Chronic suppurative otitis media in Zimbabwean school children: a cross-sectional study

Objective
Chronic suppurative otitis media is a major cause of disabling childhood hearing loss, especially in low-income countries. Estimates on its prevalence in sub-Saharan Africa range from the lowest to the highest in the world (less than one per cent to more than five per cent). However, the prevalence of chronic suppurative otitis media in Zimbabwe is largely unknown. This study aimed to determine the prevalence of paediatric chronic suppurative otitis media and other middle-ear pathology in rural Zimbabwe.
MethodA cross-sectional study was performed in primary school children aged 4–13 years from the rural province of Mashonaland East. Participants underwent video otoscopy and tympanometry.
ResultsOut of 451 examined children, two (0.4 per cent) had chronic suppurative otitis media. Acute otitis media was present in one (0.2 per cent), otitis media with effusion was present in five (1.1 per cent) and scarring was present in 69 (15.3 per cent).
ConclusionChronic suppurative otitis media and otitis media sequelae were surprisingly uncommon in this sample of rural primary school children in Zimbabwe. More studies, preferably population-based, are needed to enable more precise estimates of chronic suppurative otitis media prevalence in Zimbabwe.

Introduction
Cardiometabolic diseases are under-diagnosed and under-treated in psychiatric inpatients. A common solution is implementation of screening guidelines. However, we often forget to ask the essential question: Why were our patients underdiagnosed in the first place?
ObjectiveTo identify causes of under-diagnosing and under-treatment in the context of local clinical practices.
MethodAll consecutive admissions (n=63) over 93 days were screened for hypertension, diabetes, dyslipidemias and abdominal obesity. Patients identified with both (a) undiagnosed and untreated disease and (b) admission on a separate occasion within the previous two years to the ward in question, were selected for a retrospective patient file review. These patients (n=32) had their medical records scrutinized for information on how each cardiometabolic measure was assessed at previous admission.
ResultsRelevant examination were performed in 38% of cases and test results were abnormal for 54% of these. Abnormal test results were evaluated in 14% of cases and none of the evaluated results were acted upon.
ConclusionRigorous examination and testing of patients is only of value if clinicians have the essential knowledge to interpret the results and feel responsibility to do so. These barriers must be broken down before implementing screening guidelines in routine clinical practice.

Introduction:
Psychotic depression (PD) is classified as a sybtype of severe depression in the current diagnostic manuals. Accordingly, it is a common conception that psychotic features in depression arise as a consequence of depressive severity.
Objectives:To determine whether the severity of depression and psychosis correlate in accordance with the “severity-psychosis” hypothesis and to detect potential differences in clinical features of psychotic and non-psychotic depression (non-PD).
Aims:We aimed to answer the following questions:
Does the clinical profile differ between patients with PD and non-PD?
Is the severity of depression and psychosis correlated in patients with depression?
Methods:Quantitative analysis of Health of the Nation Outcome Scales (HoNOS) scores from all patients admitted to a Danish general psychiatric hospital between 2000 and 2010 due to a severe depressive episode.
Results:A total of 357 patients with severe depression, of which 125 (35%) were of the psychotic subtype, formed the study sample. Mean HoNOS scores at admission differed significantly between patients with non-PD and PD on the items hallucinations and delusions (non-PD = 0.33 vs. PD = 1.37, p < 0.001), aggression (non-PD = 0.20 vs. PD = 0.36, p = 0.044) and on the total score (non-PD = 10.55 vs. PD = 11.87, p = 0.024). the HoNOS scores on the two items “depression” and “hallucinations and delusions” were very weakly correlated (Spearman coefficient = 0.12).
Conclusions:The results suggest that the severity of depression is unlikely to be the key determinant for the development of psychosis and supports the hypothesis that the psychotic- and non-psychotic subtypes of depression are in fact distinct clinical syndromes.
Light as an Aid for Recovery in Psychiatric Inpatients: A Randomized Controlled Effectiveness Pilot Trial

Introduction
Electric indoor lighting can disturb sleep and increase depressive symptoms; both common complaints in psychiatric inpatients.
AimsTo improve quality of sleep in patients using an indoor hospital lighting environment simulating nature in intensity, color, and circadian timing.
MethodsInvestigator-blinded parallel group randomized controlled effectiveness trial supplied with qualitative interviews in an inpatient psychiatric ward with fully automatic and adjustable lighting. Admitted patients received a room with a naturalistic lighting environment (intervention group) or lighting as usual (control group). The primary outcome was the Pittsburg Sleep Quality Index and secondary outcomes included the Major Depression Inventory and WHO-five Well- Being Index.
ResultsIn this ongoing trial, we included 28 patients (16 treated and 12 controls). Patients in the intervention group reported higher subjective sleep quality and sleep efficiency, lower use of sleep medication (mean difference, 4.68 mg; 95% CI, 0.54; 53.5), fewer depressive symptoms (mean difference, 5; 95% CI,–2; 13), but lower well-being (difference,–4 percentage points; 95% CI,–20; 16), compared with the control group. At discharge, fewer patients in the intervention group had experienced use of involuntary treatment. Qualitative data indicated no side effects apart from issues in performing indoor leisure activities in dim light.
ConclusionsA naturalistic lighting environment was safe and improved sleep and mood in our small patient sample. The trial integrated well with routine clinical care and our sample reflected the heterogeneity of the target population (Funded by Region Midtjylland and others; Clinicaltrials.gov number, NCT02653040)

In order to exploit potentials of 20–40% reduction of herbicide use, as documented by use of Decision Support Systems (DSS), where requirements for manual field inspection constitute a major obstacle, large numbers of digital pictures of weed infestations have been collected and analysed manually by crop advisors. Results were transferred to: 1) DSS, which determined needs for control and connected, optimized options for control returned options for control and 2) convolutional, neural networks, which in this way were trained to enable automatic analysis of future pictures, which support both field- and site-specific integrated weed management.

Introduction
The Learning Health System Network clinical data research network includes academic medical centers, health-care systems, public health departments, and health plans, and is designed to facilitate outcomes research, pragmatic trials, comparative effectiveness research, and evaluation of population health interventions.
MethodsThe Learning Health System Network is 1 of 13 clinical data research networks assembled to create, in partnership with 20 patient-powered research networks, a National Patient-Centered Clinical Research Network.
Results and ConclusionsHerein, we describe the Learning Health System Network as an emerging resource for translational research, providing details on the governance and organizational structure of the network, the key milestones of the current funding period, and challenges and opportunities for collaborative science leveraging the network.
The introduction of emergency cricothyroidotomy simulation training in Zimbabwe contributed to the saving of two lives

Background:
In developing countries with limited access to ENT services, performing emergency cricothyroidotomy in patients with upper airway obstruction may be a life-saving last resort. An established Danish–Zimbabwean collaboration of otorhinolaryngologists enrolled Zimbabwean doctors into a video-guided simulation training programme on emergency cricothyroidotomy. This paper presents the positive effect of this training, illustrated by two case reports.
Case reports:A 56-year-old female presented with upper airway obstruction due to a rapidly progressing infectious swelling of the head and neck progressing to cardiac arrest. Cardiopulmonary resuscitation was initiated and a secure surgical airway was established via an emergency cricothyroidotomy, saving the patient. A 70-year-old male presented with upper airway obstruction secondary to intubation for an elective procedure. When extubated, the patient exhibited severe stridor followed by respiratory arrest. Re-intubation attempts were unsuccessful and emergency cricothyroidotomy was performed to secure the airway, preserving the life of the patient.
Conclusion:Emergency cricothyroidotomy training should be considered for all surgeons, anaesthetists and, eventually, emergency and recovery room personnel in developing countries. A video-guided simulation training programme on emergency cricothyroidotomy in Zimbabwe proved its value in this regard.

Abundant tubular macrofossils occur in finely laminated siltstones and shales of the 548–542 Ma Schwarzrand Subgroup, Nama Group, Namibia. The Nama tubes occur in both the Vingerbreek and Feldschuhhorn members commonly in dense populations and always in fine-grained, lower shore-face lithologies deposited below fair-weather wave base. The tubes are preserved mostly as compressed casts and molds that range in width from 0.6 to 2.1 mm; apparently incomplete specimens reach lengths up to 10 cm. All specimens show sinuous bending and occasional brittle fracture, indicating an original construction of strong but flexible organic matter. Feldschuhhorn specimens preserve fine longitudinal pleats or folds that record pliant organic walls, but the older Vingerbreek populations do not. Similarly, some specimens in the Feldschuhhorn Member display branching, while Vingerbreek tubes do not. The abundant Feldschuhhorn tubes are assigned to the widespread Ediacaran problematicum Vendotaenia antiqua; however, the distinctive Vingerbreek population remains in open nomenclature. The most abundant fossils in Nama rocks, these tubes resemble populations in Ediacaran successions from Russia, China, Spain, and elsewhere. Beyond their local importance, then, such tubes may turn out to be the most abundant record of Ediacaran life.
Evaluation of in situ valine production by Bacillus subtilis in young pigs

Mutants of Bacillus subtilis can be developed to overproduce Val in vitro. It was hypothesized that addition of Bacillus subtilis mutants to pig diets can be a strategy to supply the animal with Val. The objective was to investigate the effect of Bacillus subtilis mutants on growth performance and blood amino acid (AA) concentrations when fed to piglets. Experiment 1 included 18 pigs (15.0±1.1 kg) fed one of three diets containing either 0.63 or 0.69 standardized ileal digestible (SID) Val : Lys, or 0.63 SID Val : Lys supplemented with a Bacillus subtilis mutant (mutant 1). Blood samples were obtained 0.5 h before feeding and at 1, 2, 3, 4, 5 and 6 h after feeding and analyzed for AAs. In Experiment 2, 80 piglets (9.1±1.1 kg) were fed one of four diets containing 0.63 or 0.67 SID Val : Lys, or 0.63 SID Val : Lys supplemented with another Bacillus subtilis mutant (mutant 2) or its parent wild type. Average daily feed intake, daily weight gain and feed conversion ratio were measured on days 7, 14 and 21. On day 17, blood samples were taken and analyzed for AAs. On days 24 to 26, six pigs from each dietary treatment were fitted with a permanent jugular vein catheter, and blood samples were taken for AA analysis 0.5 h before feeding and at 1, 2, 3, 4, 5 and 6 h after feeding. In experiment 1, Bacillus subtilis mutant 1 tended (P<0.10) to increase the plasma levels of Val at 2 and 3 h post-feeding, but this was not confirmed in Experiment 2. In Experiment 2, Bacillus subtilis mutant 2 and the wild type did not result in a growth performance different from the negative and positive controls. In conclusion, results obtained with the mutant strains of Bacillus subtilis were not better than results obtained with the wild-type strain, and for both strains, the results were not different than the negative control.

The objective was to study the effect of type of concentrate with varying starch and fibre content on growth and gastrointestinal development in preweaned dairy calves. Thirty-two newborn Danish Holstein male calves were allocated to four treatment groups in eight blocks of four calves. An experimental low-starch, high-molasses, high-fibre (EXP) concentrate or a traditional high-starch (TRA) concentrate were fed either at a high (HIGH; 2 × 3.2 kg/day) or a low (LOW; 2 × 1.6 kg/day) whole milk allowance in a 2 × 2 factorial design. TRA contained 350 and EXP 107 g starch/kg dry matter (DM), whereas the NDF content was 136 and 296 g/kg DM, respectively. Metabolizable energy (ME) was 11.2 and 12.2 MJ ME/kg DM in EXP and TRA, respectively. All calves had free access to artificially dried grass hay (9.8 MJ ME/kg DM). Four calves were culled during the experiment. The calves were euthanized either at 38 (12 calves) or 56 days (16 calves) of age. Evaluated across both slaughter ages, there was no difference between TRA and EXP in concentrate and hay intake, rumen weight and papillation. EXP resulted in increased villi number in duodenum and jejunum compared with TRA. Concentrate intake and reticulo-rumen weight was higher for LOW compared with HIGH milk allowance, whereas live weight gain was 20% lower. The results show that a low-starch, high-molasses, high-fibre concentrate with 8% lower ME content tended to reduce daily gain compared with a traditional calf starter concentrate, but resulted in similar ruminal development in preweaned calves both on a high and a low milk allowance fed along with grass hay. Furthermore, the results suggest that the experimental concentrate stimulated intestinal villi growth over that of the traditional concentrate.

The European Model for Inhabited Areas (ERMIN) was developed to allow a user to explore different recovery options following the contamination of an urban environment with radioactive material and to refine an appropriate strategy for the whole region affected. The input data include a description of the environment, initial deposition of radionuclides on to a reference surface and a description of countermeasures. Output information includes the average doses to members of the public from external exposure to gamma and beta radiation from deposited radionuclides and inhalation of resuspended radioactivity, the contamination on urban surfaces, the activity concentration in air from resuspension, the doses to workers undertaking the recovery work, the quantity and activity of waste generated and the cost and work required to implement the countermeasure. ERMIN has been designed to be implemented as a tool that supports the approach of decision-makers and allows the area to be broken down into smaller regions where different conditions prevail and different countermeasure packages are enacted.
Associations between water-treatment methods and diarrhoea in HIV-positive individuals

This manuscript extends our previously published work (based on data from one clinic) on the association between three drinking water-treatment modalities (boiling, filtering, and bottling) and diarrhoeal disease in HIV-positive persons by incorporating data from two additional clinics collected in the following year. We conducted a cross-sectional survey of drinking water patterns, medication usage, and episodes of diarrhoea among HIV-positive persons attending clinics associated with the San Francisco Community Consortium. We present combined results from our previously published work in one clinic (n = 226) with data from these two additional clinics (n = 458). In this combined analysis we employed logistic regression and marginal structural modelling of the data. The relative risk of diarrhoea for ‘always’ vs. ‘never’ drinking boiled water was 0.68 (95% CI 0.45–1.04) and for ‘always’ vs. ‘never’ drinking bottled water was 1.22 (95% CI 0.82–1.82). Drinking filtered water was unrelated to diarrhoea [1.03 (95% CI 0.78, 1.35) for ‘always’ vs. ‘never’ drinking filtered water]. Adjustment for confounding did not have any notable effect on the point estimates (0.61, 1.35 and 0.98 for boiled, bottled, and filtered water respectively, as defined above). The risk of diarrhoea was lower among those consuming boiled water but this finding was not statistically significant. Because of these findings, the importance of diarrhoea in immunocompromised individuals, and the limitations of cross-sectional data further prospective investigations of water consumption and diarrhoea among HIV-positive individuals are needed.
Risk factors in HIV-associated diarrhoeal disease: the role of drinking water, medication and immune status

In a cross-sectional survey of 226 HIV-infected men, we examined the occurrence of diarrhoea and its relationship to drinking water consumption patterns, risk behaviours, immune status and medication use. Diarrhoea was reported by 47% of the respondents. Neither drinking boiled nor filtered water was significantly associated with diarrhoea (OR = 0·5 [0·2, 1·6], 1·2 [0·6, 2·5] respectively), whereas those that drank bottled water were at risk for diarrhoea (OR = 3·0 [1·1, 7·8]). Overall, 47% always or often used at least one water treatment. Of the 37% who were very concerned about drinking water, 62% had diarrhoea, 70% always or often used at least one water treatment. An increase in CD4 count was protective only for those with a low risk of diarrhoea associated with medication (OR = 0·6 [0·5, 0·9]). A 30% attributable risk to diarrhoea was estimated for those with high medication risk compared to those with low medication risk. The significant association between concern with drinking water and diarrhoea as well as between concern with drinking water and water treatment suggests awareness that drinking water is a potential transmission pathway for diarrhoeal disease. At the same time we found that a significant portion of diarrhoea was associated with other sources not related to drinking water such as medication usage.

Size, or its commonly used proxy live weight, is a necessary input when calculating the energy requirements of an animal. It is also a major factor in determining the intake capacity of an animal. The sole use of live weight as a determinant of size incorporates the implicit assumption that body fat and body protein mass are equivalent for the purposes of calculating energy requirements and intake capacity. Recent evidence indicates that this is not so in either case (Birnie et al., 2000; Friggens et al., 1998). The use of live weight may be acceptable where it can be reasonably assumed that there is a stable relationship between body fat and protein. However, when making breed or parity comparisons there is no reason to assume a stable relationship between body fat and protein. Meaningful comparisons can be made if live weights can be adjusted for differences in body fat content. In the applied context this means adjusting to a standard body condition score. This study provided the opportunity to examine the relationships between condition score and live weight in three breeds across three parities.
