Seroconversion and seroprevalence of TORCH infections in a pregnant women cohort study, Mombasa, Kenya, 2017–2019

Women infected during pregnancy with TORCH (Toxoplasmosis, Other, Rubella, Cytomegalovirus, and Herpes simplex viruses) pathogens have a higher risk of adverse birth outcomes including stillbirth / miscarriage because of mother-to-child transmission. To investigate these risks in pregnant women in Kenya, we analyzed serum specimens from a pregnancy cohort study at three healthcare facilities. A sample of 481 participants was selected for TORCH pathogen antibody testing to determine seroprevalence. A random selection of 285 from the 481 participants was selected to measure seroconversion. These sera were tested using an IgG enzyme-linked immunosorbent assay against 10 TORCH pathogens. We found that the seroprevalence of all but three of the 10 TORCH pathogens at enrollment was >30%, except for Bordetella pertussis (3.8%), Treponema pallidum (11.4%), and varicella zoster virus (0.5%). Conversely, very few participants seroconverted during their pregnancy and were herpes simplex virus type 2 (n = 24, 11.2%), parvovirus B19 (n = 14, 6.2%), and rubella (n = 12, 5.1%). For birth outcomes, 88% of the participant had live births and 12% had stillbirths or miscarriage. Cytomegalovirus positivity at enrolment had a statistically significant positive association with a live birth outcome (p = 0.0394). Of the 10 TORCH pathogens tested, none had an association with adverse pregnancy outcome.

Decumulation Pathways are proposed to help achieve better retirement outcomes for those with Defined Contribution (DC) pensions. The DC fund is split into two parts, in proportions of the consumer’s choice. Most is allocated to the Pension Fund to provide a lifetime income, while the rest is placed in the Flexible Fund for flexible access and/or to leave as a legacy. The Flexible Fund is invested in flexi-access drawdown. The Pension Fund is invested in a guaranteed annuity, Collective Defined Contribution, or a Pooled Pension Fund which maintains individual DC funds but pools longevity risk between participants. An illustrative standard Decumulation Pathway is intended as a default solution, or can be tailored by the consumer. It uses the Pooled Pension Fund, an automated withdrawal strategy which ensures a lifetime income is provided and one that aims to increase in line with inflation, and a moderate risk investment strategy. The standard approach is evaluated using various metrics, indicating that it has as a strong chance of providing a higher income than could be obtained from an annuity or drawdown, with limited downside risk.
Early intensification of backyard poultry systems in the tropics: a case study

Poultry production is an important way of enhancing the livelihoods of rural populations, especially in low- and middle-income countries (LMICs). As poultry production in LMICs remains dominated by backyard systems with low inputs and low outputs, considerable yield gaps exist. Intensification can increase poultry productivity, production and income. This process is relatively recent in LMICs compared to high-income countries. The management practices and the constraints faced by smallholders trying to scale-up their production, in the early stages of intensification, are poorly understood and described. We thus investigated the features of the small-scale commercial chicken sector in a rural area distant from major production centres. We surveyed 111 commercial chicken farms in Kenya in 2016. We targeted farms that sell the majority of their production, owning at least 50 chickens, partly or wholly confined and provided with feeds. We developed a typology of semi-intensive farms. Farms were found mainly to raise dual-purpose chickens of local and improved breeds, in association with crops and were not specialized in any single product or market. We identified four types of semi-intensive farms that were characterized based on two groups of variables related to intensification and accessibility: (i) remote, small-scale old farms, with small flocks, growing a lot of their own feed; (ii) medium-scale, old farms with a larger flock and well located in relation to markets and (iii) large-scale recently established farms, with large flocks, (iii-a) well located and buying chicks from third-party providers and (iii-b) remotely located and hatching their own chicks. The semi-intensive farms we surveyed were highly heterogeneous in terms of size, age, accessibility, management, opportunities and challenges. Farm location affects market access and influences the opportunities available to farmers, resulting in further diversity in farm profiles. The future of these semi-intensive farms could be compromised by several factors, including the competition with large-scale intensive farmers and with importations. Our study suggests that intensification trajectories in rural areas of LMICs are potentially complex, diverse and non-linear. A better understanding of intensification trajectories should, however, be based on longitudinal data. This could, in turn, help designing interventions to support small-scale farmers.

Background
Surgery for CHD has been slow to develop in parts of the former Soviet Union. The impact of an 8-year surgical assistance programme between an emerging centre and a multi-disciplinary international team that comprised healthcare professionals from developed cardiac programmes is analysed and presented.
Material and methodsThe international paediatric assistance programme included five main components – intermittent clinical visits to the site annually, medical education, biomedical engineering support, nurse empowerment, and team-based practice development. Data were analysed from visiting teams and local databases before and since commencement of assistance in 2007 (era A: 2000–2007; era B: 2008–2015). The following variables were compared between periods: annual case volume, operative mortality, case complexity based on Risk Adjustment for Congenital Heart Surgery (RACHS-1), and RACHS-adjusted standardised mortality ratio.
ResultsA total of 154 RACHS-classifiable operations were performed during era A, with a mean annual case volume by local surgeons of 19.3 at 95% confidence interval 14.3–24.2, with an operative mortality of 4.6% and a standardised mortality ratio of 2.1. In era B, surgical volume increased to a mean of 103.1 annual cases (95% confidence interval 69.1–137.2, p<0.0001). There was a non-significant (p=0.84) increase in operative mortality (5.7%), but a decrease in standardised mortality ratio (1.2) owing to an increase in case complexity. In era B, the proportion of local surgeon-led surgeries during visits from the international team increased from 0% (0/27) in 2008 to 98% (58/59) in the final year of analysis.
ConclusionsThe model of assistance described in this report led to improved adjusted mortality, increased case volume, complexity, and independent operating skills.
Buruli ulcer in southern Côte D’ivoire: dynamic schemes of perception and interpretation of modes of transmission

Buruli ulcer (BU) belongs to the group of neglected tropical diseases and constitutes a public health problem in many rural communities in Côte d’Ivoire. The transmission patterns of this skin infection are poorly defined, hence the current study aimed to contribute to the understanding, perceptions and interpretations of its mode of transmission using a socio-environmental approach. Social and environmental risk factors that may expose people to infection, and the dynamics of local transfer of knowledge and practices related to BU, were assessed in two endemic locations in southern Côte d’Ivoire, i.e. Taabo and Daloa. Data were generated by the administration of a household questionnaire (N=500) between February and June 2012 to assess how the population perceived transmission of BU, focus group discussions with local communities (N=8) to analyse ideologies regarding transmission patterns and semi-structured interviews with patients or their parents, former BU patients and traditional healers (N=30). The interviewees’ empirical knowledge of the disease was found to be close to its biomedical description. Their aetiological perception of the disease was linked to natural (e.g. dirty water, insects) and supernatural (e.g. witchcraft, fate) causes. Some informants attributed the spread of the disease to recently immigrated neighbouring communities whose arrival coincided with an increase in reported BU cases. However, the general consensus seemed to be that the main mode of transmission was contact with infested soil or ulcerated wounds. The participants were aware that BU was a socio-environmental problem in these endemic areas, offering a good starting point for educational campaigns for at-risk communities. Buruli ulcer control programmes should therefore include educational campaigns and Water, Sanitation and Hygiene (WASH) interventions for those at risk in affected communities.
Mechanisms of subglacial cavity filling in Glacier de Tête Rousse, French Alps

The deadliest outburst flood from an englacial cavity occurred on Glacier de Tête Rousse in the Mont Blanc area, French Alps, in 1892. A subglacial reservoir was discovered in the same glacier in 2010 and drained artificially in 2010, 2011 and 2012 to protect the 3000 inhabitants downstream. The mechanism leading to the spontaneous refilling of the cavity following these pumping operations has been analyzed. For this purpose, the subglacial water volume changes between 2010 and 2013 were reconstructed. The size of the cavity following the pumping was found to have decreased from 53500 m3 in 2010 to 12 750 m3 in 2013. Creep and the partial collapse of the cavity roof explain a large part of the volume loss. Analysis of cavity filling showed a strong relationship between measured surface melting and the filling rate, with a time delay of 4–6 hours. A permanent input of 15 m3 d−1, not depending on surface melt, was also found. The meltwater and rain from the surface is conveyed to bedrock through crevasses and probably through a permeable layer of rock debris at the glacier bed. The drainage pathway permeability was estimated at 0.054 ms−1 from water discharge measurements and dye-tracing experiments.

The climatic snow line is a little above the highest peaks in this region (Mir Samir, 5 809 m, lat. 35° 35′ N., long. 70° 10′ E.) but steep north-facing slopes provide a topographie reason for small glaciers persisting, with a snow line a little below 4 900 in (16 000 ft). Snow accumulation at this height averages 1 300 kg m−2, most of this falling in late winter and spring. Gross ablation reaches 40 kg m−2 d−1. Net ablation (July–August), measured as stream discharge, averages 3 600 m3 d−1 = 9 kg m−2 d−1. Slow recession is the average condition. Halt stages at 4 800, 4 600 and 4 000 m are interpreted from the topography. Dating of moraines by lichen distribution indicates for the highest of these a minimum age of 400 years.
The vertical flux of heat and of water vapour were calculated through an air layer 30–100 cm over a melting firn surface for 24 h. The source of heat available at the surface expressed as a percentage of the total was: net radiation 87, condensation 5, forced convection 1, conduction through snow 6 and unexplained 1. The heat sink was: long-wave radiation 4, evaporation 6, forced convection 6, conduction into snow 2 and melting 82%.

Nutrigenomics is the study of how constituents of the diet interact with genes, and their products, to alter phenotype and, conversely, how genes and their products metabolise these constituents into nutrients, antinutrients, and bioactive compounds. Results from molecular and genetic epidemiological studies indicate that dietary unbalance can alter gene–nutrient interactions in ways that increase the risk of developing chronic disease. The interplay of human genetic variation and environmental factors will make identifying causative genes and nutrients a formidable, but not intractable, challenge. We provide specific recommendations for how to best meet this challenge and discuss the need for new methodologies and the use of comprehensive analyses of nutrient–genotype interactions involving large and diverse populations. The objective of the present paper is to stimulate discourse and collaboration among nutrigenomic researchers and stakeholders, a process that will lead to an increase in global health and wellness by reducing health disparities in developed and developing countries.

Summary
Background and objective: Effective and early treatment of postoperative pain and nausea have become pivotal for the early discharge of patients after tonsillectomy. Opioid-based analgesia is standard practice but the use of non-steroidal anti-inflammatory drugs is discouraged due to their platelet inhibiting properties. The cyclo-oxygenase-2 selective non-steroidal anti-inflammatory drugs are effective analgesics and do not affect platelet function. We hypothesized that premedication with cyclo-oxygenase-2 selective non-steroidal anti-inflammatory drug in addition to paracetamol would provide effective analgesia and decrease opioid consumption during early recovery from tonsillectomy. Methods: In a randomized, placebo-controlled study of adult tonsillectomy patients (n = 40) one group (R-group; n = 20) was premedicated with paracetamol 1.5 g and rofecoxib 50 mg and a control group (P-group; n = 20) was premedicated with paracetamol 1.5 g and placebo. Morphine was used as rescue medication. Postoperative (24 h) pain scores (0–10), morphine consumption as well as intraoperative blood loss were recorded. Results: We found no overall difference in pain scores between the groups but significantly more patients in the placebo group had pain scores >5 within the first 8 h. The rofecoxib group consumed less morphine during the first 12 h. A lower intraoperative blood loss was observed in the rofecoxib group. Conclusion: Our results suggest an early although clinically minor analgesic benefit of the addition of a cyclo-oxygenase-2 selective non-steroidal anti-inflammatory drug to paracetamol as premedication for adult tonsillectomy.

A vaccination programme offering hepatitis B (HBV) vaccine at reception into prison has been introduced into selected prisons in England and Wales. Over the coming years it is anticipated this vaccination programme will be extended. A model has been developed to assess the potential impact of the programme on the vaccination coverage of prisoners, ex-prisoners, and injecting drug users (IDUs). Under a range of coverage scenarios, the model predicts the change over time in the vaccination status of new entrants to prison, current prisoners and IDUs in the community. The model predicts that at baseline in 2012 57% of the IDU population will be vaccinated with up to 72% being vaccinated depending on the vaccination scenario implemented. These results are sensitive to the size of the IDU population in England and Wales and the average time served by an IDU during each prison visit. IDUs that do not receive HBV vaccine in the community are at increased risk from HBV infection. The HBV vaccination programme in prisons is an effective way of vaccinating this hard-to-reach population although vaccination coverage on prison reception must be increased to achieve this.

We report the synthesis of organized colloidal semiconductor nanocrystal / carbon singlewalled nanotube hybrid nanostructures. The synthetic protocol described here avoids the need for covalent chemical modification of carbon nanotube (CNT) surfaces. Specifically, InP quantum dots (QDs) and CdSe QDs were found to strongly adsorb onto the surfaces of carbon single-walled nanotubes (SWNTs) by gentle heating in organic solvents. Transmission electron microscopy (TEM) was used to characterize the semiconductor nanocrystal (NC) / SWNT assemblies, and revealed that the surfaces of the SWNT bundles template the adsorption of the NCs from solution. Small QDs were found to randomly absorb onto SWNTs, while larger QDs self-assembled into long linear chains. The nature of binding and ordering was investigated by simply considering van der Waals (vdW) forces for both NC-SWNT and NC-NC interactions. Quantum rods (QRs) were also found to adsorb along the nanotube surfaces. These findings have important implications for the synthesis of NC / SWNT hybrid nanostructures.

Fracture surfaces of a recently developed Zr41.2Ti13.8Cu12.5Ni10.0Be22.5 (at.%) bulk metallic glass were investigated using a three-dimensional surface reconstruction technique. Stereoscopic scanning electron microscopy of both fatigue and overload fracture surfaces permitted the creation of digital elevation models that were used to quantify important fracture surface features. Characterization of the surfaces revealed striations of nearly constant spacing on fatigue surfaces and a vein morphology characteristic in amorphous metals on the overload fracture surfaces. Additionally, at the onset of critical failure, crack-tip openings of ˜16 μm were observed that were consistent with measured values of fracture toughness. Interestingly, at the onset of fracture, deformation was confined to one side of the fracture plane, possibly because of the asymmetric emission of shear bands from the crack tip, consistent with the highly inhomogeneous nature of deformation in this alloy.

The fracture and fatigue properties of the Zr41.2Ti13.8Cu12.5Ni10Be22.5 (at.%) bulk metallic glass alloy have been examined. The plane-strain fracture toughness of the fully amorphous alloy was found to exceed 50 MPa√m, although results were sensitive to strain rate, showed significant variability and were influenced by the presence of residual stresses following processing. Fracture surfaces exhibited a characteristic vein morphology, consistent with micromechanical models for meniscus instabilities. Local melting was evident, consistent with the emission of light during rupture and very high local temperatures (>1000 K) measured during fracture. Upon partial or complete crystallization, the alloy was severely embrittled, with toughnesses dropping to ∼1 MPa√m and the hardness increasing by ∼10%. Under cyclic loading, crack-propagation behavior in the amorphous structure was similar to that observed in polycrystalline metals; the crack-advance mechanism was associated with alternating crack-tip blunting and resharpening, as evidenced by presence of fatigue striations. Conversely, the (unnotched) stress-life (S/N) properties were markedly different. Crack initiation and subsequent growth occurred quite readily due to the lack of microstructural barriers that would normally provide local crack-arrest points. This resulted in a very low fatigue limit of∼4% the ultimate tensile strength.
Mechanism for Light Emission During Fracture of a Zr-Ti-Cu-Ni-Be Bulk Metallic Glass: Temperature Measurements in Air and Nitrogen

Light emitted during rupture of Zr41.2Ti13.8Cu12.5 Ni10Be22.5 (at%) bulk metallic glass has been investigated. Charpy V-notch specimens fractured in a pendulum impact apparatus were used to excite light emission. Spectra acquired from rupture in air exhibited a single broad peak and were fit to a blackbody temperature of ∼3175 K. In nitrogen, however, light emission was no longer visible to the eye. In this case, the captured light was at least four orders of magnitude less intense, and spectra were shifted to the red with an effective blackbody temperature of ∼1400 K. Fracture surfaces generated in both air and nitrogen exhibited local melting, providing further evidence of intense heating during fracture. Based on these observations we argue that the intense light emission in air is associated with pyrolysis of fresh material exposed during rupture. Results were compared to preliminary observations of light emission from ribbons of a Zr57Nb5Ni12.6Cu15.4Al10 (at%) glass.

The lichen vegetation of forty-five acid watercourses in England is described and related to ecological variables. The zonation scheme adopted for chalk and limestone streams that involves four overlapping bands of lichen vegetation related to length of submergence is also appropriate for this habitat. The flora involves around 90 species, a number of which are rarely recorded specialists. Evidence is presented that substratum chemistry is a more powerful ecological factor than water chemistry. An attempt has been made to relate the lichen flora to homogeneous stream segments from the headwaters down to the alluvial lowlands. An investigation into the effects of water quality showed that both suspended solids (silting) and eutrophication are highly deleterious to the lichen flora; the submerged and fluvial mesic zones are the first to be affected. The zonation across streams in England is compared with those reported from Germany and found to be closely similar.

Over the last century, the distribution of Teloschistes flavicans has contracted from being widespread in the southern half of England and Wales to being limited to South-West England with outlying populations in Pembrokeshire and North Wales. Twelve core sites have been identified where the species is well established: ten of these are saxicolous/terricolous habitats on windy coastal cliff tops; the other two are lines of sycamore trees near the coast. At 39 further localities, some inland, the species is in very small amounts (often on one tree) and vulnerable to extinction. It is normally a member of the Parmelietum revolutae or Ramalinetum scopularis associations. The conservation of the species is discussed.
