Summary
The 21st Century Cures Act preserved the FDA’s authority to regulate several categories of software that incorporate artificial intelligence/machine learning (AI/ML) techniques. The agency’s draft guidance on Clinical Decision Support Software proposed an approach for regulating CDS software and shed light on the FDA’s approach to genomic software. This chapter explains why the FDA’s proposed approach to regulating software is unlikely to preempt failure-to-warn suits. It then turns to design-defect suits, focusing on adaptive AI/ML software that continues to redesign itself throughout the product lifecycle. How will common defenses work when the state of the art is in the robotic “mind” of an AI/ML algorithm? We also reflect on broader impacts on patients, clinicians, software developers, and healthcare systems and ask whether the FDA is the “right” regulator to address the unresolved ethical and medical practice concerns that surround this software.
Developing resilience and promoting positive mental health strategies in university students

Aims
Suicide is one of the leading causes of death in young people living in Australia, accounting for 7.3% of all deaths among individuals aged 15–19 years. Historically, high levels of suicide have been recorded in Australian university students. This project aims to develop and test a massive online course-program (MOOC) for university students, underpinned by literature and strength-based suicide prevention principles, building resilience and awareness of mental health promoting activities.
MethodA scoping review of the literature was undertaken to explore the effectiveness of current suicide prevention programs for undergraduate university students, and the effective elements contributing to the success of these programs. Six electronic databases were searched to identify relevant literature. Further, mental health consumers and university students were involved in co-producing the content of the six modules of the ‘Talk-to-me' MOOC.
ResultNine articles were included in the review, discussing four types of programs including; gatekeeping, education, promotional messaging and online consultation. It was apparent from this review that there is a significant dearth of interventions and programs currently available to reduce the risk of suicide among undergraduate students, with many of the programs having limited efficacy. Despite this, a number of program elements were identified as beneficial to preventing suicide among post-secondary students including upskilling of students, and improving resilience, and self-management. These findings and further consultation with mental health consumers and undergraduate university students underpinned the development of the content of the ‘Talk-to-me' MOOC which is tailored to meet the needs of university students. The MOOC contains six modules: Mental fitness; strategies to increase mental fitness; self-harm; suicidal behaviour in young adults; interventions for suicidal behaviour; and, gatekeeper interventions. Two case study senarios depicting mental health challenges commonly experienced by yound adults portraying appropriate crisis communication skills were developed and filmed complementing the six ‘Talk-to-me' modules.
ConclusionOverall, studies included in the review provide evidence to suggest that preventative programs, incorporating an educational component may be effective to be used in the MOOC to improving help-seeking behaviours among post-secondary education students. Findings from this review have underpinned the development of the ‘Talk-to-Me' MOOC which was launched in March 2020. To date this MOOC has enrolled over 45,000 participants from over 150 countries, with the average age of users being 24 years. Collectively, this line of work highlights that MOOCs are an effective means of mental health promotion to young adults.

Summary
The science of studying diamond inclusions for understanding Earth history has developed significantly over the past decades, with new instrumentation and techniques applied to diamond sample archives revealing the stories contained within diamond inclusions. This chapter reviews what diamonds can tell us about the deep carbon cycle over the course of Earth’s history. It reviews how the geochemistry of diamonds and their inclusions inform us about the deep carbon cycle, the origin of the diamonds in Earth’s mantle, and the evolution of diamonds through time.

Background/Objective:
Apolipoprotein E (APOE) E4 is the main genetic risk factor for Alzheimer’s disease (AD). Due to the consistent association, there is interest as to whether E4 influences the risk of other neurodegenerative diseases. Further, there is a constant search for other genetic biomarkers contributing to these phenotypes, such as microtubule-associated protein tau (MAPT) haplotypes. Here, participants from the Ontario Neurodegenerative Disease Research Initiative were genotyped to investigate whether the APOE E4 allele or MAPT H1 haplotype are associated with five neurodegenerative diseases: (1) AD and mild cognitive impairment (MCI), (2) amyotrophic lateral sclerosis, (3) frontotemporal dementia (FTD), (4) Parkinson’s disease, and (5) vascular cognitive impairment.
Methods:Genotypes were defined for their respective APOE allele and MAPT haplotype calls for each participant, and logistic regression analyses were performed to identify the associations with the presentations of neurodegenerative diseases.
Results:Our work confirmed the association of the E4 allele with a dose-dependent increased presentation of AD, and an association between the E4 allele alone and MCI; however, the other four diseases were not associated with E4. Further, the APOE E2 allele was associated with decreased presentation of both AD and MCI. No associations were identified between MAPT haplotype and the neurodegenerative disease cohorts; but following subtyping of the FTD cohort, the H1 haplotype was significantly associated with progressive supranuclear palsy.
Conclusion:This is the first study to concurrently analyze the association of APOE isoforms and MAPT haplotypes with five neurodegenerative diseases using consistent enrollment criteria and broad phenotypic analysis.

Summary
Introduction
The purpose of this chapter is to provide an introduction to a paradigm shift in ways of thinking about current recordkeeping environments. We begin by explaining recordkeeping informatics, a continuum-based approach to managing authoritative information in the ever shifting, complex and technologically challenging times that confront all of us, including organizations.
We then develop these ideas through a case study of recordkeeping requirements for those who as children experience out-of-home care as a result of child welfare and protection policies. Out-of-home care is the term used in Australia for ‘the care of children and young people up to 18 years who are unable to live with their families, often due to child abuse and neglect. It involves the placement of a child or young person with alternate caregivers on a short- or long-term basis’ (Australian Institute of Family Studies, 2015). These experiences place lifelong identity, memory and accountability needs on the governments and organizations providing these services. Our case study will explore the macro and micro challenges that arise for individuals, organizations, governments and societies as a demonstration of the utility of recordkeeping informatics in designing archival futures in which multiple rights in records are embedded.
Recordkeeping informatics
The development of recordkeeping informatics has been underway for the past eight years. The original motivation for this work was the need for an up-to-date textbook based on records continuum thinking to support teaching and learning for contemporary recordkeeping. The second edition of Jay Kennedy and Cherryl Schauder's Records Management: a guide to corporate record keeping (1998), is the only title currently in print to fit the bill, but its publication date means that the content, although still valid, is inevitably dated. During our first meeting to discuss the outline of an updated text, it became clear that it would not be possible to address contemporary recordkeeping requirements adequately within the constraints of what had developed as a discipline and occupation in the paper world.
The definition of informatics that we use explains the discipline as ‘the science of information. It studies the representation, processing, and communication of information in natural and artificial systems. Since computers, individuals and organizations all process information, informatics has computational, cognitive and social aspects’ (Fourman, 2003, 237–8).

The Randolph Glacier Inventory (RGI) is a globally complete collection of digital outlines of glaciers, excluding the ice sheets, developed to meet the needs of the Fifth Assessment of the Intergovernmental Panel on Climate Change for estimates of past and future mass balance. The RGI was created with limited resources in a short period. Priority was given to completeness of coverage, but a limited, uniform set of attributes is attached to each of the ~198 000 glaciers in its latest version, 3.2. Satellite imagery from 1999–2010 provided most of the outlines. Their total extent is estimated as 726 800 ± 34 000 km2. The uncertainty, about ±5%, is derived from careful single-glacier and basin-scale uncertainty estimates and comparisons with inventories that were not sources for the RGI. The main contributors to uncertainty are probably misinterpretation of seasonal snow cover and debris cover. These errors appear not to be normally distributed, and quantifying them reliably is an unsolved problem. Combined with digital elevation models, the RGI glacier outlines yield hypsometries that can be combined with atmospheric data or model outputs for analysis of the impacts of climatic change on glaciers. The RGI has already proved its value in the generation of significantly improved aggregate estimates of glacier mass changes and total volume, and thus actual and potential contributions to sea-level rise.

Objective
We sought to analyse the variation in the incidence of patent ductus arteriosus over three recent time points and characterise ductal ligation practices in preterm infants in the United States, adjusting for demographic and morbidity factors.
MethodsUsing the Kids’ Inpatient Database from 2003, 2006, and 2009, we identified infants born at ⩽32 weeks of gestation with International Classification of Diseases, Ninth Revision diagnosis of patent ductus arteriosus and ligation code. We examined patient and hospital characteristics and identified patient and hospital variables associated with ligation.
ResultsOf 182,610 preterm births, 30,714 discharges included a patent ductus arteriosus diagnosis. The rate of patent ductus arteriosus diagnosis increased from 14% in 2003 to 21% in 2009 (p<0.001). A total of 4181 ligations were performed, with an overall ligation rate of 14%. Ligation rate in infants born at ⩽28 weeks of gestation was 20% overall, increasing from 18% in 2003 to 21% in 2009 (p<0.001). The ligation rate varied by state (4–28%), and ligation was associated with earlier gestational age, associated diagnoses, hospital type, teaching hospital status, and region (p<0.001).
ConclusionThe rates of patent ductus arteriosus diagnosis and ligation have increased in the recent years. Variation exists in the practice of patent ductus arteriosus ligation and is influenced by patient and non-patient factors.

Long-term variability in North Sea zooplankton off the Northumberland coast: productivity of small copepods and analysis of trophic interactions

All groups of meso- and macro-zooplankton in the North Sea off Northumberland, at a depth of 53 m, were studied during a 15-year period (1969–83); copepod productivity was estimated from biomass and growth rates. Phytoplankton were seasonally bi-modal with peaks in April and August–October; copepods were uni-modal peaking in June–July. The predatory zooplankters: larval fish, decapods, ctenophores, medusae (the summer-autumn predators) peaked between May and September, while chaetognaths and euphausiids (the winter predators) peaked in December–January. Copepods and the summer-autumn predators were seasonally and inter-annually positively correlated, and declined in abundance from 1974 to 1980. Euphausiids and chaetognaths on the contrary increased in abundance during these years, and were seasonally and inter-annually negatively correlated to the copepods. The mean annual abundance of copepods was positively related to the previous winter's minimum, and inversely related to the abundance of chaetognaths and euphausiids. Annual copepod productivity averaged 1260 kJ m-2 year-1, and showed no relationship to other groups of plankton.

Observations have been made of the surface currents off the Northumberland coast over a period of 12 months, using a free-floating buoy. A mean vector current was found, setting 114° at o·i knot. Currents flowed into the quadrant to the right of downwind except when this would have directed them ashore. A relationship is established between wind speed and current speed.

Investigations of the water movements off the southern Northumberland coast have until now been confined to the surface region. While important for navigational and similar purposes such investigations can give us no more than an indication of the total water flow along the coast nor can they elucidate the effects of currents on planktonic organisms which spend part or all of their existence away from the surface. It is for the latter reason that the series of investigations of sea surface currents made aboard the R.V. ‘Alexander Meek’ in 1956–7 (Evans, 1957) has now been supplemented by further series at two deeper levels.

Optical SETI at Lick Observatory is characterized by its robust approach to initial detections. Our three-detector system has distinguished itself by successful rejection of nearly all false positive signals. We present observational progress, discuss use of data analysis procedures such as FFTs and analysis of double coincidences, and mention plans to upgrade our instrumentation.

Background
An acceptable, standardised outcome measure to assess efficacy and effectiveness is needed across multiple disciplines offering psychological therapies.
AimsTo present psychometric data on reliability, validity and sensitivity to change for the CORE–OM (Clinical Outcomes in Routine Evaluation – Outcome Measure).
MethodA 34-item self-report instrument was developed, with domains of subjective well-being, symptoms, function and risk. Analysis includes internal reliability, test–retest reliability, socio-demographic differences, exploratory principal-component analysis, correlations with other instruments, differences between clinical and non-clinical samples and assessment of change within a clinical group.
ResultsInternal and test–retest reliability were good (0.75–0.95), as was convergent validity with seven other instruments, with large differences between clinical and non-clinical samples and good sensitivity to change.
ConclusionsThe CORE–OM is a reliable and valid instrument with good sensitivity to change. It is acceptable in a wide range of practice settings.

Background
Measurement is the foundation of evidence-based practice. Advances in measurement procedures should extend to psychotherapy practice.
AimsTo review the developments in measurement relevant to psychotherapy.
MethodDomains reviewed are: (a) interventions; (b) case formulation; (c) treatment integrity; (d) performance (including adherence, competence and skilfulness); (e) treatment definitions; (f) therapeutic alliance; and (g) routine outcome measurement.
ResultsModern methods of measurement can support ‘evidence-based practice’ for psychological treatments. They also support ‘practice-based evidence’, a complementary paradigm to improve clinical effectiveness in routine practice via the infrastructure of Practice Research Networks (PRNs).
ConclusionsAdvances in measurement derived from psychotherapy research support a model of professional self-management (practice-based evidence) which is widely applicable in psychiatry and medicine.
