Pilot Study of an Internet-Based, Simulated Teachable Moment for Smoking Cessation

Introduction
The internet has the potential to overcome geographic limitations for smoking cessation interventions, but further telehealth-based studies of utility are required.
AimsTo investigate the efficacy of an internet-based version of a quit smoking approach using a personalised video to create a simulated teachable moment.
MethodsSmokers within Australia were recruited through a dedicated website. After consent, eligible subjects, aged ≥30 years with a non-smoking partner, uploaded pictures of themselves, their partner and family, to be inserted into a video depicting the subject having a heart attack due to smoking, with consequences to them and their family. Nicotine replacement therapy (NRT) began prior to the quit attempt. The video was shown during two videoconference counselling sessions, with follow-up phone calls and text messaging support. Smoking status at 6 months by self-report (primary endpoint) was verified by partner/proxy and salivary cotinine (NicAlert™).
Results/FindingsSeventy seven smokers were screened, of whom 50 were eligible, and 17 of these (34%) were enrolled; 11 men and 6 women, aged 41.5 ± 6.9 years, daily cigarette consumption 20.8 ± 8.9, Heaviness of Smoking Index score 3.7 ± 1.7. Participants reported feeling personally involved with the video (5.9 ± 1.1), which felt real (5.8 ± 1.1) and emotionally moving (5.6 ± 1.5) [7-point Likert Scale]. A similar video response was reported by the four participants (24%), who due to bandwidth limitations, watched the video after the counselling session instead of during it. Non-smoking rates at 6 months were 65% (11/17) by self-report with proxy confirmation, and 47% (8/17) by self-report with biologic confirmation. Three non-smokers by self-report could not provide a valid NicAlert™ result due to current NRT use. One participant who by self-report smoked once in the prior 14-days was assessed as a non-smoker by both proxy and NicAlert™.
ConclusionsThis pilot study demonstrates efficacy for an internet-based version of a quit smoking program based on creating a simulated teachable moment. The findings provide support for further research into this technique, with the internet enabling greater reach than face-to-face.

Iceberg calving and basal melting are the two primary mass loss processes from the Antarctic ice sheet, accounting for approximately equal amounts of mass loss. Basal melting under ice shelves has been increasingly well constrained in recent work, but changes in iceberg calving rates remain poorly quantified. Here we examine the processes that precede iceberg calving, and focus on initiation and propagation of ice-shelf rifts. Using satellite imagery from the Moderate Resolution Imaging Spectroradiometer (MODIS) and the Multi-angle Imaging Spectroradiometer (MISR), we monitored five active rifts on the Amery Ice Shelf, Antarctica, from 2002 to 2014. We found a strong seasonal component: propagation rates were highest during (austral) summer and nearly zero during winter. We found substantial variability in summer propagation rates, but found no evidence that the variability was correlated with large-scale environmental drivers, such as atmospheric temperature, winds or sea-ice concentration. We did find a positive correlation between large propagation events and the arrival of tsunamis in the region. The variability appears to be related to visible structural boundaries within the ice shelf, e.g. suture zones or crevasse fields. This suggests that a complete understanding of rift propagation and iceberg calving needs to consider local heterogeneities within an ice shelf.

Unilateral neglect, a lack of awareness for one side of space, is a common and debilitating consequence of stroke. Previous work has identified a relationship between enduring left neglect and diminished general alertness and shown that increasing alertness can temporarily reduce the severity of the spatial bias. In that research, alertness was modulated by loud tones or with pharmacological stimulants. Here we examine whether cognitive, endogenously driven changes can produce similar short-term improvements. Time-pressure is associated with increased subjective arousal and increased activation in cortical regions associated with alertness. Here five patients completed a spatial cancellation task with and without instructions regarding a time limit. Significant reductions in neglect severity were observed when patients believed that they were acting under time-pressure, despite the conditions being equivalent in the actual (unlimited) time available. Functional imaging work has highlighted the secondary effects that damage to networks mediating alertness can have on structurally intact spatial systems. The results here suggest that activation of presumably spared function in these damaged networks can induce moment-by-moment changes in spatial function and, crucially, that this can be achieved using entirely endogenous means. Background
It has been suggested that there is frontal lobe involvement in schizophrenia, and that it may be lateralised and gender-specific.
AimsTo clarify the structure of the frontal lobes in schizophrenia in a postmortem series.
MethodThe volume of white matter and cortical components of the frontal lobes was measured in brains of controls and patients with schizophrenia using planimetry and the Cavalieri principle. The components measured were: superior frontal gyrus, middle frontal gyrus, a composite of inferior frontal gyrus and orbito-frontal cortex, as well as total frontal lobe cortex and white matter. In addition, the anterior cingulate gyrus was measured.
ResultsNo diagnosis, gender, diagnosis × side, diagnosis × gender or diagnosis × gender × side interactions were observed in the volume of any of the components, the grey matter as a whole or the white matter. No evidence for volumetric inter-group differences was found for the anterior cingulate gyrus.
ConclusionsSuch structural abnormalities as are present in the frontal lobes are more subtle than straightforward alterations in tissue volume; they may include changes in shape and the pattern of gyral folding.

Our target article discussed how emerging knowledge of the physiological processes involved in the control of saccadic eye movements provided the basis for a functional framework in which to understand the programming of such movements. The commentators raised many interesting issues in their varied responses that ranged from detailed discussion of the physiological substrate through issues of saccade control in reading. New evidence at the physiological level demonstrates that some elaborations are needed to the framework we proposed. Most clearly, the spatial selection process operates in a manner different from our suggestion of an increase in activity in the salience map. Some commentators make the interesting and welcome proposal that the functional processes we outline may in fact be implemented with an even more unified physiological substrate (continuity between collicular fixation and build-up cells) than we envisaged. Extensions to the framework are discussed involving the planning of sequential saccades, saccades made in crossmodal situations, the influences of learning and memory, and binocular saccades. We consider carefully the commentaries proposing explicit attentional and/or executive processes in the programming of saccades. We integrate the comments of researchers investigating saccade control in neurological and neuropsychiatric patients and finally discuss whether the framework can account for saccades made in the course of reading.

During active vision, the eyes continually scan the visual environment using saccadic scanning movements. This target article presents an information processing model for the control of these movements, with some close parallels to established physiological processes in the oculomotor system. Two separate pathways are concerned with the spatial and the temporal programming of the movement. In the temporal pathway there is spatially distributed coding and the saccade target is selected from a “salience map.” Both pathways descend through a hierarchy of levels, the lower ones operating automatically. Visual onsets have automatic access to the eye control system via the lower levels. Various centres in each pathway are interconnected via reciprocal inhibition. The model accounts for a number of well-established phenomena in target-elicited saccades: the gap effect, express saccades, the remote distractor effect, and the global effect. High-level control of the pathways in tasks such as visual search and reading is discussed; it operates through spatial selection and search selection, which generally combine in an automated way. The model is examined in relation to data from patients with unilateral neglect.

A previous report by Crow of a left-sided increase in temporal horn volume in schizophrenia implies a left-sided loss of tissue.
AimsTo elucidate the structural nature of schizophrenia.
MethodThe volume of grey matter in the temporal pole and inferior, middle and superior temporal gyri was measured, in addition to the total volume of grey and white matter, in the temporal lobes of the brains of 29 patients with schizophrenia and 27 controls.
ResultsWe found a significant left-sided reduction in the superior temporal gyrus in both males and females with schizophrenia, which was related to increasing age of onset in the males. The total volume of temporal lobe grey and white matter was also significantly reduced. Although being more marked on the left than the right, the lateralisation for these total grey and white measures (by contrast with the superior temporal gyrus alone) did not attain formal statistical significance.
ConclusionsConfirmation of a lateralised reduction in the superior temporal gyrus, which is differentially related to age of onset according to gender, adds to evidence that the changes in schizophrenia are in systems that are lateralised. The findings implicate language as the relevant function.

The taxonomy proposed by Pessoa et al. should be extended to include “pathological” completion phenomena in patients with unilateral brain damage. Patients with visual field defects (hemianopias) may “complete” whole figures, while patients with parietal lobe damage may “complete” partial figures. We argue that the former may be consistent with the brain “filling-in” information, and the latter may be consistent with the brain ignoring the absence of information.
