The effect of older age on outcomes of rTMS treatment for treatment-resistant depression

Clinical outcomes of repetitive transcranial magnetic stimulation (rTMS) for treatment of treatment-resistant depression (TRD) vary widely and there is no mood rating scale that is standard for assessing rTMS outcome. It remains unclear whether TMS is as efficacious in older adults with late-life depression (LLD) compared to younger adults with major depressive disorder (MDD). This study examined the effect of age on outcomes of rTMS treatment of adults with TRD. Self-report and observer mood ratings were measured weekly in 687 subjects ages 16–100 years undergoing rTMS treatment using the Inventory of Depressive Symptomatology 30-item Self-Report (IDS-SR), Patient Health Questionnaire 9-item (PHQ), Profile of Mood States 30-item, and Hamilton Depression Rating Scale 17-item (HDRS). All rating scales detected significant improvement with treatment; response and remission rates varied by scale but not by age (response/remission ≥ 60: 38%–57%/25%–33%; <60: 32%–49%/18%–25%). Proportional hazards models showed early improvement predicted later improvement across ages, though early improvements in PHQ and HDRS were more predictive of remission in those < 60 years (relative to those ≥ 60) and greater baseline IDS burden was more predictive of non-remission in those ≥ 60 years (relative to those < 60). These results indicate there is no significant effect of age on treatment outcomes in rTMS for TRD, though rating instruments may differ in assessment of symptom burden between younger and older adults during treatment.
Distance and percentage distance from median BMI as alternatives to BMI z score

BMI z (BMIz) score based on the Centers for Disease Control and Prevention growth charts is widely used, but it is inaccurate above the 97th percentile. We explored the performance of alternative metrics based on the absolute distance or % distance of a child’s BMI from the median BMI for sex and age. We used longitudinal data from 5628 children who were first examined <12 years to compare the tracking of three BMI metrics: distance from median, % distance from median and % distance from median on a log scale. We also explored the effects of adjusting these metrics for age differences in the distribution of BMI. The intraclass correlation coefficient (ICC) was used to compare tracking of the metrics. Metrics based on % distance (whether on the original or log scale) yielded higher ICCs compared with distance from median. The ICCs of the age-adjusted metrics were higher than that of the unadjusted metrics, particularly among children who were (1) overweight or had obesity, (2) younger and (3) followed for >3 years. The ICCs of the age-adjusted metrics were also higher compared with that of BMIz among children who were overweight or obese. Unlike BMIz, these alternative metrics do not have an upper limit and can be used for assessing BMI in all children, even those with very high BMIs. The age-adjusted % from median (on a log or linear scale) works well for all ages, while unadjusted % from median is better limited to older children or short follow-up periods.
31 Intracranial growing teratoma syndrome (IGTS): An international retrospective study

BACKGROUND: IGTS is a rare phenomenon of paradoxical germ cell tumor (GCT) growth during or following treatment despite normalization of tumor markers. We sought to evaluate the frequency, clinical characteristics and outcome of IGTS in patients in 21 North-American and Australian institutions. METHODS: Patients with IGTS diagnosed from 2000-2017 were retrospectively evaluated. RESULTS: Out of 739 GCT diagnoses, IGTS was identified in 33 patients (4.5%). IGTS occurred in 9/191 (4.7%) mixed-malignant GCTs, 4/22 (18.2%) immature teratomas (ITs), 3/472 (0.6%) germinomas/germinomas with mature teratoma, and in 17 secreting non-biopsied tumours. Median age at GCT diagnosis was 10.9 years (range 1.8-19.4). Male gender (84%) and pineal location (88%) predominated. Of 27 patients with elevated markers, median serum AFP and Beta-HCG were 70 ng/mL (range 9.2-932) and 44 IU/L (range 4.2-493), respectively. IGTS occurred at a median time of 2 months (range 0.5-32) from diagnosis, during chemotherapy in 85%, radiation in 3%, and after treatment completion in 12%. Surgical resection was attempted in all, leading to gross total resection in 76%. Most patients (79%) resumed GCT chemotherapy/radiation after surgery. At a median follow-up of 5.3 years (range 0.3-12), all but 2 patients are alive (1 succumbed to progressive disease, 1 to malignant transformation of GCT). CONCLUSION: IGTS occurred in less than 5% of patients with GCT and most commonly after initiation of chemotherapy. IGTS was more common in patients with IT-only on biopsy than with mixed-malignant GCT. Surgical resection is a principal treatment modality. Survival outcomes for patients who developed IGTS are favourable.
Niche structure of marine sponges from temperate hard-bottom habitats within Gray's Reef National Marine Sanctuary

Many species of marine sponges on tropical reefs host abundant and diverse symbiont communities capable of varied metabolic pathways. While such communities may confer a nutritional benefit to some hosts (termed High Microbial Abundance (HMA) sponges), other sympatric species host only sparse symbiont communities (termed Low Microbial Abundance (LMA) sponges) and obtain a majority of their C and N from local sources. Sponge communities are widespread across large latitudinal gradients, however, and recent evidence suggests that these symbioses may also extend beyond the tropics. We investigated the role that symbionts play in the ecology of sponges from the temperate, hard-bottom reefs of Gray's Reef National Marine Sanctuary by calculating the niche size (as standard ellipse area (SEAc)) and assessing the relative placement of five HMA and four LMA sponge species within bivariate (δ13C and δ15N) isotopic space. Although photosymbiont abundance was low across most of these species, sponges were widespread across isotopic niche space, implying that microbial metabolism confers an ecological benefit to temperate sponges by expanding host metabolic capability. To examine how these associations vary across a latitudinal gradient, we also compared the relative placement of temperate and tropical conspecifics within isotopic space. Surprisingly, shifts in sponge δ13C and δ15N values between these regions suggest a reduced reliance on symbiont-derived nutrients in temperate sponges compared with their tropical conspecifics. Despite this, symbiotic sponges in temperate systems likely have a competitive advantage, allowing them to grow and compete for space within these habitats.

A new instrument, the Why It Happened questionnaire, was developed for assessing characterological and behavior self-blame in two samples of third- through ninth-grade children (Sample 1: n = 121; sample 2: n = 322). Children's responses were very consistent within specific classes of negative events but only moderately consistent across time. Exploratory and confirmatory factor analyses of open-ended and self-rating responses, respectively, revealed that children's self-blame attributions were only moderately consistent across different classes of negative events, and that children's tendency to make characterological and behavioral attributions were unrelated for academic events and moderately correlated for social events. Multiple regression revealed that characterological self-blame about academic and social events were not significantly related to self-reported depression until ninth grade. Implications for cognitive diathesis-stress models of depression for children are discussed.

Excavations of sites spanning the Beaker to early Roman periods at Stackpole Warren, Dyfed, are described. The sites are in an area of blown sand which enhanced their preservation and led to the separation of several horizons. The earliest is a buried soil beneath the blown sand which contained Mesolithic to Bronze Age artefacts. At site A, there was a roundhouse associated with Early Bronze Age pottery and dated to 1620±70 and 1400±70 BC uncal., and two other roundhouses, one possibly of Beaker age. After a period of soil formation, a ritual complex of Later Bronze Age date was established, this contemporary with the earliest besanding of the area; it included a stone setting of more than 2000 small stones, an alignment of small water-worn stones and a standing stone. A cremation gave a latest date of 940±70 BC uncal. Other Later Bronze Age activity is recorded at site G/J in the form of a rectangular enclosure, possibly unfinished.
Late Iron Age to early Romano-British settlement was present at sites A and B, consisting of scatters of occupation debris, burnt mounds, cooking pits, hearths and houses, some of stone, some of timber, all taking place in an area being intermittently besanded.
Peripheral to the religious and domestic sites, a field system was excavated. The earliest phase was a linear earthwork from which a C14 date of 400±70 BC uncal. was obtained from charcoal in the ditch. After the decay of this, rectangular fields with stone walls were laid out, one along the line of the erstwhile earthwork, this taking place around the end of the Iron Age as dated by C14 of charcoal directly beneath a wall to 90±70 BC uncal. Some of the fields had been cultivated by a succession of cross- and one-way-ploughing, others used for cattle.
An assemblage of 763 flints included a few Mesolithic artefacts but was mostly of Late Neolithic and Bronze Age date. A succession of ceramic assemblages included a small Middle Neolithic group (4 vessels), two distinct Beaker groups, one early (Lanting and van der Waals steps 1–3 (8 vessels), one late (steps 3–6) (45 vessels), an Early Bronze Age group of collared urns (43 vessels) and a Later Bronze Age group (26 vessels).
Environmental data was not prolific but there was a small quantity of animal bone, mostly cattle and sheep, and cereal grain, mostly barley with some wheat. Marine molluscs were present but sparsely utilized and there was no other indication of the exploitation of the coastal resources such as seals, birds, fish andiseaweed. Land Mollusca indicated open country from the Iron Age onwards when the record begins.
The importance of the site is in the ritual complex from site A, the succession of Iron Age/Romano-British occupation horizons, the succession of ceramic assemblages, the field system and the fact that blown sand horizons have allowed the preservation and separation of the sequence much of which would have been at best conflated in to a single horizon or at worst destroyed. Otherwise, there is no evidence that the site was in any way special with regard to the relationship of human activity and sand deposition until the Middle Ages when the area was used as a rabbit warren. Nor was the coastal location important, at least as could be determined by the results. This was a representative of a succession of later prehistoric farming communities and their various domestic, ritual and sepulchral activities in lowland Dyfed.
237Np aND 239Pu Solution Behavior During Hydrothermal Testing of Simulated Nuclear Waste Glass with Basalt and Steel

A series of hydrothermal experiments were carried out on 237Np- and 239Pu-doped PNL 76–68 glass, synthetic basalt groundwater, basalt, and cast steel. These hydrothermal experiments are part of the Basalt Waste Isolation Project investigation of the interactions of waste package components in a basalt repository. Experiments of three months duration were conducted in Dickson-type rocking autoclaves at 200°C and 30 MPa, with an initial fluid to solid weight ratio of 10:1. All solid materials were ground and sieved to a narrow size fraction. The experiments carried out were: glass and groundwater; glass, basalt, and groundwater; glass, steel, and groundwater; and glass, steel, basalt, and groundwater. Unfiltered, 4000 Â filtered, and 18 Å filtered solutions were analyzed to determine the concentrations of radionuclides in solution and those associated with colloids.
The quantities of 237Np and 239Pu in solution were very small. Worstcase calculations indicate that 0.01% or less of the total radionuclide inventories were present in solution. The highest solution concentrations of the actinide dopants were observed in the experiments with basalt, even though smaller amounts of glass were dissolved than in the experiments without basalt. The observed differences in the solution concentrations of 237Np and 239Pu were probably controlled by differences in the reaction products, which were clinoptilolite in the experiments with basalt, and an Fe-Zn smectite clay in the experiments without basalt.
Investigation of the Hydrothermal Interaction of 99Tc-Doped Glass with Basalt Repository Nuclear Waste Package Components

Hydrothermal experiments using basalt-repository nuclear waste package components have been carried out at 200°C and 30 MPa using 99Tc-doped PNL 76–68 waste glass. This work was conducted in support of the Basalt Waste Isolation Project. The experiments were carried out in rocking autoclaves that allowed for periodic solution sampling. Preliminary results that illustrated the effect of basalt on 99Tc solution behavior were discussed previously[l]. In this paper, we continue those observations by discussing the additional experiments that investigated the effects of steel. We also include the post-experiment solid phase analyses from all test configurations.
The effect of steel on glass dissolution was observed to be minimal when basalt was not present, i.e., there were no discernible differences in the amount of glass dissolved (based on boron release), with or without steel present.
The 99Tc solution concentration results showed that basalt, steel, and a combination of basalt and steel have an ability to dramatically lower the concentration of 99Tc in the solution, probably through a redox mechanism[2]. Solid run product analyses showed that without basalt present, a gel-like secondary phase consisting of an iron, zinc clay with an apparent smectite structure formed. When basalt was present clinoptilolite formed as the major secondary phase, due primarily to the presence of aluminum in the basalt mesostasis. Separation of the various solid run products and residual initial solid phases has not yet been achieved. Such a separation would facilitate the identification of the phase or phases with which 99Tc was associated. The solution results indicated that 99Tc, a potentially mobile radionuclide, may be incorporated in a relatively insoluble phase in the environment of a basalt repository. In addition, no synergistic effects between waste package components were observed that would increase the concentration of 99Tc in solution.
