Evaluation of the impact of patient involvement in health technology assessments: A scoping review

Objectives
While involving patients in health technology assessment (HTA) has become increasingly common and important around the world, little is known about the optimal methods of evaluating patients’ involvement (PI) in HTA. This scoping review was undertaken to provide an overview of currently available methods for the evaluation of PI, specifically the impact of PI on HTA recommendations.
MethodsA literature search was conducted using nine databases as well as a grey literature search of the websites of 26 organizations related to the conduct, practice or research of HTA to identify articles, reports and abstracts related to the evaluation of PI impact in HTA.
ResultsWe identified 1,248 unique citations, six of which met our eligibility criteria. These six records (five articles, and one report) were all published after 2012. Four assessed the impact of patient experience submissions on final HTA recommendations; one evaluated the impact of direct involvement on HTA committees, and one assessed impact of multiple forms of involvement. Methods of evaluation included quantitative analyses of reimbursement decisions, qualitative interviews with those directly involved in an assessment, surveys of patient groups and committee members, and the review of HTA reports.
ConclusionsQuantitative evaluation of PI based on associations with funding decisions may not be feasible or fully capture the relevant impact of PI in the assessment of health technologies. Rather, a combination of both qualitative and quantitative strategies may allow for the most comprehensive assessment of the impact of PI on HTA recommendations when possible.

In September 2016, the annual meeting of the International Union for Quaternary Research’s Loess and Pedostratigraphy Focus Group, traditionally referred to as a LoessFest, met in Eau Claire, Wisconsin, USA. The 2016 LoessFest focused on “thin” loess deposits and loess transportation surfaces. This LoessFest included 75 registered participants from 10 countries. Almost half of the participants were from outside the United States, and 18 of the participants were students. This review is the introduction to the special issue for Quaternary Research that originated from presentations and discussions at the 2016 LoessFest. This introduction highlights current understanding and ongoing work on loess in various regions of the world and provides brief summaries of some of the current approaches/strategies used to study loess deposits.
Apical Dominance in Rhizomes of Quackgrass (Elytrigia repens): Inhibitory Effect of Scale Leaves

Surgical experiments were conducted on cultured five-node apical rhizome segments of quackgrass. Removal of scale leaves promoted an initial burst of growth within the axillary buds but did not support the continued growth of buds as effectively as removal of the rhizome apex. Replacement of detached scale leaves over denuded buds temporarily repressed the promotive effect of scale leaf removal. Aqueous extracts of scale leaf material inhibited apical growth in rhizome segments but did not inhibit bud growth. Anatomical sections revealed that removal of scale leaves promoted development of buds: cells enlarged, vascular tissues differentiated, and new nodes began to form within 4 days of the removal of scale leaves. It is suggested that scale leaves contribute to apical dominance by inhibiting the initial development of axillary buds.
Exposure of bivalve shellfish to titania nanoparticles under an environmental-spill scenario: Encounter, ingestion and egestion

Nanoparticles have applications in a diverse range of products including medications, detergents, cosmetics, paint, sunscreen and electronics, with an economic worth projected to reach $2.5 trillion dollars in 2015. Research into the effects of manufactured nanomaterials on the environment, however, has failed to keep pace with the high volume of commercial production. Whereas a number of studies have examined the effects of nanoparticles on aquatic species, little work has focused on the way in which benthic marine species encounter, ingest and depurate these materials. The purpose of this study was to examine the ingestion and depuration of titania nanoparticles (anatase) by the blue mussel (Mytilus edulis) and the eastern oyster (Crassostrea virginica) during a spill scenario (an acute exposure to elevated concentrations). Bivalves were exposed to nanoparticles either incorporated into marine snow, an environmentally relevant medium for pollutants, or added directly to seawater at a concentration of 4.5 mg L−1 for 2 h. After feeding, the animals were transferred to filtered seawater and allowed to depurate. Faeces and tissues were collected at 0, 6, 24, 72 and 120 h, post-exposure, and analysed for concentrations of titanium by inductively coupled plasma-mass spectrometry. Results indicated that the capture and ingestion of titania nanoparticles by both species was not dependent on the method of delivery (incorporated into marine snow or freely suspended). Additionally, greater than 90% of the titania nanoparticles, on average, were eliminated from the tissues after 6 h, and only trace amounts remained after 72 h. These data demonstrate that mussels and oysters readily ingest titania nanoparticles, but rapidly depurate the material within hours of an acute exposure suggesting that little would be transferred to secondary consumers including humans. Further research is required to determine if other species of suspension-feeders handle titania nanoparticles in a manner similar to bivalves.

A comparative study has been made of the progress in the control of ovine echinococcosis caused by Echinococcus granulosus and the ovine cysticercoses caused by Taenia hydatigena and T. ovis in New Zealand. The methods of control included an educational and a dog-dosing programme. In the Styx field trial, the Otago/Southland Surveillance Programme and the national control programme, E. granulosus declined towards extinction. In contrast, this same control effort transformed the cysticercoses from hyper-endemic to endemic status with focal epidemics. In this unstable state, the density-dependent constraint was negligible and superinfection occurred.
A Randomized, Controlled Trial of a New Vascular Catheter Flush Solution (Minocycline-EDTA) in Temporary Hemodialysis Access

Background and Objective:
We previously demonstrated that minocycline-EDTA was efficacious at preventing catheter-related bloodstream infections (BSIs) in three patients with recurrent infections. This study compared heparin with minocycline-EDTA as flush solutions used among dialysis patients with central venous catheters, a high-risk group for catheter-related BSI.
Methods:Patients were enrolled within 72 hours of catheter insertion and randomized to receive heparin or minocycline-EDTA as a flush after each dialysis session. Each syringe containing flush solution was wrapped in orange plastic to conceal the type of solution it contained. Patients were observed for evidence of infection and catheter thrombosis. After catheters were removed, cultures were performed to determine whether microbial colonization had occurred.
Results:During a 14-month period, 60 patients were enrolled (30 in each group). The two groups had similar demographics and underlying diseases. Catheter survival at 90 days was 83% for the minocycline-EDTA group versus 66% for the heparin group (P = .07). Significant catheter colonization, a surrogate measure of catheter-related infection, was significantly more frequent in the heparin group (9 of 14 vs 1 of 11; P= .005). There was only one catheter-related bacteremia and it occurred in the heparin group.
Conclusions:When compared with heparin, minocycline-EDTA had a better 90-day catheter survival (P = .07) and a decreased rate of catheter colonization. This pilot study warrants a larger prospective, randomized trial (Infect Control Hosp Epidemiol 2005;26:520-524).
Dispersal of Staphylococcus aureus Into the Air Associated With a Rhinovirus Infection

Objective:
To determine whether healthy adult nasal carriers of Staphylococcus aureus can disperse S. aureus into the air after rhinovirus infection.
Design:We investigated the “cloud” phenomenon among adult nasal carriers of S. aureus experimentally infected with a rhinovirus. Eleven volunteers were studied for 16 days in an airtight chamber wearing street clothes, sterile garb, or sterile garb plus surgical mask; rhinovirus inoculation occurred on day 2. Daily quantitative air, nasal, and skin cultures for S. aureus; cold symptom assessment; and nasal rhinovirus cultures were performed.
Setting:Wake Forest University School of Medicine, Winston-Salem, North Carolina.
Participants:Wake Forest University undergraduate or graduate students who had persistent nasal carriage of S. aureus for 4 or 8 weeks.
Results:After rhinovirus inoculation, dispersal of S. aureus into the air increased 2-fold with peak increases up to 34-fold. Independent predictors of S. aureus dispersal included the time period after rhinovirus infection and wearing street clothes (P < .05). Wearing barrier garb but not a mask decreased dispersal of S. aureus into the air (P < .05).
Conclusion:Virus-induced dispersal of S. aureus into the air may have an important role in the transmission of S. aureus and other bacteria.

During the past decade, 'partnership' has become a watchword for regional environmental management (Bronars & Michaels 1997). Partnerships, in the context of environmental conservation, can be defined as collaborative efforts to achieve shared management objectives which engage, but are not limited to, non-profit groups, for-profit companies, government agencies, and landowners.

Confocal laser scanning microscopy (CLSM) has been used to study asphaltene aggregation kinetics by employing the microscope's automated acquisition to generate time-lapsed projection maps of aggregating asphaltenes in the autofluorescent matrix of crude oil. Heavy crude oils contain asphaltene particles resulting in the production of optically observable micron-sized asphaltene aggregates. These aggregates form as a result of attractive interactions induced by mixing the heavy crude oil with a poor solvent. This technique has been employed to determine the volume fraction of aggregated asphaltenes, ϕagg, and the time evolution of this phenomenon. The measurements cover a range of various concentrations of asphaltene volume fractions of the heavy asphaltenic oil, ϕm, from ϕm =0.001 to ϕm =0.4.
At each ϕm,after the mixtures have been made, approximately 20 μl of the crude oil is placed in a 20 μm deep flat-well quartz cell and immediately placed on a microscope stage.

Confocal laser scanning microscopy (CLSM) has been used to generate three dimensional projection maps of less fluorescent domains caused by asphaltene aggregates in the autofluorescent matrix of crude oil. Heavy crude oils contain asphaltene particles resulting in the production of optically observable micron sized asphaltene aggregates. This technique has been employed to determine the volume fraction of aggregated asphaltenes, øagg, and the time evolution of this phenomenon. The measurements cover a range of various concentrations of asphaltene volume fractions of the heavy asphaltenic oil, øm, from øm = 0 to øm = 0.6.
Examining crude oils for asphaltenes aggregates using CLSM presents a challenge due to the strong absorption of the oils. This is because some molecules of the crude oil fluoresce when excited by optical laser light used as a source in the CLSM. Other molecules of the crude oils absorb light at similar wavelengths.

Recognition of distinctive ceramics by visual inspection is the norm in surface survey - especially when ceramic types are as varied and colourful as those of Islamic medieval pottery. Here petrographic analysis is used successfully to define the ceramic categories and the contributions of local and imported wares.
