Conducting a pediatric randomized clinical trial during a pandemic: A shift to virtual procedures

Background/Objective:
Prior to the COVID-19 pandemic, our research group initiated a pediatric practice-based randomized trial for the treatment of childhood obesity in rural communities. Approximately 6 weeks into the originally planned 10-week enrollment period, the trial was forced to pause all study activity due to the COVID-19 pandemic. This pause necessitated a substantial revision in recruitment, enrollment, and other study methods in order to complete the trial using virtual procedures. This descriptive paper outlines methods used to recruit, enroll, and manage clinical trial participants with technology to obtain informed consent, obtain height and weight measurements by video, and maintain participant engagement throughout the duration of the trial.
Methods:The study team reviewed the IRB records, protocol team meeting minutes and records, and surveyed the site teams to document the impact of the COVID-19 shift to virtual procedures on the study. The IRB approved study changes allowed for flexibility between clinical sites given variations in site resources, which was key to success of the implementation.
Results:All study sites faced a variety of logistical challenges unique to their location yet successfully recruited the required number of patients for the trial. Ultimately, virtual procedures enhanced our ability to establish relationships with participants who were previously beyond our reach, but presented several challenges and required additional resources.
Conclusion:Lessons learned from this study can assist other study groups in navigating challenges, especially when recruiting and implementing studies with rural and underserved populations or during challenging events like the pandemic.

Sealings recovered from the Omo M10 temple, a provincial center of the Andean Tiwanaku state (AD 500–1100), and from the Muru Ut Pata neighborhood of the Tiwanaku capital, as well as a signet ring from the Akapana East complex of Tiwanaku's highland capital, shed light on the hitherto undocumented use of seals and sealings in Central Andean complex society. The identification of Tiwanaku sealings related to the signet ring seal has implications for understanding the transmission of identity, authority, and authenticity over time and distance in early Andean states.
A new percussion hammer mechanism for a borehole deployable subglacial sediment corer

Subglacial sediments have the potential to reveal information about the controls on glacier flow, changes in ice-sheet history and characterise life in those environments. Retrieving sediments from beneath the ice, through hot water drilled access holes at remote field locations, present many challenges. Motivated by the need to minimise weight, corer diameter and simplify assembly and operation, British Antarctic Survey, in collaboration with UWITEC, developed a simple mechanical percussion corer. At depths over 1000 m however, manual operation of the percussion hammer is compromised by the lack of clear operator feedback at the surface. To address this, we present a new auto-release-recovery percussion hammer mechanism that makes coring operations depth independent and improves hammer efficiency. Using a single rope tether for both the corer and hammer operation, this modified percussion corer is relatively simple to operate, easy to maintain, and has successfully operated at a depth of >2130 m.
Impacts of invasive rats and tourism on a threatened island bird: the Palau Micronesian Scrubfowl

Invasive predators have decimated island biodiversity worldwide. Rats (Rattus spp.) are perhaps the greatest conservation threat to island fauna. The ground nesting Palau Micronesian Scrubfowl Megapodius laperouse senex (Megapodiidae) inhabits many of the islands of Palau’s Rock Island Southern Lagoon Conservation Area (RISL) in the western Pacific. These islands are also heavily visited by tourists and support populations of introduced rats, both of which may act as added stressors for the scrubfowl. Using passive chew-tag and call playback surveys on five tourist-visited and five tourist-free islands, we investigated if rats and tourists negatively affect scrubfowl, and if higher rat activity is associated with tourist presence. Rat detection probability and site occupancy were significantly higher on tourist visited (89% and 99%, respectively) compared to tourist-free islands (52% and 73%). Scrubfowl were detected at significantly more stations on tourist-free (93%) than tourist visited (47%) islands and their relative abundance was higher (2.66 and 1.58 birds per station, respectively), although not statistically significantly. While rat occupancy probability likewise had a non-significant negative effect on scrubfowl numbers across islands, our results show a negative relationship between tourist presence and scrubfowl in the RISL. Our findings also suggest that rat populations may be augmented by tourist visitation in the RISL. Although this situation may not seriously affect the scrubfowl, it may be highly detrimental to populations of other threatened island landbirds.

Objective:
To develop and validate the Discrepancy-based Evidence for Loss of Thinking Abilities (DELTA) score. The DELTA score characterizes the strength of evidence for cognitive decline on a continuous spectrum using well-established psychometric principles for improving detection of cognitive changes.
Methods:DELTA score development used neuropsychological test scores from the Alzheimer’s Disease Neuroimaging Initiative (ADNI) cohort (two tests each from Memory, Executive Function, and Language domains). We derived regression-based normative reference scores using age, gender, years of education, and word-reading ability from robust cognitively normal ADNI participants. Discrepancies between predicted and observed scores were used for calculating the DELTA score (range 0–15). We validated DELTA scores primarily against longitudinal Clinical Dementia Rating-Sum of Boxes (CDR-SOB) and Functional Activities Questionnaire (FAQ) scores (baseline assessment through Year 3) using linear mixed models and secondarily against cross-sectional Alzheimer’s biomarkers.
Results:There were 1359 ADNI participants with calculable baseline DELTA scores (age 73.7 ± 7.1 years, 55.4% female, 100% white/Caucasian). Higher baseline DELTA scores (stronger evidence of cognitive decline) predicted higher baseline CDR-SOB (ΔR2 = .318) and faster rates of CDR-SOB increase over time (ΔR2 = .209). Longitudinal changes in DELTA scores tracked closely and in the same direction as CDR-SOB scores (fixed and random effects of mean + mean-centered DELTA, ΔR2 > .7). Results were similar for FAQ scores. High DELTA scores predicted higher PET-Aβ SUVr (ρ = 324), higher CSF-pTau/CSF-Aβ ratio (ρ = .460), and demonstrated PPV > .9 for positive Alzheimer’s disease biomarker classification.
Conclusions:Data support initial development and validation of the DELTA score through its associations with longitudinal functional changes and Alzheimer’s biomarkers. We provide several considerations for future research and include an automated scoring program for clinical use.
Aligning systems science and community-based participatory research: A case example of the Community Health Advocacy and Research Alliance (CHARA)

Partnered research may help bridge the gap between research and practice. Community-based participatory research (CBPR) supports collaboration between scientific researchers and community members that is designed to improve capacity, enhance trust, and address health disparities. Systems science aims to understand the complex ways human-ecological coupled systems interact and apply knowledge to management practices. Although CBPR and systems science display complementary principles, only a few articles describe synergies between these 2 approaches. In this article, we explore opportunities to utilize concepts from systems science to understand the development, evolution, and sustainability of 1 CBPR partnership: The Community Health Advocacy and Research Alliance (CHARA). Systems science tools may help CHARA and other CBPR partnerships sustain their core identities while co-evolving in conjunction with individual members, community priorities, and a changing healthcare landscape. Our goal is to highlight CHARA as a case for applying the complementary approaches of CBPR and systems science to (1) improve academic/community partnership functioning and sustainability, (2) ensure that research addresses the priorities and needs of end users, and (3) support more timely application of scientific discoveries into routine practice.

Background
Obesity is a major challenge for people with schizophrenia.
AimsWe assessed whether STEPWISE, a theory-based, group structured lifestyle education programme could support weight reduction in people with schizophrenia.
MethodIn this randomised controlled trial (study registration: ISRCTN19447796), we recruited adults with schizophrenia, schizoaffective disorder or first-episode psychosis from ten mental health organisations in England. Participants were randomly allocated to the STEPWISE intervention or treatment as usual. The 12-month intervention comprised four 2.5 h weekly group sessions, followed by 2-weekly maintenance contact and group sessions at 4, 7 and 10 months. The primary outcome was weight change after 12 months. Key secondary outcomes included diet, physical activity, biomedical measures and patient-related outcome measures. Cost-effectiveness was assessed and a mixed-methods process evaluation was included.
ResultsBetween 10 March 2015 and 31 March 2016, we recruited 414 people (intervention 208, usual care 206) with 341 (84.4%) participants completing the trial. At 12 months, weight reduction did not differ between groups (mean difference 0.0 kg, 95% CI −1.6 to 1.7, P = 0.963); physical activity, dietary intake and biochemical measures were unchanged. STEPWISE was well-received by participants and facilitators. The healthcare perspective incremental cost-effectiveness ratio was £246 921 per quality-adjusted life-year gained.
ConclusionsParticipants were successfully recruited and retained, indicating a strong interest in weight interventions; however, the STEPWISE intervention was neither clinically nor cost-effective. Further research is needed to determine how to manage overweight and obesity in people with schizophrenia.

Vulnerability of megapodes (Megapodiidae, Aves) to climate change and related threats

Aspects of species life histories may increase their susceptibility to climate change. Owing to their exclusive reliance on environmental sources of heat for incubation, megapodes may be especially vulnerable. We employed a trait-based vulnerability assessment to weigh their exposure to projected climate variables of increasing temperatures, fluctuating rainfall and sea level rise and their biological sensitivity and capacity to adapt. While all 21 species were predicted to experience at least a 2 °C increase in mean annual temperature, 12 to experience a moderate or greater fluctuation in rainfall and 16 to experience rising seas, the most vulnerable megapodes are intrinsically rare and range restricted. Species that employ microbial decomposition for incubation may have an adaptive advantage over those that do not and may be more resilient to climate change. The moderate microclimate necessary for mound incubation, however, may in some areas be threatened by anthropogenic habitat loss exacerbated by warmer and seasonally drier conditions. As with many avian species, little is known about the capacity of megapodes to adapt to a changing climate. We therefore recommend that future research efforts investigate megapode fecundity, gene flow and genetic connectivity at the population level to better determine their adaptive capacity.
Maternal fish consumption during pregnancy and smoking behavioural patterns

n-3 Highly unsaturated fatty acids (HUFA), are essential components of neuronal membranes and mediate a range of complex bioactive properties including gene expression, myelination, cell-signalling and dopaminergic function. Deficits in n-3 HUFA have been linked to increased risks for addictive disorders, thus we posited that lower fish consumption would be associated with greater risks for perinatal smoking among 9640 mothers enroled in the Avon Longitudinal Study of Parents and Children. We used univariable and multivariable regression models to examine relationships between self-reported prenatal dietary intakes of n-3 HUFA-rich foods (fish and shellfish) and maternal smoking; outcomes included cessation and the number of cigarettes smoked per d. Both before and during pregnancy, there was consistent evidence (P<0·001) of protective fish intake–smoking associations; relative to mothers reporting no fish consumption, those who reported some fish consumption (<340 g/week) and high fish consumption (340 g+/week) at 32 weeks of gestation showed lower likelihoods of smoking (adjusted P values <0·001). Respective OR for these relationships were 0·87 (95% CI 0·77, 0·97) and 0·73 (95% CI 0·61, 0·86). Although the prevalence of smoking diminished, from a high of 31·6% (pre-pregnancy) to a low of 18·7% (second trimester), the magnitude of fish intake–smoking associations remained stable following adjustment for confounders. These observations suggest that greater fish or n-3 HUFA consumption should be evaluated as an intervention to reduce or prevent smoking in randomised clinical trials.
Late-Season Weed Escape Survey Reveals Discontinued Atrazine Use Associated with Greater Abundance of Broadleaf Weeds

Atrazine has been used for control of many weeds, primarily broadleaf weeds, in U.S. corn fields since 1957. Recently, the adoption of glyphosate-resistant corn hybrids have led to glyphosate eclipsing atrazine as the most commonly used herbicide in corn production. However, the evolution and spread of glyphosate-resistant weeds is a major concern. Atrazine use in Wisconsin is prohibited in 102 areas encompassing 0.49 million ha where total chlorinated residues were found in drinking water wells at concentrations > 3 μg L−1. Atrazine has been prohibited in many of those areas for > 10 yr, providing an opportunity to evaluate weed community composition differences due to herbicide regulation. In question, has the abundance of broadleaf weeds increased, coupled with an increased reliance on glyphosate, where atrazine use has been discontinued? To answer this, an online questionnaire was distributed to Wisconsin growers in June and then weeds present in 343 fields in late July through mid-September in 2012 and 2013 were counted. Data were summarized for frequency, uniformity, density, and relative abundance to compare weed community composition in fields with discontinued vs. recent atrazine use. Growers used glyphosate in 70 vs. 54% of fields with discontinued vs. recent atrazine use, respectively (P = 0.021). Moreover, broadleaf weeds were found more frequently, (73 vs. 61%; P = 0.03), they had 50% greater in-field uniformity (P = 0.002), and density was 0.4 vs. 0.19 plants m−2 (i.e., twofold greater; P < 0.0001) in discontinued vs. recent atrazine-use fields. Changes were most evident with troublesome glyphosate-resistant broadleaf weeds such as Amaranthus species and giant ragweed. In conclusion, weed community composition consisted of more broadleaf weeds in fields where atrazine has not been used in the recent decade coupled with greater glyphosate use. These results provide evidence of negative long-term implications for glyphosate resistance where growers increased reliance on glyphosate in place of atrazine.

Two broad aims drive weed science research: improved management and improved understanding of weed biology and ecology. In recent years, agricultural weed research addressing these two aims has effectively split into separate subdisciplines despite repeated calls for greater integration. Although some excellent work is being done, agricultural weed research has developed a very high level of repetitiveness, a preponderance of purely descriptive studies, and has failed to clearly articulate novel hypotheses linked to established bodies of ecological and evolutionary theory. In contrast, invasive plant research attracts a diverse cadre of nonweed scientists using invasions to explore broader and more integrated biological questions grounded in theory. We propose that although studies focused on weed management remain vitally important, agricultural weed research would benefit from deeper theoretical justification, a broader vision, and increased collaboration across diverse disciplines. To initiate change in this direction, we call for more emphasis on interdisciplinary training for weed scientists, and for focused workshops and working groups to develop specific areas of research and promote interactions among weed scientists and with the wider scientific community.
Does Timing Influence the Utility of Reduced Atrazine Rates for Proactive Resistance Management?

Atrazine is an important herbicide for broadleaf weed control in corn. Use rates have declined in many corn production systems due to environmental concerns and the availability of other effective herbicides, especially glyphosate in glyphosate-resistant hybrids. However, using multiple effective herbicide modes of action is ever more important because occurrence of herbicide-resistant weeds is increasing. An experiment to compare application timings of reduced rates of atrazine to benefit resistance management in broadleaf weeds while protecting corn yield was conducted in Wisconsin across four site-years in 2012 and 2013. Herbicide treatments consisted of five atrazine rate and timing combinations and three POST base herbicides: glyphosate, glufosinate, and tembotrione. Metolachlor was applied PRE at 2.1 kg ai ha−1 for grass control in all treatments. A linear regression model estimated that atrazine rates ≥ 1.0 kg ai ha−1 applied PRE would prevent exposure of common lambsquarters plants to POST herbicides, but giant ragweed and velvetleaf exposure was not influenced by timing. Corn yield was also not influenced by atrazine rate and timing combinations at the α = 0.05 level; however, at P = 0.06, corn yield was greater for atrazine applied PRE at 1.1 kg ha−1 than for atrazine applied PRE at 0.5 kg ha−1, POST at 1.1 kg ha−1, or not at all. In summary, higher rates of atrazine applied PRE may improve yield, as reported by others, but this study concludes reduced rates of atrazine (i.e., ≤ 1.1 kg ha−1) applied to corn in a POST tank mixture combination provided more consistent control of giant ragweed, velvetleaf, and common lambsquarters compared with atrazine applied PRE. This information should help direct atrazine application timing applied POST when applied at low rates to improve proactive herbicide resistance management.

The anticipated release of EnlistTM cotton, corn, and soybean cultivars likely will increase the use of 2,4-D, raising concerns over potential injury to susceptible cotton. An experiment was conducted at 12 locations over 2013 and 2014 to determine the impact of 2,4-D at rates simulating drift (2 g ae ha−1) and tank contamination (40 g ae ha−1) on cotton during six different growth stages. Growth stages at application included four leaf (4-lf), nine leaf (9-lf), first bloom (FB), FB + 2 wk, FB + 4 wk, and FB + 6 wk. Locations were grouped according to percent yield loss compared to the nontreated check (NTC), with group I having the least yield loss and group III having the most. Epinasty from 2,4-D was more pronounced with applications during vegetative growth stages. Importantly, yield loss did not correlate with visual symptomology, but more closely followed effects on boll number. The contamination rate at 9-lf, FB, or FB + 2 wk had the greatest effect across locations, reducing the number of bolls per plant when compared to the NTC, with no effect when applied at FB + 4 wk or later. A reduction of boll number was not detectable with the drift rate except in group III when applied at the FB stage. Yield was influenced by 2,4-D rate and stage of cotton growth. Over all locations, loss in yield of greater than 20% occurred at 5 of 12 locations when the drift rate was applied between 4-lf and FB + 2 wk (highest impact at FB). For the contamination rate, yield loss was observed at all 12 locations; averaged over these locations yield loss ranged from 7 to 66% across all growth stages. Results suggest the greatest yield impact from 2,4-D occurs between 9-lf and FB + 2 wk, and the level of impact is influenced by 2,4-D rate, crop growth stage, and environmental conditions.

The USDA's Market Access Program (formerly Market Promotion Program) recently underwent a major change to redirect all branded products export promotion funds to small domestic firms and cooperatives. The redirection responded to criticisms by the General Accounting Office of past allocations of branded products export promotion funds to large, experienced exporters. This study uses a firm-level analysis to examine whether firm size and export experience matter in how effectively firms use the promotion funds to increase their revenues. The results support neither the GAO criticisms nor the recent program redirection.

Background
Trials evaluating efficacy of omega-3 highly unsaturated fatty acids (HUFAs) in major depressive disorder report discrepant findings.
AimsTo establish the reasons underlying inconsistent findings among randomised controlled trials (RCTs) of omega-3 HUFAs for depression and to assess implications for further trials.
MethodA systematic bibliographic search of double-blind RCTs was conducted between January 1980 and July 2014 and an exploratory hypothesis-testing meta-analysis performed in 35 RCTs including 6665 participants receiving omega-3 HUFAs and 4373 participants receiving placebo.
ResultsAmong participants with diagnosed depression, eicosapentaenoic acid (EPA)-predominant formulations (>50% EPA) demonstrated clinical benefits compared with placebo (Hedge's G = 0.61, P<0.001) whereas docosahexaenoic acid (DHA)-predominant formulations (>50% DHA) did not. EPA failed to prevent depressive symptoms among populations not diagnosed for depression.
ConclusionsFurther RCTs should be conducted on study populations with diagnosed or clinically significant depression of adequate duration using EPA-predominant omega-3 HUFA formulations.
