A systematic review of real-world healthcare resource use and costs of Clostridioides difficile infections

Objective:
To conduct a systematic review of published real-world evidence describing the cost and healthcare resource use for Clostridiodes difficile infection (CDI) in the United States.
Methods:A systematic literature review was conducted searching for terms for CDI and healthcare costs. Titles of articles and abstracts were reviewed to identify those that met study criteria. Studies were evaluated to examine overall design and comparison groups in terms of healthcare resource use and cost for CDI.
Results:In total, 28 articles met the inclusion criteria. Moreover, 20 studies evaluated primary CDI or did not specify, and 8 studies1–8 evaluated both primary CDI and recurrent (rCDI). Data from Medicare were used in 6 studies. Nearly all studies used a comparison group, either controls without CDI (N = 20) or comparison between primary CDI and rCDI (N = 7). Two studies examined costs of rCDI by the number of recurrences. Overall, the burden of CDI is significant, with higher aggregate costs for patients with rCDI. Compared with non-CDI controls, hospital length of stay increased in patients with both primary and rCDI compared to patients without CDI. Patients with primary CDI cost healthcare systems $24,000 more than patients without CDI. Additionally, 2 studies that evaluated the impact of recurrence among those patients with an index case of CDI demonstrated significantly higher direct all-cause medical costs among those with rCDI compared to those without.
Conclusion:CDI, and particularly rCDI, is a costly condition with hospitalizations being the main cost driver.

This study assessed the health and welfare implications of feeding pigs a dry or liquid diet when housed in either fully-slatted or straw-based accommodation. Between April and October 2002, 1024 (Large White × Landrace) × Large White pigs, housed in pens of 32, were fed ad libitum from 34 kg to slaughter at 104 kg liveweight. Data were collected on a range of welfare parameters. Feeding system affected only respiratory health losses. Lameness and tail-biting tended to be more prevalent health conditions in the fully-slatted system, while in the straw-based system pigs showed significantly more enteric and respiratory disease. There were no significant treatment effects on skin lesions or bursitis of the hock. Liquid fed pigs had poorer hygiene scores than dry fed pigs, especially in straw-based housing. Liquid feeding reduced activity level and investigatory behaviours directed towards other pigs. Pigs with straw spent a large proportion of their time manipulating it. Pigs without straw were less active and spent more time manipulating the pen hardware. In post-slaughter assessments, there were no systems differences in lung lesions or osteochondrosis, but other measures differed between housing or feeding systems; pigs with straw had more severe toe erosions on the foot, while pigs without straw had more severe heel erosions. Gastric lesions were more pronounced with dry feeding and in the fully-slatted system. The results highlight the relative health and welfare advantages and disadvantages of these systems for finishing pigs.

The application of passive microwave radiometry to the remote sensing of snow properties is based on the ratio of emitted to scattered portions of the upwelling radiation. Increased scattering is indicative of increased snow amount, i.e. the number of snow grains present. However, scattering is also directly proportional to snow grain-size for a given snow amount. Current snow cover retrieval algorithms produce inaccurate results when snow grain-sizes are unusually large. Therefore, it is necessary to characterize snow grain-size on a regional scale (and perhaps local scale in extreme situations) in order to adjust passive microwave algorithms. Preliminary analysis indicates that: (1) algorithms are not as sensitive to the presence of large grain-sizes as the initial theory had indicated; (2) standard deviation of grain-size diameters throughout the total snow cover may often be less than 0.5 mm, thus average grain-size data may often serve to characterize the detailed stratigraphy of the total snow cover; (3) conditions in subfreezing snow which produce grain-sizes that greatly exceed a mean diameter value of 1–2 mm result from snow cover/climate relationships which can be modelled/monitored on a regional scale. A preliminary method is investigated for selecting snow retrieval algorithms according to prevailing regional-scale grain-size.

1. The outbreak of typhoid fever in the Ecclefechan Special Water District affected 55 per 1000 of the inhabitants (primary cases only).
2. The evidence available indicated the public water supply as the vehicle of infection.
3. A typhoid carrier was found employed in the water-collecting area.
4. Examination of the water pipes after excavation showed that surface pollution might gain access to the pipes conveying spring water to the collecting tank.
5. Secondary cases were few in number, and so far there is no evidence that foci of infection remain as sequelae of the outbreak.

Micro-scale Focused Ion Beam (FIB) machined cantilevers were manufactured in single crystal copper, polycrystalline copper and a copper-bismuth alloy. These were imaged and tested in bending using a nanoindenter. Cantilevers machined inside a single grain of polycrystalline copper were tested to determine their (anisotropic) Young's modulus: results were in good agreement with values calculated from literature values for single crystal elastic constants. The size dependence of yield behavior in the Cu microcantilevers was also investigated. As the thickness of the specimen was reduced from 23μm to 1.7μm the yield stress increased from 300MPa to 900MPa. Microcantilevers in Cu-0.02%Bi were manufactured containing a single grain boundary of known character, with a FIB-machined sharp notch on the grain boundary. The cantilevers were loaded to fracture allowing the fracture toughness of grain boundaries of different misorientations to be determined.

We investigated the relationship between environmental temperature and reported Salmonella infections in 10 European populations. Poisson regression adapted for time-series data was used to estimate the percentage change in the number of cases associated with a 1 °C increase in average temperature above an identified threshold value. We found, on average, a linear association between temperature and the number of reported cases of salmonellosis above a threshold of 6 °C. The relationships were very similar in The Netherlands, England and Wales, Switzerland, Spain and the Czech Republic. The greatest effect was apparent for temperature 1 week before the onset of illness. The strongest associations were observed in adults in the 15–64 years age group and infection with Salmonella Enteritidis (a serotype of Salmonella). Our findings indicate that higher temperatures around the time of consumption are important and reinforce the need for further education on food-handling behaviour.

There is a need to identify finishing systems for pigs that meet the requirements of both pig producers and society. These require that a system is economically efficient, but also takes due account of animal welfare, food safety and environmental considerations. As part of an integrated investigation, this study assessed the health and welfare implications of giving pigs either dry or liquid feed when housed in either fully-slatted or straw-based accommodation.

The magnetic properties of alkali-doped zeolites have been investigated using magnetic resonance spectroscopy. Although paramagnetic centres have been detected, the predominant species, in many cases, is spin-paired.

Recent work1 has highlighted the possibility that through the introduction of metals into the one-dimensional channels of zeolite L, it may be feasible to engineer charge transport along the channels to produce a unique compound comprising a precise, assembled array of ultrafine, atomic-scale conducting wires embedded within the aluminosilicate framework. Using electron spin resonance (ESR), and microwave cavity perturbation measurements, we examine the properties of these remarkable materials as a function of composition as they approach the insulator to metal transition.

The effect of the weaning diet on the subsequent colonic metabolism of bran and pectin in the adult rat has been investigated. Feeding a fibre-reduced diet on its own or supplemented with bran (WB) and pectin (P) from weaning (fibre-reduced (weaning)) was compared with introducing the same diet to age-matched rats reared on a standard laboratory diet from weaning (fibre-reduced (6 weeks)). The effects of the diets on colonic metabolism were measured by wet and dry caecal contents and stool weights, caecal sac weight, and caecal and faecal short-chain fatty acids (SCFA). Final body-weights were greater for fibre-reduced (6 weeks) and fibre-reduced (6 weeks)+P groups, but not fibre-reduced (6 weeks)+WB, than those of the fibre-reduced (weaning) rats. Rats fed on fibre-reduced (6 weeks) diet had a higher total caecal SCFA content than fibre-reduced (weaning) control rats. Fibre-reduced (weaning)+P-fed rats had a threefold higher caecal concentration of both propionate and butyrate than the matched fibre-reduced (6 weeks)+P group. Fibre-reduced (weaning)+WB animals had a significantly higher butyrate caecal concentration compared with their matched fibre-reduced (6 weeks)+WB group. Fibre-reduced (weaning)+P-fed rats had a lower faecal output than the fibre-reduced (6 weeks)+P rats. There was no difference in faecal output in rats fed on either fibre-reduced (6 weeks)+ WB or fibre-reduced (weaning)+WB. The faecal concentration of SCFA was in general higher in the rats fed on fibre-reduced (weaning) alone, +P, or +WB than in those fed on fibre-reduced (6 weeks) alone, +P or +WB. Faecal output of total and individual SCFA was increased on the fibre-reduced (weaned)+WB diet compared with fibre-reduced (6 weeks)+WB-fed animals. The diet at weaning may be important in determining the pathways of caecal bacterial metabolism in the adult rat. In studying the effect of a dietary fibre on caecal metabolism and faecal output, when the diet is changed appears to be important

In a well-known passage Plutarch (quoting Dikairchos) tells us that when Theseus landed in Delos with Ariadne, daughter of Nlinos, on his return from Crete, he and his young companions danced a dance which was called ‘The Crane’. This dance, as I hope to show, was an integral part of a cultural pattern of sacrificial and funerary ritual which was widespread throughout Europe and Asia and extended in modified form at least as far as Malekula in the New Hebrides. Unquestionably it was associated with the Labyrinth. Its winding figures represented the hero's wanderings in the Labyrinth on Crete.

No foreigner learning French fails to remark the use of jeunes gens as the plural of jeune homme. The French, for whom this plural is a part of their linguistic inheritance, usually accept it unquestioningly, but in the Mélanges Antoine Thomas Professor Oscar Bloch of the Lycée Buffon at Paris notes the abnormality of the expression and seeks to explain its origin. The cause of the disappearance of a plural jeunes hommes is, he thinks, to be found in a hesitation to employ, in a combination of the two words where the separate value of each is still in evidence, the linked s. In other combinations where such a linking to hommes occurs, such as les bonshommes, les gentilshommes, we have, says Mr. Bloch, true compound words in which the separate value of the elements has been lost.

In March, 1925, Clark and Collip (1) published a modification of Tisdall's method for the estimation of calcium. They had made extensive use of their method, and claimed that it gave an error of not more than 2%.
