, and in 2007 with areas of higher grass. However, between 2007 and 2009 there was a significant decline in grass cover and height, a 40% decline in number of birds recorded along repeated transects, and a contraction of 38% in the occupied area of the Liben Plain. Moreover, the cover of bare ground increased more in areas where the species was recorded in 2007 than at random points, suggesting a more rapid degradation of the best sites. There was also a loss to arable agriculture of 8% of the grassland present in 2007. Invading fennel plants increased in number and area on the plain but did not appear to influence the distribution of the lark. An analysis of NDVI showed that grassland deterioration could not be explained by drought, and the most likely explanation is that grassland quality is suffering from overgrazing. Predictive modelling suggests that, apart from a smaller and politically insecure area some 500 km to the north-east near Somalia, there is no suitable habitat for this species elsewhere in the Horn of Africa. As a matter of extreme urgency, cattle exclosures need to be established on the Liben Plain to allow grassland regeneration. This may require the ploughing of land to reduce soil compaction and re-sowing with local grass species. In the longer term, further degradation of the plain should be prevented by, for example, clearing encroaching scrub to increase grassland area and reduce grazing pressure, and by developing sustainable rangeland management practices. These actions have the full and active support of local pastoralists.
Characterization of Megatrypanum trypanosomes from European Cervidae

Megatrypanum trypanosomes have been isolated from a number of different European Cervidae, but on the basis of morphology it has not been possible to define the species to which these isolates belong. We isolated Trypanosoma (Megatrypanum) theileri from 10 cattle, and Megatrypanum trypanosomes from 11 fallow deer (Cervus dama), 9 red deer (Cervus elaphus), and 4 roe deer (Capreolus capreolus) by blood culture on a biphasic medium (NNN agar slopes). Trypanosomes were propagated in Schneider's Drosophila medium and characterized by isoenzyme analysis and molecular karyotyping. Isocitrate dehydrogenase and phosphoglucomutase were visualized after starch gel electrophoresis of trypanosome lysates. By cluster analysis of this data all isolates from deer were clearly separated from the T. (M.) theileri isolates from cattle. Isolates from roe deer were different not only from T. (M.) theileri but also from the other deer isolates. Isolates from fallow deer and red deer were grouped together. Thus, there are probably at least two different species of Megatrypanum trypanosomes in the three Cervidae. One parasitizing roe deer, the other, apparently less host specific species, infecting red deer and fallow deer. Separation of the chromosomes of Megatrypanum trypanosomes by pulsed-field gradient gel electrophoresis (PFGE) showed that each isolate contained a large number (> 18) of chromosomes ranging in size from 300 to > 2200 kb. The molecular karyotypes were similar for all isolates, although no isolate was identical to another.

Autophosphorylation by protein kinases has been implicated as an important control mechanism in signal transduction and growth regulatory pathways in mammalian cells. We have set out to investigate whether any such autophosphorylating protein kinase activities can be found in Trypanosoma brucei. In order to do this, we have developed a system for characterizing such protein kinase activities using an in vitro assay. This assay was carried out by fractionation of trypanosome lysates using isoelectric focusing gel electrophoresis followed by incubation of the gel in γ32P-labelled nucleotide triphosphate and subsequent autoradiography. We have identified two classes of autophosphorylating protein kinase activities. In the first class all were dependent on ATP as the phosphate donor substrate and were all found to have a molecular size of 60 kDa. Differences in the activity of these protein kinases were observed between the bloodstream and procyclic life-cyle stages. Furthermore, the addition of mammalian epidermal growth factor to bloodstream stage lysates stimulated an additional activity. The second class of autophosphorylating protein kinases utilized GTP as the phosphate donor and were all found to be 90 kDa in size. Stage-specific differences were also observed in the activity of these protein kinases.
Evidence that the mechanism of gene exchange in Trypanosoma brucei involves meiosis and syngamy

All pairwise combinations of three cloned stocks of Trypanosoma brucei (STIB 247L, STIB 386AA and TREU 927/4) were co-transmitted through tsetse flies (Glossina morsitans) and screened for the production of hybrid trypanosomes. Clones of metacyclic and bloodstream trypanosomes from flies harbouring mature infections containing hybrid trypanosomes were established and screened for several isoenzyme and restriction fragment length polymorphisms. For each of the three combinations of parents, some progeny clones were observed to be of a phenotype and genotype indicating that genetic exchange had occurred during development of the trypanosomes in flies. These hybrid clones shared three salient features: (1) where the parents were homozygous variants the progeny were heterozygous, (2) where one of the parents was heterozygous, allelic segregation was observed and (3) the progeny clones were shown to be recombinant when two or more markers for which one of the parents was heterozygous were examined. These results are consistent with the progeny being an F1 in a diploid mendelian genetic system involving meiosis and syngamy. Our observations show that all possible combinations of the three stocks may undergo genetic exchange. A marker analysis of a series of clones each derived from single metacyclic trypanosomes showed that individual flies transmit a mixture of trypanosome genotypes corresponding to F1 progeny and to parental types, indicating that genetic exchange was a non-obligatory event in the life-cycle of the trypanosome. In addition, a preliminary analysis of the phenotype of procyclic stage trypanosomes derived from flies infected with two stocks, indicates that genetic exchange is unlikely to occur at this stage.

Two clones of East African Trypanosoma brucei, with distinct homozygous isoenzyme patterns for one of three enzymes examined, were cotransmitted through the tsetse fly vector Glossina morsitans centralis. Flies with mature infections were individually fed on mice and the subsequent bloodstream form populations analysed for the presence of hybrid trypanosomes by isoenzyme analysis. Several combinations have previously been detected using this approach (Schweizer, Tait & Jenni, 1988; Sternberg et al. 1989). Four clones were isolated from one of the hybrid-containing populations. They showed a hybrid phenotype, as would be expected for the F1 progeny in a diploid Mendelian system. The analysis of the progeny clones, using two gene probes which detect restriction fragment length polymorphisms between the two parental stocks, showed that alleles had segregated at each locus and given rise to three different non-parental combinations of alleles in the hybrid progeny. Characterization of the hybrid progeny clones by PFGE (pulsed field gradient gel electrophoresis) revealed that all progeny clones were recombinant for the intermediate size chromosomes. From the analysis of the segregation of the larger chromosomes, marked by P0K (phosphoglycerate kinase) and CP (cysteine protease) gene probes, it was inferred that the progeny clones did not result from a direct fusion of diploid cells. Results with the PGK probe fit into a classical system with meiosis and subsequent fusion of the nuclei to form diploid progeny. On the other hand, blots with the CP probe as well as some of the ethidium bromide stained PFGE gels revealed the existence of non-parental size chromosomes in some of the hybrid progeny. This phenomenon was observed previously (Gibson, 1989) and further investigation is required to elucidate the mechanism.

The inheritance of variant surface antigens in Trypanosoma brucei has been determined by identifying variable antigen types (VATs) in each of two cloned parental stocks and then examining the presence and abundance of these VATs in hybrid progeny produced when these stocks undergo genetic exchange during co-transmission through tsetse flies. Nine VATs have been identified from the repertoire of the parental stock STIB 247L and 5 VATs have been identified from the parental stock STIB 386AA; the identified VATs were exclusive to each stock. Their inheritance was elucidated using two assays. In the first, repertoire antisera (RAS) containing antibody specificities to many different VATs were raised in rabbits to the 2 parental stocks and 6 progeny clones. The presence of VAT-specific antibodies in these RAS was then determined by antibody-dependent complement-mediated lysis. In the second assay, the 2 parental stocks and 4 hybrid progeny clones were each independently transmitted through tsetse flies and VATs observed using VAT-specific antisera in indirect immunofluorescence of metacyclic trypanosomes and in bloodstream forms of fly-bitten mice. The results from both assays showed that (1) both metacyclic- and bloodstream-VATs were inherited into the progeny, (2) each hybrid progeny clone contained some VATs from both parents, (3) hybrids did not express all the VATs from either parent, (4) there was little apparent pattern as to which VATs had been inherited and which had not and (5) the VAT repertoires of the hybrid progeny appeared to be larger than those of the parents. In addition, two results indicated that control of VAT expression remains unaltered after genetic exchange. Firstly, the immunofluorescence results showed that VATs present in hybrid trypanosomes were expressed at the same stage during an infection and at approximately the same prevalence as in the parent. Secondly, a double-labelling experiment using direct immunofluorescence indicated that individual hybrid trypanosomes did not generally simultaneously express more than one VAT. Taken together, these results demonstrate that recombinant VAT repertoires are created when trypanosomes undergo genetic exchange and that genetic exchange is a mechanism whereby the generation of new serodemes can occur.
Semantic priming in patients with right frontal lobe lesions

Patients with unilateral, right frontal lobe damage (N = 13) and matched controls (N = 20) performed a task of lexical ambiguity resolution in order to explore the contribution of right frontal regions to lexical-semantic priming. Word triplets consisting of balanced homographs were presented to participants in four conditions: concordant, discordant, neutral, and unrelated. Controls demonstrated facilitation for concordant meanings of homographs, as evidenced by their faster reaction times in the concordant relative to the unrelated (baseline) condition, as well as a lack of facilitation for the discordant meaning relative to the neutral and concordant conditions. Results in patients with right frontal lobe damage differed depending on the site of the lesion. Patients with lesions restricted to the right medial frontal lobe only showed facilitation in the neutral condition, while those with lesions encroaching upon the right dorsolateral region demonstrated facilitation of both discordant and concordant meanings relative to the baseline condition. These results support a role for the right frontal lobe in semantic priming and suggest possible specialization within the right prefrontal cortex for the processing of lexical-semantic information. Nucleation of AlN on the (7×7) Reconstructed Silicon (1 1 1) Surface

The (7×7) reconstructed (1 1 1) surface of silicon is found to be an excellent surface for the nucleation of epitaxial aluminum nitride, despite the +23.4% misfit in the AlN/Si system. AlN nucleated above the (7×7) to (1×1) transition temperature (830°C) is found to contain 30° misoriented grains, while films nucleated below the transition temperature are single orientation. Optimized aluminum nitride films grown on (7×7) silicon surfaces make excellent substrates for GaN heteroepitaxy.
Nonuniform Morphology and Luminescence Properties of a Molecular Beam Epitaxy GaN Film from Atomic Force Microscopy, Scanning Electron Microscopy and Cathodoluminescence

Complex faceted features of micrometer sizes and with intense luminescence rise 200-300 nm above the surface of a GaN thin film grown by molecular beam epitaxy on (0001) sapphire. Cathodoluminescence measurements at room temperature and at 8K were used to investigate the luminescence properties of these microfeatures in comparison with those of the background GaN material. The morphology of the micro-features was studied by scanning electron microscopy and by atomic force microscopy.
GaN and related compounds have been considered as promising materials for light emitting devices in the short wavelength visible and UV spectral regions because of their direct, wide band gaps and high luminescence efficiency. Remarkable, rapid success has been achieved in developing devices based on III-V nitrides, which allows these material systems to rival other systems under development [1]. Blue-green LEDs fabricated from InGaN/AlGaN double-heterostructure layers on sapphire substrates by Nichia Chemical Industries in Japan are commercially available, despite the presence of high extended defect densities in the 1010 /cm2 range [2]. In December 1995, an important milestone in the development of nitride lasers was reached; Nakamura et al. in Nichia demonstrated the first nitride-based laser diode which operated at 417 nm under pulsed conditions at room temperature [3].
The morphology and spatial distribution of luminescence in GaN thin films have been investigated by several research groups [4], [5], [6], although most efforts have been directed to developing appropriate growth conditions for improved film quality. From observations by scanning electron microscopy (SEM) and atomic force microscopy (AFM), Trager-Cowan et al. described an MBE-grown GaN film which contained an assembly of oriented hexagonal crystallites rising above a background of polycrystalline or amorphous material [4]. All the crystallites, oriented in a similar fashion, had roughly the same sizes, 1 or 2 μm across a hexagonal face and about 1 μm high. These crystallites were much brighter than the surrounding background material in panchromatic low-temperature cathodoluminescence (CL) images. Trager-Cowan et al. concluded that the crystallites were of better quality than the background material. They also observed a green emission band, attributed to impurities, which became weaker for higher electron beam voltages, generating luminescence from deeper in the film. From this observation, they concluded that higher quality material is located closer to the film's outer surface. Spatial variation of the luminescence efficiency from MOCVD-grown films has also been observed by Ponce et al[5]. in their room-temperature CL microscopy studies. Their results showed significant nonuniformities in both the band-edge and yellow band emissions. Although they reported no faceted island structures, one of their samples had “marked surface features” and consisted of hexagonal crystals 10 to 50 μm in diameter. The crystals gave strong band-to-band (364 nm) luminescence.
In this paper, the morphology of a GaN film grown on (0001) sapphire by MBE is characterized by SEM and AFM, and CL measurements are used to investigate the luminescence properties of the film at room temperature and at 8K. Some regions of this film are similar to the one described by Trager-Cowan et al. [4] in having micron sized, brightly luminescent islands. The hexagonal facet angles and bright luminescence of islands in our film also resemble the larger scale, more regularly shaped hexagonal crystals of Ponce et al. [5]. These regions lie on the boundary of excess gallium growth conditions. Our results differ from those of Trager-Cowan in terms of the island morphologies and the spectral character of luminescence from the islands and from the background material. Possible causes of the large variations in luminescence efficiency are discussed.
Epitaxial Growth and Orientation of GaN on (1 0 0) γ-LiAlO2

The (1 0 0) face of γ-LiAlO2 has attracted attention as a possible substrate for GaN epitaxial growth. This is partly because this face has an excellent lattice and structural match to (1 0 0) GaN. This orientation would have a misfit of only −1.4% along the c-direction and −0.1% along the b-direction of LiAlO2. We find that in practice this orientation relationship does not occur; instead, (0 0 0 1) oriented GaN grows with a small tilt (0.6° towards the c-direction) between the film and substrate. Although the misfit along the substrate b direction is large (−6.3%) for this orientation, the tilt perfectly accommodates the −1.4% misfit in the c direction. We present characterization of these films by RHEED, X-ray diffraction, and TEM. We propose that the tilt is driven by a reduction of interface energy which occurs in polar, incoherent interfaces.
