Objectives: Studies in PD have traditionally focused on motor features, however, interest in non-motor manifestations has increased resulting in improved knowledge regarding the prognosis of the disease. Although several studies have explored the incidence of dementia in PD cohorts, these studies have been conducted mainly in reference centers in high-income countries (HIC). In this study we aimed to analyze the prevalence of cognitive impairment in people with parkinsonism and PD and its association with incident dementia in a population- based study, of elderly from six Latin American countries.
Methods: This report consists of the analysis of data from a follow-up of 12,865 elderly people aged 65 years or older, carried out by 10/66 Dementia Research Group. Residents of urban and rural areas, from six low and middle- income countries (Cuba, Dominican Republic, Puerto Rico, Venezuela, Mexico and Peru). Exposures include parkinsonism and PD defined according to the UK Parkinson’s Disease Society Brain Bank diagnostic criteria. Cognitive impairment was the main exposure and dementia was measured through the dementia diagnosis algorithm from 10/66 DRG.
Results: At baseline, the overall prevalence of cognitive impairment was 14% (n = 1,581), in people with parkinsonism and PD, it was of 30.0% and 26.2%, respectively. Parkinsonism and PD were individually associated with prevalent and incident dementia after controlling for age, sex, and education. The pooled odds ratios from a fixed-effects meta-analysis were 2.2 (95% CI: 1.9 – 2.6) for parkinsonism and 1.9 (95% CI: 1.4 – 2.4) for PD. Regarding incident dementia, the pooled sub-Hazard ratio estimated using a competing risk model was 1.5 (95% CI: 1.2 –1.9) for parkinsonism and 1.5 (95% CI: 1.0 – 2.2) forPD.
Conclusions: Parkinsonism and PD were associated cross-sectionally with the presence of cognitive impairment, and prospectively with incident dementia in elderly people in the community population of Latin America studied. Systematic screening for cognitive impairment and dementia with valid tools in PD patients may help with earlier detection of those at highest risk for adverse outcomes. Identifying modifiable risk factors could potentially lead to efficient interventions even in advanced stages of PD.
Background: Limited knowledge exists about the association between Parkinsonism or Parkinson’s disease (PD) and cognitive impairment and dementia in Latin America.
Objectives: The study aimed to determine the cross-sectional and prospective associations between Parkinsonism and PD with cognitive impairment and dementia in a large multi-country cohort in Latin America.
Methods: The 10/66 is a prospective, observational cohort study. This population-based cohort study was based in six Latin American countries: Cuba, Dominican Republic, Puerto Rico, Venezuela, Mexico, and Peru. The study includes 12,865 participants from six countries, including residents aged 65 years and living in urban and rural catchment areas. Exposures included diagnosed Parkinsonism and PD defined according to the United Kingdom Parkinson’s Disease Society Brain Bank diagnostic criteria. Cognitive impairment was the main outcome measure for cross-sectional analysis and dementia was used to measure the prospective association with the exposures. Logistic regression models were used to explore the association between Parkinsonism/PD with cognitive impairment at baseline. Competing risk models were used to assess the prospective association between Parkinsonism/PD with incident dementia accounting for competing risk of mortality. Individual country analyses were combined via fixed-effect meta-analysis.
Results: At baseline, the prevalence of cognitive impairment in people with Parkinsonism and PD was 30% and 26.2%, respectively. Parkinsonism (OR 2.2 (95%CI 1.9 – 2.6)) and PD (1.9 (95%CI 1.4 – 2.4)) were individually associated with baseline and incident cognitive impairment after accounting for age, sex, and education, after pooling. In competing risk models, the pooled sub- hazard ratios for dementia in the fixed effect metanalysis were 1.5 (95%CI 1.2 – 1.9) for parkinsonism and 1.5 (95%CI 1.0 – 2.2) for PD.
Conclusions: Parkinsonism and PD were cross-sectionally associated with cognitive impairment and prospectively associated with incident dementia in Latin America. Routine screening for cognitive impairment and dementia with validated tools in PD patients may aid earlier detection of those at greater risk ofadverseoutcomes.
The intercalating growth of new silicate layers or metal hydroxide layers in the interlayer space of other clay minerals is known from various mixed-layer clay minerals such as illite-smectite (I-S), chlorite-vermiculite, and mica-vermiculite. In a recent study, the present authors proposed that smectite-group minerals can be synthesized from solution as new 2:1 silicate layers within the low-charge interlayers of rectorite. That study showed how oxalate catalyzes the crystallization of saponite from a silicate gel at low temperatures (60ºC) and ambient pressure. As an extension of this work the aim of the present study was to test the claim that new 2:1 silicate layers can be synthesized as new intercalating layers in the low-charge interlayers of rectorite and whether oxalate could promote such an intercalation synthesis. Two experiments were conducted at 60ºC and atmospheric pressure. First, disodium oxalate solution was added to a suspension of rectorite in order to investigate the effects that oxalate anions have on the structure of rectorite. In a second experiment, silicate gel of saponitic composition (calculated interlayer charge -0.33 eq/O10(OH)2) was mixed with a suspension of rectorite and incubated in disodium oxalate solution. The synthesis products were extracted after 3 months and analyzed by X-ray diffraction and high-resolution transmission electron microscopy (HRTEM). The treatment of ultrathin sections with octadecylammonium (nC =18) cations revealed the presence of 2:1 layer silicates with different interlayer charges that grew from the silicate gel. The oxalate-promoted nucleation of saponite and talc crystallites on the rectorite led to the alteration and ultimately to the destruction of the rectorite structure. The change was documented in HRTEM lattice-fringe images. The crystallization of new 2:1 layer silicates also occurred within the expandable interlayers of rectorite but not as new 2:1 silicate layers parallel to the previous 2:1 silicate layers. Instead, they grew independently of any orientation predetermined by the rectorite crystal substrate and their crystallization was responsible for the destruction of the rectorite structure.
The COVID-19 pandemic has disproportionally affected traditionally marginalized groups. Both the Delta and Omicron variants raised concern amongst public health officials due to potentially higher infectivity rates and disease severity than prior variants. This study sought to compare disease severity between adults infected with the Omicron variant and adults infected with the Delta variant who presented to the Emergency Department at an academic, safety-net hospital in Virginia.
Methods:
This retrospective cohort study used electronic medical record data of patients who presented to the Emergency Department and received a positive SARS-CoV-2 test between September 1, 2021, and January 31, 2022. Positive tests were stratified by genotypic variant through whole genome sequencing. Participants with the Omicron variant were propensity scores matched with individuals with the Delta variant.
Results:
Among 500 Delta and 500 Omicron participants, 279 propensity score-matched pairs were identified. Participants were predominantly unvaccinated, with medical comorbidities, and self-identified as Black. Individuals infected with the Delta variant had more severe disease compared to those with the Omicron variant, regardless of vaccination status. Patients with kidney, liver, and respiratory disease, as well as cancer, are at higher risk for severe disease. Patients with 2 doses of COVID-19 immunization trended toward less severe disease.
Conclusions:
Overall, these data further support the literature regarding the disproportionate effects of the COVID-19 pandemic on vulnerable patient populations – such as those with limited access to care, people of color, and those with chronic medical conditions – and can be used to inform public health interventions.
Assessment of mass-produced animal-housing equipment can serve as a basis for improving animal welfare. A number of European countries have adopted various legal approaches to such assessment. In Germany, welfare assessment of housing equipment is voluntary, but minimum standards can be set by regulation for the assessment procedure and for the qualifications of the persons involved. From a scientific perspective, the time and resource constraints pose some problems, particularly as they apply to a voluntary procedure. For reasons of practicability, certain compromises will be required. Nevertheless, it is important to ensure that each assessment procedure is based upon scientific principles and considers animal welfare aspects to a sufficient extent. A proposal for the minimum standards of an assessment procedure has been elaborated by the Animal Welfare Committee of the German Agricultural Society (Deutsche Landwirtschafts-Gesellschaft, DLG), a shortened version of which is presented here. The animal welfare impact of such a regulated but voluntary procedure will be less than that of an obligatory assessment; however, the relatively flexible approach may still significantly contribute to the improvement of welfare aspects of livestock housing.
Access to and utilisation of antenatal care (ANC) is important for optimising health and nutrition during pregnancy. This study aimed to assess adherence to and factors associated with ANC and antenatal supplement use among Laotian women, and consider culturally appropriate strategies to increase micronutrient intakes. Mother–child (aged 21 d to <18 months) dyads (n 699) enrolled in a hospital-based prospective cohort study with the community comparison group in Luang Prabang province were interviewed about their antenatal history, supplement use, household sociodemographic and dietary practices, including postpartum food avoidances. Ninety percent of women (mean age 24⋅7 ± 6⋅3 years) reported receiving ANC during their pregnancy, with the majority reporting four to seven contacts, while 84⋅6 and 17⋅3 % reported supplement use during pregnancy and lactation, respectively. Adequate ANC contacts (≥8) and supplement use was more likely among women with complete primary education and from higher socioeconomic status households, and less likely among women belonging to ethnic minority populations and those who delivered their child at home. All women continued to consume salt while adhering to postpartum food avoidances; however, 58⋅5 and 38⋅7 % of habitual consumers restricted fish and soy sauces, respectively. Eighty-six percent of women reported they would be willing to take supplements when adhering to postpartum dietary restrictions. Overall, women's reported ANC attendance and antenatal supplement use was suboptimal. Understanding predictors of and barriers to ANC and supplement use may help implement effective public health strategies to improve adherence. Alongside targeted supplementation, salt fortification with micronutrients may be a viable population-wide intervention that needs further evaluation.
The following position statement from the Union of the European Phoniatricians, updated on 25th May 2020 (superseding the previous statement issued on 21st April 2020), contains a series of recommendations for phoniatricians and ENT surgeons who provide and/or run voice, swallowing, speech and language, or paediatric audiology services.
Objectives
This material specifically aims to inform clinical practices in countries where clinics and operating theatres are reopening for elective work. It endeavours to present a current European view in relation to common procedures, many of which fall under the aegis of aerosol generating procedures.
Conclusion
As evidence continues to build, some of the recommended practices will undoubtedly evolve, but it is hoped that the updated position statement will offer clinicians precepts on safe clinical practice.
Some studies found that providing micronutrient powder (MNP) causes adverse health outcomes, but modifying factors are unknown. We aimed to investigate whether Fe status and inherited Hb disorders (IHbD) modify the impact of MNP on growth and diarrhoea among young Lao children. In a double-blind controlled trial, 1704 children of age 6–23 months were randomised to daily MNP (with 6 mg Fe plus fourteen micronutrients) or placebo for about 36 weeks. IHbD, and baseline and final Hb, Fe status and anthropometrics were assessed. Caregivers provided weekly morbidity reports. At enrolment, 55·6 % were anaemic; only 39·3 % had no sign of clinically significant IHbD. MNP had no overall impact on growth and longitudinal diarrhoea prevalence. Baseline Hb modified the effect of MNP on length-for-age (LAZ) (P for interaction = 0·082). Among children who were initially non-anaemic, the final mean LAZ in the MNP group was slightly lower (–1·93 (95 % CI –1·88, –1·97)) v. placebo (–1·88 (95 % CI –1·83, –1·92)), and the opposite occurred among initially anaemic children (final mean LAZ –1·90 (95 % CI –1·86, –1·94) in MNP v. –1·92 (95 % CI –1·88, –1·96) in placebo). IHbD modified the effect on diarrhoea prevalence (P = 0·095). Among children with IHbD, the MNP group had higher diarrhoea prevalence (1·37 (95 % CI 1·17, 1·59) v. 1·21 (95 % CI 1·04, 1·41)), while it was lower among children without IHbD who received MNP (1·15 (95 % CI 0·95, 1·39) v. 1·37 (95 % CI 1·13, 1·64)). In conclusion, there was a small adverse effect of MNP on growth among non-anaemic children and on diarrhoea prevalence among children with IHbD.
We assessed the impact of an embedded electronic medical record decision-support matrix (Cerner software system) for the reduction of hospital-onset Clostridioides difficile. A critical review of 3,124 patients highlighted excessive testing frequency in an academic medical center and demonstrated the impact of decision support following a testing fidelity algorithm.
The spatial-intensity profile of light reflected during the interaction of an intense laser pulse with a microstructured target is investigated experimentally and the potential to apply this as a diagnostic of the interaction physics is explored numerically. Diffraction and speckle patterns are measured in the specularly reflected light in the cases of targets with regular groove and needle-like structures, respectively, highlighting the potential to use this as a diagnostic of the evolving plasma surface. It is shown, via ray-tracing and numerical modelling, that for a laser focal spot diameter smaller than the periodicity of the target structure, the reflected light patterns can potentially be used to diagnose the degree of plasma expansion, and by extension the local plasma temperature, at the focus of the intense laser light. The reflected patterns could also be used to diagnose the size of the laser focal spot during a high-intensity interaction when using a regular structure with known spacing.
X-ray fluorescence analysis (XFA) is a new method for the spatial localization and quantification of herbicide residue on leaf surfaces. The distribution pattern of propanil (3′,4′-dichloropropionanilide) on the surface of prune (Prunus domestica L.) leaves sprayed to simulate drift is easily mapped using XFA. Residue distribution maps may be electronically enhanced. Enhancement does not qualitatively change the information content of the map when done with discretion.
Distribution patterns of foliar applied herbicides can be studied by x-ray element mapping provided the herbicide contains an atom with an atomic number of 11 (sodium) or greater. The suitability of an herbicide for element mapping increases as the number of similar detectable atoms per molecule increases. Herbicides containing one detectable atom per molecule provide usable element maps at concentrations of 1.12 kg/ha and higher. Inaccurate element maps can result from: (1) Formulation components containing detectable atoms the same as those of the active ingredient. (2) Herbicide volatility during analysis due to reduced pressures and heating. (3) Specimen topography preventing x-ray detection at some locations on the plant surface. (4) Herbicide concentrations that are below the minimum detectable limit. If adequate precautions are followed in the use of x-ray element mapping, a wide range of uses exist in weed science.
The distribution pattern of MCPA ([(4-chloro-o-tolyl)oxy] acetic acid) on leaf surfaces of three species was studied using the cathodoluminescence detection mode of a scanning electron microscope. On low-wax-content sugarbeet (Beta vulgaris L.) leaves MCPA concentrated in the depressions over the anticlinal cell walls when applied at high volumes (748 and 374 L/ha). At low volumes (23 L/ha), numerous small deposits of MCPA were randomly distributed over both anticlinal and periclinal walls. These distinct patterns were independent of herbicide concentration. Regardless of spray volumes, MCPA remaining on the waxy leaf surfaces of cabbage (Brassica oleracea L.) coalesced into small thick deposits. Large spray drops from high application volumes shattered on impact with the stellate hairs of turkey mullein (Eremocarpus setigerus Benth.) resulting in some MCPA reaching the leaf surface. Spray drops from low application volumes did not shatter but lodged on the hairs with very little reaching the leaf surface.
The effects of chloracetamides on protein synthesis were studied both in vivo and in vitro. Four chloracetamide herbicides, alachlor [2-chloro-2′,6′-diethyl-N-(methoxymethyl)acetanilide], metolachlor [2-chloro-N-(2-ethyl-6-methylphenyl)-N-(2-methoxy-1-methylethyl)acetamide], CDAA (N–N-diallyl-2-chloroacetamide), and propachlor (2-chloro-N-isopropylacetanilide) were tested for inhibition of [3H]-leucine incorporation into protein. Incorporation of 3H-leucine into trichloroacetic acid (TCA)-insoluble protein was inhibited in oat (Avena sativa L. ‘Victory’) seedlings grown in sand culture and treated 12 h at 1 × 10−4M with these chloracetamides. The herbicides were also tested in a cell-free protein synthesizing system containing polyribosomes purified from oat root cytoplasm. These herbicides had no effect on the rates of polypeptide elongation nor on the synthesis of specific polypeptides when herbicides (1 × 10−4M) were added directly to the system. Polypeptide formation was inhibited 89% when 1 × 10−4M cycloheximide was added during translation. Cytoplasmic polyribosomes were isolated from oat roots treated 12 h with 1 × 10−4M herbicide. Translation rates and products were not altered when these polyribosomes were added to the in vitro system. Protein synthesis is inhibited when tested in an in vivo system; however, the inhibition does not occur during the translation of mRNA into protein.
The distribution patterns of several herbicide formulations sprayed on adaxial leaf surfaces were determined using scanning electron microscopy coupled with cathodoluminescence and x-ray microanalysis. The sodium and amine salts of MCPA {[(4-chloro-o-tolyl) oxy] acetic acid} sprayed on sugar beet (Beta vulgaris L.) leaves appeared as discrete deposits above the anticlinal cell walls that represented the location of spray drops that adhered to the leaf. When the sodium salt was applied to bermudagrass [Cynodon dactylon (L.) Pers.], the pattern of distribution was the same; however, each deposit was significantly smaller. The iso-octyl ester of MCPA coalesced into numerous, small, thick deposits on the cuticle of sugar beet leaves. The distribution of a wettable powder formulation of atrazine [2-chloro-4-(ethylamino)-6-(isopropylamino-s-triazine] appeared as uniform deposits over the anticlinal and periclinal cell walls that represented the location of aqueous spray drops after application. When a flowable formulation of atrazine was applied, there was a significant preferential accumulation of the herbicide at the edges of the separate deposits. One commercial formulation of propanil (3′,4′-dichloropropionanilide) yielded deposits that were crystalline, one that was partially crystalline, and one that was noncrystalline.
The single-celled green alga Chlamydomonas eugametos was found to be well suited for detecting growth inhibitor activity of chemicals. Tests consisted of solubilizing technical grade compounds in 20-ml aliquots of a 1 × 105 cells/ml zoospore culture. Commercially available growth inhibitor herbicides significantly inhibited synchronous cell population increases within 48 h at concentrations ranging from 1 × 10−4 to 1 × 10−7M. Examples of compounds that inhibited Chlamydomonas more than 50% were butylate (S-ethyl diisobutylthiocarbamate) and diallate [S-(2,3-dichloroallyl)diisopropylthiocarbamate] at 1 × 10−4M, CDEC [2 chloroallyl diethyldithiocarbamate] and propham (isopropyl carbanilate) at 1 × 10−5M, alachlor [2-chloro-2′,6′-diethyl-N-(methoxymethyl)acetanilide] and trifluralin [α,α,α-trifluoro-2,6-dinitro-N,N-dipropyl-p-toluidine] at 1 × 10−6 M, and bifenox [methyl 5-(2,4-dichlorophenoxy)-2-nitrobenzoate] at 1 × 10−7M. Ethanol, dimethylsulfoxide (DMSO), or acetone can be used to solubilize herbicides in the aqueous medium. DMSO and ethanol are not detrimental to the cells if the concentration is kept at 1% v/v or less.
The effects of varying concentrations and duration of alachlor [2-chloro-2′,6′diethyl-N-(methoxymethyl)acetanilide] and metolachlor [2-chloro-N-(2-ethyl-6-methylphenyl)-N-(2-methoxy-1-methylethyl)acetamide] treatment on root growth, cell division, and cell enlargement were studied. Peas (Pisum sativum L. ‘Alaska’) and oats (Avena sativa L. ‘Victory’) were treated from 0 to 48 h with concentrations ranging from 1 × 10-8 to 1 × 10-3 M of each herbicide. After 48 h, average growth rates were significantly inhibited at concentrations of 1 × 10-7 M alachlor and 5 × 10−8 M metolachlor, and 5 × 10−7 M alachlor and 1 × 10-6 M metolachlor for peas and oats, respectively. When growth inhibitions were examined across time at concentrations greater than these, the degree of growth inhibition was a function of both concentration and duration of treatment. Often the greatest decrease in growth occurred between 0 and 12 h. Mitotic indices of root tip squashes from pea roots and paraffin sections from oat roots were determined. There was a significant reduction in the mitotic indices of pea roots treated for 48 h with 5 × 10−6 M alachlor or 1 × 10-5 M metolachlor. After a 30-h treatment, the mitotic indices of oat roots were significantly reduced by 1 × 10−7 M metolachlor and 1 × 10−6 M alachlor. Significant inhibition of elongation of etiolated oat coleoptiles were observed at 5 × 10−6 M alachlor (27%) and 5 × 10−5 M metolachlor (30%). Inhibition of pea hypocotyl elongation did not occur at concentrations below 5 × 10−4 M. It was concluded that the growth inhibition of plants caused by alachlor and metolachlor results from both an inhibition of cell division and cell enlargement.
One to 5.6 μM DCPA (dimethyl tetrachloroterephthalate) inhibited oat (Avena sativa L. ‘Victory’) root growth within 12 to 18 h. Treated roots were severely stunted and swollen. An analysis of cell division in roots treated with DCPA revealed a disruption of normal mitosis after prophase. Metaphase, anaphase, and telophase division figures were absent 8 to 10 h after treatment with 5.6 μM DCPA. In contrast, a 24-h treatment with 5.6 μM DCPA was necessary to eliminate prophase division figures. The number of aberrant division figures increased concomitantly with the reduction in normal division figures. The predominant type of aberrant division figure was a condensed prophase. When the aberrant division cycle was completed and cells entered interphase, the dispersed chromosomes coalesced to form large, polymorphic nuclei and, occasionally, micronuclei. Approximately 60% of the outer four tiers of cells in roots treated with 5.6 μM DCPA developed abnormal cell walls. These data suggest that DCPA causes root growth inhibition by disrupting several processes involving organized microtubules.
Hans Jacob Christoffel von Grimmelshausen (ca. 1621-1676) is the most significant (and still readable) author of seventeenth-century German novels. His Abenteuerlicher Simplicius Simplicissimusremains the one German novel of its time that has attained the stature of "world literature": its unique mix of violent action and solitary reflection, its superlative humor, its realistic portrayalof a peasant turned soldier turned hermit has made it the longest-running bestseller in German literature. Read by students and scholars in comparative literature, history, and German, and by those interested in the development of the picaresque novel in Europe, the work and its "Continuations" have increasingly occupied scholars around the world, who have in recent years shown it to be a work ofsubtle structure and characterization, bearing the imprint of the most advanced political thinking of the time, and showing the influences of some of the most significant works of world literature, including Cervantes' Don Quixote and Barclay's Argenis. This volume of essays by leading Grimmelshausen scholars from Germany, the United States, and England provides analyses of significant topics in his life and works, including questions of genre, structure, satire, allegory, narratology, political thought, religion, morality, humor, realism, and mortality.
