Background: A subset of patients experience poor outcomes following anterior cervical discectomy and fusion (ACDF). Our study aimed to identify postoperative trajectories of disability, neck/arm pain and determine baseline measures that predict subgroup membership. Methods: Patients with cervical spondylotic radiculopathy undergoing ACDF are presented. Prognostic factors comprised demographic, health and surgery-related variables. Study outcomes were trajectories of neck disability index scores, numeric rating scales for neck/arm pain modeled with latent-class growth analysis. Associations were explored using robust Poisson models and reported with risk ratios and 95% confidence intervals. Results: Patients (N = 352; mean (SD) age = 50.9(9.5) years, 43.8% female) identified trajectories for disability (excellent=45.3%,fair=39.2%,poor=15.5%),arm pain (excellent=24.5%,good=52.0%,poor=23.5%),and neck pain (excellent=13.7%,good=63.1%,poor=23.2%). Greater physical and mental health-related quality of life were associated with a reduced risk of poor outcome(per SD,0.40[0.30,0.53]-0.80[0.65,0.99]), while higher risk for depression (per SD, 1.36[1.12,1.65]-2.26[1.84,2.78]), longer wait time(per 90 days, 1.31[1.05,1.63]-1.64[1.20,2.24]), and longer procedure time (per 30 min,1.07[1.03,1.10]-1.08[1.05,1.12]) were associated with an increased risk of poor outcome for all outcomes. Poor disability was increased with self-reported depression(3.03[1.76,5.21]), greater neck-to-arm pain ratio (2.63 [1.28 to 5.40]), ASA score > 2(2.26[1.33,3.83]), and preoperative opiates (2.05[1.18,3.56]), while preoperative physiotherapy (0.51[0.30, 0.88]), spinal injections (0.48[0.23 to 0.98]), and regular exercise (0.44 [0.24, 0.79]) decreased risk. Receiving compensation and smoking were associated with poor outcome for neck pain. Remaining candidate prognostic factors were not associated with clinical outcome. Conclusions: Perioperative factors were shown to decrease risk of poor outcomes for pain and disability two years following ACDF.

Introduction
Craving in negative emotional situations (negative craving) is commonly associated with relapse and heavy alcohol use. Elevated dynorphin levels were associated with negative emotions, while variations in the OPRK1 and PDYN genes encoding OPRK1 receptor and dynorphins were associated with alcohol dependence.
ObjectivesTo investigate potential overlap in the genetic factors underlying, negative craving and alcohol dependence.
AimsExamine the association of the negative craving and genetic variation in the OPRK1 and PDYN genes.
Methods13 PDYN and 10 OPRK1 Single Nucleotide Polymorphisms (SNPs), including those previously reported to be associated with alcohol dependence were genotyped in 196 alcohol dependent subjects. The raw scores of the negative subscale of Inventory of Drug Taking Situations (IDTS) were utilized as a quantitative measure of negative craving. Logistic regression models were used to test for associations after controlling for age and gender.
ResultsGene-level haplotype testing demonstrated significant association of negative craving with variation in PDYN (p < 0.05) but not OPRK1 gene. The rs2281285 - rs199794 haplotype showed significant association (p = 0.0236) with negative craving, while rs2235749 - rs10485703 haplotype showed marginally significant association (p = 0.055). This replicates previous findings of association between these haplotypes and alcohol dependence. Negative craving was also associated with PDYN rs2281285 variant (p = 0.012) with estimated effect size of 6.95 (SE = 2.75). This new association finding was not significant after correction for multiple testing (p = 0.18).
ConclusionsOur findings support association of PDYN sequence variation with negative craving in alcohol dependent subjects. Future studies should investigate functional mechanisms of this association.

In Cameroon, there is a national programme engaged in the control of schistosomiasis and soil-transmitted helminthiasis. In certain locations, the programme is transitioning from morbidity control towards local interruption of parasite transmission. The volcanic crater lake villages of Barombi Mbo and Barombi Kotto are well-known transmission foci and are excellent context-specific locations to assess appropriate disease control interventions. Most recently they have served as exemplars of expanded access to deworming medications and increased environmental surveillance. In this paper, we review infection dynamics through time, beginning with data from 1953, and comment on the short- and long-term success of disease control. We show how intensification of local control is needed to push towards elimination and that further environmental surveillance, with targeted snail control, is needed to consolidate gains in preventive chemotherapy as well as empower local communities to take ownership of interventions.

OBJECTIVE
To determine the scope, source, and mode of transmission of a multifacility outbreak of extensively drug-resistant (XDR) Acinetobacter baumannii.
DESIGNOutbreak investigation.
SETTING AND PARTICIPANTSResidents and patients in skilled nursing facilities, long-term acute-care hospital, and acute-care hospitals.
METHODSA case was defined as the incident isolate from clinical or surveillance cultures of XDR Acinetobacter baumannii resistant to imipenem or meropenem and nonsusceptible to all but 1 or 2 antibiotic classes in a patient in an Oregon healthcare facility during January 2012–December 2014. We queried clinical laboratories, reviewed medical records, oversaw patient and environmental surveillance surveys at 2 facilities, and recommended interventions. Pulsed-field gel electrophoresis (PFGE) and molecular analysis were performed.
RESULTSWe identified 21 cases, highly related by PFGE or healthcare facility exposure. Overall, 17 patients (81%) were admitted to either long-term acute-care hospital A (n=8), or skilled nursing facility A (n=8), or both (n=1) prior to XDR A. baumannii isolation. Interfacility communication of patient or resident XDR status was not performed during transfer between facilities. The rare plasmid-encoded carbapenemase gene blaOXA-237 was present in 16 outbreak isolates. Contact precautions, chlorhexidine baths, enhanced environmental cleaning, and interfacility communication were implemented for cases to halt transmission.
CONCLUSIONSInterfacility transmission of XDR A. baumannii carrying the rare blaOXA-237 was facilitated by transfer of affected patients without communication to receiving facilities.

Objective.
To establish a statewide network to detect, control, and prevent the spread of carbapenem-resistant Enterobacteriaceae (CRE) in a region with a low incidence of CRE infection.
Design.Implementation of the Drug Resistant Organism Prevention and Coordinated Regional Epidemiology (DROP-CRE) Network.
Setting and Participants.Oregon infection prevention and microbiology laboratory personnel, including 48 microbiology laboratories, 62 acute care facilities, and 140 long-term care facilities.
Methods.The DROP-CRE working group, comprising representatives from academic institutions and public health, convened an interdisciplinary advisory committee to assist with planning and implementation of CRE epidemiology and control efforts. The working group established a statewide CRE definition and surveillance plan; increased the state laboratory capacity to perform the modified Hodge test and polymerase chain reaction for carbapenemases in real time; and administered surveys that assessed the needs and capabilities of Oregon infection prevention and laboratory personnel. Results of these inquiries informed CRE education and the response plan.
Results.Of 60 CRE reported from November 2010 through April 2013, only 3 were identified as carbapenemase producers; the cases were not linked, and no secondary transmission was found. Microbiology laboratories, acute care facilities, and long-term care facilities reported lacking carbapenemase testing capability, reliable interfacility communication, and CRE awareness, respectively. Survey findings informed the creation of the Oregon CRE Toolkit, a state-specific CRE guide booklet.
Conclusions.A regional epidemiology surveillance and response network has been implemented in Oregon in advance of widespread CRE transmission. Prospective surveillance will determine whether this collaborative approach will be successful at forestalling the emergence of this important healthcare-associated pathogen.

The Millimetre Astronomy Legacy Team 90 GHz (MALT90) survey aims to characterise the physical and chemical evolution of high-mass star-forming clumps. Exploiting the unique broad frequency range and on-the-fly mapping capabilities of the Australia Telescope National Facility Mopra 22 m single-dish telescope1, MALT90 has obtained 3′ × 3′ maps towards ~2 000 dense molecular clumps identified in the ATLASGAL 870 μm Galactic plane survey. The clumps were selected to host the early stages of high-mass star formation and to span the complete range in their evolutionary states (from prestellar, to protostellar, and on to $\mathrm{H\,{\scriptstyle {II}}}$ regions and photodissociation regions). Because MALT90 mapped 16 lines simultaneously with excellent spatial (38 arcsec) and spectral (0.11 km s−1) resolution, the data reveal a wealth of information about the clumps’ morphologies, chemistry, and kinematics. In this paper we outline the survey strategy, observing mode, data reduction procedure, and highlight some early science results. All MALT90 raw and processed data products are available to the community. With its unprecedented large sample of clumps, MALT90 is the largest survey of its type ever conducted and an excellent resource for identifying interesting candidates for high-resolution studies with ALMA.

Foods and dietary patterns that enhance satiety may provide benefit to consumers. The aim of the present review was to describe, consider and evaluate research on potential benefits of enhanced satiety. The proposal that enhanced satiety could only benefit consumers by a direct effect on food intake should be rejected. Instead, it is proposed that there is a variety of routes through which enhanced satiety could (indirectly) benefit dietary control or weight-management goals. The review highlights specific potential benefits of satiety, including: providing appetite control strategies for consumers generally and for those who are highly responsive to food cues; offering pleasure and satisfaction associated with low-energy/healthier versions of foods without feeling ‘deprived’; reducing dysphoric mood associated with hunger especially during energy restriction; and improved compliance with healthy eating or weight-management efforts. There is convincing evidence of short-term satiety benefits, but only probable evidence for longer-term benefits to hunger management, possible evidence of benefits to mood and cognition, inadequate evidence that satiety enhancement can promote weight loss, and no evidence on which consumers would benefit most from satiety enhancement. The appetite-reducing effects of specific foods or diets will be much more subtle than those of pharmaceutical compounds in managing hunger; nevertheless, the experience of pharmacology in producing weight loss via effects on appetite suggests that there is potential benefit of satiety enhancement from foods incorporated into the diet to the consumer.

A survey of the Milky Way disk and the Magellanic System at the wavelengths of the 21-cm atomic hydrogen (H i) line and three 18-cm lines of the OH molecule will be carried out with the Australian Square Kilometre Array Pathfinder telescope. The survey will study the distribution of H i emission and absorption with unprecedented angular and velocity resolution, as well as molecular line thermal emission, absorption, and maser lines. The area to be covered includes the Galactic plane (|b| < 10°) at all declinations south of δ = +40°, spanning longitudes 167° through 360°to 79° at b = 0°, plus the entire area of the Magellanic Stream and Clouds, a total of 13 020 deg2. The brightness temperature sensitivity will be very good, typically σT≃ 1 K at resolution 30 arcsec and 1 km s−1. The survey has a wide spectrum of scientific goals, from studies of galaxy evolution to star formation, with particular contributions to understanding stellar wind kinematics, the thermal phases of the interstellar medium, the interaction between gas in the disk and halo, and the dynamical and thermal states of gas at various positions along the Magellanic Stream.

An understanding of all environmental effects resulting from control actions is important in the selection of a method of managing insect populations. Effects may extend beyond the year of treatment and can modify management decisions. In 1978, a nuclear polyhedrosis virus (NPV) and Bacillus thuringiensis Berliner (B.t.) were tested as aerial spray applications against the western spruce budworm, Choristoneura occidentalis Freeman, on Douglas-fir, Pseudotsuga menziesii (Mirb.) Franco, near Cache Creek, British Columbia. The impact on population densities and on the incidence of NPV was measured in the treated and non-treated areas for 2 vears following the year of application, and is reported here.

A polyhedrosis virus was found in collections of Bruce spanworm, Operophtera bruceata (Hulst), larvae from two locations in British Columbia in 1957 (O. N. Morris, pers. comm.). Bruce spanworm was prevalent in the Maritime Provinces and Quebec in 1963 and 1964 and a polyhedrosis virus was credited with terminating this outbreak (Forbes et al. 1964, 1965; Martineau 1964, 1965). A detailed examination of the virus from Quebec showed that it was a singly-embedded (unicapsid) nuclear polyhedrosis virus (Smirnoff 1964) which is classified as Baculovirus subgroup A (Matthews 1979).
A 2-YEAR STUDY OF VIRUS – CHEMICAL INSECTICIDE COMBINATION IN THE INTEGRATED CONTROL OF THE SPRUCE BUDWORM, CHORISTONEURA FUMIFERANA (TORTRICIDAE: LEPIDOPTERA)

During 1972, spruce budworm infested white spruce and balsam fir trees were sprayed aerially with a low dose of fenitrothion (0.25 oz AI/ac), entomopox virus (EPV) at 7.6 × 1010 polyhedra/ac, nuclear polyhedrosis virus (NPV) at 2.7 × 1011 polyhedra/ac, or each virus–fenitrothion combination. Fenitrothion (active ingredient), NPV, and EPV deposited at ground level at 5%, 31%, and 42%, respectively, of the amounts emitted.
In the year of application, NPV + fenitrothion was highly effective in population reduction and foliage protection especially on; balsam fir although a higher virus infection rate was found on white spruce than on balsam fir. EPV + fenitrothion also provided a high level of foliage protection and apparently reduced surviving female:male sex ratio to 1:2 compared with the normal 1:1 ratio. NPV + insecticide caused highest larval mortality, highest incidence of virus, lowest rate of moth emergence, fewest progeny, and lowest rate of progeny survival. The natural incidence of microsporidia was low in all plots.
In the year following application, the NPV + insecticide treated plot again showed lowest population density, highest larval mortality and incidence of virus, and low defoliation and egg mass density. However, there was a higher proportion of viable eggs deposited than in the previous year. Egg parasitism by Trichogramma minutum increased by 1.5 to 4.2% in plots treated with virus only and declined by 1.6 to 10.5% in insecticide treated plots and by 1.2% in untreated check plot. The transmission of the virus from one year to the next is considered to be of paramount importance in the future use of this pathogen in spruce budworm control.
Statewide Validation of Hospital-Reported Central Line–Associated Bloodstream Infections: Oregon, 2009

Background.
Mandatory reporting of healthcare-associated infections is common, but underreporting by hospitals limits meaningful interpretation.
Objective.To validate mandatory intensive care unit (ICU) central line–associated bloodstream infection (CLABSI) reporting by Oregon hospitals.
Design.Blinded comparison of ICU CLABSI determination by hospitals and health department–based external reviewers with group adjudication.
Setting.Forty-four Oregon hospitals required by state law to report ICU CLABSIs.
Participants.Seventy-six patients with ICU CLABSIs and a systematic sample of 741 other patients with ICU-related bacteremia episodes.
Methods.External reviewers examined medical records and determined CLABSI status. All cases with CLABSI determinations discordant from hospital reporting were adjudicated through formal discussion with hospital staff, a process novel to validation of CLABSI reporting.
Results.Hospital representatives and external reviewers agreed on CLABSI status in 782 (96%) of 817 bacteremia episodes (k = 0.77 [95% confidence interval (CI), 0.70-0.84]). Among the 27 episodes identified as CLABSIs by external reviewers but not reported by hospitals, the final status was CLABSI in 16 (59%). The measured sensitivities of hospital ICU CLABSI reporting were 72% (95% CI, 62%-81%) with adjudicated CLABSI determination as the reference standard and 60% (95% CI, 51%-69%) with external review alone as the reference standard (P = .07). Validation increased the statewide ICU CLABSI rate from 1.21 (95% CI, 0.95-1.51) to 1.54 (95% CI, 1.25-1.88) CLABSIs/1,000 central line–days; ICU CLABSI rates increased by more than 1.00 CLABSI/1,000 central line–days in 6 (14%) hospitals.
Conclusions.Validating hospital CLABSI reporting improves accuracy of hospital-based CLABSI surveillance. Discussing discordant findings improves the quality of validation.

The purpose of the study was to determine patterns of diet use among middle-aged Australian men and women and the relationships between these different usage patterns and demographic characteristics, health status and health habits. A cross-sectional mail survey was conducted among a random sample of 2975 people aged 40–71 years in Victoria, Australia. A total of 1031 usable questionnaires were obtained which included information about the use of diets (e.g. low-fat and low-salt) during the past 3 months along with demographic information, health status and health habits. Based on the responses about the use of thirteen diets for both sexes, latent class analysis was employed to identify the optimal number of use of diets and the assignment of participants to particular groups. Three types of diet uses were identified and provisionally named: diet use, selected diet use and non-diet use. This classification was associated with demographics, health status and health habits, and these associations differed between men and women. The findings suggest that nutrition education programmes should be tailored to the different needs of the diet use groups.

Contractile, histochemical and biochemical properties of the triceps surae were compared in 13 aerobically trained male subjects aged 63 to 76 years. Electrical stimulation of the triceps surae was used to determine muscle twitch, tetanic, and fatigue parameters. From these tests, twitch tension (Pt), time to peak tension (TPT), half relaxation time (RT), tetanic tensions at 10(Po10), 20(Po20), and 50(Po50) Hz and a fatigue index (FI) were calculated. Muscle samples from the belly of the lateral head of the gastrocnemius were obtained using the needle biopsy technique. A portion of each sample was tested histochemically for myosin ATPase (pH 4.30, 4.58 and 10.00) and NADH-tetrazolium reductase in order to classify fibre types (ST, FTa, FTb) and to determine fibre areas. The remainder of each sample was analysed for succinic dehydrogenase (SDH) and phosphofructokinase (PFK) enzyme activities. Significant correlations were found between fibre areas (both ST and FT) and Po10/Po50 and FI. No significant relationships were noted between Pt, 1/2RT and MVC and any histochemical parameter. SDH and PFK activities did not correlate significantly with any histochemical or physiological parameter. Regular endurance exercise apparently does not retard the decline in contractile properties seen with the aging process. Substrate related regulatory enzyme activities (PFK) and marker enzyme activities (SDH) from skeletal muscle of the elderly are much lower than activities found in younger subjects, despite regular aerobic exercise.

Two hundred and forty neurotic out-patients were allocated to no diagnostic category but were rated in terms of observer and self ratings of both depression and anxiety at weekly intervals during a 4-week trial of amitriptyline, diazepam, amitriptyline and diazepam, and placebo. The sample could not be divided into anxious and depressed groups on the basis of the clinical picture. The outcome tended to be good irrespective of medication but the significant drug effects found were due to amitriptyline and concerned measures of anxiety as well as depression. It is concluded that a distinction between anxiety and depression in neurotic out-patients is of no practical value with regard to drug treatment.
Seroprevalence of hepatitis B and C in a Merseyside hospital for the mentally handicapped

This study reports the prevalence of antibodies to hepatitis B virus (HBV) and C virus (HCV). and the frequency of potential exposure to these viruses among patients and staff in six long-stay wards of a hospital caring for mentally handicapped adults from the Mersey region. A retrospective survey of risk behaviour among 134 patients and questionnaire survey of 75 nursing staff was performed. Serum samples from both groups were tested for HBV markers and patient sera for antibodies to HCV by enzyme-linked immunosorbent assay (ELISA). None of the 102 patients tested had antibodies against HCV. although 17 had detectable antibody to HBV core (anti-HBc). Seven out of the 17 were positive for HBV surface antigen. None was positive for IgM antibody to HBV core. Only 1 out of 61 staff had anti-HBc and none was positive for surface antigen. Twenty-nine of 75 (39%) staff reported bites sufficient to break the skin and 52 (69%) significant other injuries from patients; 25 (31%) of staff had not received HBV vaccination. None of the patients had received HBV vaccine.
We conclude that HCV does not appear to be a major hazard in this closed community but the prevalence of HBV markers indicating past exposure among patients is high, vaccine uptake is incomplete and incidents which may allow viral transmission are frequent.
