In view of the increasing complexity of both cardiovascular implantable electronic devices (CIEDs) and patients in the current era, practice guidelines, by necessity, have become increasingly specific. This document is an expert consensus statement that has been developed to update and further delineate indications and management of CIEDs in pediatric patients, defined as ≤21 years of age, and is intended to focus primarily on the indications for CIEDs in the setting of specific disease categories. The document also highlights variations between previously published adult and pediatric CIED recommendations and provides rationale for underlying important differences. The document addresses some of the deterrents to CIED access in low- and middle-income countries and strategies to circumvent them. The document sections were divided up and drafted by the writing committee members according to their expertise. The recommendations represent the consensus opinion of the entire writing committee, graded by class of recommendation and level of evidence. Several questions addressed in this document either do not lend themselves to clinical trials or are rare disease entities, and in these instances recommendations are based on consensus expert opinion. Furthermore, specific recommendations, even when supported by substantial data, do not replace the need for clinical judgment and patient-specific decision-making. The recommendations were opened for public comment to Pediatric and Congenital Electrophysiology Society (PACES) members and underwent external review by the scientific and clinical document committee of the Heart Rhythm Society (HRS), the science advisory and coordinating committee of the American Heart Association (AHA), the American College of Cardiology (ACC), and the Association for European Paediatric and Congenital Cardiology (AEPC). The document received endorsement by all the collaborators and the Asia Pacific Heart Rhythm Society (APHRS), the Indian Heart Rhythm Society (IHRS), and the Latin American Heart Rhythm Society (LAHRS). This document is expected to provide support for clinicians and patients to allow for appropriate CIED use, appropriate CIED management, and appropriate CIED follow-up in pediatric patients.

Background:
Multicentre research databases can provide insights into healthcare processes to improve outcomes and make practice recommendations for novel approaches. Effective audits can establish a framework for reporting research efforts, ensuring accurate reporting, and spearheading quality improvement. Although a variety of data auditing models and standards exist, barriers to effective auditing including costs, regulatory requirements, travel, and design complexity must be considered.
Materials and methods:The Congenital Cardiac Research Collaborative conducted a virtual data training initiative and remote source data verification audit on a retrospective multicentre dataset. CCRC investigators across nine institutions were trained to extract and enter data into a robust dataset on patients with tetralogy of Fallot who required neonatal intervention. Centres provided de-identified source files for a randomised 10% patient sample audit. Key auditing variables, discrepancy types, and severity levels were analysed across two study groups, primary repair and staged repair.
Results:Of the total 572 study patients, data from 58 patients (31 staged repairs and 27 primary repairs) were source data verified. Amongst the 1790 variables audited, 45 discrepancies were discovered, resulting in an overall accuracy rate of 97.5%. High accuracy rates were consistent across all CCRC institutions ranging from 94.6% to 99.4% and were reported for both minor (1.5%) and major discrepancies type classifications (1.1%).
Conclusion:Findings indicate that implementing a virtual multicentre training initiative and remote source data verification audit can identify data quality concerns and produce a reliable, high-quality dataset. Remote auditing capacity is especially important during the current COVID-19 pandemic.

Background/Aims:
Registry-based trials have emerged as a potentially cost-saving study methodology. Early estimates of cost savings, however, conflated the benefits associated with registry utilisation and those associated with other aspects of pragmatic trial designs, which might not all be as broadly applicable. In this study, we sought to build a practical tool that investigators could use across disciplines to estimate the ranges of potential cost differences associated with implementing registry-based trials versus standard clinical trials.
Methods:We built simulation Markov models to compare unique costs associated with data acquisition, cleaning, and linkage under a registry-based trial design versus a standard clinical trial. We conducted one-way, two-way, and probabilistic sensitivity analyses, varying study characteristics over broad ranges, to determine thresholds at which investigators might optimally select each trial design.
Results:Registry-based trials were more cost effective than standard clinical trials 98.6% of the time. Data-related cost savings ranged from $4300 to $600,000 with variation in study characteristics. Cost differences were most reactive to the number of patients in a study, the number of data elements per patient available in a registry, and the speed with which research coordinators could manually abstract data. Registry incorporation resulted in cost savings when as few as 3768 independent data elements were available and when manual data abstraction took as little as 3.4 seconds per data field.
Conclusions:Registries offer important resources for investigators. When available, their broad incorporation may help the scientific community reduce the costs of clinical investigation. We offer here a practical tool for investigators to assess potential costs savings.

We describe an algorithm that can fit the properties of the dwarf galaxy progenitor of a tidal stream, given the properties of that stream. We show that under ideal conditions (the Milky Way potential, the orbit of the dwarf galaxy progenitor, and the functional form of the dwarf galaxy progenitor are known exactly), the density and angular width of stars along the stream can be used to constrain the mass and radial profile of both the stellar and dark matter components of the progenitor dwarf galaxy that was ripped apart to create the stream. Our provisional fit for the parameters of the dwarf galaxy progenitor of the Orphan Stream indicates that it is less massive and has fewer stars than previous works have indicated.

Background:
Determining infectious cross-transmission events in healthcare settings involves manual surveillance of case clusters by infection control personnel, followed by strain typing of clinical/environmental isolates suspected in said clusters. Recent advances in genomic sequencing and cloud computing now allow for the rapid molecular typing of infecting isolates.
Objective:To facilitate rapid recognition of transmission clusters, we aimed to assess infection control surveillance using whole-genome sequencing (WGS) of microbial pathogens to identify cross-transmission events for epidemiologic review.
Methods:Clinical isolates of Staphylococcus aureus, Enterococcus faecium, Pseudomonas aeruginosa, and Klebsiella pneumoniae were obtained prospectively at an academic medical center, from September 1, 2016, to September 30, 2017. Isolate genomes were sequenced, followed by single-nucleotide variant analysis; a cloud-computing platform was used for whole-genome sequence analysis and cluster identification.
Results:Most strains of the 4 studied pathogens were unrelated, and 34 potential transmission clusters were present. The characteristics of the potential clusters were complex and likely not identifiable by traditional surveillance alone. Notably, only 1 cluster had been suspected by routine manual surveillance.
Conclusions:Our work supports the assertion that integration of genomic and clinical epidemiologic data can augment infection control surveillance for both the identification of cross-transmission events and the inclusion of missed and exclusion of misidentified outbreaks (ie, false alarms). The integration of clinical data is essential to prioritize suspect clusters for investigation, and for existing infections, a timely review of both the clinical and WGS results can hold promise to reduce HAIs. A richer understanding of cross-transmission events within healthcare settings will require the expansion of current surveillance approaches.
Bahiagrass (Paspalum notatum) Seedhead Suppression Following Consecutive Yearly Applications of Plant Growth Retardants

Field trials evaluated sequential applications of plant growth retardants (PGRs) to bahiagrass for a 3-yr period at two Florida locations: Newberry (north Florida) and Okeechobee (south Florida). Seven PGRs from various chemical families were used: glyphosate, fluazifop, sethoxydim, maleic hydrazide, trinexapac-ethyl, imazapic, and imazaquin. In 1991 and 1992, PGRs reduced turf color for 2 to 4 wk after treatment (WAT) at the Newberry location but remained above acceptable levels (> 5.0) and recovered by 6 WAT. In 1993, glyphosate, fluazifop, and maleic hydrazide reduced turf color below acceptable levels (< 5.0) 4 WAT, but turf color was acceptable by 6 WAT. Turf color was slightly reduced following single and sequential imazapic applications in Okeechobee; however, turf recovered by 8 WAT. Cumulative seedhead suppression at both locations increased during the 3 yr resulting in at least a 60% reduction in seedhead numbers. Seedhead production each year was 10 to 50% less at 12 WAT in treated plots than in the previous year. Single applications of imazapic and imazapic + imazaquin provided 85 to 100% seedhead suppression for 12 WAT and 100% suppression for up to 16 wk following sequential applications made 8 wk after the initial application (WAI). Single applications of glyphosate, fluazifop, and maleic hydrazide provided ≥ 70% seedhead suppression for 4 to 8 WAT. Sequential applications of fluazifop made 8 WAI provided > 70% control of seedheads through 8 to 16 WAI for 3 yr.
Absorption, Translocation, and Metabolism of Acifluorfen and Lactofen in Pitted Morningglory (Ipomoea lacunosa) and Ivyleaf Morningglory (Ipomoea hederacea)

Experiments were conducted to determine 14C absorption, translocation, and metabolism by pitted morningglory (Ipomoea lacunosa L. # IPOLA) and ivyleaf morningglory [Ipomoea hederacea (L.) Jacq. # IPOHE] foliarly treated with 14C-acifluorfen [5-(2-chloro-4-(trifluoromethyl)phenoxy)-2-nitrobenzoic acid] or 14C-lactofen [(±)-2-ethoxy-1-methyl-2-oxoethyl-5-(2-chloro-4-(trifluoromethyl)phenoxy)-2-dinitrobenzoate]. Seventy-one to 84% of applied 14 C-acifluorfen was recovered in leaf water wash of ivyleaf morningglory compared to 32 to 46% from pitted morningglory. Sixty-four percent of applied 14C-lactofen was recovered in leaf water wash 96 h after treatment in both morningglory species. Thirty-five to 37% more 14C from applied 14C-acifluorfen was found in pitted morningglory treated leaves than ivyleaf morningglory treated leaves. Less than 28% of applied 14C-lactofen was extracted from treated leaves of either morningglory species. Translocation and metabolism of 14C-acifluorfen and 14C-lactofen were minimal in both morningglory species.

Cotton [Gossypium hirsutum (L.) ‘Hancock’] lint yield or quality was not adversely affected under weed free conditions by three consecutive annual preemergence applications of up to 1.68 kg/ha fluometuron [1,1-dimethyl-3-(a,a,a-trifluoro-m-tolyl)urea] alone or in combination with 0.84 kg/ha trifluralin (a,a,a-trifluoro-2,6-dinitro-N,N-dipropyl-p-toluidine) preplant incorporated on silt loam soils of the Memphis catena containing less than 1% organic matter. Similar applications of fluometuron at 3.36 kg/ha delayed maturity of cotton plants and ultimately reduced lint yields. Trifluralin treatments tended to overcome the detrimental effects of high rates of fluometuron. There was no effect on fiber development and maturation because fiber length and fineness (micronaire) were virtually identical for all treatments. The effect on maturity as determined by percent first harvest was a reflection of slower plant growth and fruiting and not of the development of the fiber itself. These results indicate no detrimental effects on cotton yield, maturity, and quality from annual applications of these herbicides at rates recommended and normally used in Tennessee.

The general appearance of plants was similar for 14 accessions of velvetleaf (Abutilon theophrasti Medic. ♯ ABUTH) obtained from Stoneville, MS (33° 25’ N), Rosemount, MN (44° 43’ N), and various locations in between. All accessions were equally susceptible to bentazon [3-isopropyl-1H-2,1,3-benzothiadiazin-4(3H)-one 2,2-dioxide]. Variation in maturity characteristics was found among the accessions in the field at Rosemount; southern accessions began seed capsule production later and continued it longer than did northern accessions. Velvetleaf accessions from Stoneville and Rosemount both produced seeds in the field at Weslaco, TX (26° 09’ N), but at Fairbanks, AK (64° 49’ N), neither accession produced seeds before being killed by frost.
Cross-Scale Assessment of Potential Habitat Shifts in a Rapidly Changing Climate

We assessed the ability of climatic, environmental, and anthropogenic variables to predict areas of high-risk for plant invasion and consider the relative importance and contribution of these predictor variables by considering two spatial scales in a region of rapidly changing climate. We created predictive distribution models, using Maxent, for three highly invasive plant species (Canada thistle, white sweetclover, and reed canarygrass) in Alaska at both a regional scale and a local scale. Regional scale models encompassed southern coastal Alaska and were developed from topographic and climatic data at a 2 km (1.2 mi) spatial resolution. Models were applied to future climate (2030). Local scale models were spatially nested within the regional area; these models incorporated physiographic and anthropogenic variables at a 30 m (98.4 ft) resolution. Regional and local models performed well (AUC values > 0.7), with the exception of one species at each spatial scale. Regional models predict an increase in area of suitable habitat for all species by 2030 with a general shift to higher elevation areas; however, the distribution of each species was driven by different climate and topographical variables. In contrast local models indicate that distance to right-of-ways and elevation are associated with habitat suitability for all three species at this spatial level. Combining results from regional models, capturing long-term distribution, and local models, capturing near-term establishment and distribution, offers a new and effective tool for highlighting at-risk areas and provides insight on how variables acting at different scales contribute to suitability predictions. The combinations also provides easy comparison, highlighting agreement between the two scales, where long-term distribution factors predict suitability while near-term do not and vice versa.
Change in agitation in Alzheimer's disease in the placebo arm of a nine-week controlled trial

Background:
Placebo responses raise significant challenges for the design of clinical trials. We report changes in agitation outcomes in the placebo arm of a recent trial of citalopram for agitation in Alzheimer's disease (CitAD).
Methods:In the CitAD study, all participants and caregivers received a psychosocial intervention and 92 were assigned to placebo for nine weeks. Outcomes included Neurobehavioral Rating Scale agitation subscale (NBRS-A), modified AD Cooperative Study-Clinical Global Impression of Change (CGIC), Cohen-Mansfield Agitation Inventory (CMAI), the Neuropsychiatric Inventory (NPI) Agitation/Aggression domain (NPI A/A) and Total (NPI-Total) and ADLs. Continuous outcomes were analyzed with mixed-effects modeling and dichotomous outcomes with logistic regression.
Results:Agitation outcomes improved over nine weeks: NBRS-A mean (SD) decreased from 7.8 (3.0) at baseline to 5.4 (3.2), CMAI from 28.7 (6.7) to 26.7 (7.4), NPI A/A from 8.0 (2.4) to 4.9 (3.8), and NPI-Total from 37.3 (17.7) to 28.4 (22.1). The proportion of CGI-C agitation responders ranged from 21 to 29% and was significantly different from zero. MMSE improved from 14.4 (6.9) to 15.7 (7.2) and ADLs similarly improved. Most of the improvement was observed by three weeks and was sustained through nine weeks. The major predictor of improvement in each agitation measure was a higher baseline score in that measure.
Conclusions:We observed significant placebo response which may be due to regression to the mean, response to a psychosocial intervention, natural course of symptoms, or nonspecific benefits of participation in a trial.

The economic feasibility of soybeans, grain sorghum, and corn in annualrotation with winter wheat using reduced tillage and no-tillage systems inthe Central Great Plains was evaluated, with continuous wheat and grainsorghum also analyzed. Net returns were calculated using simulated yield andprice distributions based on historical yields, two historical annual priceseries, and 2011 costs. Impaired flexibility in the use of substrates for energy production in the heart is implicated in cardiomyopathy. We investigated the effect of maternal protein restriction during pregnancy in rats on the transcription of key genes in cardiac lipid and carbohydrate metabolism in the offspring. Rats were fed protein-sufficient or protein-restricted (PR) diets during pregnancy. Triacylglycerol concentration in adult (day 105) heart was altered by maternal protein intake contingent on post-weaning fat intake and sex. mRNA expression of peroxisomal proliferator-activated receptor (PPAR)-α and carnitine palmitoyltransferase-1 was increased by the maternal PR diet in adult, but not neonatal, offspring. PPARα promoter methylation was lower in adult and neonatal heart from PR offspring. These findings suggest that prenatal nutrition alters the future transcriptional regulation of cardiac energy metabolism in the offspring through changes in epigenetic regulation of specific genes. However, changes in gene functional changes may not be apparent in early life.
Maternal protein restriction with or without folic acid supplementation during pregnancy alters the hepatic transcriptome in adult male rats

Feeding pregnant rats a protein-restricted (PR) diet induces altered expression of candidate genes in the liver of the adult offspring, which can be prevented by supplementation of the PR diet with folic acid (PRF). We investigated the effect of maternal nutrition during pregnancy on the liver transcriptome in their adult male offspring. Pregnant rats were fed control, PR or PRF diets. Male offspring were killed on day 84. The liver transcriptome was analysed by microarray (six livers per maternal dietary group) followed by post hoc analysis of relative mRNA levels and gene ontology. These results were confirmed for selected genes by real-time RT-PCR. There were 311 genes that differed significantly ( ≥ 1·5-fold change; P < 0·05) between PR offspring (222 increased) and control offspring, while 191 genes differed significantly between PRF offspring (forty-five increased) compared with offspring of control dams. There were sixteen genes that were significantly altered in both PR and PRF offspring compared with controls. Ion transport, developmental process, and response to reactive oxygen species (RROS) and steroid hormone response (SHR) ontologies were altered in PR offspring. Folic acid supplementation prevented changes within RROS and SHR response pathways, but not in ion transport or developmental process. There was no effect of maternal PR on mRNA expression of imprinted genes. Insulin 1 and Pleckstrin homology-like domain family A member 2 were increased significantly in PRF compared with PR offspring. The present findings show that the pattern of induced changes in the adult liver transcriptome were dependent on maternal protein and folic acid intakes during pregnancy.
