Introduction
The recent release of powerful next-generation sequencing platforms, which can provide whole exome sequencing (WES) or whole genome sequencing (WGS) in quicker timelines and at reduced costs, has resulted in proposals for these diagnostic testing methods to be routinely integrated into clinical practice in multiple settings. However, the complexities of these diagnostic approaches, and the minimal comparative evidence available on them, creates difficulties in the evaluation of their diagnostic performance. Novel approaches need to be developed to improve the health technology assessment (HTA) of WES and WGS.
MethodsSeveral HTAs on genetic testing and the use of WES or WGS in fetal medicine were reviewed. Information on factors associated with this diagnostic modality that affect typical test accuracy assessment (e.g., sensitivity and specificity) was extracted. The multiple steps required for completing a WES or WGS test, and the potential for the introduction of errors (type I or type II) at each of these steps, were mapped and examples provided. The clinical and economic implications associated with imperfect and uncertain test accuracy were described.
ResultsLimited data on analytical and clinical validity were identified. WES and WGS are multistep processes and errors were found in sampling, molecular sequencing, bioinformatic filtering, and variant interpretation; therefore, the assumption that WES or WGS is 100 percent sensitive or specific is not reasonable. Although alternative evidence-based estimates are unlikely to be available, the inevitability of such errors, and their implications in terms of comparative effectiveness, safety, and cost effectiveness, should be described in HTAs.
ConclusionsWhile unknown diagnostic accuracy remains an issue with WES and WGS testing, formal sensitivity analysis of test performance characteristics should be conducted as part of HTAs. A checklist has been developed to assist those involved in HTA and policy to understand the potential for inaccurate test results in clinical practice, and the risk-benefit implications of these diagnostic errors for patients.

Aim:
To critically appraise the literature to determine availability and identify the cultural responsiveness of infant resuscitation education for Aboriginal and Torres Strait Islander populations.
Background:Despite overall reductions in infant mortality in the last two decades, Aboriginal people have some of the highest rates of infant mortality of any developed nation. One of the key factors that has attributed to improvements in infant mortality rates is parent and carer education around risk factors and actions of first responders. Identifying gaps in the current basic first-aid initiatives available to Aboriginal communities may contribute to developing resources to contribute to reductions in Aboriginal neonatal mortality rates.
Method:The review used key terms and Boolean operators across an 11-month time frame searching for research articles utilising the databases of CINAHL, Scopus, Ovid Emcare, Informit, Pubmed and Proquest. After review, 39 articles met the inclusion criteria, 25 articles were discarded due to irrelevant material and 14 articles were included in the structured literature review. The search process was developed using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines. Articles were assessed for validity and inclusion using the Critical Appraisal Skills Program checklist.
Results:Research literature relating to First Nation community-based CPR and first-aid education programmes in Canada, USA, India, UK and Europe, Asia and Africa were identified; however, none pertaining specifically to CPR and first-aid education in Australian Aboriginal communities were found.
Discussion:Despite the lack of research evidence relating to infant cardiopulmonary resuscitation (CPR) education for Australian Aboriginal populations, the reviewed studies noted the importance of culturally responsive education designed in collaboration with First Nation peoples, using novel ways of teaching CPR, that align with the language, culture and needs of the communities it is intended for.
Conclusion:Further research is required to create a framework for the delivery of culturally responsive infant resuscitation education for Australian Aboriginal parents and communities.

The relationship between simulated milrinone exposure and hypotension in children

Introduction:
Hypotension is an adverse event that may be related to systemic exposure of milrinone; however, the true exposure–safety relationship is unknown.
Methods:Using the Pediatric Trials Network multicentre repository, we identified children ≤17 years treated with milrinone. Hypotension was defined according to age, using the Pediatric Advanced Life Support guidelines. Clinically significant hypotension was defined as hypotension with concomitant lactate >3 mg/dl. A prior population pharmacokinetic model was used to simulate milrinone exposures to evaluate exposure–safety relationships.
Results:We included 399 children with a median (quarter 1, quarter 3) age of 1 year (0,5) who received 428 intravenous doses of milrinone (median infusion rate 0.31 mcg/kg/min [0.29,0.5]). Median maximum plasma milrinone concentration was 110.7 ng/ml (48.4,206.2). Median lowest systolic and diastolic blood pressures were 74 mmHg (60,85) and 35 mmHg (25,42), respectively. At least 1 episode of hypotension occurred in 178 (45%) subjects; clinically significant hypotension occurred in 10 (2%). The maximum simulated milrinone plasma concentrations were higher in subjects with clinically significant hypotension (251 ng/ml [129,329]) versus with hypotension alone (86 ng/ml [44, 173]) versus without hypotension (122 ng/ml [57, 208], p = 0.002); however, this relationship was not retained on multivariable analysis (odds ratio 1.01; 95% confidence interval 0.998, 1.01).
Conclusions:We successfully leveraged a population pharmacokinetic model and electronic health record data to evaluate the relationship between simulated plasma concentration of milrinone and systemic hypotension occurrence, respectively, supporting the broader applicability of our novel, efficient, and cost-effective study design for examining drug exposure–response and –safety relationships.
Constructions of older people's identities in Indonesian regional ageing policies: the impacts on micro and macro experiences of ageing

As Indonesia experiences rapid growth of the ageing population, the government's attention has turned to the wellbeing of older people. This study aims to review critically the construction of older people's identity and care within regional ageing policies in Indonesia. Working from a critical gerontology perspective, a critical discourse analysis of 16 regional ageing policies identified two constructions, labelled ‘material’ and ‘cultural’ ageing, which were used to position older people. The analysis showed that ‘material ageing’ positions older people at the intersection of ‘decline’ and ‘successful ageing’ discourses, while ‘cultural ageing’ positions older people's welfare at the intersection of ‘public responsibility’ and ‘family obligation’ discourses. These discursive constructions in the policy documents have both micro (interpersonal) and macro (structural) constructive effects. At the micro-level, the regional ageing policies stand at a crossroad between empowering and marginalising older people and their families. While the dominant discourse of ‘successful ageing’ encourages older people to be healthy, it marginalises those who do not, or cannot, meet its criteria, undermining a rehabilitative approach as a policy priority. In addition, the rights of the family are overlooked, despite being a pivotal element of cultural ageing. At the macro level, a moral dilemma appears in defining the public and private domains of older people's welfare. Eligibility requirements for state assistance (due to budgetary constraints) ensure that elder care is often relegated to the private sphere, without support. Recommendations for policy improvement are discussed, including the recognition of families’ rights and the importance of local cultural practices in providing care for older people.

The role that vitamin D plays in pulmonary function remains uncertain. Epidemiological studies reported mixed findings for serum 25-hydroxyvitamin D (25(OH)D)–pulmonary function association. We conducted the largest cross-sectional meta-analysis of the 25(OH)D–pulmonary function association to date, based on nine European ancestry (EA) cohorts (n 22 838) and five African ancestry (AA) cohorts (n 4290) in the Cohorts for Heart and Aging Research in Genomic Epidemiology Consortium. Data were analysed using linear models by cohort and ancestry. Effect modification by smoking status (current/former/never) was tested. Results were combined using fixed-effects meta-analysis. Mean serum 25(OH)D was 68 (sd 29) nmol/l for EA and 49 (sd 21) nmol/l for AA. For each 1 nmol/l higher 25(OH)D, forced expiratory volume in the 1st second (FEV1) was higher by 1·1 ml in EA (95 % CI 0·9, 1·3; P<0·0001) and 1·8 ml (95 % CI 1·1, 2·5; P<0·0001) in AA (Prace difference=0·06), and forced vital capacity (FVC) was higher by 1·3 ml in EA (95 % CI 1·0, 1·6; P<0·0001) and 1·5 ml (95 % CI 0·8, 2·3; P=0·0001) in AA (Prace difference=0·56). Among EA, the 25(OH)D–FVC association was stronger in smokers: per 1 nmol/l higher 25(OH)D, FVC was higher by 1·7 ml (95 % CI 1·1, 2·3) for current smokers and 1·7 ml (95 % CI 1·2, 2·1) for former smokers, compared with 0·8 ml (95 % CI 0·4, 1·2) for never smokers. In summary, the 25(OH)D associations with FEV1 and FVC were positive in both ancestries. In EA, a stronger association was observed for smokers compared with never smokers, which supports the importance of vitamin D in vulnerable populations.

Vapor movement of synthetic auxin herbicides can injure desirable plants outside the treatment zone. Vapor movement of the synthetic auxin herbicides aminocyclopyrachlor and aminocyclopyrachlor methyl was compared with that of the relatively volatile herbicide dicamba and the low volatile herbicide aminopyralid with a soybean bioassay under greenhouse and field conditions. Soybean is very sensitive to these active ingredients. Under greenhouse conditions, 82 (61 to 104) mg ae ha−1 of aminocyclopyrachlor, 26 (18 to 33) mg ae ha−1 of aminocyclopyrachlor methyl, 82 (69 to 95) mg ae ha−1 of aminopyralid, and 61 (47 to 75) mg ae ha−1 of dicamba produced an estimated 25% visual soybean phytotoxicity response when soybean was treated POST at the V3 growth stage (GR25 [95% confidence interval]). In field studies, aminocyclopyrachlor, aminocyclopyrachlor methyl, and aminopyralid were applied at 70 g ae ha−1 and dicamba was applied at 560 g ae ha−1 (labeled application rates) to soybean at the V3 growth stage. All herbicides were applied within an enclosed chamber (3 m by 3 m by 1 m) to mitigate movement of spray droplets. The enclosures were removed shortly after spray application and soybean response immediately surrounding the treated area was recorded in each of eight directions approximately 10 d after treatment. On the basis of bioassay responses, relative amount of vapor movement was dicamba > aminocyclopyrachlor methyl > aminopyralid ≈ aminocyclopyrachlor. Vapor movement of aminocyclopyrachlor was very low indicating that the risk of phytotoxic response of sensitive plants due to volatility of aminocyclopyrachlor is negligible.

Urban ethnic minority youth are often exposed to high levels of aggression and violence. As such, many aggression intervention programs that have been designed with suburban nonethnic minority youth have been used or slightly adapted in order to try and meet the needs of high-risk urban youth. The current study contributes to the literature base by examining how well a range of social–cognitive, emotional distress and victimization, and prosocial factors are related to youth aggression in a sample of urban youth. This study utilized data gathered from 109 9- to 15-year-old youth (36.7% male; 84.4% African American) and their parents or caregivers. A series of hierarchical multiple regressions were fit predicting youth aggression from social–cognitive variables, victimization and distress, and prosocial variables, controlling for youth gender and age. Each set of variables explained a significant and unique amount of the variance in youth aggressive behavior. The full model including all predictors accounted for 41% of the variance in aggression. Models suggest that youth with stronger beliefs supportive of violence, youth who experience more overt victimization, and youth who experience greater distress in overtly aggressive situations are likely to be more aggressive. In contrast, youth with higher self-esteem and youth who endorse greater leadership efficacy are likely to be less aggressive. Contrary to hypotheses, hostile attributional bias and knowledge of social information processing, experience of relational victimization, distress in relationally aggressive situations, and community engagement were not associated with aggression. Our study is one of the first to address these important questions for low-income, predominately ethnic minority urban youth, and it has clear implications for adapting aggression prevention programs to be culturally sensitive for urban African American youth.

We tested the hypothesis that the conophagous Douglas-fir cone gall midge, Contarinia oregonensis Foote (Diptera: Cecidomyiidae), responds to infrared (IR) radiation and other electromagnetic wavelengths associated with cones of Douglas-fir, Pseudotsuga menziesii (Mirbel) Franco (Pinaceae). Early-season (March–April) thermographic images showed that cone orientation (upright, horizontal, pendant) and cone colour (green, purple, green/purple) did not affect apparent cone temperature (inferred from thermographic images). Tree components significantly differed in apparent temperature with foliage being coolest and branches warmest. There was no significant difference in the number of larvae in cones of different colours, and adult midges were equally attracted to traps painted green or purple, suggesting that cone colour does not affect oviposition decisions by gravid females. Adult midges were more strongly attracted to warm traps with IR frequency emissions higher than the background than to cold traps with IR frequency emissions lower than the background. They were also more strongly attracted to warm branch-shaped traps than to warm can-shaped traps. Collectively, these data indicate that the shape and IR attributes of Douglas-fir branches may serve as foraging cues for C. oregonensis.

Several single nucleotide polymorphisms (SNPs) in candidate genes of DNA repair and hormone pathways have been reported to be associated with endometrial cancer risk. We sought to confirm these associations in two endometrial cancer case-control sample sets and used additional data from an existing genome-wide association study to prioritize an additional SNP for further study. Five SNPs from the CHEK2, MGMT, SULT1E1 and SULT1A1 genes, genotyped in a total of 1597 cases and 1507 controls from two case-control studies, the Australian National Endometrial Cancer Study and the Polish Endometrial Cancer Study, were assessed for association with endometrial cancer risk using logistic regression analysis. Imputed data was drawn for CHEK2 rs8135424 for 666 cases from the Study of Epidemiology and Risk factors in Cancer Heredity study and 5190 controls from the Wellcome Trust Case Control Consortium. We observed no association between SNPs in the MGMT, SULT1E1 and SULT1A1 genes and endometrial cancer risk. The A allele of the rs8135424 CHEK2 SNP was associated with decreased risk of endometrial cancer (adjusted per-allele OR 0.83; 95%CI 0.70-0.98; p = .03) however this finding was opposite to that previously published. Imputed data for CHEK2 rs8135424 supported the direction of effect reported in this study (OR 0.85; 95% CI 0.65–1.10). Previously reported endometrial cancer risk associations with SNPs from in genes involved in estrogen metabolism and DNA repair were not replicated in our larger study population. This study highlights the need for replication of candidate gene SNP studies using large sample groups, to confirm risk associations and better prioritize downstream studies to assess the causal relationship between genetic variants and cancer risk. Our findings suggest that the CHEK2 SNP rs8135424 be prioritized for further study as a genetic factor associated with risk of endometrial cancer.

We administered the Multiscale Depression Inventory (MDI) and the Beck Depression Inventory (BDI) to 84 multiple sclerosis (MS) patients, 101 patients diagnosed with major depression and 87 nonmedical, nonpsychiatric controls. The MDI consists of three separate depression scales measuring mood, vegetative, and evaluative symptoms. We found that: (a) MS patients did not significantly differ from the controls in mood symptoms, (b) the depression prevalence rate in MS patients was significantly lower when measured by the mood scale (17·7%) than by the BDI (30·5%) or MDI total score (26·6%), and (c) MS patients showed significantly less mood disturbance than a non-MS comparison group matched on BDI measured depression severity. We suggest that the inclusion of nonmood symptoms in self-report depression scales may artificially raise both prevalence rates and severity ratings of MS related depression and that the most valid measure of depression in MS is mood disturbance. Measuring The Work Functions Of PVD TaN, TaSiN And TiSiN Films With A Schottky Diode CV Technique For Metal Gate CMOS Applications

The metal gate process becomes a promising candidate for sub-65nm CMOS, due to the elimination of polysilicon depletion effects, and the possibility of adjusting the CMOS threshold voltage without more threshold implants. Our goal is to process mteal films with tunable work functions, in order to meet the demand of sub-65nm metal gate CMOS.
PVD TaN films are deposited with various processing conditions. Auger analysis shows that by changing the nitrogen flow rate and the plasma power, the nitrogen content in the TaN films can be adjusted. In order to accurately determine the work function of these TaN materials, we have developed a Schottky Diode CV technique (or Metal-Silicon CV, or MS-CV). This approach not only improves the accuracy of the metal work function measurement, compared with the traditional MOS-CV technique (which is affected by the thickness and quality of the oxide), but also simplifies the fabrication.
With the MS-CV's, we have successfully measured the work functions of Ni and Co, and compared the data with published references. The work function of PVD TaN actually decreases with higher nitrogen content, according to the Auger data and the MS-CV measurement, ranging from 3.42 – 4.20 Volts. The MS-CV technique is shown to be independent to the size of the capacitors, and is little affected by the measurement frequency. By changing the frequency from 100KHz to 1MHz, the error in the work function is less than 50mV.
