Design hackathons offer a unique research opportunity to study time-pressured collaborative design. At the same time, research on design hackathons faces unique methodological challenges, prompting the exploration of new research approaches. This paper proposes a new data-collection framework that leverages a virtual format of hackathon events and enables a deeper insight into hackathon dynamics. The framework applicability is presented through a case study of the IDEA challenge hackathon, in which different intrusive and non-intrusive data collection approaches were used.

Bedside clinical neurologic assessment utilisation in paediatric cardiac intensive care units

Introduction
Neurodevelopmental disabilities in children with CHD can result from neurologic injury sustained in the cardiac ICU when children are at high risk of acute neurologic injury. Physicians typically order and specify frequency for serial bedside nursing clinical neurologic assessments to evaluate patients’ neurologic status.
Materials and methodsWe surveyed cardiac ICU physicians to understand how these assessments are performed, and the attitudes of physicians on the utility of these assessments. The survey contained questions regarding assessment elements, assessment frequency, communication of neurologic status changes, and optimisation of assessments.
ResultsSurveys were received from 50 institutions, with a response rate of 86%. Routine clinical neurologic assessments were reported to be performed in 94% of institutions and standardised in 56%. Pupillary reflex was the most commonly reported assessment. In all, 77% of institutions used a coma scale, with Glasgow Coma Scale being most common. For patients with acute brain injury, 82% of institutions reported performing assessments hourly, whereas assessment frequency was more variable for low-risk and high-risk patients without overt brain injury. In all, 84% of respondents thought their current practice for assessing and monitoring neurologic status was suboptimal. Only 41% felt that the Glasgow Coma Scale was a valuable tool for assessing neurologic function in the cardiac ICU, and 91% felt that a standardised approach to assessing pre-illness neurologic function would be valuable.
ConclusionsRoutine nursing neurologic assessments are conducted in most surveyed paediatric cardiac ICUs, although assessment characteristics vary greatly between institutions. Most clinicians rated current neurologic assessment practices as suboptimal.

West Antarctic climate and surface mass balance (SMB) records are sparse. To fill this gap, regional atmospheric climate modelling is useful, providing that such models are employed at sufficiently high horizontal resolution and coupled with a snow model. Here we present the results of a high-resolution (5.5 km) regional atmospheric climate model (RACMO2) simulation of coastal West Antarctica for the period 1979–2015. We evaluate the results with available in situ weather observations, remote-sensing estimates of surface melt, and SMB estimates derived from radar and firn cores. Moreover, results are compared with those from a lower-resolution version, to assess the added value of the resolution. The high-resolution model resolves small-scale climate variability invoked by topography, such as the relatively warm conditions over ice-shelf grounding zones, and local wind speed accelerations. Surface melt and SMB are well reproduced by RACMO2. This dataset will prove useful for picking ice core locations, converting elevation changes to mass changes, for driving ocean, ice-sheet and coupled models, and for attributing changes in the West Antarctic Ice Sheet and shelves to changes in atmospheric forcing.

Fibroblast growth factor (FGF)-2 (basic) is a potent angiogenic molecule involved in tumor progression, and is one of several growth factors with a central role in ovarian carcinogenesis. We hypothesized that common single nucleotide polymorphisms (SNPs) in the FGF2 gene may alter angiogenic potential and thereby susceptibility to ovarian cancer. We analyzed 25 FGF2 tgSNPs using five independent study populations from the United States and Australia. Analysis was restricted to non-Hispanic White women with serous ovarian carcinoma (1269 cases and 2829 controls). There were no statistically significant associations between any FGF2 SNPs and ovarian cancer risk. There were two nominally statistically significant associations between heterozygosity for two FGF2 SNPs (rs308379 and rs308447; p < .05) and serous ovarian cancer risk in the combined dataset, but rare homozygous estimates did not achieve statistical significance, nor were they consistent with the log additive model of inheritance. Overall genetic variation in FGF2 does not appear to play a role in susceptibility to ovarian cancer.

Executive summary
Nature of the problem
Observations of atmospheric reactive nitrogen (Nr) deposition are severely restricted in spatial extent and type. The chain of processes leading to atmospheric deposition emissions, atmospheric dispersion, chemical transformation and eventual loss from the atmosphere is extremely complex and therefore currently, observations can only address part of this chain.
Approaches
Modelling provides a way of estimating atmospheric transport and deposition of Nr at the European scale. A description of the different model types is provided.
Current deposition estimates from models are compared with observations from European air chemistry monitoring networks.
The main focus of the chapter is at the European scale; however, both local variability and and intercontinental Nr transfers are also addressed.
Key findings/state of knowledge
Atmospheric deposition is a major input of Nr for European terrestrial and freshwater ecosystems as well as coastal sea areas.
Models are key tools to integrate our understanding of atmospheric chemistry and transport, and are essential for estimating the spatial distribution of deposition, and to support the formulation of air pollution control strategies.
Our knowledge of the reliability of models for deposition estimates is, however, limited, since we have so few observational constraints on many key parameters.
Total Nr deposition estimates cannot be directly assessed because of a lack of measurements, especially of the Nr dry deposition component. Differences among European regional models can be significant, however, e.g. 30% in some areas, and substantially more than this for specific locations.

Summary
Appropriate pediatric CPR differs from that in adults, because children are anatomically and physiologically different from adults. In addition, the pathogenesis of the cardiac arrests and the most common rhythm disturbances are different in children. In contrast to adults, children rarely suffer sudden ventricular fibrillation (VF) cardiac arrest from coronary artery disease. The causes of pediatric arrests are more diverse and are usually secondary to profound hypoxia or asphyxia due to respiratory failure or circulatory shock. Prolonged hypoxia and acidosis impair cardiac function and ultimately lead to cardiac arrest. By the time the arrest occurs, all organs of the body have generally suffered significant hypoxic-ischemic insults.
Importantly, children of various ages exhibit developmental changes that affect cardiac and respiratory physiology before, during, and after cardiac arrest. For example, newborns undergoing transitional physiological changes during emergence from an environment of amniotic fluid to a gaseous environment certainly differ from adolescents. Similarly, newborns and infants have much less cardiac and respiratory reserve, and higher pulmonary vascular resistance than do older children. Moreover, many children who experience in-hospital cardiac arrest have pre-existing developmental challenges and other organ dysfunction. Finally, pediatrics is developmental medicine, and pediatric neurological tools that are appropriate at one age may not be accurate or valid at another age.
Perhaps the most profound difference between child and adult cardiac arrest is the devastating effect of the death of a child on a family. Coping with a sudden unexpected death is always difficult. When the victim is a child, the loss tends to be even more oppressive. We do not expect children to die before their parents and thus are not prepared for it.

Summary
Drowning refers to submersion and immersion. This chapter focuses on submersion and reviews the epidemiology, pathophysiology, and treatment of this mainly respiratory problem. Hypothermia-related immersion issues are described in Chapter 49.
Epidemiology
Circumstances of drowning vary around the world; from healthy toddlers to desperate boat refugees, and from beach to bathtub. According to the WHO report, each year between 350 000 and 450 000 persons die from drowning. In addition, in some years, over 500 000 persons have drowned in floods and tsunamis. Most drowned victims are children, and the potential years of life lost are immense. Within this global perspective, 97% of all drowning occurs in South East Asia, the Pacific, and Africa. In some areas, the drowning rate is as high as 400 persons per 100 000 inhabitants. The leading cause of drowning in these areas is multifactorial. Leisure, work, transport, and collecting water for household purposes occur in the surroundings of water. Swimming skills are lacking, as is the knowledge on how to perform rescue, first aid, or basic life support (BLS). Also, prevention efforts, rescue resources, or communication equipment are poor.
In the Western world, a combination of socioeconomic factors, legislation, multifaceted prevention programs, improved rescue techniques, and up-to-date medical systems have resulted in a 10- to 20-fold decrease in drowning rates during the last 50 years. The death rate is between 0.1 and 2.5 per 100 000 inhabitants.

Background
We explored the course of broadly defined eating disorders during pregnancy in the Norwegian Mother and Child Cohort Study (MoBa) at the Norwegian Institute of Public Health.
MethodA total of 41 157 pregnant women, enrolled at approximately 18 weeks' gestation, had valid data from the Norwegian Medical Birth Registry. We collected questionnaire-based diagnostic information on broadly defined anorexia nervosa (AN), and bulimia nervosa (BN), and eating disorders not otherwise specified (EDNOS). EDNOS subtypes included binge eating disorder (BED) and recurrent self-induced purging in the absence of binge eating (EDNOS-P). We explored rates of remission, continuation and incidence of BN, BED and EDNOS-P during pregnancy.
ResultsPrepregnancy prevalence estimates were 0·1% for AN, 0·7% for BN, 3·5% for BED and 0·1% for EDNOS-P. During early pregnancy, estimates were 0·2% (BN), 4·8% (BED) and 0·1% (EDNOS-P). Proportions of individuals remitting during pregnancy were 78% (EDNOS-P), 40% (BN purging), 39% (BED), 34% (BN any type) and 29% (BN non-purging type). Additional individuals with BN achieved partial remission. Incident BN and EDNOS-P during pregnancy were rare. For BED, the incidence rate was 1·1 per 1000 person-weeks, equating to 711 new cases of BED during pregnancy. Incident BED was associated with indices of lower socio-economic status.
ConclusionsPregnancy appears to be a catalyst for remission of some eating disorders but also a vulnerability window for the new onset of broadly defined BED, especially in economically disadvantaged individuals. Vigilance by health-care professionals for continuation and emergence of eating disorders in pregnancy is warranted.

The high level of meat and saturated fat consumption in the USA and other high-income countries exceeds nutritional needs and contributes to high rates of chronic diseases such as cardiovascular disease, diabetes mellitus and some cancers. Affluent citizens in middle- and low-income countries are adopting similar high-meat diets and experiencing increased rates of these same chronic diseases. The industrial agricultural system, now the predominant form of agriculture in the USA and increasingly world-wide, has consequences for public health owing to its extensive use of fertilisers and pesticides, unsustainable use of resources and environmental pollution. In industrial animal production there are public health concerns surrounding feed formulations that include animal tissues, arsenic and antibiotics as well as occupational health risks and risks for nearby communities. It is of paramount importance for public health professionals to become aware of and involved in how our food is produced.
Collagenous Materials Enhance Healing of Chronic Skin Ulcers

Type I collagen in a porous sponge form attracts fibroblasts in culture and accelerates repair of animal wounds. This study examines the effect of type I collagen sponge and flakes on healing of chronic skin ulcers. Patients included in this study had skin ulcers.
Patients included in this study had skin ulcers characterized by loss of dermis and epidermis without exposure of muscle, tendon or bone. Patients showing evidence of systemic infection or patients with ulcers that decreased in area during an initial three week observation period were excluded.
Three out of seven patients treated with a collagen sponge and twelve out of fourteen patients treated with collagen flakes showed a 40% decrease in wound area after six weeks of treatment. In comparison, eighteen control patients showed no change in wound area over the same time interval. These results suggest that collagen flakes are effective in initiating healing of chronic skin ulcers.
