This paper describes how to compute algorithmically certain twisted signature invariants of a knot $K$ using twisted Blanchfield forms. An illustration of the algorithm is implemented on $(2,q)$-torus knots. Additionally, using satellite formulas for these invariants, we also show how to obstruct the sliceness of certain iterated torus knots.

For every $k \geq 2$ and $n \geq 2$, we construct n pairwise homotopically inequivalent simply connected, closed $4k$-dimensional manifolds, all of which are stably diffeomorphic to one another. Each of these manifolds has hyperbolic intersection form and is stably parallelisable. In dimension four, we exhibit an analogous phenomenon for spin$^{c}$ structures on $S^2 \times S^2$. For $m\geq 1$, we also provide similar $(4m-1)$-connected $8m$-dimensional examples, where the number of homotopy types in a stable diffeomorphism class is related to the order of the image of the stable J-homomorphism $\pi _{4m-1}(SO) \to \pi ^s_{4m-1}$.

Objectives:
To explore associations between maternal pre-pregnancy exposure to arsenic in diet and non-cardiac birth defects.
Design:This is a population-based, case–control study using maternal responses to a dietary assessment and published arsenic concentration estimates in food items to calculate average daily total and inorganic arsenic exposure during the year before pregnancy. Assigning tertiles of total and inorganic arsenic exposure, logistic regression analysis was used to estimate OR for middle and high tertiles, compared to the low tertile.
Setting:US National Birth Defects Prevention Study, 1997–2011.
Participants:Mothers of 10 446 children without birth defects and 14 408 children diagnosed with a non-cardiac birth defect.
Results:Maternal exposure to total dietary arsenic in the middle and high tertiles was associated with a threefold increase in cloacal exstrophy, with weak positive associations (1·2–1·5) observed either in both tertiles (intercalary limb deficiency) or the high tertile only (encephalocele, glaucoma/anterior chamber defects and bladder exstrophy). Maternal exposure to inorganic arsenic showed mostly weak, positive associations in both tertiles (colonic atresia/stenosis, oesophageal atresia, bilateral renal agenesis/hypoplasia, hypospadias, cloacal exstrophy and gastroschisis), or the high (glaucoma/anterior chamber defects, choanal atresia and intestinal atresia stenosis) or middle (encephalocele, intercalary limb deficiency and transverse limb deficiency) tertiles only. The remaining associations estimated were near the null or inverse.
Conclusions:This exploration of arsenic in diet and non-cardiac birth defects produced several positive, but mostly weak associations. Limitations in exposure assessment may have resulted in exposure misclassification. Continued research with improved exposure assessment is recommended to identify if these associations are true signals or chance findings.

Background
Pharmacogenomic testing has emerged to aid medication selection for patients with major depressive disorder (MDD) by identifying potential gene-drug interactions (GDI). Many pharmacogenomic tests are available with varying levels of supporting evidence, including direct-to-consumer and physician-ordered tests. We retrospectively evaluated the safety of using a physician-ordered combinatorial pharmacogenomic test (GeneSight) to guide medication selection for patients with MDD in a large, randomized, controlled trial (GUIDED).
Materials and MethodsPatients diagnosed with MDD who had an inadequate response to ≥1 psychotropic medication were randomized to treatment as usual (TAU) or combinatorial pharmacogenomic test-guided care (guided-care). All received combinatorial pharmacogenomic testing and medications were categorized by predicted GDI (no, moderate, or significant GDI). Patients and raters were blinded to study arm, and physicians were blinded to test results for patients in TAU, through week 8. Measures included adverse events (AEs, present/absent), worsening suicidal ideation (increase of ≥1 on the corresponding HAM-D17 question), or symptom worsening (HAM-D17 increase of ≥1). These measures were evaluated based on medication changes [add only, drop only, switch (add and drop), any, and none] and study arm, as well as baseline medication GDI.
ResultsMost patients had a medication change between baseline and week 8 (938/1,166; 80.5%), including 269 (23.1%) who added only, 80 (6.9%) who dropped only, and 589 (50.5%) who switched medications. In the full cohort, changing medications resulted in an increased relative risk (RR) of experiencing AEs at both week 4 and 8 [RR 2.00 (95% CI 1.41–2.83) and RR 2.25 (95% CI 1.39–3.65), respectively]. This was true regardless of arm, with no significant difference observed between guided-care and TAU, though the RRs for guided-care were lower than for TAU. Medication change was not associated with increased suicidal ideation or symptom worsening, regardless of study arm or type of medication change. Special attention was focused on patients who entered the study taking medications identified by pharmacogenomic testing as likely having significant GDI; those who were only taking medications subject to no or moderate GDI at week 8 were significantly less likely to experience AEs than those who were still taking at least one medication subject to significant GDI (RR 0.39, 95% CI 0.15–0.99, p=0.048). No other significant differences in risk were observed at week 8.
ConclusionThese data indicate that patient safety in the combinatorial pharmacogenomic test-guided care arm was no worse than TAU in the GUIDED trial. Moreover, combinatorial pharmacogenomic-guided medication selection may reduce some safety concerns. Collectively, these data demonstrate that combinatorial pharmacogenomic testing can be adopted safely into clinical practice without risking symptom degradation among patients.

Background:
The Genomics Used to Improve DEpresssion Decisions (GUIDED) trial assessed outcomes associated with combinatorial pharmacogenomic (PGx) testing in patients with major depressive disorder (MDD). Analyses used the 17-item Hamilton Depression (HAM-D17) rating scale; however, studies demonstrate that the abbreviated, core depression symptom-focused, HAM-D6 rating scale may have greater sensitivity toward detecting differences between treatment and placebo. However, the sensitivity of HAM-D6 has not been tested for two active treatment arms. Here, we evaluated the sensitivity of the HAM-D6 scale, relative to the HAM-D17 scale, when assessing outcomes for actively treated patients in the GUIDED trial.
Methods:Outpatients (N=1,298) diagnosed with MDD and an inadequate treatment response to >1 psychotropic medication were randomized into treatment as usual (TAU) or combinatorial PGx-guided (guided-care) arms. Combinatorial PGx testing was performed on all patients, though test reports were only available to the guided-care arm. All patients and raters were blinded to study arm until after week 8. Medications on the combinatorial PGx test report were categorized based on the level of predicted gene-drug interactions: ‘use as directed’, ‘moderate gene-drug interactions’, or ‘significant gene-drug interactions.’ Patient outcomes were assessed by arm at week 8 using HAM-D6 and HAM-D17 rating scales, including symptom improvement (percent change in scale), response (≥50% decrease in scale), and remission (HAM-D6 ≤4 and HAM-D17 ≤7).
Results:At week 8, the guided-care arm demonstrated statistically significant symptom improvement over TAU using HAM-D6 scale (Δ=4.4%, p=0.023), but not using the HAM-D17 scale (Δ=3.2%, p=0.069). The response rate increased significantly for guided-care compared with TAU using both HAM-D6 (Δ=7.0%, p=0.004) and HAM-D17 (Δ=6.3%, p=0.007). Remission rates were also significantly greater for guided-care versus TAU using both scales (HAM-D6 Δ=4.6%, p=0.031; HAM-D17 Δ=5.5%, p=0.005). Patients taking medication(s) predicted to have gene-drug interactions at baseline showed further increased benefit over TAU at week 8 using HAM-D6 for symptom improvement (Δ=7.3%, p=0.004) response (Δ=10.0%, p=0.001) and remission (Δ=7.9%, p=0.005). Comparatively, the magnitude of the differences in outcomes between arms at week 8 was lower using HAM-D17 (symptom improvement Δ=5.0%, p=0.029; response Δ=8.0%, p=0.008; remission Δ=7.5%, p=0.003).
Conclusions:Combinatorial PGx-guided care achieved significantly better patient outcomes compared with TAU when assessed using the HAM-D6 scale. These findings suggest that the HAM-D6 scale is better suited than is the HAM-D17 for evaluating change in randomized, controlled trials comparing active treatment arms.

Background
Major depressive disorder (MDD) is a leading cause of disease burden worldwide, with lifetime prevalence in the United States of 17%. Here we present the results of the first prospective, large-scale, patient- and rater-blind, randomized controlled trial evaluating the clinical importance of achieving congruence between combinatorial pharmacogenomic (PGx) testing and medication selection for MDD.
Methods1,167 outpatients diagnosed with MDD and an inadequate response to ≥1 psychotropic medications were enrolled and randomized 1:1 to a Treatment as Usual (TAU) arm or PGx-guided care arm. Combinatorial PGx testing categorized medications in three groups based on the level of gene-drug interactions: use as directed, use with caution, or use with increased caution and more frequent monitoring. Patient assessments were performed at weeks 0 (baseline), 4, 8, 12 and 24. Patients, site raters, and central raters were blinded in both arms until after week 8. In the guided-care arm, physicians had access to the combinatorial PGx test result to guide medication selection. Primary outcomes utilized the Hamilton Depression Rating Scale (HAM-D17) and included symptom improvement (percent change in HAM-D17 from baseline), response (50% decrease in HAM-D17 from baseline), and remission (HAM-D17<7) at the fully blinded week 8 time point. The durability of patient outcomes was assessed at week 24. Medications were considered congruent with PGx test results if they were in the ‘use as directed’ or ‘use with caution’ report categories while medications in the ‘use with increased caution and more frequent monitoring’ were considered incongruent. Patients who started on incongruent medications were analyzed separately according to whether they changed to congruent medications by week8.
ResultsAt week 8, symptom improvement for individuals in the guided-care arm was not significantly different than TAU (27.2% versus 24.4%, p=0.11). However, individuals in the guided-care arm were more likely than those in TAU to achieve remission (15% versus 10%; p<0.01) and response (26% versus 20%; p=0.01). Remission rates, response rates, and symptom reductions continued to improve in the guided-treatment arm until the 24week time point. Congruent prescribing increased to 91% in the guided-care arm by week 8. Among patients who were taking one or more incongruent medication at baseline, those who changed to congruent medications by week 8 demonstrated significantly greater symptom improvement (p<0.01), response (p=0.04), and remission rates (p<0.01) compared to those who persisted on incongruent medications.
ConclusionsCombinatorial PGx testing improves short- and long-term response and remission rates for MDD compared to standard of care. In addition, prescribing congruency with PGx-guided medication recommendations is important for achieving symptom improvement, response, and remission for MDD patients.

Given a link $L$, the Blanchfield pairing $\text{Bl(}L\text{)}$ is a pairing that is defined on the torsion submodule of the Alexander module of $L$. In some particular cases, namely if $L$ is a boundary link or if the Alexander module of $L$ is torsion, $\text{Bl(}L\text{)}$ can be computed explicitly; however no formula is known in general. In this article, we compute the Blanchfield pairing of any link, generalizing the aforementioned results. As a corollary, we obtain a new proof that the Blanchfield pairing is Hermitian. Finally, we also obtain short proofs of several properties of $\text{Bl(}L\text{)}$.

Visible and infrared satellite images reveal numerous lineations on the Siple Coast region of West Antarctica. We used 5 MHz ice-penetrating radar to probe the interior and the bed of the ice sheet beneath a lineation at the boundary between Engelhardt Ice Ridge and flat-ice terrain to the south of the Kamb Ice Stream (KIS) outlet. Results show curved reflectors that emerge from the bed beneath 600 m thick ice. The tops of the reflectors extend about 100m into the ice above the bed, where they become almost horizontal. Apparent reflectivity of the horizontal section is about 20 dB less than that of the bed. We conclude that the likely cause of such strong reflection is sea water that was accreted into basal crevasses when the flat-ice terrain was floating. Internal layers are warped downward just downslope from the basal reflectors. It is thought that the downwarping was caused by localized basal melting in the past. The spatial pattern of downwarping suggests that localized basal melting was stronger on the north side than on the south side of KIS; apparently ice/ocean interactions on the two sides of KIS were different.
Bed reflectivity beneath inactive ice streams in West Antarctica

Radio-echo sounding (RES) techniques are used to examine spatial changes in bed reflectivity across relict ice streams inWest Antarctica. Measurements from adjacent interstream ridges are used to correct the measured power returned from the bed for attenuation and losses due to geometric spreading, scattering and absorption. RES measurements near boreholes drilled on Ice Stream C (ISC) indicate high coefficients of bed reflectivity (R > 0.1) in locations where the bed was thawed and boreholes connected to the basal water system, and low reflectivity coefficients (R < 0.02) at locations that were frozen and not connected. Intermediate values of bed reflectivity were measured at locations where the connection to the basal water system was weak. Measurements across four relict margins show that bed reflectivity usually jumps from low to high values several kilometers inside the outermost buried crevasses. We interpret this to be a transition from frozen to thawed basal conditions and discuss implications of these observations.

Radio-echo soundings were collected on Black Rapids Glacier, Alaska, USA, from mid-May to mid-July 1993 to investigate spring speed-up and summer slowdown including high-speed events associated with three lake drainages. Temporal changes in echo power from all depths were highly correlated, indicating a strong effect from varying amounts of near-surface water. Evaluation of bed reflectivity was corrected for this effect based on the time variation of spatially stable patterns of internal scattering identified using principal component analysis. Hourly time series collected at two fixed locations over the deepest part of two valley cross sections showed no detectable change in bed reflection power (<5%) or phase (<0.05 rad). Reoccupation of fixed locations toward the margins at several-day intervals revealed changes in bed power reflectivity up to 50%, but with no definable relation to lake drainages. Theoretical analyses indicate that changes in reflectivity of <5% from a rock bed constrain basal water thickness changes to centimeter scale or less. Conductive basal till degrades the constraint to decimeter scale or more. Changes in bed reflectivity of 50% indicate probable absence of thick conductive till at such locations, and that the changes were caused by centimeter to decimeter changes in equivalent water thickness.

Taking the signature of the closure of a braid defines a map from the braid group to the integers. In 2005, Gambaudo and Ghys expressed the homomorphism defect of this map in terms of the Meyer cocycle and the Burau representation. In the present paper, we simultaneously extend this result in two directions, considering the multivariable signature of the closure of a coloured tangle. The corresponding defect is expressed in terms of the Maslov index and of the Lagrangian functor defined by Turaev and the first-named author.

Background: People with dementia have a range of needs that are met by informal caregivers. A DVD-based training program was developed using research-based strategies for memory and communication in dementia. The effectiveness of the training on the caregiver experience and the well-being of the person with dementia was evaluated.
Methods: A pre-test/post-test controlled trial was undertaken with caregiver–care-recipient dyads living in the community. Measures of the carers’ knowledge of memory and communication strategies, burden, positive perceptions of caregiving, and perceptions of problem behaviors were taken pre- and three months post-intervention. The depression and well-being of the person with dementia were also evaluated. Satisfaction with the training and feedback were measured.
Results: Twenty-nine dyads (13 training group, 16 control group) participated. Bonferroni's correction was made to adjust for multiple comparisons, setting α at 0.00385. A significant improvement was found in caregivers’ knowledge for the training group compared to the control group (p = 0.0011). The training group caregivers reported a reduction in the frequency of care recipient disruptive behaviors (p = 0.028) and increased perceptions of positive aspects of caregiving (p = 0.039), both at a level approaching significance. The training group care recipients had increased frequency of verbally communicated depressive behaviors at a level approaching significance (p = 0.0126). The frequency of observed depressive behaviors was not significantly different between groups.
Conclusions: This approach to training for caregivers of people with dementia appears promising for its impact on knowledge and the caregiving experience. Further research could monitor the impact of the training on broader measures of depression and well-being, with a larger sample.
