Increasing fibre intake in the UK: lessons from the Danish Whole Grain Partnership

Diets deficient in fibre are reported globally. The associated health risks of insufficient dietary fibre are sufficiently grave to necessitate large-scale interventions to increase population intake levels. The Danish Whole Grain Partnership (DWP) is a public–private enterprise model that successfully augmented whole-grain intake in the Danish population. The potential transferability of the DWP model to Slovenia, Romania and Bosnia-Herzegovina has recently been explored. Here, we outline the feasibility of adopting the approach in the UK. Drawing on the collaborative experience of DWP partners, academics from the Healthy Soil, Healthy Food, Healthy People (H3) project and food industry representatives (Food and Drink Federation), this article examines the transferability of the DWP approach to increase whole grain and/or fibre intake in the UK. Specific consideration is given to the UK’s political, regulatory and socio-economic context. We note key political, regulatory, social and cultural challenges to transferring the success of DWP to the UK, highlighting the particular challenge of increasing fibre consumption among low socio-economic status groups – which were also most resistant to interventions in Denmark. Wholesale transfer of the DWP model to the UK is considered unlikely given the absence of the key ‘success factors’ present in Denmark. However, the DWP provides a template against which a UK-centric approach can be developed. In the absence of a clear regulatory context for whole grain in the UK, fibre should be prioritised and public–private partnerships supported to increase the availability and acceptability of fibre-rich foods.

Background
Cognitive behavior therapy (CBT) is effective for most patients with a social anxiety disorder (SAD) but a substantial proportion fails to remit. Experimental and clinical research suggests that enhancing CBT using imagery-based techniques could improve outcomes. It was hypothesized that imagery-enhanced CBT (IE-CBT) would be superior to verbally-based CBT (VB-CBT) on pre-registered outcomes.
MethodsA randomized controlled trial of IE-CBT v. VB-CBT for social anxiety was completed in a community mental health clinic setting. Participants were randomized to IE (n = 53) or VB (n = 54) CBT, with 1-month (primary end point) and 6-month follow-up assessments. Participants completed 12, 2-hour, weekly sessions of IE-CBT or VB-CBT plus 1-month follow-up.
ResultsIntention to treat analyses showed very large within-treatment effect sizes on the social interaction anxiety at all time points (ds = 2.09–2.62), with no between-treatment differences on this outcome or clinician-rated severity [1-month OR = 1.45 (0.45, 4.62), p = 0.53; 6-month OR = 1.31 (0.42, 4.08), p = 0.65], SAD remission (1-month: IE = 61.04%, VB = 55.09%, p = 0.59); 6-month: IE = 58.73%, VB = 61.89%, p = 0.77), or secondary outcomes. Three adverse events were noted (substance abuse, n = 1 in IE-CBT; temporary increase in suicide risk, n = 1 in each condition, with one being withdrawn at 1-month follow-up).
ConclusionsGroup IE-CBT and VB-CBT were safe and there were no significant differences in outcomes. Both treatments were associated with very large within-group effect sizes and the majority of patients remitted following treatment.
The last interglacial sea-level high stand on the southern Cape coastline of South Africa

The continental margin of southern South Africa exhibits an array of emergent marginal marine sediments permitting the reconstruction of long-term eustatic sea-level changes. We report a suite of optical luminescence ages and supplementary amino acid racemization data, which provide paleosea-level index points for three sites on this coastline. Deposits in the Swartvlei and Groot Brak estuaries display tidal inlet facies overlain by shoreface or eolian facies. Contemporary facies relations suggest a probable high stand 6.0-8.5 m above modern sea level (amsl). At Cape Agulhas, evidence of a past sea-level high stand comprises a gravel beach (ca. 3.8 m amsl) and an overlying sandy shoreface facies (up to 7.5 m amsl). OSL ages between 138±7 ka and 118±7 ka confirm a last interglacial age for all marginal marine facies. The high stand was followed by a sea-level regression that was associated with the accumulation of eolian dunes dating to between 122±7 ka and 113±6 ka. These data provide the first rigorous numerical age constraints for last interglacial sea-level fluctuations in this region, revealing the timing and elevation of the last interglacial high stand to broadly mirror a number of other far-field locations.

The effects of varying dietary digestible protein (DP) and digestible energy (DE) content on performance, nutrient retention efficiency and the de novo lipogenesis of DP origin were examined in triplicate groups of gilthead sea bream (Sparus aurata), fed nine extruded experimental diets. In order to trace the metabolic fate of dietary protein, 1·8 % fishmeal was replaced with isotope-labelled whole protein (>98 % 13C). The experiment was divided into a growth period lasting 89 d, growing fish from approximately 140 to 350 g, followed by a 3 d period feeding isotope-enriched diets. Isotope ratio MS was applied to quantify the 13C enrichment of whole-body lipid from dietary DP. Between 18·6 and 22·4 % of the carbon derived from protein was recovered in the lipid fraction of the fish, and between 21·6 and 30·3 % of the total lipid deposited could be attributed to dietary protein. DP retention was significantly improved by reductions in dietary DP:DE ratio, while the opposite was true for apparent digestible lipid retention. Both overall DE retention and whole-body proximate composition of whole fish were largely unaffected by dietary treatments, while feed conversion ratios were significantly improved with increasing dietary energy density. The present study suggests that gilthead sea bream efficiently utilises dietary nutrients over a wide range of DP:DE ratios and energy densities. In addition, they appear to endeavour a certain body energy status rather than maximising growth, which in the present trial was apparent from inherently high de novo lipogenesis originating from DP.

The effects of replacing a digestible energy source from fat (fish oil) with carbohydrate (wheat starch) on performance, glycogenesis and de novo lipogenesis was examined in triplicate groups of juvenile gilthead sea bream (Sparus aurata), fed four extruded experimental diets. In order to trace the metabolic fate of dietary starch, 0·7 % wheat starch was replaced with isotope-labelled starch (>98 % 13C). Fish were fed the experimental diets for three consecutive 10 d periods, and isotope ratio MS was applied to quantify 13C enrichment of liver and whole-body glycogen and lipid pools over the three feeding periods. Glycogenesis originating from dietary starch accounted for up to 68·8 and 38·8 % of the liver and whole-body glycogen pools, respectively, while up to 16·7 % of the liver lipid could be attributed to dietary starch. Between 5 and 8 % of dietary starch carbon was recovered in whole-body lipid, and estimated deposition rates of de novo synthesised lipid originating from starch ranged from 18·7 to 123·7 mg/kg biomass per d. Dietary treatments did not significantly affect growth, feed performance or body composition of the fish, while the hepatosomatic index and glycogen content of whole fish and livers correlated directly with dietary starch inclusion level. The study suggests that gilthead sea bream efficiently synthesises glycogen from both dietary starch and endogenous sources. In contrast, lipogenesis from carbon derived from starch seems to play a minor role in overall lipid synthesis and deposition under the specified experimental conditions.

Systemic corticosteroids are a mainstay of treatment for many pediatric medical conditions. Although their impact on the central nervous system has been well-studied in animal models and adults, less is known about such effects in pediatric populations. The current study investigated acute effects of corticosteroids on memory, executive functions, emotion, and behavior in children and adolescents with inflammatory bowel disease (IBD). Patients 8–17 years with IBD (Crohn's disease, CD; ulcerative colitis, UC) on high-dose prednisone (n = 33) and IBD patients in remission off steroids (n = 33) completed standardized neuropsychological tests and behavior rating scales. In the IBD sample as a whole, few steroid effects were found for laboratory cognitive measures, but steroid-treated patients were rated as exhibiting more problems with emotional, and to a lesser extent with cognitive function in daily life. Steroid effects, assessed by laboratory measures and questionnaires, were more prevalent in CD than UC patients; UC patients on steroids sometimes performed better than controls. Sleep disruption also predicted some outcomes, diminishing somewhat the magnitude of the steroid effects. Corticosteroid therapy can have acute effects on cognition, emotion, and behavior in chronically ill children; the clinical and long-term significance of these effects require further investigation. The host range of a mixture of Choristoneura fumiferana (Clemens) nucleopolyhedroviruses (CfMNPV and CfDefNPV) was investigated using a per os bioassay of larvae of 29 species of Lepidoptera and adult males of Megachile rotundata (F.) (Hymenoptera: Megachilidae). Using a whole-genomic DNA probe, positive results were obtained in 8 of 10 Tortricidae: Archips cerasivorana (Fitch), Choristoneura fractivittana (Clemens), C. fumiferana, Choristoneura occidentalis Freeman, Choristoneura pinus pinus Freeman, Choristoneura rosaceana (Harris), Clepsis persicana (Fitch), and Cydia pomonella (L.); one Crambidae: Ostrinia nubilalis (Hübner); one arctiine Erebidae: Estigmene acrea (Drury); and two Noctuidae: Oligia illocata (Walker) and Pyrrhia exprimens (Walker). Mortality rates were highest among C. fumiferana, C. occidentalis, C. pinus pinus, A. cerasivorana, and C. pomonella. Sequenced polymerase chain reaction (PCR) amplicons from infected individuals from several species confirmed that the primer sets amplified the target viruses. CfMNPV was consistently found in virus-fed C. fumiferana; whereas, CfDefNPV was present only occasionally. The presence of CfMNPV and CfDefNPV in A. cerasivorana was confirmed by PCR and DNA sequencing. Significant treatment-mortality rates were induced in the noctuids P. exprimens and Acronicta impleta Walker; PCR determined that both viruses were present in treated P. exprimens but only CfMNPV was present in A. impleta. No virus was detected in M. rotundata.
