CBT for difficult-to-treat depression: self-regulation model

Background:
Cognitive behavioural therapy (CBT) is an effective treatment for depression but a significant minority of clients do not complete therapy, do not respond to it, or subsequently relapse. Non-responders, and those at risk of relapse, are more likely to have adverse childhood experiences, early-onset depression, co-morbidities, interpersonal problems and heightened risk. This is a heterogeneous group of clients who are currently difficult to treat.
Aim:The aim was to develop a CBT model of depression that will be effective for difficult-to-treat clients who have not responded to standard CBT.
Method:The method was to unify theory, evidence and clinical strategies within the field of CBT to develop an integrated CBT model. Single case methods were used to develop the treatment components.
Results:A self-regulation model of depression has been developed. It proposes that depression is maintained by repeated interactions of self-identity disruption, impaired motivation, disengagement, rumination, intrusive memories and passive life goals. Depression is more difficult to treat when these processes become interlocked. Treatment based on the model builds self-regulation skills and restructures self-identity, rather than target negative beliefs. A bespoke therapy plan is formed out of ten treatment components, based on an individual case formulation.
Conclusions:A self-regulation model of depression is proposed that integrates theory, evidence and practice within the field of CBT. It has been developed with difficult-to-treat cases as its primary purpose. A case example is described in a concurrent article (Barton et al., 2022) and further empirical tests are on-going.

Objective:
Examine impact of audit and feedback on antipsychotic prescribing in schizophrenia over 4.5-years.
Methods:Clinical files in three outpatient psychiatric services in Auckland, New Zealand were reviewed at two time-points (March-2000;October-2004). After the first audit, feedback was provided to all three services. Baseline prescribing variations between services were found for antipsychotic combinations and atypical prescribing, in particular clozapine. In two services audit and feedback continued with two interim reviews (October-2001;March-2003). Specific feedback and interventions targeting clozapine use were introduced in both services. No further audit or feedback occurred in the third service until the final audit. Data were collected (patient characteristics, diagnosis, antipsychotic treatment) and analysed at each audit.
Results:Three prescribing variables (antipsychotic monotherapy, atypical and clozapine use) were consistent with practice recommendations at the final audit (85.7%, 82.7% and 34.5% respectively) and had changed in the desired direction for all three services over 4.5-years. At baseline there were differences between the three services. One service had baseline prescribing variables closest to recommendations, was actively involved in audit, and improved further. The second service, also actively involved in audit had baseline prescribing variables further from recommendations but improved the most. The service not involved in continuing audit and feedback made smaller changes, and atypical and clozapine use at endpoint were significantly lower despite at baseline being comparable to the service which improved the most.
Conclusions:We found high intensity audit and feedback was an effective intervention in closing the gap between recommended and clinical practice for antipsychotic prescribing.

Introduction
Despite improvements in the medical and surgical management of infants with CHD, growth failure before surgery in many infants continues to be a significant concern. A nutritional pathway was developed, the aim of which was to provide a structured approach to nutritional care for infants with CHD awaiting surgery.
Materials and methodsThe modified Delphi process was development of a nutritional pathway; initial stakeholder meeting to finalise draft guidelines and develop questions; round 1 anonymous online survey; round 2 online survey; regional cardiac conference and pathway revision; and final expert meeting and pathway finalisation.
ResultsPaediatric Dietitians from all 11 of the paediatric cardiology surgical centres in the United Kingdom contributed to the guideline development. In all, 33% of participants had 9 or more years of experience working with infants with CHD. By the end of rounds 1 and 2, 76 and 96% of participants, respectively, were in agreement with the statements. Three statements where consensus was not achieved by the end of round 2 were discussed and agreed at the final expert group meeting.
ConclusionsNutrition guidelines were developed for infants with CHD awaiting surgery, using a modified Delphi process, incorporating the best available evidence and expert opinion with regard to nutritional support in this group.

The effect of 3,6-dichloro-o-anisic acid (dicamba) on common purslane (Portulaca oleracea L.) was studied under growth room conditions. Common purslane was quite sensitive to dicamba. The lethal rate depended on the age of the plants and increased with their development. The visual responses included bending of young stems, epinasty of leaves, and inhibition of nyctinasty, followed by necrosis of leaves, defoliation, swelling of stems, and death of the growing tips with loss of apical dominance. Stem elongation was inhibited and fresh weight of tops was markedly reduced by dicamba application to either the roots or foliage. Carboxyl-labeled dicamba-7-14C was absorbed slowly by common purslane leaves. However, translocation appeared rapid, with accumulation of dicamba observed in the young tissues. A small amount of dicamba was exuded through the roots. There was no indication of dicamba degradation in common purslane during 8 days following treatment.

OBJECTIVE
To investigate an outbreak of Pseudomonas aeruginosa infections and colonization in a neonatal intensive care unit.
DESIGNInfection control assessment, environmental evaluation, and case-control study.
SETTINGNewly built community-based hospital, 28-bed neonatal intensive care unit.
PATIENTSNeonatal intensive care unit patients receiving care between June 1, 2013, and September 30, 2014.
METHODSCase finding was performed through microbiology record review. Infection control observations, interviews, and environmental assessment were performed. A matched case-control study was conducted to identify risk factors for P. aeruginosa infection. Patient and environmental isolates were collected for pulsed-field gel electrophoresis to determine strain relatedness.
RESULTSIn total, 31 cases were identified. Case clusters were temporally associated with absence of point-of-use filters on faucets in patient rooms. After adjusting for gestational age, case patients were more likely to have been in a room without a point-of-use filter (odds ratio [OR], 37.55; 95% confidence interval [CI], 7.16–∞). Case patients had higher odds of exposure to peripherally inserted central catheters (OR, 7.20; 95% CI, 1.75–37.30) and invasive ventilation (OR, 5.79; 95% CI, 1.39–30.62). Of 42 environmental samples, 28 (67%) grew P. aeruginosa. Isolates from the 2 most recent case patients were indistinguishable by pulsed-field gel electrophoresis from water-related samples obtained from these case-patient rooms.
CONCLUSIONSThis outbreak was attributed to contaminated water. Interruption of the outbreak with point-of-use filters provided a short-term solution; however, eradication of P. aeruginosa in water and fixtures was necessary to protect patients. This outbreak highlights the importance of understanding the risks of stagnant water in healthcare facilities.

Background:
Eustachian tube dysfunction is a poorly defined condition associated with various symptoms and it can predispose to middle-ear disease. Balloon dilation Eustachian tuboplasty has been proposed as a treatment for Eustachian tube dysfunction.
Objective:To evaluate the subjective and objective outcomes of balloon dilation Eustachian tuboplasty in patients with recurrent, previously treated chronic Eustachian tube dysfunction.
Methods:The study was conducted on 11 patients (13 ears) who had undergone previous unsuccessful medical and surgical treatment. Tympanometry was the primary outcome measure. Secondary outcome measures included pure tone audiogram assessment and seven-item Eustachian Tube Dysfunction Questionnaire score.
Results:Balloon dilation Eustachian tuboplasty resulted in significant improvements in 11 patients’ subjective but not objective outcome measures.
Conclusion:The objective abnormality and subjective symptoms in Eustachian tube dysfunction may represent two distinct pathological processes, which may nevertheless influence and exacerbate each other.

A depth-averaged finite-difference numerical model has been used to make a preliminary study of the tides under the Filchner–Ronne Ice Shelf. Open boundary conditions were specified using the global ocean model of Schwiderski. Tidal constituents for the two principal semi-diurnal constituents M2 and S2, and the two principal diurnal constituents Ο1 and K1 were extracted from computed sea-surface elevations by harmonic analysis. Measured values near to the grounding line could only be reproduced satisfactorily by increasing the bottom friction coefficient under the ice to 50 times the open-ocean value. This destroys any agreement near the ice front or at pelagic sites. It is thought that a friction coefficient which varies with distance under the ice would be able to reproduce better all the available measurements. More tidal measurements are required to validate any model of the region with model experiments being used to help pinpoint possible sites for instrument deployment.
Assortative epibiosis of leatherback, olive ridley and green sea turtles in the Eastern Tropical Pacific

Sea turtles host a diverse array of epibionts, yet it is not well understood what factors influence epibiont community composition. To test whether epibiont communities of sea turtles are influenced by the hosts’ nesting or foraging habitats, we characterized the epibiota of leatherback, olive ridley and green turtles nesting at a single location on the Pacific coast of Costa Rica. We also compared the epibiota of these turtles to conspecific populations nesting elsewhere in the East Pacific. If epibiont communities are influenced by nesting habitats, we predicted that sympatrically nesting turtles would have comparable epibiont taxa. Alternatively, if epibiont communities are influenced by foraging habitats, we predicted the diversity of epibiont taxa should reflect the type and diversity of the hosts’ foraging habitats. We identified 18 epibiont taxa from 18 leatherback, 19 olive ridley and six green turtles. Epibiont diversity was low on leatherbacks (four taxa), but higher for olive ridley and green turtles (12 and nine epibiont taxa respectively). The epibiont communities of olive ridley and green turtles were not statistically different, but both were different from leatherbacks. In addition, conspecific sea turtles from other nesting locations hosted more similar epibiont communities than sympatrically nesting, non-conspecifics. We conclude that epibiont diversity of nesting sea turtles is partially linked to the diversity of their foraging habitats. We also conclude that the surface properties of the skin and carapace of these turtles may contribute to the uniqueness of leatherback turtle epibiont communities and the similarities between olive ridley and green turtle epibiont communities.
Communitywide cryptosporidiosis outbreak associated with a surface water-supplied municipal water system – Baker City, Oregon, 2013

Cryptosporidium, a parasite known to cause large drinking and recreational water outbreaks, is tolerant of chlorine concentrations used for drinking water treatment. Human laboratory-based surveillance for enteric pathogens detected a cryptosporidiosis outbreak in Baker City, Oregon during July 2013 associated with municipal drinking water. Objectives of the investigation were to confirm the outbreak source and assess outbreak extent. The watershed was inspected and city water was tested for contamination. To determine the community attack rate, a standardized questionnaire was administered to randomly sampled households. Weighted attack rates and confidence intervals (CIs) were calculated. Water samples tested positive for Cryptosporidium species; a Cryptosporidium parvum subtype common in cattle was detected in human stool specimens. Cattle were observed grazing along watershed borders; cattle faeces were observed within watershed barriers. The city water treatment facility chlorinated, but did not filter, water. The community attack rate was 28·3% (95% CI 22·1–33·6), sickening an estimated 2780 persons. Watershed contamination by cattle probably caused this outbreak; water treatments effective against Cryptosporidium were not in place. This outbreak highlights vulnerability of drinking water systems to pathogen contamination and underscores the need for communities to invest in system improvements to maintain multiple barriers to drinking water contamination.

Sire and liveweight affect feed intake and methane emissions of sheep confined in respiration chambers

Daily methane production and feed intake were measured on 160 adult ewes, which were the progeny of 20 sires and 3 sire types (Merino, dual-purpose and terminal) from a genetically diverse flock. All animals were housed in individual pens and fed a 50/50 mix of chaffed lucerne and oaten hays at 20 g/kg liveweight (LW), with feed refusals measured for at least 10 days before the first of three 22-h measurements in respiration chambers (RC). Feed was withdrawn at 1600 h on the day before each RC test to encourage the ewes to eat the entire ration provided for them in the RC. After the first 1-day RC test, the sheep were returned to their pens for a day, then given a second 1-day RC test, followed by another day in their pens, then a third RC test. After all animals had been tested, they were ranked according to methane emissions adjusted for feed intake in the RC and on the previous day, enabling 10 low and 10 high methane animals to be chosen for repeat measurement. No variation between sires nor consistent effects of LW on feed eaten (%FE, expressed as per cent of feed offered) was evident in the 10 days before the first RC measurement. However, significant differences between sires (equivalent to an estimated heritability of 41%) were identified for %FE during the 2nd and 3rd days of RC testing (2 and 4 days after the initial RC test). The analysis of all data showed that methane emissions in the RC were related to feed intake on the day of testing and the two previous days (all P<0.0005). Before correcting for feed intake on previous days, there was some variation between sires in methane yield, equivalent to an estimated heritability of 9%. Correction for feed intake on the 2 previous days halved the residual variation, allowing other effects to be detected, including effects of LW, twins reared as singles, test batch, RC and test-day effects, but estimated sire variation fell to zero. In order to avoid potential biases, statistical models of methane emissions in the RC need to consider potential confounding factors, such as those identified as significant in this study.

A load moving on sea ice, whether the weight of a vehicle or the pressure exerted by a low-flying aircraft, produces a deflection which can in extreme cases cause ice failure. The magnitude and shape of the deflection profile depends on the weight and speed of the vehicle) and also the the ice thickness and properties, with flexuralgraviry waves radiating from the source at speeds above a critical value. This wave pattern was studied in detail on flat, snow-free sea ice in McMurdo Sound, Antarctica. Surface strain was measured directly and microcracking activity monitored to correlate measured strain with possible generation of dangerous cracks. Speeds of up to 28 m sec-1 (60 mph) were achieved with a pickup truck, and up to 80 m sec-1 with a US Navy C131 aircraft. Initial comparison between theory and experimental results is very encouraging.
