Introduction
The Inventory of Academic Sources of Stress in Medical Education (IASSME) evaluates the presence and intensity of the main sources of academic stress for Portuguese Medicine students in five dimensions: Course demands/CD, Human demands/HD, Lifestyle/LS, Academic competition/AC, and Academic adjustment/AA.
ObjectivesTo further validate the ISSME using Confirmatory Factor Analysis and to analyze[ATP1] the psychometric properties of a new version including additional sources of stress.
MethodsParticipants were 666 Portuguese medicine (82.6%) and dentistry (17.4%) students (81.8% girls); they answered an online survey including the ISSME and other validated questionnaires: Maslach Burnout Inventory – Students Survey (MBI-SS) and Depression Anxiety and Stress Scales (DASS).
ResultsConfirmatory Factor Analysis showed that the second order model composed of five factors (the original structure by Loureiro et al. 2008), but excluding item 11 (loading=.371), presented good fit indexes (χ2/df=3.274; RMSEA=.0581, p<.001; CFI=.917; TLI=.904, GFI=.919). The Cronbach’s alfas were α=.897 for the total and from α=.669 (F2-HD) to α=.859 (F1-CD) for the dimensions. The expanded version, including two additional items related to lack of interest in medicine/dentistry (F6, α=.543) and two additional COVID-19 stress-related-items (F7, α=.744) also showed acceptable fit indexes (χ2/df=3.513; RMSEA=.061, p<.001; CFI=.88.; TLI=.866, GFI=.892). This new version’s α was of .896. Pearson correlations between ISSME and the other measures were significant (p<.01) and high: >.55 with DASS and >.50 with MBI-SS. Girls presented significantly higher ISSME scores. F6 score was significantly higher in dentistry students.
ConclusionsThis further validation study underlines that IASSME presents good validity (construct and convergent) and reliability.

The increasing pressure of anthropogenic development in areas with high natural value poses a huge challenge for wildlife conservation worldwide. The Tagus estuary in southern Portugal is among the most important wetlands for migratory shorebirds in the East Atlantic Flyway (EAF). However, in 2020 the Portuguese government approved the construction of the new Lisbon international airport at the heart of the Tagus estuary. Intense aircraft traffic, flying at low altitudes during both approach and take-off, is expected to cross the estuary, overlapping to a great extent the important intertidal foraging areas. Here, we aim to quantify the potential loss of conservation value of the intertidal areas of the Tagus estuary for shorebirds resulting from the disturbance (noise) caused by overflying aircraft. Using a comprehensive dataset of wintering shorebird abundance and distribution in the whole intertidal estuarine area we first identified priority areas for conservation using a spatial prioritization approach. We then performed a replacement cost analysis by excluding the areas likely to become unsuitable or severely underused by birds due to intense air traffic noise. Our results suggest that the implementation of the new Lisbon airport may lead to a loss of up to 30% of the conservation value of the Tagus estuary in terms of intertidal feeding areas of wintering birds alone. The global impact will likely be greater when effects on supratidal roosts, as well as on passage birds, are also considered. The Tagus estuary, which is internationally important for six of the 10 species included in our analysis, is just one of a network of already depleted sites along the EAF. Thus, negative impacts on bird populations on the Tagus estuary will have repercussions and undermine conservation efforts elsewhere. The plight of shorebirds at the Tagus estuary is thus a matter of international conservation concern.
Association between early weight gain and later adiposity in Sri Lankan adolescents

Early growth pattern is increasingly recognized as a determinant of later obesity. This study aimed to identify the association between weight gain in early life and anthropometry, adiposity, leptin, and fasting insulin levels in adolescence. A cross-sectional study was conducted in 366 school children aged 11–13 years. Weight, height, and waist circumference (WC) were measured. Fat mass (FM) was assessed using bioelectrical impedance analysis. Blood was drawn after a 12-h fast for insulin and leptin assay. Birth weight and weight at 6 months and at 18 months were extracted from Child Health Development Records. An increase in weight SD score (SDS) by ≥0.67 was defined as accelerated weight gain. Linear mixed-effects modeling was used to predict anthropometry, adiposity, and metabolic outcomes using sex, pubertal status, accelerated weight gain as fixed factors; age, birth weight, and family income as fixed covariates, and school as a random factor. Children with accelerated weight gain between birth and 18 months had significantly higher body mass index (BMI) SDS, WC SDS, height SDS, %FM, fat mass index (FMI), fat free mass index (FFMI), and serum leptin levels in adolescence. Accelerated weight gain between 6 and 18 months was associated with higher BMI SDS, WC SDS, %FM, and FMI, but not with height SDS or FFMI. Accelerated weight gain at 0–6 months, in children with low birth weight, was associated with higher height SDS, BMI SDS, WC SDS, %FM, and FMI; in children with normal birth weight, it was associated with BMI SDS, WC SDS, height SDS, and FFMI, but not with %FM or FMI. Effects of accelerated weight gain in early life on anthropometry and adiposity in adolescence varied in different growth windows. Accelerated weight gain during 6–18 months was associated with higher FM rather than linear growth. Effects of accelerated weight gain between 0 and 6 months varied with birth weight.

Introduction
The HCV has been associated with significant impairment of quality of life, irrespective of the degree of liver injury. The association of this disease with psychiatric comorbidities, especially depression, has been well described in the scientific literature. However, despite also significantly affect the quality of life of individuals, the Posttraumatic Stress Disorder (PTSD) has not been well studied in this population.
Objective/aimEvaluate whether individuals perceive the liver disease as a potentially traumatic experience and investigate the impact of PTSD diagnosis on health-related quality of life in hepatitis C virus-infected subjects.
MethodsWe conducted a cross-sectional survey of 127 consecutive hepatitis C-infected outpatients. We investigated Traumatic Experiences and the subject's perception about the disease using the Trauma History Questionnaire. PTSD and others psychiatry diagnoses were assessed trough M.I.N.I. PLUS. Quality of life was assessed by the Short-Form 36.
ResultsApproximately 38.6% of the patients perceived the hepatitis C as a traumatic experience, 60.7% had PTSD diagnosis. It were no associate with the disease severity (been eligible for transplant), however there was a significant association with diagnosis of PTSD (p = 0.003). Roughly 22% of the sample showed PTSD diagnosis. PTSD imposed a significant impairment in quality of life of individuals in seven of the eight domains in the bivariate and multivariate analysis. This difference remained significant after adjustment for covariates such as major depressive disorder comorbidity.
ConclusionThe results suggest high prevalence of PTSD diagnosis in VHC infected patients and it impose impairment in their quality of life.

In Europe, in the late 19th century and the first half of the 20th century, new nosologic categories were formulated that could not be framed within Manic-Depressive Psycosis or Kraeplins' Demencia Praecox: Bouffée delirante by the French, Cycloid Psychosis by the German, Reactive and Schizophreniform Psychosis by the Scandinavians. These are concepts that are currently classified in the ICD-10 under Acute and Transitory Psychotic Disorders.
Cycloid Psychosis are characterized by an abrupt onset and polimorfic symptomatology namely delusions, hallucinations in every sensory modality, confusion, akinesia or hyperkinesia, feelings of ecstasy, which can vary with time. They have a recurring course, rapid remission followed by the reinstatement of the premorbid personality.
We present a case study of a female, 26 year old patient. The first psychotic episode occurred 2 years ago and was followed by complete recovery. Recently, there was a recrudescence of the symptomatology. She presented with a polimorfic symptoms, alternating between mutism and psychomotor agitation, disorientation, incoherent speech and almost total insomnia. There was an absence of response to antipsychotic medication. Therefore, the patient was submitted to electroconvulsive therapy with an overall improvement of symptoms, maintaining some puerility. The aim of this case study was to draw attention to the diagnosis of Cycloid Psychosis, and it confirms the efficacy of electroconvulsive therapy in its treatment.
The psychiatry recruitment crisis across Europe: Evaluation by the European Federation of psychiatric trainees

Introduction
Recruitment of medical students and junior doctors in to psychiatry is a long-standing concern in many countries, with low proportions of medical graduates choosing it as a specialty and ongoing stigma from within the medical profession. In some countries the reverse problem is the case, with too many doctors wishing to enter psychiatry, and insufficient training places available.
ObjectivesTo understand the current situation within Europe with regards to recruitment in to psychiatry and to identify existing recruitment initiatives.
MethodsThe European Federation of Psychiatric Trainees conducts an annual survey of all member organisations. A delegate of each national association of psychiatric trainees is asked to identify if their country has a problem with recruitment and if so, whether there were too many or too few applicants for training places. Delegates from countries with recruitment initiatives were contacted to provide further details.
ResultsIn 2014, a total of 31 countries completed the survey, with 17 stating that too few medical practitioners choose psychiatry. In total 8 countries with recruitment problems reported that initiatives exist to encourage doctors to enter psychiatric training. Of these, 7 responded to describe the initiatives, which included national recruitment strategies, financial incentives, careers fairs, mentoring schemes and a whole host of other projects.
ConclusionsRecruitment in to psychiatry remains a serious problem in a significant proportion of European countries, but a wide range of initiatives exist which aim to combat this shortfall. It will be important over the coming years to establish which initiatives are most effective at increasing recruitment.

In the framework of coupled cell systems, a coupled cell network describes graphically the dynamical dependencies between individual dynamical systems, the cells. The fundamental network of a network reveals the hidden symmetries of that network. Subspaces defined by equalities of coordinates which are flow-invariant for any coupled cell system consistent with a network structure are called the network synchrony subspaces. Moreover, for every synchrony subspace, each network admissible system restricted to that subspace is a dynamical system consistent with a smaller network called a quotient network. We characterize networks such that: the network is a subnetwork of its fundamental network, and the network is a fundamental network. Moreover, we prove that the fundamental network construction preserves the quotient relation and it transforms the subnetwork relation into the quotient relation. The size of cycles in a network and the distance of a cell to a cycle are two important properties concerning the description of the network architecture. In this paper, we relate these two architectural properties in a network and its fundamental network.
Duration of untreated illness in a cross-diagnostic sample of obsessive-compulsive disorder, panic disorder, and social anxiety disorder

Objective
In this study, we compared duration of untreated illness (DUI) in obsessive-compulsive disorder (OCD), panic disorder (PD), and social anxiety disorder (SAD) patients and investigated its correlates, both within specific diagnoses and across the whole sample.
MethodsEighty-eight patients (33 OCD, 24 SAD, and 31 PD) had their diagnosis confirmed by the Mini International Neuropsychiatric Interview, were assessed for treatment-seeking variables, and were evaluated with instruments aimed at quantifying transdiagnostic features (i.e., the Cause subscale of the Illness Perception Questionnaire–Mental Health and the Anxiety Sensitivity Index–Revised) and severity of illness (i.e., Beck Depression and Anxiety Inventories, the Dimensional Obsessive-Compulsive Scale, the Panic and Agoraphobia Scale, and the Social Phobia Inventory).
ResultsThe only differences between groups with short (<2 years) versus long (>2 years) DUI were greater fear of public display of anxiety in the first group and greater social avoidance in the second group. The DUI was significantly different between groups that sought treatment after the onset of illness, with OCD patients having longer DUI than PD patients and shorter DUI than SAD patients. Further, DUI correlated negatively with the perception of OCD being caused by stress and positively with severity of panic-related disability in SAD patients, but not in PD or OCD patients.
ConclusionThere was substantial delay in treatment seeking among the anxiety and obsessive-compulsive disorder patients, particularly those with OCD or SAD. Perception of stress as a cause of OCD prompted treatment seeking, while severity of panic symptoms delayed treatment seeking.
Important Bird and Biodiversity Areas (IBAs): the development and characteristics of a global inventory of key sites for biodiversity

Important Bird and Biodiversity Areas (IBAs) are sites identified as being globally important for the conservation of bird populations on the basis of an internationally agreed set of criteria. We present the first review of the development and spread of the IBA concept since it was launched by BirdLife International (then ICBP) in 1979 and examine some of the characteristics of the resulting inventory. Over 13,000 global and regional IBAs have so far been identified and documented in terrestrial, freshwater and marine ecosystems in almost all of the world’s countries and territories, making this the largest global network of sites of significance for biodiversity. IBAs have been identified using standardised, data-driven criteria that have been developed and applied at global and regional levels. These criteria capture multiple dimensions of a site’s significance for avian biodiversity and relate to populations of globally threatened species (68.6% of the 10,746 IBAs that meet global criteria), restricted-range species (25.4%), biome-restricted species (27.5%) and congregatory species (50.3%); many global IBAs (52.7%) trigger two or more of these criteria. IBAs range in size from < 1 km2 to over 300,000 km2 and have an approximately log-normal size distribution (median = 125.0 km2, mean = 1,202.6 km2). They cover approximately 6.7% of the terrestrial, 1.6% of the marine and 3.1% of the total surface area of the Earth. The launch in 2016 of the KBA Global Standard, which aims to identify, document and conserve sites that contribute to the global persistence of wider biodiversity, and whose criteria for site identification build on those developed for IBAs, is a logical evolution of the IBA concept. The role of IBAs in conservation planning, policy and practice is reviewed elsewhere. Future technical priorities for the IBA initiative include completion of the global inventory, particularly in the marine environment, keeping the dataset up to date, and improving the systematic monitoring of these sites.

BirdLife International´s Important Bird and Biodiversity Areas (IBA) Programme has identified, documented and mapped over 13,000 sites of international importance for birds. IBAs have been influential with governments, multilateral agreements, businesses and others in: (1) informing governments’ efforts to expand protected area networks (in particular to meet their commitments through the Convention on Biological Diversity); (2) supporting the identification of Ecologically or Biologically Significant Areas (EBSAs) in the marine realm, (3) identifying Wetlands of International Importance under the Ramsar Convention; (4) identifying sites of importance for species under the Convention on Migratory Species and its sister agreements; (5) identifying Special Protected Areas under the EU Birds Directive; (6) applying the environmental safeguards of international finance institutions such as the International Finance Corporation; (7) supporting the private sector to manage environmental risk in its operations; and (8) helping donor organisations like the Critical Ecosystems Partnership Fund (CEPF) to prioritise investment in site-based conservation. The identification of IBAs (and IBAs in Danger: the most threatened of these) has also triggered conservation and management actions at site level, most notably by civil society organisations and local conservation groups. IBA data have therefore been widely used by stakeholders at different levels to help conserve a network of sites essential to maintaining the populations and habitats of birds as well as other biodiversity. The experience of IBA identification and conservation is shaping the design and implementation of the recently launched Key Biodiversity Areas (KBA) Partnership and programme, as IBAs form a core part of the KBA network.
Meal patterns across ten European countries – results from the European Prospective Investigation into Cancer and Nutrition (EPIC) calibration study

Objective
To characterize meal patterns across ten European countries participating in the European Prospective Investigation into Cancer and Nutrition (EPIC) calibration study.
DesignCross-sectional study utilizing dietary data collected through a standardized 24 h diet recall during 1995–2000. Eleven predefined intake occasions across a 24 h period were assessed during the interview. In the present descriptive report, meal patterns were analysed in terms of daily number of intake occasions, the proportion reporting each intake occasion and the energy contributions from each intake occasion.
SettingTwenty-seven centres across ten European countries.
SubjectsWomen (64 %) and men (36 %) aged 35–74 years (n 36 020).
ResultsPronounced differences in meal patterns emerged both across centres within the same country and across different countries, with a trend for fewer intake occasions per day in Mediterranean countries compared with central and northern Europe. Differences were also found for daily energy intake provided by lunch, with 38–43 % for women and 41–45 % for men within Mediterranean countries compared with 16–27 % for women and 20–26 % for men in central and northern European countries. Likewise, a south–north gradient was found for daily energy intake from snacks, with 13–20 % (women) and 10–17 % (men) in Mediterranean countries compared with 24–34 % (women) and 23–35 % (men) in central/northern Europe.
ConclusionsWe found distinct differences in meal patterns with marked diversity for intake frequency and lunch and snack consumption between Mediterranean and central/northern European countries. Monitoring of meal patterns across various cultures and populations could provide critical context to the research efforts to characterize relationships between dietary intake and health.

In recent years great importance has been given to the adverse effects of particulate matter (PM) in health, and several epidemiological studies correlating the airborne particles with pulmonary injury have been carried out. The adverse effect of short- and long-term expositions to PM can, indeed, cause a variety of effects, from minor effects on the respiratory system to serious oncogenic effects that can lead to precocious death and are considered one of the top environmental public health concerns. Some cytogenetic techniques, as micronucleus assay, allow to detect chromosome breakage and loss by measuring the formation of micronuclei and proved to be a good tool in the evaluation of genotoxic damage induced by PM. Thus genotoxic effects can be evaluated by micronucleus test in order to identify a gradient of potential exposure and to assess environmental monitoring. However, cross investigations involving biomarkers of genotoxicity and electron microscopy PM-induced tracheal and lung damages in natural populations are not common.
Long-term declines of wader populations at the Tagus estuary, Portugal: a response to global or local factors?

Migratory wader populations face global threats, mainly related to increasing rates of habitat loss and disturbance driven by human activities. To a large extent, the long-term survival of these populations requires the conservation of networks of sites along their migratory flyways. The Tagus estuary, Portugal, is among the most important wetlands for waders in the East Atlantic Flyway. Annual winter wader counts have been carried in this wetland since 1975 and a monthly roost-monitoring programme was implemented in 2007. Wintering populations of three out of the five most abundant species, Dunlin Calidris alpina, Grey Plover Pluvialis squatarola and Redshank Tringa totanus, showed significant population declines over the past three decades, which are most likely due to the loss and degradation of roost sites as a result of increasing human activity. The situation is unlikely to improve, as a high proportion of the wintering waders use roost sites that are situated in highly urbanised areas with no legal protection. The use of different roost sites by waders is highly variable both temporally and spatially, thus emphasizing the need for a network of good quality roost sites. Additionally, during migration, 60–80% of all the waders of the Tagus estuary concentrate at a single refuge, thus increasing the risk for wader populations during these periods.

The presence of bats in caves, attics, ceilings, and roofs is important epidemiologically as they can increase the chance of human acquisition of pathogens, including Histoplasma capsulatum. Brazilian urban areas contain many species of bats, especially insectivorous bats, that are attracted by a wide range of readily available food and shelter. From August 2003 to December 2008, we analysed 2427 bats in the São Paulo State region. Homogenates of the livers and spleens of the bats were plated on specific medium to identify animals infected with H. capsulatum. The fungus was isolated from 87 bats (3·6%). The infected bats were identified as Molossus molossus (74), Nyctinomops macrotis (10), Tadarida brasiliensis (1), Molossus rufus (1) and Eumops glaucinus (1), all insectivorous species. The data presented are a relevant contribution to the epidemiology of H. capsulatum in densely populated urban areas such as in São Paulo State, especially since histoplasmosis is not included in the mandatory disease notification system.

In vitro anti-trypanosomal activity of elatol isolated from red seaweed Laurencia dendroidea

Chagas' disease is a debilitating but comparatively neglected illness that affects about 15 million people. There is an urgent need to develop new, more effective, and less-toxic compounds. In this study, we assessed the in vitro anti-trypanosomal activity of the sesquiterpene elatol from the Brazilian red seaweed Laurencia dendroidea. We used electron microscopy to evaluate the effect of elatol on the morphology and ultrastructure of the parasite. Elatol showed a dose-dependent effect against the epimastigote, trypomastigote, and amastigote forms, with IC50 values of 45·4, 1·38, and 1·01 μm, respectively. Observation of treated intracellular amastigotes by light microscopy demonstrated a total elimination of the infection at a dose of 3·0 μm. In addition, the compound did not affect the red blood cells, and the CC50 value for LLCMK2 cells was 27·0 μm. Transmission and scanning electron micrographs showed aberrant-shaped cells and breaks in the plasma membrane, prominent swollen mitochondria, and extensive formation of cytoplasmic vacuoles in all the forms. This is the first report of the anti-trypanosomal effect of the sesquiterpene elatol.
Increased susceptibility to Strongyloides venezuelensis in mice due to Mycobacterium bovis co-infection which modulates production of Th2 cytokines

An estimated quarter of the world's population possesses an infection caused by gastrointestinal nematodes, which induce a Th2 type immune response. Concomitant infection of nematodes with Mycobacterium tuberculosis, which induces a predominantly Th1 type response, is very frequent in tropical and subtropical regions. This study examined immune responses of BALB/c mice infected with Strongyloides venezuelensis and then co-infected with Mycobacterium bovis. The number of worms in the intestine, eggs in feces, cytokine production in lungs and intestine and the expression of CD80, CD86, CTLA-4 and CD28 cell markers on pulmonary cells were analysed. Our results indicate that co-infected mice had an increased parasite burden, which correlates with elevated IFN-γ and IL-10 cytokine production and decreased IL-4 and IL-13. Moreover, decreased expression of CD80 and increased expression of CTLA-4 were observed in co-infected mice. Our data point out that susceptibility to Strongyloides venezuelensis infection is increased by Mycobacterium bovis co-infection, resulting in higher parasite survival.
