Evaluation of emergency department ultrasound machines for the presence of occult blood

Objectives
Bedside ultrasound in the emergency department is a common diagnostic tool, especially when evaluating trauma patients. Many trauma patients have blood on their chest and abdomen that may contact the probe during examination. The primary aim of this study was to investigate whether occult blood contamination was present on the emergency department ultrasound machine, both after daily use and after use in trauma.
MethodsFor a period of 31 days, the ultrasound machine at the trauma centre emergency department in Saskatoon, Saskatchewan, was tested once daily and following all Level 1 traumas. The ultrasound machine probes and keyboard were swabbed, and contamination was detected using a commercially available phenolphthalein blood testing kit. Any visible blood contamination was also noted. The machine was then cleaned following each positive test and re-tested to ensure the absence of contamination.
ResultsOver the study period, the ultrasound machine tested positive for occult blood contamination on 10% of daily tests and on 43% of assessments after its use in trauma. The curvilinear probe was most frequently contaminated (daily, 6%; trauma, 26%), followed by the keyboard (daily, 3%; trauma, 26%), but both lacked visible contamination.
ConclusionsIn this single centre study, there was evidence of occult blood on the emergency department ultrasound machine after both routine use and major trauma cases, highlighting the need for a standardized cleaning and disinfection protocol.

The Randolph Glacier Inventory (RGI) is a globally complete collection of digital outlines of glaciers, excluding the ice sheets, developed to meet the needs of the Fifth Assessment of the Intergovernmental Panel on Climate Change for estimates of past and future mass balance. The RGI was created with limited resources in a short period. Priority was given to completeness of coverage, but a limited, uniform set of attributes is attached to each of the ~198 000 glaciers in its latest version, 3.2. Satellite imagery from 1999–2010 provided most of the outlines. Their total extent is estimated as 726 800 ± 34 000 km2. The uncertainty, about ±5%, is derived from careful single-glacier and basin-scale uncertainty estimates and comparisons with inventories that were not sources for the RGI. The main contributors to uncertainty are probably misinterpretation of seasonal snow cover and debris cover. These errors appear not to be normally distributed, and quantifying them reliably is an unsolved problem. Combined with digital elevation models, the RGI glacier outlines yield hypsometries that can be combined with atmospheric data or model outputs for analysis of the impacts of climatic change on glaciers. The RGI has already proved its value in the generation of significantly improved aggregate estimates of glacier mass changes and total volume, and thus actual and potential contributions to sea-level rise.

Analysis of an exhaustive data base of Namurian ammonoid shell characters indicates that the morphology of the Goniatitida can be explained in terms of functional constraints, resulting particularly from hydrostatic and hydrodynamic properties. Modes of life ranging from benthic to pelagic are inferred on this basis for various goniatitid morphotypes; all morphologic features and facies associations are normally compatible with these inferences. Neutral buoyancy is shown to have been likely for all goniatitids. By contrast, the prolecanitids (Order Agoniatitida) exhibit a number of hydrostatic and morphologic anomalies; these anomalies are not explicable using the same principles and remain problematic. This is noteworthy, in that prolecanitids survived the Permian/Triassic extinctions and gave rise to the diverse ceratitic radiation in the Triassic.
The applicability of these results to ammonoids outside the Namurian is assessed, and, in particular, morphologic parallels with Mesozoic ammonites are discussed.

Morphologic analysis of 281 species of ammonoids from Great Britain, the North American mid-continent, and the South Urals, at eight successive levels within the Namurian Series (ca. 18 Myr duration), using bivariate plots and principal-components analysis, permits definition of morphologic diversity and identification of morphotypic patterns in time and space. Namurian ammonoids exhibit the same general range of shell geometry that characterizes ammonoids as a whole; there were few post-Namurian innovations in the basic geometry of planispiral ammonoids. Within this overall range of geometry, there are eight preferred morphotypes: two were phylogenetically monopolized by long-ranging forms; three were generalized and reoccur in successive horizons; two others were homeomorphically utilized at different times by different lineages; and one represents morphologic innovation followed by radiation. Such patterns seem to represent combined effects of function, phylogeny, and ecology. Synchronous variations in isolated successions suggest global controls such as eustatic sea-level fluctuations, whereas provincial differences in diversity may be attributable to paleogeographic and ecologic factors. We predict that the Namurian record of ammonoid morphologic diversity and change will be found to be distinctive and differentiable from earlier and later intervals.

Identification of Fragile Microscopic Structures during Mineral Transformations in Wet Supercritical CO2

This study examines the nature of highly fragile reaction products that form in low water content supercritical carbon dioxide (scCO2) using a combination of focus ion beam/scanning electron microscopy, confocal Raman spectroscopy, helium ion microscopy (HeIM), and transmission electron microscopy (TEM). HeIM images show these precipitates are fragile rosettes. Using the TEM revealed details on the interfacial structure between the newly formed surface precipitates and the underlying initial solid phases. Detailed microscopy analysis revealed that growth of the precipitates either followed a tip growth mechanism, with precipitates forming directly on the forsterite surface if the initial solid was nonporous (natural forsterite) or growth from the surface of the precipitates, where fluid was conducted through the porous (nanoforsterite) agglomerates to the growth center. Identification of the mechanism of formation of hydrated/hydroxylated magnesium carbonate compound phases is a key factor in unraveling the impact of water recycling on mineral reactivity in low water content scCO2 solutions, which has received a great deal of attention as a result of the potential for CO2 to act as an atmospheric greenhouse gas. Techniques used here to examine these fragile structures are also used to examine a wide range of fragile material surfaces.

The origin of Illinois Episode (OIS 6) glacial ridges (formerly: ‘Ridged Drift’) in the Kaskaskia Basin of southwestern Illinois is controversial despite a century of research. Two studied ridges, containing mostly fluvial sand (OSL ages: ~ 150 ± 19 ka), with associated debris flows and high-angle reverse faults, are interpreted as ice-walled channels. A third studied ridge, containing mostly fine-grained till, is arcuate and morainal. The spatial arrangement of various ridge types can be explained by a glacial sublobe in the Kaskaskia Basin, with mainly fine-grained ridges along the sublobe margins and coarse-grained glaciofluvial ridges in a paleodrainage network within the sublobe interior. Illinois Episode till fabric and striation data demonstrate southwesterly ice flow that may diverge near the sublobe terminus. The sublobe likely formed as glacial ice thinned and receded from its maximum extent. The Kaskaskia Basin contains some of the best-preserved Illinois Episode constructional glacial landforms in the North American midcontinent. Such distinctive features probably result from ice flow and sedimentation into this former lowland, in addition to minimal postglacial erosion. Other similar OIS 6 glacial landforms may exist in association with previously unrecognized sublobes in the midcontinent, where paleo-lowlands might also have focused glacial sedimentation.

Isotope tracer studies, particularly radiocarbon measurements, play a key role in biological, nutritional, and environmental research. Accelerator mass spectrometry (AMS) is now the most sensitive detection method for 14C, but AMS is not widely used in kinetic studies of humans. Part of the reason is the expense, but costs would decrease if AMS were used more widely. One component in the cost is sample preparation for AMS. Biological and environmental samples are commonly reduced to graphite before they are analyzed by AMS. Improvements and mechanization of this multistep procedure is slowed by a lack of organized educational materials for AMS sample preparation that would allow new investigators to work with the technique without a substantial outlay of time and effort. We present a detailed sample preparation protocol for graphitizing biological samples for AMS and include examples of nutrition studies that have used this procedure.

Nutrigenomics is the study of how constituents of the diet interact with genes, and their products, to alter phenotype and, conversely, how genes and their products metabolise these constituents into nutrients, antinutrients, and bioactive compounds. Results from molecular and genetic epidemiological studies indicate that dietary unbalance can alter gene–nutrient interactions in ways that increase the risk of developing chronic disease. The interplay of human genetic variation and environmental factors will make identifying causative genes and nutrients a formidable, but not intractable, challenge. We provide specific recommendations for how to best meet this challenge and discuss the need for new methodologies and the use of comprehensive analyses of nutrient–genotype interactions involving large and diverse populations. The objective of the present paper is to stimulate discourse and collaboration among nutrigenomic researchers and stakeholders, a process that will lead to an increase in global health and wellness by reducing health disparities in developed and developing countries.

The formation of self-assembled quantum dots (QD) is of increasing interest for applications in optical, nanoelectronic, biological and quantum computing systems. From the perspective of fabrication technology, there are great advantages if the whole device can be made using a single Si substrate. Furthermore, GeSi is a model semiconductor system for fundamental studies of growth and material properties. In practice, as the MBE growth of heterostructures is inherently a non-equilibrium process, the formation of self-assembled nanostructures is both complex and sensitive to growth and overgrowth conditions. The morphology, structure and composition of QDs can all change during growth. It is therefore crucial to understand their structures at different stages of growth at the atomic scale. Here, the characterization of QD growth using high-resolution high angle annular dark field (HAADF) scanning transmission electron microscopy (STEM) imaging is presented. Both the formation of uncapped QDs and the effect of the encapsulation are investigated, and the morphological and compositional evolution of the QDs and wetting layers are observed directly at the atomic scale for the first time. During encapsulation, the Ge content in the centres of the QD remains unchanged, despite significant intermixing, lateral spreading and a laterally inhomogeneous Ge distribution inside the Ge QD. The initial non-uniform wetting layer for the uncapped Ge QD becomes uniform after encapsulation, and a 3-monolayer-thick core with ∼ 60% Ge content is formed in the 2 nm-thick wetting layer with an average Ge content of ∼ 30%. The results were obtained by direct analysis of the Z-contrast STEM imaging without involving complex image simulations.

The factors affecting optical gain were studied for Er-doped BaTiO3 thin film waveguides. Er-doped BaTiO3 with dopant concentrations of 0.3 – 9 at.% was deposited by metal-organic chemical vapor deposition. The luminescence efficiency was maximized by optimizing the growth temperature and erbium concentration as well as by post-deposition annealing. Stimulated emission was studied using the pump-probe technique over the spectral range of 1,520-1,550 nm. The factors affecting optical gain were studied for Er-doped BaTiO3 thin film waveguides. Er-doped BaTiO3 with dopant concentrations of 0.3 – 9 at.% was deposited by metal-organic chemical vapor deposition. The luminescence efficiency was maximized by optimizing the growth temperatureand erbium concentration as well as by post-deposition annealing. Stimulated emission was studied using the pump-probe technique over the spectral range of 1,520-1,550 nm. A maximum differential gain of 3 dB/cm wasmeasured at 1,540 nm in an 8 mm long, 8 μm wide ridge waveguide.
