Multiple burdens of malnutrition and relative remoteness in rural Ecuadorian communities

Objective:
Social and economic changes associated with new roads can bring about rapid nutritional transitions. To study this process, we: (1) describe trends in adult overweight and obesity (OW/OB) among rural Afro-Ecuadorians over time and across a gradient of community remoteness from the nearest commercial centre; (2) examine the relationship between male and female adult OW/OB and factors associated with market integration such as changing livelihoods and (3) examine the co-occurrence of adult OW/OB and under-five stunting and anaemia.
Design:Adult anthropometry was collected through serial case–control studies repeated over a decade across twenty-eight communities. At the same time, anthropometry and Hb were measured for all children under 5 years of age in every community.
Setting:Northern coastal Ecuador.
Participants:Adults (n 1665) and children under 5 years of age (n 2618).
Results:From 2003 and 2013, OW/OB increased from 25·1 % to 44·8 % among men and 59·9 % to 70·2 % among women. The inverse relationship between remoteness and OW/OB in men was attenuated when adjusting for urban employment, suggesting that livelihoods mediated the remoteness–OW/OB relationship. No such relationship was observed among women. Communities with a higher prevalence of male OW/OB also had a greater prevalence of stunting, but not anaemia, in children under 5 years of age.
Conclusions:The association between male OW/OB and child stunting at the community level, but not the household level, suggests that changing food environments, rather than household- or individual-level factors, drove these trends. A closer examination of changing socio-economic structures and food environments in communities undergoing rapid development could help mitigate future public health burdens.

Aims.
A substantial proportion of persons with mental disorders seek treatment from complementary and alternative medicine (CAM) professionals. However, data on how CAM contacts vary across countries, mental disorders and their severity, and health care settings is largely lacking. The aim was therefore to investigate the prevalence of contacts with CAM providers in a large cross-national sample of persons with 12-month mental disorders.
Methods.In the World Mental Health Surveys, the Composite International Diagnostic Interview was administered to determine the presence of past 12 month mental disorders in 138 801 participants aged 18–100 derived from representative general population samples. Participants were recruited between 2001 and 2012. Rates of self-reported CAM contacts for each of the 28 surveys across 25 countries and 12 mental disorder groups were calculated for all persons with past 12-month mental disorders. Mental disorders were grouped into mood disorders, anxiety disorders or behavioural disorders, and further divided by severity levels. Satisfaction with conventional care was also compared with CAM contact satisfaction.
Results.An estimated 3.6% (standard error 0.2%) of persons with a past 12-month mental disorder reported a CAM contact, which was two times higher in high-income countries (4.6%; standard error 0.3%) than in low- and middle-income countries (2.3%; standard error 0.2%). CAM contacts were largely comparable for different disorder types, but particularly high in persons receiving conventional care (8.6–17.8%). CAM contacts increased with increasing mental disorder severity. Among persons receiving specialist mental health care, CAM contacts were reported by 14.0% for severe mood disorders, 16.2% for severe anxiety disorders and 22.5% for severe behavioural disorders. Satisfaction with care was comparable with respect to CAM contacts (78.3%) and conventional care (75.6%) in persons that received both.
Conclusions.CAM contacts are common in persons with severe mental disorders, in high-income countries, and in persons receiving conventional care. Our findings support the notion of CAM as largely complementary but are in contrast to suggestions that this concerns person with only mild, transient complaints. There was no indication that persons were less satisfied by CAM visits than by receiving conventional care. We encourage health care professionals in conventional settings to openly discuss the care patients are receiving, whether conventional or not, and their reasons for doing so.

Background
The treatment gap between the number of people with mental disorders and the number treated represents a major public health challenge. We examine this gap by socio-economic status (SES; indicated by family income and respondent education) and service sector in a cross-national analysis of community epidemiological survey data.
MethodsData come from 16 753 respondents with 12-month DSM-IV disorders from community surveys in 25 countries in the WHO World Mental Health Survey Initiative. DSM-IV anxiety, mood, or substance disorders and treatment of these disorders were assessed with the WHO Composite International Diagnostic Interview (CIDI).
ResultsOnly 13.7% of 12-month DSM-IV/CIDI cases in lower-middle-income countries, 22.0% in upper-middle-income countries, and 36.8% in high-income countries received treatment. Highest-SES respondents were somewhat more likely to receive treatment, but this was true mostly for specialty mental health treatment, where the association was positive with education (highest treatment among respondents with the highest education and a weak association of education with treatment among other respondents) but non-monotonic with income (somewhat lower treatment rates among middle-income respondents and equivalent among those with high and low incomes).
ConclusionsThe modest, but nonetheless stronger, an association of education than income with treatment raises questions about a financial barriers interpretation of the inverse association of SES with treatment, although future within-country analyses that consider contextual factors might document other important specifications. While beyond the scope of this report, such an expanded analysis could have important implications for designing interventions aimed at increasing mental disorder treatment among socio-economically disadvantaged people.
Effect of 2,4-DB on White Clover Seed Production

Seed fields of white clover (Trifolium repens L. ‘New Zealand’ and ‘Louisiana S-1′) were treated with the dimethylamine salt and the isooctyl ester of 4-(2,4-dichlorophenoxy)-butyric acid (2,4-DB). Rates of application were 0.56, 1.12, and 1.68 kg/ha. Applications were made at the vegetative, bud, and flowering stages of growth. All treatments applied at the vegetative and bud stages of growth significantly reduced white clover seed production. Reductions in clover seed production usually became more severe as the rate of 2,4-DB increased.
Clean Grass Seed Crops Established with Activated Carbon Bands and Herbicides

Activated carbon, applied over the seeded row in a band 2.5 cm wide, protected grasses from herbicides applied preemergence. The carbon rate required to protect the grasses varied with the grass species, the herbicide, and the rate of herbicide application. Most of the herbicides controlled weeds effectively between the rows. The herbicides usually controlled weeds less effectively within the carbon bands. Diuron [3-(3,4-dichlorophenyl)-1,1-dimethylurea] was inactivated most readily by the carbon and showed the greatest safety margin on the crop species. Atrazine [2-chloro-4-(ethylamino)-6-(isopropylamino)-s-triazine], simazine [2-chloro-4,6-bis(ethylamino)-s-triazine], and terbacil (3-tert-butyl-5-chloro-6-methyluracil) required higher carbon rates for crop protection than diuron and showed a much narrower safety margin on crops. Use of this technique made possible the effective control of weeds during establishment and, subsequently, the production of high quality seed in the first seed crop.
Effect of Picloram on Production and Quality of Seed in Several Grasses

Twelve field trials were conducted in western Oregon during 1963 to 1966 to evaluate the effect of 4-amino-3,5,6-trichloropicolinic acid (picloram) on the seed production and seed quality of eight grass species. Seed production of creeping bentgrass (Agrostics palustris Huds., vars. Penncross and Seaside), Colonial bentgrass (Agrostis tenuis Sibth., var. Highland), perennial ryegrass (Lolium perenne L., var. Linn), Italian ryegrass (Lolium multiflorum Lam., var. Gulf), and tall fescue (Festuca arundinacea Schreb., var. Alta) treated with picloram was not statistically different from the untreated check. Seed production of Colonial bentgrass (Agrostis tenuis Sibth., var. Astoria) was reduced by picloram at rates of 1.0 and 1.5 lb/A. Seed production of Kentucky bluegrass (Poa pratensis L., var. Newport) was not statistically different from the untreated check when picloram was applied in October but was significantly lower than the check following some March treatments. Seed production of orchardgrass (Dactylis glomerata L., var. Potomac) and creeping red fescue (Festuca rubra L., var. Pennlawn) was reduced by most picloram treatments made either in October or March. Germination of the creeping red fescue seed was significantly reduced when picloram was applied at 2 lb/A in March. None of the treatments affected seedling development.

Background
Traumatic events are common globally; however, comprehensive population-based cross-national data on the epidemiology of posttraumatic stress disorder (PTSD), the paradigmatic trauma-related mental disorder, are lacking.
MethodsData were analyzed from 26 population surveys in the World Health Organization World Mental Health Surveys. A total of 71 083 respondents ages 18+ participated. The Composite International Diagnostic Interview assessed exposure to traumatic events as well as 30-day, 12-month, and lifetime PTSD. Respondents were also assessed for treatment in the 12 months preceding the survey. Age of onset distributions were examined by country income level. Associations of PTSD were examined with country income, world region, and respondent demographics.
ResultsThe cross-national lifetime prevalence of PTSD was 3.9% in the total sample and 5.6% among the trauma exposed. Half of respondents with PTSD reported persistent symptoms. Treatment seeking in high-income countries (53.5%) was roughly double that in low-lower middle income (22.8%) and upper-middle income (28.7%) countries. Social disadvantage, including younger age, female sex, being unmarried, being less educated, having lower household income, and being unemployed, was associated with increased risk of lifetime PTSD among the trauma exposed.
ConclusionsPTSD is prevalent cross-nationally, with half of all global cases being persistent. Only half of those with severe PTSD report receiving any treatment and only a minority receive specialty mental health care. Striking disparities in PTSD treatment exist by country income level. Increasing access to effective treatment, especially in low- and middle-income countries, remains critical for reducing the population burden of PTSD.

A trend toward greater body size in dizygotic (DZ) than in monozygotic (MZ) twins has been suggested by some but not all studies, and this difference may also vary by age. We analyzed zygosity differences in mean values and variances of height and body mass index (BMI) among male and female twins from infancy to old age. Data were derived from an international database of 54 twin cohorts participating in the COllaborative project of Development of Anthropometrical measures in Twins (CODATwins), and included 842,951 height and BMI measurements from twins aged 1 to 102 years. The results showed that DZ twins were consistently taller than MZ twins, with differences of up to 2.0 cm in childhood and adolescence and up to 0.9 cm in adulthood. Similarly, a greater mean BMI of up to 0.3 kg/m2 in childhood and adolescence and up to 0.2 kg/m2 in adulthood was observed in DZ twins, although the pattern was less consistent. DZ twins presented up to 1.7% greater height and 1.9% greater BMI than MZ twins; these percentage differences were largest in middle and late childhood and decreased with age in both sexes. The variance of height was similar in MZ and DZ twins at most ages. In contrast, the variance of BMI was significantly higher in DZ than in MZ twins, particularly in childhood. In conclusion, DZ twins were generally taller and had greater BMI than MZ twins, but the differences decreased with age in both sexes.

For over 100 years, the genetics of human anthropometric traits has attracted scientific interest. In particular, height and body mass index (BMI, calculated as kg/m2) have been under intensive genetic research. However, it is still largely unknown whether and how heritability estimates vary between human populations. Opportunities to address this question have increased recently because of the establishment of many new twin cohorts and the increasing accumulation of data in established twin cohorts. We started a new research project to analyze systematically (1) the variation of heritability estimates of height, BMI and their trajectories over the life course between birth cohorts, ethnicities and countries, and (2) to study the effects of birth-related factors, education and smoking on these anthropometric traits and whether these effects vary between twin cohorts. We identified 67 twin projects, including both monozygotic (MZ) and dizygotic (DZ) twins, using various sources. We asked for individual level data on height and weight including repeated measurements, birth related traits, background variables, education and smoking. By the end of 2014, 48 projects participated. Together, we have 893,458 height and weight measures (52% females) from 434,723 twin individuals, including 201,192 complete twin pairs (40% monozygotic, 40% same-sex dizygotic and 20% opposite-sex dizygotic) representing 22 countries. This project demonstrates that large-scale international twin studies are feasible and can promote the use of existing data for novel research purposes.

Background
Studies of the impact of mental disorders on educational attainment are rare in both high-income and low- and middle-income (LAMI) countries.
AimsTo examine the association between early-onset mental disorder and subsequent termination of education.
MethodSixteen countries taking part in the World Health Organization World Mental Health Survey Initiative were surveyed with the Composite International Diagnostic Interview (n=41 688). Survival models were used to estimate associations between DSM–IV mental disorders and subsequent non-attainment of educational milestones.
ResultsIn high-income countries, prior substance use disorders were associated with non-completion at all stages of education (OR 1.4–15.2). Anxiety disorders (OR=1.3), mood disorders (OR=1.4) and impulse control disorders (OR=2.2) were associated with early termination of secondary education. In LAMI countries, impulse control disorders (OR=1.3) and substance use disorders (OR=1.5) were associated with early termination of secondary education.
ConclusionsOnset of mental disorder and subsequent non-completion of education are consistently associated in both high-income and LAMI countries.

Cross-sectional studies report an inverse association between BMI and serum carotenoid concentration. The present study examined the prospective association between BMI and the serum concentration of five carotenoids in the Coronary Artery Risk Development in Young Adults (CARDIA) study. Serum carotenoids (α-carotene, β-carotene, β-cryptoxanthin, zeaxanthin/lutein, lycopene), BMI, dietary intake, physical activity and dietary supplement use were measured at years 0 and 7 in 3071 black and white male and female participants, who were either persistent smokers or non-smokers. Among non-smokers, year 0 BMI predicted year 7 serum carotenoid levels: obese subjects (BMI ≥30kg/m2) had an average concentration of the sum of four carotenoids (α-carotene +β-carotene + zeaxanthin/lutein +β-cryptoxanthin)that was 22% lower than the concentration among subjects with a BMI of less than 22kg/m2. In contrast, the sum of carotenoids among smokers was only 6% lower. Relationships between BMI and serum lycopene were weak. The change from year 0 to year 7 in serum carotenoids, except for lycopene, was inversely associated with the change in BMI among non-smokers but not among smokers. Parallel findings were observed for BMI and serum γ-glutamyl transferase level. In summary, the observation that BMI predicted the evolution of serum carotenoids during a 7-year follow-up among young non-smoking adults is consistent with the hypothesis that carotenoids are decreased in protecting against oxidative stress generated by adipose tissue, while smokers maintain a minimal level of serum carotenoids independent of adiposity. The results for lycopene were, however, discordant from those of the other carotenoids.

A person with a moderate or severe motor disability will often use specialised or adapted tools to assist their interaction with a general environment. Such tools can assist with the movement of a person's arms so as to facilitate manipulation, can provide postural supports, or interface to computers, wheelchairs or similar assistive technologies. Designing such devices with programmable stiffness and damping may offer a better means for the person to have effective control of their surroundings.
This paper addresses the possibility of designing some assistive technologies using impedance elements that can adapt to the user and the circumstances. Two impedance elements are proposed. The first, based on magnetic particle brakes, allows control of the damping coefficient in a passive element. The second, based on detuning the P-D controller in a servo-motor mechanism, allows control of both stiffness and damping. Such a mechanical impedance can be modulated to the conditions imposed by the task in hand. The limits of linear theory are explored and possible uses of programmable impedance elements are proposed.

Four herbicide treatments, 1,1′-dimethyl-4,4′-bipyridinium salt (paraquat), 6,7-dihydrodipyrido(1,2-a:2’,1′-c)pyrazidiinium salt (diquat), isopropyl N-phenylcarbamate (IPC) + butoxyethanol ester of 2,4-dichlorophenoxyacetic acid (2,4-D), and 3-amino-l,2,4-triazole + ammonium thiocyanate (amitrole-T) were successfully applied before Janaury 15 for control of weeds and volunteer crop plants. Grasses for seed production were sown in March without disturbing the soil. An application of paraquat at seeding greatly increased subsequent seed yields. Little additional germination of contaminating weed grasses occurred after the grasses were planted and they became established rapidly. By the following October most species could be treated with 3-(3,4-dichlorophenyl)-1,1-dimethylurea (diuron) to control winter annual weeds. Where weeds were thus effectively controlled during the establishment period, excellent stands resulted which produced high yields of weed-free seed in the first year of seed production.
Selective Control of Downy Brome and Rattail Fescue in Irrigated Perennial Grass Seed Fields of Central Oregon

Downy brome (Bromus tectorum L.), and rattail fescue (Festuca myuros L.) were controlled by 2-methoxy-3,6-dichlorobenzoic acid (dicamba) and 2,4-bis(isoproplyamino)-6-methylmercapto-s-triazine (prometryne). Both herbicides gave 70–100% control when applied soon after the first postharvest irrigation. Dicamba was somewhat less effective when applied before this irrigation. Neither herbicide reduced seed yields when applied before or immediately after the first postharvest irrigation, but both herbicides caused some significant seed yield reductions when treatments were delayed until after considerable crop regrowth had occurred.
