Lessons learned from over a decade of data audits in international observational HIV cohorts in Latin America and East Africa

Introduction:
Routine patient care data are increasingly used for biomedical research, but such “secondary use” data have known limitations, including their quality. When leveraging routine care data for observational research, developing audit protocols that can maximize informational return and minimize costs is paramount.
Methods:For more than a decade, the Latin America and East Africa regions of the International epidemiology Databases to Evaluate AIDS (IeDEA) consortium have been auditing the observational data drawn from participating human immunodeficiency virus clinics. Since our earliest audits, where external auditors used paper forms to record audit findings from paper medical records, we have streamlined our protocols to obtain more efficient and informative audits that keep up with advancing technology while reducing travel obligations and associated costs.
Results:We present five key lessons learned from conducting data audits of secondary-use data from resource-limited settings for more than 10 years and share eight recommendations for other consortia looking to implement data quality initiatives.
Conclusion:After completing multiple audit cycles in both the Latin America and East Africa regions of the IeDEA consortium, we have established a rich reference for data quality in our cohorts, as well as large, audited analytical datasets that can be used to answer important clinical questions with confidence. By sharing our audit processes and how they have been adapted over time, we hope that others can develop protocols informed by our lessons learned from more than a decade of experience in these large, diverse cohorts.

Background: We aim to assess the effect of simultaneous acute code stroke activation(ACSA) in patients undergoing reperfusion therapies in the emergency department on home time at 90 days. Methods: We assessed ACSA over 20 months from the QuICR(Quality Improvement and Clinical Research Alberta Stroke Program) Registry. We defined Simultaneous reperfusion therapy as, ACSA within 60 min of the arrival of any patient receiving intravenous thrombolysis or ACSA within 150 min of the arrival of any patient receiving endovascular thrombectomy (based on the Canadian Triage and Acuity Scale, average localdoor-to-needle and door-to-puncture times)Results: A total of 2607 ACSA occurred at a mean±SD of 130.8±17.1 per month during the study period. 545 (20.9%) underwent acute reperfusion therapy with a mean age of 70.6±14.2 years, 45.9%(n=254) were female and a median (IQR) NIHSS of 13(8-18). Simultaneous reperfusion therapies occurred in 189(34.6%). There was no difference in the median door-to-CT time between the simultaneous (16, 11-23 min) and non-simultaneous (15, 11–21 min, p=0.3) activations. There was no difference in the median home time at 90 days between the two groups. Conclusions: Simultaneous ACSA occurs in one-third of patients receiving acute reperfusion therapies. An optimal workflow may help mitigate the clinical and system burden associated with simultaneity.

More than 50 million older people worldwide are suffering from dementia, and this number is estimated to increase to 150 million by 2050. Greater caregiver burdens and financial impacts on the healthcare system are expected as we wait for an effective treatment for dementia. Researchers are constantly exploring new therapies and screening approaches for the early detection of dementia. Artificial intelligence (AI) is widely applied in dementia research, including machine learning and deep learning methods for dementia diagnosis and progression detection. Computerized apps are also convenient tools for patients and caregivers to monitor cognitive function changes. Furthermore, social robots can potentially provide daily life support or guidance for the elderly who live alone. This review aims to provide an overview of AI applications in dementia research. We divided the applications into three categories according to different stages of cognitive impairment: (1) cognitive screening and training, (2) diagnosis and prognosis for dementia, and (3) dementia care and interventions. There are numerous studies on AI applications for dementia research. However, one challenge that remains is comparing the effectiveness of different AI methods in real clinical settings.

Background: Acute Necrotizing Encephalopathy (ANEC) is a rare neuroinflammatory disorder involving the deep grey matter following viral infection and has been associated with the RANBP2 gene. We aimed to evaluate clinical and imaging features in ANEC patients. Methods: This retrospective chart review of ANEC patients (2012-2020) seen at a tertiary pediatric center included analysis of outcomes including ANE-Severity Score, Expanded Disability Status Scale (EDSS) and the modified Rankin Scale (mRS), semi-quantitative imaging scores (degree of swelling or hemorrhage rated 0 (none)-5 (severe/massive)), and dichotomous outcomes including RANBP2 gene status, influenza status. Results: 20 patients were included (Avg. age at presentation 3.5 yrs IQR=3.56., F:M 2.33:1). 3/20 experienced recurrences. All patients with recurrences were positive for RANBP2 mutations. 10/20 patients were influenza positive. 7/20 were RANBP2 mutation positive. We observed higher likelihood of hemorrhage in influenza-positive compared to negative patients (W=78, p=0.048). Kaplan-Meier survival curve analysis revealed that patients without brainstem lesions were more likely to reach minimal/no disability (EDSS<=2) than patients with brainstem lesions (p=0.035). Conclusions: Hemorrhage is more likely to be seen in children with ANEC who are positive for influenza. RANBP2 status was predictive of relapse but not predictive of overall outcome.

Objective:
The coronavirus disease 2019 (COVID-19) pandemic has resulted in shortages of personal protective equipment (PPE), underscoring the urgent need for simple, efficient, and inexpensive methods to decontaminate masks and respirators exposed to severe acute respiratory coronavirus virus 2 (SARS-CoV-2). We hypothesized that methylene blue (MB) photochemical treatment, which has various clinical applications, could decontaminate PPE contaminated with coronavirus.
Design:The 2 arms of the study included (1) PPE inoculation with coronaviruses followed by MB with light (MBL) decontamination treatment and (2) PPE treatment with MBL for 5 cycles of decontamination to determine maintenance of PPE performance.
Methods:MBL treatment was used to inactivate coronaviruses on 3 N95 filtering facepiece respirator (FFR) and 2 medical mask models. We inoculated FFR and medical mask materials with 3 coronaviruses, including SARS-CoV-2, and we treated them with 10 µM MB and exposed them to 50,000 lux of white light or 12,500 lux of red light for 30 minutes. In parallel, integrity was assessed after 5 cycles of decontamination using multiple US and international test methods, and the process was compared with the FDA-authorized vaporized hydrogen peroxide plus ozone (VHP+O3) decontamination method.
Results:Overall, MBL robustly and consistently inactivated all 3 coronaviruses with 99.8% to >99.9% virus inactivation across all FFRs and medical masks tested. FFR and medical mask integrity was maintained after 5 cycles of MBL treatment, whereas 1 FFR model failed after 5 cycles of VHP+O3.
Conclusions:MBL treatment decontaminated respirators and masks by inactivating 3 tested coronaviruses without compromising integrity through 5 cycles of decontamination. MBL decontamination is effective, is low cost, and does not require specialized equipment, making it applicable in low- to high-resource settings.
A period of 10 weeks of increased protein consumption does not alter faecal microbiota or volatile metabolites in healthy older men: a randomised controlled trial

Diet has a major influence on the composition and metabolic output of the gut microbiome. Higher-protein diets are often recommended for older consumers; however, the effect of high-protein diets on the gut microbiota and faecal volatile organic compounds (VOC) of elderly participants is unknown. The purpose of the study was to establish if the faecal microbiota composition and VOC in older men are different after a diet containing the recommended dietary intake (RDA) of protein compared with a diet containing twice the RDA (2RDA). Healthy males (74⋅2 (sd 3⋅6) years; n 28) were randomised to consume the RDA of protein (0⋅8 g protein/kg body weight per d) or 2RDA, for 10 weeks. Dietary protein was provided via whole foods rather than supplementation or fortification. The diets were matched for dietary fibre from fruit and vegetables. Faecal samples were collected pre- and post-intervention for microbiota profiling by 16S ribosomal RNA amplicon sequencing and VOC analysis by head space/solid-phase microextraction/GC-MS. After correcting for multiple comparisons, no significant differences in the abundance of faecal microbiota or VOC associated with protein fermentation were evident between the RDA and 2RDA diets. Therefore, in the present study, a twofold difference in dietary protein intake did not alter gut microbiota or VOC indicative of altered protein fermentation.

Background and Aims:
Literature about suicide behavior often shows equity between suicide attempt and deliberate self harm (DSH). The objective of this study is to differentiate suicide attempt from DSH regarding the frequency, methods used, reasons, and purposes.
Methods:Data were collected from 1,551 junior high school students from Mexico City Downtown (2003 and 2006 academic generations). The questionnaire included the CIP (Cédula de Indicadores Parasuicidas: Parasuicide Indicators Schedule), suicide ideation, depression, and others. Participation in the study was voluntary and anonymous.
Results:DSH overall prevalence was 5.8% male and 20.5% female, including 3.9% and 16.7% of ambivalent suicide attempt, and 0.7% and 6.1% of suicide attempt with death purpose. Most of the boys who attempted suicide used sharp objects. Reasons in both sexes were mostly related to interpersonal area, and the purpose was to stop suffering. The most frequent method used in DSH was sharp objects; reasons for boys were in the emotional area and in the interpersonal for girls. The purpose was continue living.
Conclusions:It is important to distinguish between suicide attempt and DSH to obtain more precise information about the problems and its characteristics. Results show that both of these are a considerable matter among adolescents; tough they have different characteristics, with a considerable suicide risk. Mental health promotion and attention must begin during elementary school in order to prevent more serious distress and illness during adolescence and adult age.
EPA-0298 – Pregabalin Abuse Among Opiate Addicted Patients

Introduction:
Pregabalin is a novel GABA-analogue approved for the treatment of partial onset seizures, neuropathic pain, and general anxiety disorder. Pregabalin has been classified as a Schedule V drug with a low risk of inflicting abuse or addiction. However, some publications have indicated that pregabalin may have a potential for abuse among patients with past or current opiate addiction.
Aims:We hypothesized that pregabalin might be abused my patients who were undergoing an opiate replacement therapy and never had an indication for taking pregabalin on medical grounds.
Methods:Urine specimens from 124 patients with opiate dependency syndrome and from 111 patients with other addiction disorders (alcohol, benzodiazepines, cannabis, amphetamines) were screened for pregabalin by means of a mass spectrometer analysis.
Results:We found 12.1% of all urine specimens from patients with opiate addiction to be positive for pregabalin. None of the patients concerned had a medical indication for using pregabalin. In the control group, 2.7% of the patients were tested positively for pregabalin, due to their taking it regularly for chronic pain or general anxiety.
Conclusions:Our data suggest that pregabalin is liable to be abused among patients with opiate dependency syndrome. Thus, vigilance and caution are called for when patients with a past or current opiate dependency are exposed to treatment with pregabalin.

Background
There has been increasing evidence that chronic low-grade inflammation is associated with mood disorders. However, the findings have been inconsistent because of heterogeneity across studies and methodological limitations. Our aim is to prospectively evaluate the bi-directional associations between inflammatory markers including interleukin (IL)-6, tumor necrosis factor (TNF)-α and high sensitivity C-reactive protein (hsCRP) with mood disorders.
MethodsThe sample consisted of 3118 participants (53.7% women; mean age: 51.0, s.d. 8.8 years), randomly selected from the general population, who underwent comprehensive somatic and psychiatric evaluations at baseline and follow-up (mean follow-up duration = 5.5 years, s.d. 0.6). Current and remitted mood disorders including bipolar and major depressive disorders (MDD) and its subtypes (atypical, melancholic, combined atypical and melancholic, and unspecified) were based on semi-structured diagnostic interviews. Inflammatory biomarkers were analyzed in fasting blood samples. Associations were tested by multiple linear and logistic regression models.
ResultsCurrent combined MDD [β = 0.29, 95% confidence interval (CI) 0.03–0.55] and current atypical MDD (β = 0.32, 95% CI 0.10–0.55) at baseline were associated with increased levels of hsCRP at follow-up. There was little evidence for inflammation markers at baseline predicting mood disorders at follow-up.
ConclusionsThe prospective unidirectional association between current MDD subtype with atypical features and hsCRP levels at follow-up suggests that inflammation may be a consequence of this condition. The role of inflammation, particularly hsCRP that is critically involved in cardiovascular diseases, warrants further study. Future research that examines potential influences of medications on inflammatory processes is indicated.
Sorption of Bentazon and Degradation Products in Two Mississippi Soils

Bentazon degradation in soil typically proceeds with development of bound residue. Low sorption of bentazon suggests that this residue consists of degradation products; however, there is little data on the sorption behavior of these products. This study was undertaken to determine the sorption of bentazon and the degradation products 2-amino-N-isopropyl benzamide, 2-aminobenzoic acid, and N-methyl bentazon in Dundee silt loam and Sharkey clay, two common agricultural soils of the Mississippi Delta. Greater sorption of bentazon and degradation products in the Sharkey soil was related to finer texture and higher organic C content. Isotherms were nonlinear, with sorption increasing in the order bentazon ≪ 2-amino-N-isopropyl benzamide < N- methyl bentazon. In general, methanol extraction indicated reversible sorption of these compounds. Therefore, it is unlikely that sorption of either 2-amino-N-isopropyl benzamide or N-methyl bentazon contributes to bound residue. In contrast, essentially all of the 14C sorbed from radiolabeled 2-aminobenzoic acid solution remained bound after methanol extraction. However, due to degradation of 2-aminobenzoic acid, it was not possible to conclude the extent to which this compound contributed to bound 14C. Sorption of 14C from 2-aminobenzoic acid exceeded that of N-methyl bentazon, and at low initial concentrations (≤ 20 μM) was nearly 1000-fold greater than bentazon sorption.
Developing an Empirical Yield-Prediction Model Based on Wheat and Wild Oat (Avena fatua) Density, Nitrogen and Herbicide Rate, and Growing-Season Precipitation

To develop a more complete understanding of the ecological factors that regulate crop productivity, we tested the relative predictive power of yield models driven by five predictor variables: wheat and wild oat density, nitrogen and herbicide rate, and growing-season precipitation. Existing data sets were collected and used in a meta-analysis of the ability of at least two predictor variables to explain variations in wheat yield. Yield responses were asymptotic with increasing crop and weed density; however, asymptotic trends were lacking as herbicide and fertilizer levels were increased. Based on the independent field data, the three best-fitting models (in order) from the candidate set of models were a multiple regression equation that included all five predictor variables (R2 = 0.71), a double-hyperbolic equation including three input predictor variables (R2 = 0.63), and a nonlinear model including all five predictor variables (R2 = 0.56). The double-hyperbolic, three-predictor model, which did not include herbicide and fertilizer influence on yield, performed slightly better than the five-variable nonlinear model including these predictors, illustrating the large amount of variation in wheat yield and the lack of concrete knowledge upon which farmers base their fertilizer and herbicide management decisions, especially when weed infestation causes competition for limited nitrogen and water. It was difficult to elucidate the ecological first principles in the noisy field data and to build effective models based on disjointed data sets, where none of the studies measured all five variables. To address this disparity, we conducted a five-variable full-factorial greenhouse experiment. Based on our five-variable greenhouse experiment, the best-fitting model was a new nonlinear equation including all five predictor variables and was shown to fit the greenhouse data better than four previously developed agronomic models with an R2 of 0.66. Development of this mathematical model, through model selection and parameterization with field and greenhouse data, represents the initial step in building a decision support system for site-specific and variable-rate management of herbicide, fertilizer, and crop seeding rate that considers varying levels of available water and weed infestation.
Selection of a Barley Yield Model Using Information–Theoretic Criteria

Empirical models of crop–weed competition are integral components of bioeconomic models, which depend on predictions of the impact of weeds on crop yields to make cost-effective weed management recommendations. Selection of the best empirical model for a specific crop–weed system is not straightforward, however. We used information–theoretic criteria to identify the model that best describes barley yield based on data from barley–wild oat competition experiments conducted at three locations in Montana over 2 yr. Each experiment consisted of a complete addition series arranged as a randomized complete block design with three replications. Barley was planted at 0, 0.5, 1, and 2 times the locally recommended seeding rate. Wild oat was planted at target infestation densities of 0, 10, 40, 160, and 400 plants m−2. Twenty-five candidate yield models were used to describe the data from each location and year using maximum likelihood estimation. Based on Akaike's Information Criterion (AIC), a second-order small-sample version of AIC (AICc), and the Bayesian Information Criterion (BIC), most data sets supported yield models with crop density (Dc), weed density (Dw), and the relative time of emergence of the two species (T) as variables, indicating that all variables affected barley yield in most locations. AIC, AICc, and BIC selected identical best models for all but one data set. In contrast, the Information Complexity criterion, ICOMP, generally selected simpler best models with fewer parameters. For data pooled over years and locations, AIC, AICc, and BIC strongly supported a single best model with variables Dc, Dw, T, and a functional form specifying both intraspecific and interspecific competition. ICOMP selected a simpler model with Dc and Dw only, and a functional form specifying interspecific, but no intraspecific, competition. The information–theoretic approach offers a rigorous, objective method for choosing crop yield and yield loss equations for bioeconomic models.

Africa is experiencing a rapid increase in adult obesity and associated cardiometabolic diseases (CMDs). The H3Africa AWI-Gen Collaborative Centre was established to examine genomic and environmental factors that influence body composition, body fat distribution and CMD risk, with the aim to provide insights towards effective treatment and intervention strategies. It provides a research platform of over 10 500 participants, 40–60 years old, from Burkina Faso, Ghana, Kenya and South Africa. Following a process that involved community engagement, training of project staff and participant informed consent, participants were administered detailed questionnaires, anthropometric measurements were taken and biospecimens collected. This generated a wealth of demographic, health history, environmental, behavioural and biomarker data. The H3Africa SNP array will be used for genome-wide association studies. AWI-Gen is building capacity to perform large epidemiological, genomic and epigenomic studies across several African counties and strives to become a valuable resource for research collaborations in Africa.

The properties of high frequency capillary waves generated by steep gravity waves on deep water have been measured with a high resolution laser optical slope gauge. The results have been compared with the steady theory of Longuet-Higgins (1963). Good qualitative agreement is obtained. However, the quantitative predictions of the capillary wave slopes cannot be verified by the data because the theory requires knowledge of an idealized quantity - the crest curvature of the gravity wave in the absence of surface tension - which cannot be measured experimentally.
