In view of the increasing complexity of both cardiovascular implantable electronic devices (CIEDs) and patients in the current era, practice guidelines, by necessity, have become increasingly specific. This document is an expert consensus statement that has been developed to update and further delineate indications and management of CIEDs in pediatric patients, defined as ≤21 years of age, and is intended to focus primarily on the indications for CIEDs in the setting of specific disease categories. The document also highlights variations between previously published adult and pediatric CIED recommendations and provides rationale for underlying important differences. The document addresses some of the deterrents to CIED access in low- and middle-income countries and strategies to circumvent them. The document sections were divided up and drafted by the writing committee members according to their expertise. The recommendations represent the consensus opinion of the entire writing committee, graded by class of recommendation and level of evidence. Several questions addressed in this document either do not lend themselves to clinical trials or are rare disease entities, and in these instances recommendations are based on consensus expert opinion. Furthermore, specific recommendations, even when supported by substantial data, do not replace the need for clinical judgment and patient-specific decision-making. The recommendations were opened for public comment to Pediatric and Congenital Electrophysiology Society (PACES) members and underwent external review by the scientific and clinical document committee of the Heart Rhythm Society (HRS), the science advisory and coordinating committee of the American Heart Association (AHA), the American College of Cardiology (ACC), and the Association for European Paediatric and Congenital Cardiology (AEPC). The document received endorsement by all the collaborators and the Asia Pacific Heart Rhythm Society (APHRS), the Indian Heart Rhythm Society (IHRS), and the Latin American Heart Rhythm Society (LAHRS). This document is expected to provide support for clinicians and patients to allow for appropriate CIED use, appropriate CIED management, and appropriate CIED follow-up in pediatric patients.

SNP in the vitamin D receptor (VDR) gene is associated with risk of lower respiratory infections. The influence of genetic variation in the vitamin D pathway resulting in susceptibility to upper respiratory infections (URI) has not been investigated. We evaluated the influence of thirty-three SNP in eleven vitamin D pathway genes (DBP, DHCR7, RXRA, CYP2R1, CYP27B1, CYP24A1, CYP3A4, CYP27A1, LRP2, CUBN and VDR) resulting in URI risk in 725 adults in London, UK, using an additive model with adjustment for potential confounders and correction for multiple comparisons. Significant associations in this cohort were investigated in a validation cohort of 737 children in Manchester, UK. In all, three SNP in VDR (rs4334089, rs11568820 and rs7970314) and one SNP in CYP3A4 (rs2740574) were associated with risk of URI in the discovery cohort after adjusting for potential confounders and correcting for multiple comparisons (adjusted incidence rate ratio per additional minor allele ≥1·15, Pfor trend ≤0·030). This association was replicated for rs4334089 in the validation cohort (Pfor trend=0·048) but not for rs11568820, rs7970314 or rs2740574. Carriage of the minor allele of the rs4334089 SNP in VDR was associated with increased susceptibility to URI in children and adult cohorts in the United Kingdom.

Enlist™ cotton contains the aad-12 and pat genes that confer resistance to 2,4-D and glufosinate, respectively. Thirty-three field trials were conducted focused on Enlist cotton injury from glufosinate as affected by cotton growth stage, application rate, and single or sequential applications. Maximum injury from a single application of typical 1X (542 g ae ha-1) and 2X use rates was 3 and 13%, respectively, regardless of growth stage. Injury from sequential applications of 1X or 2X rates was equivalent to single applications. Similar injury was observed with four commercial formulations of glufosinate. Cotton yield was never affected by glufosinate. This research demonstrates Enlist™ cotton has robust resistance to glufosinate at rates at least twice the typical use rate when applied once or twice at growth stages ranging from 2 to 12 leaves.

Fiber optic sensors offer a novel approach to monitoring of fractures in concrete waste disposal vaults and offer the possibility of determining the quantity, width and location of the cracks as they form. Fiber optics can directly detect cracks if they form within the path of a fiber optic as well as monitor secondary indicators of cracking such as temperature changes and strain. When cracks form in concrete waste disposal vaults they can fill with water which has a high heat capacity, this enables cracks to be observed by monitoring temperature variations near the crack. An analytical solution for heat transfer is applied to estimate the propagation of temperature waves around cracks. It is demonstrated that discharge rates through the concrete which are less than 10-5 m3/m-s do not produce a meaningful temperature wave through the concrete. Fractures in the concrete must be larger than 0.07 cm to produce a measurable result and temperature sensors must be located within 0.5 meters of a crack to detect a change in temperature produced by seasonal groundwater flow through a crack. A distributed system of fiber optic sensors may be embedded in the concrete vault and used to monitor crack formation, temperature variations and strain.

The Self Test: a screening tool for dementia requiring minimal supervision

Background: The main purpose of this study was to correlate a newly developed, simple, but comprehensive screening test for dementia called the Self Test (ST) with the Mini-mental State Examination (MMSE), and establish the reliability and validity of the ST.
Methods:The ST was administered to 42 consecutive new referrals to the memory clinic at the Cole Neuroscience Center, University of Tennessee Medical Center (UTMC), Knoxville, TN, U.S.A. and 41 age-matched control subjects at regular intervals over a 12-month period.
Results:The total ST scores correlated well with cognitive severity as measured by the MMSE (r = 0.71, p<0.01). The area under the receiver-operating characteristic curve (AUROCC) in the group of patients with Alzheimer's disease (AD) with less than a high school education was 0.80, and 0.89 in those with at least a high school education. Reliability analysis showed a mean inter-item correlation of 0.31 for patients with AD, and 0.47 for normal participants. Cronbach's α was calculated to be 0.70 for the AD group. Test–retest reliability was determined using intraclass correlation coefficients (ICC = 0.93, p<0.01).
Conclusions: The ST is an internally consistent, reliable and valid screening test for cognitive impairment in persons exhibiting early symptoms of dementia. In addition, the ST requires minimal nonprofessional supervision and may be administered by an untrained person.
Insulin-like growth factor-I and analogues increase growth in artificially-reared neonatal pigs

Exogenous insulin-like growth factor (IGF)-I has been shown to increase growth rate in neonatal pigs while an analogue of IGF-I, long arginine (LR3) IGF-I, has been shown to be more potent than IGF-I in the rat. Therefore, two studies were conducted to determine whether IGF-I and LR3IGF-I increase growth in the artificially-reared neonatal pig. Expt 1 involved forty-two (2 kg initial weight) pigs infused with either control, IGF-I (2, 4 or 8 μg/h) or LR3IGF-I (2, 4 or 8 μg/h) infusions for 8 d. Pigs were weighed and then offered 1·7 MJ (gross energy) milk replacer/kg0·75 per d. Expt 2 involved eighteen pigs (2 kg initial weight) treated with control saline, IGF-I (8 μg/h) or LR3IGF-I (8 μg/h) infusions. After 9 d an additional pump was inserted to increase the infusion rates of each of the growth factors (16 μg/h) for a further 9 d. Cows' milk was provided ad libitum. In Expt 1 there was no overall effect of growth factors on daily weight gain or slaughter weight. However, milk intake was greater in pigs infused with growth factors (909 v. 867 g/d, P=0·027), with an apparently greater milk intake by the pigs infused with IGF-I compared with LR3IGF-I (920 v. 898 g/d, P=0·12). Infusion of LR3IGF-I decreased plasma IGF-I concentrations, but had no effect on plasma IGF-II concentrations. In Expt 2, neither IGF-I nor LR3IGF-I infusion had any effect upon daily weight gain over the first 9 d of the study. However, over the second 9 d of the study, daily weight gain was increased in LR3IGF-I-infused pigs (457 v. 386 g/d, P<0·01), but not in pigs infused with IGF-I (413 v. 386 g/d, P=0·15). Milk intake was not different during the first 9 d of the study but was significantly greater in pigs infused with growth factors over the second half of the study (3407 v. 2905 g/d, P<0·01). Plasma IGF-binding protein-3 concentrations were highly correlated (R=0·85) with average daily gain over the 3 d preceding blood sampling. In conclusion, exogenous IGF-I and particularly LR3IGF-I can increase growth rate and milk intake in artificially-reared pigs fed ad libitum but not in limit-fed piglets.

C60 Cl6 can be phenylated and arylated to give derivatives of the type C60Ar5Cl, which may be readily converted to C60Ar5. The compounds C60Ar5 and various other phenylated derivatives have been isolated from the product of reaction of [60]fullerene with bromine/ferric chloride/benzene, and partially characterised.

Corrosion products resulting from the oxidation of steel containers and iron-containing waste can alter the local geochemical environment in radioactive waste disposal vaults and trenches. In addition, corrosion products can enhance the retention of radionuclides by providing a highly sorptive media. Initial hydrous corrosion products are poorly crystalline with large surface area, high sorption capacity, and large Kds. Over time, corrosion products recrystallize by step-wise dehydration to phases with significantly smaller surface areas. The decreased surface area results in decreased sorption and the potential for remobilization of previously sorbed radionuclides. The impact of time-dependent variation in sorption on radionuclide transport is a complex function of the rate of recrystallization and the radionuclide half-life.
A mathematical model has been developed, which coupling mass transport with corrosion product recrystallization and equilibrium sorption of radionuclides. The model is applied release of radionuclides with a range of half-lives (year to 100,000s of years) from radioactive waste disposal facilities to illustrate the significance of corrosion product recrystallization on release.

Prediction of radionuclide release rates for high-level waste requires estimates of the rates of waste form alteration and formation of secondary minerals inside the failed canister. Unsaturated repository sites may promote development of a variety of chemical environments related to two phase (liquid/ vapor) transport and temperature gradients caused by radiogenic decay. A mass balance (shell balance) approach is used to estimate the effects of dripping water, evaporation, and condensation on the waste canister and the presence of saline water inside the failed waste canister. The simplified calculations predict large variability of water chemistry over spatial scales of a few centimeters. The effects of the predicted aqueous chemistry on waste form alteration, secondary mineral formation, and radionuclide solubility are examined.

One of the requirements for the performance of waste packages prescribed in 10CFR 60.113 is that the high level waste must be “substantially completely” contained for a minimum period of 300 to 1000 years. During this period, the radiation and thermal conditions in the engineered barrier system and the near-field environment are dominated by fission product decay. In the present U.S design of the engineered barrier system, the outer container plays a dominant role in maintaining radionuclide containment. A quantitative methodology for analyzing the performance of the container is described in this paper. This methodology enables prediction of the evolution of the waste package environment in terms of temperature fields, stability of liquid water on the container surface, and concentration of aggressive ions such as chloride. The initiation and propagation of localized corrosion is determined by the corrosion potential of the container material and critical potentials for localized corrosion. The coiTOsion potential is estimated from the kinetics of the anodic and cathodic reactions including oxygen diffusion through scale layers formed on the container surface. The methodology described is applicable to a wide range of metals, alloys and environmental conditions.
The Role of Oxygen Diffusion in the Release of Technetium from Reducing Cementitious Waste Forms

Cementitious materials provide an ideal geochemical environment (e.g., high pH pore fluids and large surface areas for sorption) for immobilizing nuclear waste. The inclusion of reducing agents, such as blast furnace slag (BFS) can immobilize radionuclides by forming of solid sulfide phases. Thermodynamic calculations using the MINTEQ geochemical computer code indicate that elemental sulfur present in BFS reacts with the highly mobile pertechnetate anion (TcO4−) anion to form an insoluble technetium sulfide phase (Tc2S7(8)). Initially, the waste form very effectively immobilizes technetium. However, as oxygen diffuses into the waste form, an outer zone of oxidized concrete and a shrinking core of reduced intact concrete develops. Oxidation of sulfur in the outer zone results in increased technetium concentrations in the pore fluid because Tc2S7(8) oxidizes to the mobile TcO4− anion. The TcO4− anion can then diffuse from the waste form into the environment.
A mathematical model that accounts for diffusion of oxygen into concrete coupled with oxidation of sulfur and sulfide to sulfate has been developed. This model assumes the existence of an oxidized outer layer of concrete surrounding a shrinking core of reducing intact concrete. A sharp boundary between the two zones moves slowly inward resulting in oxidation of Tc2S7(8) and subsequent release of TcO4− via aqueous diffusion in the concrete pore fluids. The model indicates that this mechanism results in a linear dependance of release with the square root of time similar to pure diffusion. In addition, the release of technetium is related to the inverse of the square root of the concentration of BFS, indicating that performance will significantly increases with the addition of approximately 20 percent BFS to the cement mix.

A near-field performance assessment (PA) was conducted for the Saltstone Disposal Facility (SDF) at the Savannah River Site in South Carolina. The analysis was conducted in four parts: general screening calculations, degradation calculations, and flow and transport through the fractured and nonfractured facility. Modeling approaches and example sensitivity analysis results from the simulations of the fractured facility are discussed. Design considerations that may not be apparent without addressing flow and transport through fractures and lessons learned during the process are also presented.

The wealhering of concrete by carbonation causes the formation of calcite solid solutions that can significantly improve concrete as a barrier to the migration of divalent radionuclide cations. Example calculations for 90Sr and 60Co indicate that release rates from carbonated concrete are five orders of magnitude lower than for intact, unweathered concrete and that the thin carbonated zone is a significant sink for these radionuclides.
