Summary
Randomized clinical trials, a large NCI-Kaiser Permanente real-world follow-up study involving over one million women, and the three US FDA registration trials clearly demonstrate that HPV primary screening for cervical cancer is safe and effective. Co-testing that combines both HPV testing and cytology offers minimal, if any, additional benefit over HPV primary testing and increases costs, requires follow-up of HPV-negative women with ASCUS and LSIL that can lead to unnecessary colposcopies/biopsies and retains the medico-legal risk of false-negative cytology results. A number of countries have adopted HPV primary screening for their national screening programs and in the United States HPV primary screening is now classified as the preferred screening approach for women >25 years by the 2020 American Cancer Society guidelines.

The coronavirus disease 2019 (COVID-19) pandemic highlighted the lack of agreement regarding the definition of aerosol-generating procedures and potential risk to healthcare personnel. We convened a group of Massachusetts healthcare epidemiologists to develop consensus through expert opinion in an area where broader guidance was lacking at the time.

Testing of asymptomatic patients for severe acute respiratory coronavirus virus 2 (SARS-CoV-2) (ie, “asymptomatic screening) to attempt to reduce the risk of nosocomial transmission has been extensive and resource intensive, and such testing is of unclear benefit when added to other layers of infection prevention mitigation controls. In addition, the logistic challenges and costs related to screening program implementation, data noting the lack of substantial aerosol generation with elective controlled intubation, extubation, and other procedures, and the adverse patient and facility consequences of asymptomatic screening call into question the utility of this infection prevention intervention. Consequently, the Society for Healthcare Epidemiology of America (SHEA) recommends against routine universal use of asymptomatic screening for SARS-CoV-2 in healthcare facilities. Specifically, preprocedure asymptomatic screening is unlikely to provide incremental benefit in preventing SARS-CoV-2 transmission in the procedural and perioperative environment when other infection prevention strategies are in place, and it should not be considered a requirement for all patients. Admission screening may be beneficial during times of increased virus transmission in some settings where other layers of controls are limited (eg, behavioral health, congregate care, or shared patient rooms), but widespread routine use of admission asymptomatic screening is not recommended over strengthening other infection prevention controls. In this commentary, we outline the challenges surrounding the use of asymptomatic screening, including logistics and costs of implementing a screening program, and adverse patient and facility consequences. We review data pertaining to the lack of substantial aerosol generation during elective controlled intubation, extubation, and other procedures, and we provide guidance for when asymptomatic screening for SARS-CoV-2 may be considered in a limited scope.

The Hierarchical Taxonomy of Psychopathology (HiTOP) has emerged out of the quantitative approach to psychiatric nosology. This approach identifies psychopathology constructs based on patterns of co-variation among signs and symptoms. The initial HiTOP model, which was published in 2017, is based on a large literature that spans decades of research. HiTOP is a living model that undergoes revision as new data become available. Here we discuss advantages and practical considerations of using this system in psychiatric practice and research. We especially highlight limitations of HiTOP and ongoing efforts to address them. We describe differences and similarities between HiTOP and existing diagnostic systems. Next, we review the types of evidence that informed development of HiTOP, including populations in which it has been studied and data on its validity. The paper also describes how HiTOP can facilitate research on genetic and environmental causes of psychopathology as well as the search for neurobiologic mechanisms and novel treatments. Furthermore, we consider implications for public health programs and prevention of mental disorders. We also review data on clinical utility and illustrate clinical application of HiTOP. Importantly, the model is based on measures and practices that are already used widely in clinical settings. HiTOP offers a way to organize and formalize these techniques. This model already can contribute to progress in psychiatry and complement traditional nosologies. Moreover, HiTOP seeks to facilitate research on linkages between phenotypes and biological processes, which may enable construction of a system that encompasses both biomarkers and precise clinical description.

Objectives:
HIV-associated neurocognitive disorders (HANDs) are prevalent in older people living with HIV (PLWH) worldwide. HAND prevalence and incidence studies of the newly emergent population of combination antiretroviral therapy (cART)-treated older PLWH in sub-Saharan Africa are currently lacking. We aimed to estimate HAND prevalence and incidence using robust measures in stable, cART-treated older adults under long-term follow-up in Tanzania and report cognitive comorbidities.
Design:Longitudinal study
Participants:A systematic sample of consenting HIV-positive adults aged ≥50 years attending routine clinical care at an HIV Care and Treatment Centre during March–May 2016 and followed up March–May 2017.
Measurements:HAND by consensus panel Frascati criteria based on detailed locally normed low-literacy neuropsychological battery, structured neuropsychiatric clinical assessment, and collateral history. Demographic and etiological factors by self-report and clinical records.
Results:In this cohort (n = 253, 72.3% female, median age 57), HAND prevalence was 47.0% (95% CI 40.9–53.2, n = 119) despite well-managed HIV disease (Mn CD4 516 (98-1719), 95.5% on cART). Of these, 64 (25.3%) were asymptomatic neurocognitive impairment, 46 (18.2%) mild neurocognitive disorder, and 9 (3.6%) HIV-associated dementia. One-year incidence was high (37.2%, 95% CI 25.9 to 51.8), but some reversibility (17.6%, 95% CI 10.0–28.6 n = 16) was observed.
Conclusions:HAND appear highly prevalent in older PLWH in this setting, where demographic profile differs markedly to high-income cohorts, and comorbidities are frequent. Incidence and reversibility also appear high. Future studies should focus on etiologies and potentially reversible factors in this setting.

Accurate assessments of population sizes and trends are fundamental for effective species conservation, particularly for social and long-lived species in which low reproductive rates, aging demographic structure and Allee effects could interact to drive rapid population declines. In the parrots (Order Psittaciformes) these life history characteristics have combined with habitat loss and capture for the pet trade to lead to widespread endangerment, with over 40% of species classified under some level of threat. Here we report the results of a population survey of one such species, the Yellow-naped Amazon, Amazona auropalliata, that is classified as ‘Endangered’ on the IUCN Red List. We conducted a comprehensive survey in June and July of 2016 of 44 night roosts of the populations in contiguous Pacific lowlands of northern Costa Rica and southern Nicaragua and compared numbers in Costa Rica to those found in a similar survey conducted in June 2005. In 2016 we counted 990 birds across 25 sites surveyed in Costa Rica and 692 birds across 19 sites surveyed in Nicaragua for a total population estimate of only 1,682 birds. Comparisons of 13 sites surveyed in both 2005 and 2016 in Costa Rica showed a strong and statistically significant decline in population numbers over the 11-year period. Assessment of group sizes approaching or leaving roosts indicated that less than 25% of groups consisted of three or more birds; there was a significantly higher proportion of these putative family groups observed in Nicaragua than Costa Rica. Taken together, these results are cause for substantial concern for the health of this species in a region that has previously been considered its stronghold, and suggest that stronger conservation action should be undertaken to protect remaining populations from capture for the pet trade and loss of key habitat.
Comparative responses of soybean (Glycine max), sicklepod (Senna obtusifolia), and Palmer amaranth (Amaranthus palmeri) to root zone and aerial temperatures

Experiments were conducted to compare germination efficiencies and vegetative growth of soybean and the competing weed species, sicklepod and Palmer amaranth, over a range of temperatures in the root zone and aerial environments. From genetic origins we hypothesized that the weeds would have a higher temperature optimum, which would help explain competitive interactions seen in the southeastern U.S. Germination experiments indicated that germination efficiency of the weeds was much more sensitive to low temperature than soybean, being markedly inhibited below 18 C. Similarly, experiments in an automated, temperature-controlled hydroponic system revealed that the weed species were less tolerant of low root zone temperature but more tolerant of high root zone temperature than soybean. At 16 C, dry weight of soybean was 74% of the control dry weight at 24 C, whereas dry weights of sicklepod and Palmer amaranth were 5 and 20% of the control, respectively. At 32 C, soybean root dry weight was only 80% of the 24 C treatment, whereas root dry weight of the weed species was not significantly different. When plants were grown at a low aerial temperature, growth of all plants was strongly inhibited] but the negative effects were somewhat more severe in the weed species than with soybean. An increase in aerial temperature from 26/22 C to 34/30 C (day/night) had a positive influence on dry matter accumulation of the weed species, stimulating sicklepod 150 to 200% and Palmer amaranth 150 to 1,600% compared to their respective controls, whereas soybean remained at about 80 to 90% of the control. All species grew taller with increasing temperature. Leaf area of the weeds increased but leaf area of soybean did not increase. Consistent with our original hypothesis, the results clearly show that the weeds, which originate from warm geographical regions, respond more negatively than soybean to low temperatures in the growth environment but more positively to high temperatures. The temperature characteristics help to explain why the intensity of weed pressure increases as the soybean growing season progresses, even after canopy closure.

The development of dicamba-tolerant and other auxin-tolerant crops will enable the use of these effective herbicides in soybean and cotton at application timings such as at planting or over-the-top that are not currently possible. This research examined the effect of various factors on detection of postapplication amounts of dicamba in the air under field conditions by coupling a sample collection system with advanced chemical analysis of those samples. The quantity of dimethylamine salt of dicamba that was detected within 48 hr after application was two times greater (P < 0.05) than the quantity of diglycoamine salt formulation based on field studies in 2009. Regardless of application timing, the amount of detected dicamba was greatest during the 0 to 12 hr time period after application. However, the total detected after 48 hr was less for evening applications (5 micrograms [µg]) compared with midday (17 µg) or morning (14 µg) applications based on 2010 field trials. Average ambient air temperature (and other weather variables) correlated with higher detection levels of dicamba in the air in the field.

There is increasing evidence that hyperenergetic diets have an impact on memory in rodents. However, it is largely unknown how diets, such as a cafeteria diet (CD), that mimic a Western-type diet act on learning and memory, in particular when fed during early stages of development. Here, we fed lactating dams a CD and exposed both male and female offspring to a novel object discrimination (NOD) task, a two-trial test of recognition memory in which rats exposed to two identical objects during a training/familiarisation trial can discriminate a novel from a familiar object during the subsequent choice trial. The choice trial was performed following inter-trial interval (ITI) delays of up to 4 h. Maternal diet did not have an impact on exploration of the objects by either sex during the familiarisation trial. Control males discriminated the novel from the familiar object, indicating intact memory with an ITI of 1 h, but not 2 or 4 h. The CD delayed this natural forgetting in male rats such that discrimination was also evident after a 2 h ITI. In contrast, control females exhibited discrimination following both 1 and 2 h ITI, but the CD impaired performance. In summary, the present study shows that maternal exposure to the CD programmes NOD in the adult. In better-performing females, dietary programming interferes with NOD, whereas NOD was improved in males after lactational CD feeding.

Trap response of cutworm moths (Abagrotis orbis) to a sex-attractant lure in grape vineyards

Larvae of Abagrotis orbis (Grote) (Lepidoptera: Noctuidae) are climbing cutworms and can damage grapevines, Vitis vinifera L. (Vitaceae), in early spring by consuming expanding buds. A sex attractant would be useful for monitoring this insect in commercial vineyards. (Z)-7-Tetradecenyl acetate and (Z)-11-hexadecenyl acetate were found in extracts of female abdominal tips. In multiple field experiments, male A. orbis were captured in traps baited with a combination of these two chemicals but not in traps baited with either chemical alone. Males were trapped from mid-September to early October in south-central Washington and south-central British Columbia. Other noctuid moths (Mamestra configurata Walker, Xestia c-nigrum (L.), and Feltia jaculifera (Guenée)) were also captured in traps baited with the A. orbis pheromone and may complicate the use of this lure to monitor A. orbis. Abagrotis discoidalis (Grote) was captured in traps baited with (Z)-7-tetradecenyl acetate but not in traps baited with the two chemicals together.

Objective.
To assess healthcare personnel (HCP) perceptions regarding implementation of sensor-based electronic systems for automated hand hygiene adherence monitoring.
Design.Using a mixed-methods approach, structured focus groups were designed to elicit quantitative and qualitative responses on familiarity, comfort level, and perceived impact of sensor-based hand hygiene adherence monitoring
Setting.A university hospital, a Veterans Affairs hospital, and a community hospital in the Midwest.
Participants.Focus groups were homogenous by HCP type, with separate groups held for leadership, midlevel management, and frontline personnel at each hospital.
Results.Overall, 89 HCP participated in 10 focus groups. Levels of familiarity and comfort with electronic oversight technology varied by HCP type; when compared with frontline HCP, those in leadership positions were significantly more familiar with (P<.01) and more comfortable with (P<.01) the technology. The most common concerns cited by participants across groups included lack of accuracy in the data produced, such as the inability of the technology to assess the situational context of hand hygiene opportunities, and the potential punitive use of data produced. Across groups, HCP had decreased tolerance for electronic collection of spatial-temporal data, describing such oversight as Big Brother.
Conclusions.While substantial concerns were expressed by all types of HCP, participants' recommendations for effective implementation of electronic oversight technologies for hand hygiene monitoring included addressing accuracy issues before implementation and transparent communication with frontline HCP about the intended use of the data.

The seventh annual Teaching and Learning Conference (TLC) was held in Philadelphia, Pennsylvania, from February 5 to 7, 2010, with 224 attendees onsite. The theme for the meeting was “Advancing Excellence in Teaching Political Science.” Using the working-group model, the TLC track format encourages in-depth discussion and debate on research dealing with the scholarship of teaching and learning.

An exploration of the utility of hypnosis in pain management among rural pain patients

Objective: Hypnosis is an adjunctive, noninvasive treatment with few side effects that can be useful in the management of chronic pain. However, it has fallen into disfavor in recent years and is often perceived by physicians as simple charlatanism. We evaluated the efficacy of this treatment as used clinically in a large, mostly rural, pain management center.
Methods: We conducted a chart review of 300 pain patients from the Pain Treatment Center of the Bluegrass who had undergone hypnosis for their pain concerns. A chart audit tool was developed consisting of basic demographics, pre- and posthypnosis pain ratings, a rating of relaxation achieved posthypnosis, and scores on the Beck Depression Inventory, Perceived Disability Scale, and the Pain Anxiety Symptom Scale.
Results: The sample consisted of 79 men (26.3%) and 221 women (73.7%) with a mean age of 46.3 years (SD = 9.9, range = 19–78). Pain levels recorded pre- and posthypnosis revealed significant improvement as a result of the intervention (mean difference = 2.5, t(1,298) = 25.9, p < .001). Patients reported an average of 49.8% improvement in relaxation level posthypnosis (SD = 24.2%) and had a mean score of 19.0 on the Beck Depression Inventory (SD = 9.9), indicating moderate levels of depression. Also, patients saw themselves as severely disabled regarding their ability to engage in physical (8.3/10) or job-related (7.7/10) activities. Attempts to identify predictors of hypnosis success were not fruitful with one exception. “Poor” responders to hypnosis reported greater levels of perceived dysfunction in their sexual functioning compared to the “good” responders, F(1,187) = 7.2, p < .01.
Significance of results: Hypnosis appears to be a viable adjunct for pain management patients, including those from rural and relatively disadvantaged backgrounds. Prospective trials are needed to examine the utility of this modality in end-of-life and palliative care patients.
