Recurrent acute otitis media: a survey of current management in England

Objective
Recurrent acute otitis media is common in children. The preferred treatment measures for recurrent acute otitis media have a mixed evidence base. This study sought to assess baseline practice across ENT departments in England.
MethodsA national telephone survey of healthcare staff was conducted. Every ENT centre in England was contacted. A telephone script was used to ask about antibiotic and grommet use and duration in recurrent acute otitis media cases.
ResultsNinety-six centres (74 per cent) provided complete information. Recurrent acute otitis media treatment across England by ENT departments varied. The antibiotic first- and second-line prophylaxis offered varies, with trimethoprim used in 33 centres and 29 centres not offering any antibiotics. The timing or choice about when to use grommets also varies, but 87 centres (91 per cent) offer grommet surgery at one stage.
ConclusionThe treatments received by children in England for recurrent acute otitis media vary by centre; collaborative research in this area is advised.

Introduction
Out of hours, there is only one on-site junior doctor. First year psychiatry trainees (CT1s) and GP trainees may have no prior experience in psychiatry. On-call shifts are therefore potentially daunting for new trainees.
ObjectivesExpand the resources available for trainees when on-call.
MethodsWe issued questionnaires to CT1s asking if they would have appreciated more information about on-call scenarios and in what format.
Based on the questionnaire results we implemented some changes. These were:
– a printed “pocket-guide” summarising common on-call scenarios;
– a training video on common on-call scenarios.
The handout was given to new trainees in February 2016 and in August 2016. The video was shown to new trainees in August 2016. Trainees provided feedback on the resources.
ResultsOf 24 CT1s, 15 (63%) were “neutral” or “disagreed” that they had felt prepared for on-calls.
CT1s wanted additional resources, especially a paper handout or phone download.
Feedback on the “pocket-guide” from trainees in February 2016 (n = 8) was positive (62.5% reported increased confidence in on-call situations). Feedback is also being collected from trainees who received the guide in August 2016.
Trainees in August 2016 (n = 36) liked the video – no trainees “disagreed” with statements asking if the video had been useful.
The video improved the confidence of trainees about on-call situations by an average of 2.8 points.
ConclusionsWe have expanded available resources relating to on-calls and improved confidence. Further improvements would include making resources more easily available in downloadable formats.

Interleukin-6 as a predictor of symptom resolution in psychological distress: a cohort study

Background
Elevated levels of interleukin-6 (IL-6) have been associated with the development of common mental disorders, such as depression, but its role in symptom resolution is unclear.
MethodWe examined the association between IL-6 and symptom resolution in a non-clinical sample of participants with psychological distress.
ResultsRelative to high IL-6 levels, low levels at baseline were associated with symptom resolution at follow-up [age- and sex-adjusted risk ratio (RR) = 1.15, 95% confidence interval (CI) 1.06–1.25]. Further adjustment for covariates had little effect on the association. Symptomatic participants with repeated low IL-6 were more likely to be symptom-free at follow-up compared with those with repeated high IL-6 (RR = 1.21, 95% CI 1.03–1.41). Among the symptomatic participants with elevated IL-6 at baseline, IL-6 decreased along with symptom resolution.
ConclusionsIL-6 is potentially related to the mechanisms underlying recovery from symptoms of mental ill health. Further studies are needed to examine these mechanisms and to confirm the findings in relation to clinical depression.

The Millimetre Astronomy Legacy Team 90 GHz (MALT90) survey aims to characterise the physical and chemical evolution of high-mass star-forming clumps. Exploiting the unique broad frequency range and on-the-fly mapping capabilities of the Australia Telescope National Facility Mopra 22 m single-dish telescope1, MALT90 has obtained 3′ × 3′ maps towards ~2 000 dense molecular clumps identified in the ATLASGAL 870 μm Galactic plane survey. The clumps were selected to host the early stages of high-mass star formation and to span the complete range in their evolutionary states (from prestellar, to protostellar, and on to $\mathrm{H\,{\scriptstyle {II}}}$ regions and photodissociation regions). Because MALT90 mapped 16 lines simultaneously with excellent spatial (38 arcsec) and spectral (0.11 km s−1) resolution, the data reveal a wealth of information about the clumps’ morphologies, chemistry, and kinematics. In this paper we outline the survey strategy, observing mode, data reduction procedure, and highlight some early science results. All MALT90 raw and processed data products are available to the community. With its unprecedented large sample of clumps, MALT90 is the largest survey of its type ever conducted and an excellent resource for identifying interesting candidates for high-resolution studies with ALMA.
Effects of cortisol on memory in women with borderline personality disorder: role of co-morbid post-traumatic stress disorder and major depression

Background
Stress and cortisol administration are known to have impairing effects on memory retrieval in healthy humans. These effects are reported to be altered in patients with major depressive disorder (MDD) and post-traumatic stress disorder (PTSD) but they have not yet been investigated in borderline personality disorder (BPD).
MethodIn a placebo-controlled cross-over study, 71 women with BPD and 40 healthy controls received either placebo or 10 mg of hydrocortisone orally before undertaking a declarative memory retrieval task (word list learning) and an autobiographical memory test (AMT). A working memory test was also applied.
ResultsOverall, opposing effects of cortisol on memory were observed when comparing patients with controls. In controls, cortisol had impairing effects on memory retrieval whereas in BPD patients cortisol had enhancing effects on memory retrieval of words, autobiographical memory and working memory. These effects were most pronounced for specificity of autobiographical memory retrieval. Patients with BPD alone and those with co-morbid PTSD showed this effect. We also found that co-morbid MDD influenced the cortisol effects: in this subgroup (BPD + MDD) the effects of cortisol on memory were absent.
ConclusionsThe present results demonstrate beneficial effects of acute cortisol elevations on hippocampal-mediated memory processes in BPD. The absence of these effects in patients with co-morbid MDD suggests that these patients differ from other BPD patients in terms of their sensitivity to glucocorticoids (GCs).

Retained austenite is an important characteristic of properly heat-treated steel components, particularly gears and shafts, that will be subjected to long-term use and wear. Normally, either X-ray diffraction or optical microscopy techniques are used to determine the volume percent of retained austenite present in steel components subjected to specific heat-treatment regimes. As described in the literature, a number of phenomenological, experimental, and calculation factors can influence the volume fraction of retained austenite determined from X-ray diffraction measurements. However, recent disagreement between metallurgical properties, microscopy, and service laboratory values for retained austenite led to a re-evaluation of possible reasons for the apparent discrepancies. Broad, distorted X-ray peaks from un-tempered martensite were found to yield unreliable integrated intensities whereas diffraction peaks from tempered samples were more amenable to profile fitting with standard shape functions, yielding reliable integrated intensities. Retained austenite values calculated from reliable integrated intensities were found to be consistent with values obtained by Rietveld refinement of the diffraction patterns. The experimental conditions used by service laboratories combined with a poor choice of diffraction peaks were found to be sources of retained austenite values containing significant bias.

Commercially farmed animals are frequently housed in conditions that impose a number of concurrent environmental stressors. For pigs housed indoors, elevated levels of mechanical noise, atmospheric ammonia and low light intensities are commonplace. This experiment examined the effects on growing pigs of chronic exposure to combinations of commercially relevant levels of these potential stressors. Four-week-old hybrid female pigs (n = 224) were housed under experimentally manipulated conditions of nominally either <5 or 20 ppm atmospheric concentration of ammonia (24 h), a light intensity of 40 lux or 200 lux (12 h) and mechanical noise at either ⩽60 or 80 dB(A) (24 h) for 15 weeks in a fully factorial arrangement (23) of treatments. The response of pigs to these environmental factors was assessed using a suite of physiological, production and behavioural measures. These included indicators of hypothalamic–pituitary–adrenal (HPA) axis activation such as salivary cortisol and adrenal morphometry, as well as body weight, food conversion efficiency and general health scores. Play behaviour was recorded as it is thought to be inversely related to stress. Chronic exposure to ammonia produced the strongest effect, shown by lower concentrations of salivary cortisol and larger adrenal cortices in the pigs reared under 20 ppm ammonia, which may have been indicative of a period of HPA activation leading to a downregulation of cortisol production. The pigs in the ammoniated rooms also performed less play behaviour than pigs in non-ammoniated rooms. There was evidence for an interaction between high noise and ammonia on the health scores of pigs and for brighter light to ameliorate the effect of ammonia on salivary cortisol. However, there was no measurable impact of these potential stressors on the productivity of the pigs or any of the other physiological parameters measured. We conclude that there should be little concern in terms of performance about the physical stressors tested here, within current European Union legal limits. However, 20 ppm ammonia may have had an adverse influence on the well-being of growing pigs. In this study, all other aspects of the pigs’ husbandry were optimal; therefore, it is possible that under less favourable conditions, more pronounced effects of ammonia, noise and dim light would be observed.

The effects of common and concurrent environmental stressors on the social behaviour of farm animals are poorly understood. Here, we report the results of a multifactorial experiment designed specifically to examine the individual, additive or interactive effects of elevated ammonia, noise and low light (LL) levels on the social behaviour of growing pigs. Social behaviour was measured in terms of the nature, frequency and duration of both initiated and response behaviours for 4 weeks following mixing of the groups. General activity patterns, group cohesion and social discrimination were also examined as a function of the environmental treatments. Elevated concentrations of atmospheric ammonia (∼20 v. <5 ppm) and LL intensity (∼40 v. 200 lux) had the most pronounced effects, particularly on the nature of social interactions, with pigs under these conditions showing more aggression in the early stages of the experiment. In addition, pigs exposed to a high level of mechanical noise representative of artificial ventilation (∼80 v. 40 dB [A]) were less submissive to aggressive acts, while pigs in ∼20 ppm ammonia showed more reciprocated aggression when in coincident LL (<40 lux). The results indicate that atmospheric ammonia at commonly experienced concentrations may undermine social stability, particularly in the presence of low lighting, though the mechanisms are currently unknown. These findings have implications for the welfare of growing pigs and hence policy makers and farmers alike, with respect to the improvement of welfare in intensive pig farming.

A potential-flow modelling of flow past an oscillating circular cylinder with separated wake is developed here based on Parkinson & Jandali's (1970) wake-source model for steady flow. The phase-averaged pressure distributions, the in-line force coefficients, as well as the drag and added-mass coefficients, for an in-line oscillating circular cylinder in a steady free-stream flow are computed using the present ‘unsteady wake-source model’. The results show that Morison's equation is in some cases a satisfactory model in the study of unsteady bluff-body aerodynamics.
The two-dimensional incompressible potential-flow model simulates the effect of flow separation in unsteady flow by placing surface sources, with time-dependent strength and angular positions on the rear wetted surface of the body, and downstream sinks to form a closed wake model in the transformed plane. The unsteady Bernoulli equation is used to obtain the time-dependent pressure distributions over the front wetted surface, from which the in-line force coefficients are obtained through integration.
The in-line force equation obtained from the present model is shown to be comprised of an uncoupled drag term and inertia terms. The corresponding hydrodynamie coefficients obtained for the case of oscillatory flow are also more realistic than those obtained in a potential-flow calculation without flow separation which gives a drag coefficient of zero and a constant inertia coefficient of 2.0. The in-line force equation is reduced to the familiar Morison's equation with some simplifications and thus provides some support to the much criticized Morison's equation in the study of unsteady separated flow.
Another interesting feature of the present model is that it enables the calculation of instantaneous drag and inertia coefficients which have not been successfully obtained previously. In the cases considered here, the variations of drag and inertia coefficients over a cycle are shown to be small and thus the Morison's equation using mean coefficients is shown to predict the in-line forces rather precisely.
The present model was compared with experimental measurements obtained by oscillating a 0.1 m diameter circular cylinder along the direction of free-stream flow. The pressure distributions and in-line force coefficients agree well with the experimental measurements for velocity ratio rω/U∞ up to 0.25, reduced velocity U∞T/d down to 50 and Keulegan—Carpenter number 2πr/d up to 17, where r, ω, T, U∞ and d are the amplitude of oscillation, angular frequency, period of oscillation, free-stream velocity and diameter of the cylinder respectively. The computed drag and inertia coefficients also agree well with those obtained experimentally by previous investigators.

Field measurements of wave orbital velocities and pressure, collected in the lower part of the water column in 7 m depth with a three-component acoustic Doppler current meter and a co-located pressure transducer, are compared to the second-order theory for weakly nonlinear surface gravity waves in arbitrary water depth (Hasselmann 1962). Pressure and velocity spectra and cross-spectra are in excellent agreement with (linear) free wave transfer functions, even at (and higher than) twice the spectral peak frequency where nonlinearities (forced secondary waves) are expected to be important. Theoretical predictions show that although secondary waves sometimes contribute a significant fraction of the energy observed at double swell and sea frequencies, their effect on velocity-pressure transfer functions is small. However, forced waves are more apparent in deviations from Gaussian statistics. Good agreement between observed and predicted third-order statistics shows that Hasselmann's weakly nonlinear theory accurately describes the secondary pressure and orbital velocity (both horizontal and vertical components) field at double swell and sea frequencies, even for moderately large (0(0.1–0.2)) values of the nonlinear perturbation parameter. Only with near-breaking swell and relatively strong nonlinearities (perturbation parameter ≈ 0.22), do the observed third-order statistics diverge significantly from Hasselmann's theory.

It is well known that plastic deformation induced by conventional forming methodssuch as rolling, drawing or extrusion can significantly increase the strength of metalsHowever, this increase is usually accompanied by a loss of ductility. For example, Fig.1 shows that with increasing plastic deformation, the yield strength of Cu and Almonotonically increases while their elongation to failure (ductility) decreases. Thesame trend is also true for other metals and alloys. Here we report an extraordinarycombination of high strength and high ductility produced in metals subject to severeplastic deformation (SPD). We believe that this unusual mechanical behavior is causedby the unique nanostructures generated by SPD processing. The combination ofultrafine grain size and high-density dislocations appears to enable deformation by newmechanisms. This work demonstrates the possibility of tailoring the microstructures ofmetals and alloys by SPD to obtain both high strength and high ductility. Materialswith such desirable mechanical properties are very attractive for advanced structuralapplications.

We present the first results from multi-site observations of the δ Scuti star XX Pyx (CD–24°7599). The observations were carried out as the 17th run of the Delta Scuti Network. We collected 583 hr of B, V time-series photometry, resulting in a detection level (4σ) in the amplitude spectrum of 0.5 mmag. We detect 6 new pulsation frequencies, bringing the total number of frequencies known in this star up 19.
