Methods for Improving Screening for Vascular Cognitive Impairment Using the Montreal Cognitive Assessment

Background:
Vascular cognitive impairment (VCI) post-stroke is frequent but may go undetected, which highlights the need to better screen cognitive functioning following a stroke.
Aim:We examined the clinical utility of the Montreal Cognitive Assessment (MoCA) in detecting cognitive impairment against a gold-standard neuropsychological battery.
Methods:We assessed cognitive status with a comprehensive battery of neuropsychological tests in 161 individuals who were at least 3-months post-stroke. We used receiver operating characteristic (ROC) curves to identify two cut points for the MoCA to maximize sensitivity and specificity at a minimum 90% threshold. We examined the utility of the Symbol Digit Modalities Test, a processing speed measure, to determine whether this additional metric would improve classification relative to the MoCA total score alone.
Results:Using two cut points, 27% of participants scored ≤ 23 and were classified as high probability of cognitive impairment (sensitivity 92%), and 24% of participants scored ≥ 28 and were classified as low probability of cognitive impairment (specificity 91%). The remaining 48% of participants scored from 24 to 27 and were classified as indeterminate probability of cognitive impairment. The addition of a processing speed measure improved classification for the indeterminate group by correctly identifying 65% of these individuals, for an overall classification accuracy of 79%.
Conclusions:The utility of the MoCA in detecting cognitive impairment post-stroke is improved when using a three-category approach. The addition of a processing speed measure provides a practical and efficient method to increase confidence in the determined outcome while minimally extending the screening routine for VCI.
Using Standardized Checklists Increase the Completion Rate of Critical Actions in an Evacuation from the Operating Room: A Randomized Controlled Simulation Study

Introduction:
Hospital evacuations of patients with special needs are extremely challenging, and it is difficult to train hospital workers for this rare event.
Hypothesis/Problem:Researchers developed an in-situ simulation study investigating the effect of standardized checklists on the evacuation of a patient under general anesthesia from the operating room (OR) and hypothesized that checklists would improve the completion rate of critical actions and decrease evacuation time.
Methods:A vertical evacuation of the high-fidelity manikin (SimMan3G; Laerdal Inc.; Norway) was performed and participants were asked to lead the team and evacuate the manikin to the ground floor after a mock fire alarm. Participants were randomized to two groups: one was given an evacuation checklist (checklist group [CG]) and the other was not (non-checklist group [NCG]). A total of 19 scenarios were run with 28 participants.
Results:Mean scenario time, preparation phase of evacuation, and time to transport the manikin down the stairs did not differ significantly between groups (P = .369, .462, and .935, respectively). The CG group showed significantly better performance of critical actions, including securing the airway, taking additional drug supplies, and taking additional equipment supplies (P = .047, .001, and .001, respectively). In the post-evacuation surveys, 27 out of 28 participants agreed that checklists would improve the evacuation process in a real event.
Conclusion:Standardized checklists increase the completion rate of pre-defined critical actions in evacuations out of the OR, which likely improves patient safety. Checklist use did not have a significant effect on total evacuation time.
Hospital-Level Variability in Broad-Spectrum Antibiotic Use for Children With Acute Leukemia Undergoing Hematopoietic Cell Transplantation

OBJECTIVE
To explore the prevalence and drivers of hospital-level variability in antibiotic utilization among hematopoietic cell transplant (HCT) recipients to inform antimicrobial stewardship initiatives.
DESIGNRetrospective cohort study using data merged from the Pediatric Health Information System and the Center for International Blood and Marrow Transplant Research.
SETTINGThe study included 27 transplant centers in freestanding children’s hospitals.
METHODSThe primary outcome was days of broad-spectrum antibiotic use in the interval from day of HCT through neutrophil engraftment. Hospital antibiotic utilization rates were reported as days of therapy (DOTs) per 1,000 neutropenic days. Negative binomial regression was used to estimate hospital utilization rates, adjusting for patient covariates including demographics, transplant characteristics, and severity of illness. To better quantify the magnitude of hospital variation and to explore hospital-level drivers in addition to patient-level drivers of variation, mixed-effects negative binomial models were also constructed.
RESULTSAdjusted hospital rates of antipseudomonal antibiotic use varied from 436 to 1121 DOTs per 1,000 neutropenic days, and rates of broad-spectrum, gram-positive antibiotic use varied from 153 to 728 DOTs per 1,000 neutropenic days. We detected variability by hospital in choice of antipseudomonal agent (ie, cephalosporins, penicillins, and carbapenems), but gram-positive coverage was primarily driven by vancomycin use. Considerable center-level variability remained even after controlling for additional hospital-level factors. Antibiotic use was not strongly associated with days of significant illness or mortality.
CONCLUSIONAmong a homogenous population of children undergoing HCT for acute leukemia, both the quantity and spectrum of antibiotic exposure in the immediate posttransplant period varied widely. Antimicrobial stewardship initiatives can apply these data to optimize the use of antibiotics in transplant patients.

First Record and Phylogenetic Significance of a Jurassic Diadematacean Sea Urchin from California

Although diadematacean sea urchins (diadematids, aspidodiadematids, and micropygids) likely diverged sometime during the Jurassic, the lack of fossils representing this group has greatly hampered progress in understanding their evolution. No unequivocal Jurassic diademataceans have been described previously from North America. We describe a new genus and species, Sierradiadema kristini, from a single fossil from the Middle Jurassic (Callovian) Colfax sequence of the Mariposa Formation exposed in the Middle Fork American River drainage of the northern Sierra Nevada, California. The specimen, although not complete, reveals details of the spination and tooth morphology often lacking in Jurassic diademataceans, along with test architecture that yields information concerning the relative timing of important events in the origins of the diadematids in particular. We explore this evolution with a phylogenetic analysis of relevant clades with Jurassic times of divergence, finding not only that Sierradiadema n. gen. is the earliest known member of a clade containing the extant Diadematidae, but that it will stimulate ongoing discussion of the putative Jurassic origins of all the diadematacean groups.

The Equalizing Effect of the Internet on Access to Research Expertise in Political Science and Economics

Recent research shows that the Internet, by sharply reducing the real costs of sharing information, has profoundly impacted collaboration between faculty in different departments. Using information on spatial-temporal Bitnet connections, Agrawal and Goldfarb (2006) find that Internet connectivity increased collaboration between those connected universities by 85%. Extensively analyzing faculty productivity at the top 25 universities during the last three decades, Kim, Morse, and Zingales (2006) report that the Internet has significantly lowered department-specific research externalities in economics and finance by giving those outside of top schools access to productive research colleagues within top schools. Concomitant with this decline in location-specific externalities, they also report that salaries at those top universities have increased since the universities are less able to appropriate any rent based on research productivity created by physically being located at that university. One of the reasons that physical location is less important is e-mail. Walsh and Maloney (2003), using information from four fields (experimental biology, mathematics, physics, and sociology), find that e-mail has significantly reduced coordination problems in research collaboration.
