Kawasaki disease: patients’ transition journey and recommendations for adult care

Background:
Children who develop coronary artery aneurysms after Kawasaki disease are at risk for cardiovascular morbidity, requiring health care transition and lifelong follow-up with an adult specialist. Follow-up losses after health care transition have been reported but without outcome and patient experience evaluation.
Objective:The Theoretical Domains Framework underpinned our aim to explore the required self-care behaviours and experiences of young adults’ post-health care transition.
Methods:A qualitative description approach was used for virtual, 1:1 interviews with 11 participants, recruited after health care transition from a regional cardiac centre in Ontario. Directed content analysis was employed.
Results:Health, psychosocial, and lifestyle challenges were compounded by a sense of loss. Six themes emerged within the Theoretical Domains Framework categories. Participants offered novel health care transition programme recommendations.
Conclusions:The realities of health care transition involve multiple, overlapping stressors for young adults with Kawasaki disease and coronary artery aneurysms. Our findings will inform a renewed health care transition programme and will include outcome evaluation.

The first demonstration of laser action in ruby was made in 1960 by T. H. Maiman of Hughes Research Laboratories, USA. Many laboratories worldwide began the search for lasers using different materials, operating at different wavelengths. In the UK, academia, industry and the central laboratories took up the challenge from the earliest days to develop these systems for a broad range of applications. This historical review looks at the contribution the UK has made to the advancement of the technology, the development of systems and components and their exploitation over the last 60 years.

Background:
Studies using acute tryptophan depletion (ATD) to examine the effects of a rapid reduction in serotonin function have shown a reduction in global cognitive status during ATD in Alzheimer's disease (AD) and Parkinson's disease (PD). Based on the severe cholinergic loss evident in dementia with Lewy bodies (DLB) and Parkinson's disease and dementia (PDD), we predicted that a reduction of global cognitive status during ATD would be greater in these conditions than in AD.
Methods:Patients having DLB or PDD underwent ATD in a double-blind, placebo-controlled, randomized, counterbalanced, crossover design.
Results:While the study intended to test 20 patients, the protocol was poorly tolerated and terminated after six patients attempted, but only four patients – three with DLB and one with PDD – completed the protocol. The Modified Mini-Mental State Examination (3MSE) score was reduced in all three DLB patients and unchanged in the PDD and dementia patient during ATD compared with placebo.
Conclusions:This reduction in global cognitive function and the poor tolerability may fit with the hypothesis that people with dementia with Lewy bodies have sensitivity to the effects of reduced serotonin function.

As part of the Z-PAndAS Keck II DEIMOS survey of resolved stars in our neighboring galaxy, Andromeda (M31), we have built up a unique data set of measured velocities and chemistries for thousands of stars in the Andromeda stellar halo, particularly probing its rich and complex substructure. In this contribution, we will discuss the structural, dynamical and chemical properties of Andromeda's dwarf spheroidal galaxies, and how there is no observational evidence for a difference in the evolutionary histories of those found on and off M31's vast plane of satellites. We will also discuss a possible extension to the most significant merger event in M31 - the Giant Southern Stream - and how we can use this feature to refine our understanding of M31's mass profile, and its complex evolution.

Andromeda XXVII is a dwarf spheroidal galaxy in the outskirts of the Andromeda galaxy (M31). It appears to be dissolving in to the Northern arc of M31, and could be the remnant of a strong tidal disruption. In the upcoming months, our spectroscopic program, which has measured velocities for multiple stars within both the dwarf galaxy progenitor and its stream (using the Keck II DEIMOS telescope, as part of the PAndAS survey), will determine velocity dispersions, scale radii and metallicities of both the dwarf and the stream. This in turn may enable us to ascertain the progenitor mass profile and determine whether it is cusped or cored.

Objective
To describe dietary changes in men participating in an obesity intervention as part of the Self-Help, Exercise and Diet using Information Technology (SHED-IT) study.
DesignAn assessor-blinded randomized controlled trial comparing Internet (n 34) v. information-only groups (n 31) with 6-month follow-up. Dietary intake assessed by FFQ, reporting usual consumption of seventy-four foods and six alcoholic beverages using a 10-point frequency scale. A single portion size (PSF) factor was calculated based on photographs to indicate usual serving sizes.
SettingThe campus community of the University of Newcastle, New South Wales, Australia.
SubjectsSixty-five overweight/obese men (43 % students, 42 % non-academic general staff, 15 % academic staff; mean age 35·9 (sd 11·1) years, mean BMI 30·6 (sd 2·8) kg/m2).
ResultsThe average PSF decreased significantly over time (χ2 = 20·9, df = 5, P < 0·001) with no differences between groups. While both groups reduced mean daily energy intake (GLM χ2 = 34·5, df = 3, P < 0·001), there was a trend towards a greater reduction in the Internet group (GLM χ2 = 3·3, P = 0·07). Both groups reduced percentage of energy from fat (P < 0·05), saturated fat (P < 0·001) and energy-dense/nutrient-poor items (P < 0·05), with no change in dietary fibre or alcohol (P > 0·05).
ConclusionsAlthough men reported some positive dietary changes during weight loss, they did not increase vegetable intakes nor decrease alcohol consumption, while saturated fat, fibre and Na intakes still exceeded national targets. Future interventions for men should promote specific food-based guidelines to target improvements in their diet-related risk factor profile for chronic diseases.

Objective
To investigate the impact of school garden-enhanced nutrition education (NE) on children’s fruit and vegetable consumption, vegetable preferences, fruit and vegetable knowledge and quality of school life.
DesignQuasi-experimental 10-week intervention with nutrition education and garden (NE&G), NE only and control groups. Fruit and vegetable knowledge, vegetable preferences (willingness to taste and taste ratings), fruit and vegetable consumption (24 h recall × 2) and quality of school life (QoSL) were measured at baseline and 4-month follow-up.
SettingTwo primary schools in the Hunter Region, New South Wales, Australia.
SubjectsA total of 127 students in Grades 5 and 6 (11–12 years old; 54 % boys).
ResultsRelative to controls, significant between-group differences were found for NE&G and NE students for overall willingness to taste vegetables (P < 0·001) and overall taste ratings of vegetables (P < 0·001). A treatment effect was found for the NE&G group for: ability to identify vegetables (P < 0·001); willingness to taste capsicum (P = 0·04), broccoli (P = 0·01), tomato (P < 0·001) and pea (P = 0·02); and student preference to eat broccoli (P < 0·001) and pea (P < 0·001) as a snack. No group-by-time differences were found for vegetable intake (P = 0·22), fruit intake (P = 0·23) or QoSL (P = 0·98).
ConclusionsSchool gardens can impact positively on primary-school students’ willingness to taste vegetables and their vegetable taste ratings, but given the complexity of dietary behaviour change, more comprehensive strategies are required to increase vegetable intake.

The Proceedings of the National Summit on Legal Preparedness for Obesity Prevention and Control is based on a two-part conceptual framework composed of public health and legal perspectives. The public health perspective comprises the six target areas and intervention settings that are the focus of the obesity prevention and control efforts of the Centers for Disease Control and Prevention (CDC).
This paper presents the legal perspective. Legal preparedness in public health is the underpinning of the framework for the four “assessment” papers and the four “action” papers that are integral to the application of public health law to any particular health issue. In addition, this paper gives real-world grounding to the legal framework through examples that illustrate the four core elements of legal preparedness in public health that are at work in obesity prevention and control.

Ancient Egyptians are thought to have been the only people in the Old World who were practising mummification in the Bronze Age (c. 2200-700 BC). But now a remarkable series of finds from a remote Scottish island indicates that Ancient Britons were performing similar, if less elaborate, practices of bodily preservation. Evidence of mummification is usually limited to a narrow range of arid or frozen environments which are conducive to soft tissue preservation. Mike Parker Pearson and his team show that a combination of microstructural, contextual and AMS 14C analysis of bone allows the identification of mummification in more temperate and wetter climates where soft tissues and fabrics do not normally survive. Skeletons from Cladh Hallan on South Uist, Western Isles, Scotland were buried several hundred years after death, and the skeletons provide evidence of post mortem manipulation of body parts. Perhaps these practices were widespread in mainland Britain during the Bronze Age.
