Hand, foot, and mouth disease (HFMD) shows spatiotemporal heterogeneity in China. A spatiotemporal filtering model was constructed and applied to HFMD data to explore the underlying spatiotemporal structure of the disease and determine the impact of different spatiotemporal weight matrices on the results. HFMD cases and covariate data in East China were collected between 2009 and 2015. The different spatiotemporal weight matrices formed by Rook, K-nearest neighbour (KNN; K = 1), distance, and second-order spatial weight matrices (SO-SWM) with first-order temporal weight matrices in contemporaneous and lagged forms were decomposed, and spatiotemporal filtering model was constructed by selecting eigenvectors according to MC and the AIC. We used MI, standard deviation of the regression coefficients, and five indices (AIC, BIC, DIC, R2, and MSE) to compare the spatiotemporal filtering model with a Bayesian spatiotemporal model. The eigenvectors effectively removed spatial correlation in the model residuals (Moran’s I < 0.2, p > 0.05). The Bayesian spatiotemporal model’s Rook weight matrix outperformed others. The spatiotemporal filtering model with SO-SWM was superior, as shown by lower AIC (92,029.60), BIC (92,681.20), and MSE (418,022.7) values, and higher R2 (0.56) value. All spatiotemporal contemporaneous structures outperformed the lagged structures. Additionally, eigenvector maps from the Rook and SO-SWM closely resembled incidence patterns of HFMD.
Objectives/Goals: Obesity has been classified as a global epidemic and origin of numerous health issues. The central hypothesis of this study is that psychological measures, DNA methylation, and gene transcription will predict obesity-related outcomes after lifestyle interventions, and such interventions may alter DNA methylation profiles. Methods/Study Population: This study consisted of a randomized-controlled trial examining the effects of lifestyle +/- stress reduction interventions in 285 highly stressed parents with obesity, followed for 2 years. Full participants received nutrition and activity counseling, and were randomized to either a stress reduction intervention or a contact control. Those who otherwise qualified for the study but unable to fully participate were included in a no intervention control group. The intervention consisted of 12 weeks of nutrition and activity counseling +/- 2-hour weekly stress reduction interventions using MBSR and CBT-based strategies. DNA methylation was assessed using Illumina EPIC arrays. Results/Anticipated Results: Using linear mixed models (LMMs), this study will first examine the hypothesis that baseline psychological measures and pre-existing methylation sites associated with obesity and glycemic control (e.g., ABCG1, ATP10A, TXNIP, SREBF1, RNF39, and SOCS3) predict changes in BMI, HOMA-IR, and HgbA1C post-intervention and at 1 and 2 year follow-ups. Using sites that demonstrate statistical significance, we will develop a polymethylation risk score predictor of change in BMI. Next, we will examine the hypothesis that interventions which reduce obesity may also lead to improvements in epigenetic aging using LMMs to determine if changes in BMI or HOMA-IR predict changes in epigenetic age acceleration over the course of the study. Discussion/Significance of Impact: This work examines whether psychological factors and/or epigenetic markers may be used in patient stratification at initiation of treatment, enabling improved treatment selection, fewer years of obesity and decreased risk of comorbidities. This proposal also asks whether lifestyle interventions impact the aging process itself.
This study demonstrates a kilowatt-level, spectrum-programmable, multi-wavelength fiber laser (MWFL) with wavelength, interval and intensity tunability. The central wavelength tuning range is 1060–1095 nm and the tunable number is controllable from 1 to 5. The wavelength interval can be tuned from 6 to 32 nm and the intensity of each channel can be adjusted independently. Maximum output power up to approximately 1100 W has been achieved by master oscillator power amplifier structures. We also investigate the wavelength evolution experimentally considering the difference of gain competition, which may give a primary reference for kW-level high-power MWFL spectral manipulation. To the best of our knowledge, this is the highest output power ever reported for a programmable MWFL. Benefiting from its high power and flexible spectral manipulability, the proposed MWFL has great potential in versatile applications such as nonlinear frequency conversion and spectroscopy.
Several novel anthropometric indices, including paediatric body adiposity index (BAIp) and triponderal mass index (TMI), have emerged as potential tools for estimating body fat in preschool children. However, their comparative validity and accuracy, particularly when compared with established indicators such as BMI, have not been thoroughly investigated. This cross-sectional study enrolled 2869 preschoolers aged 3–6 years in Wuhan, China. The non-parametric Bland–Altman analysis was employed to evaluate the agreement between BMI, BAIp and TMI with percentage of body fat (PBF), determined by bioelectrical impedance analysis (BIA), serving as the reference measure of adiposity. Additionally, receiver operating characteristic curve analysis was conducted to assess the effectiveness of BMI, BAIp and TMI in screening for obesity. BAIp demonstrated the least bias in estimating PBF, showing discrepancies of 3·64 % (95 % CI 3·40 %, 4·12 %) in boys and 3·95 % (95 % CI 3·79 %, 4·23 %) in girls. Conversely, BMI underestimated PBF by 3·89 % (95 % CI 3·70 %, 4·37 %) in boys and 4·81 % (95 % CI 4·59 %, 5·09 %) in girls, while TMI also underestimated PBF by 5·15 % (95 % CI 4·90 %, 5·52 %) in boys and 5·68 % (95 % CI 5·30 %, 5·91 %) in girls. BAIp exhibited the highest AUC values (AUC = 0·867–0·996) in boys, whereas in girls, there was no statistically significant difference between BMI (AUC = 0·936, 95 % CI 0·921, 0·948) and BAIp (AUC = 0·901, 95 % CI 0·883, 0·916) in girls (P = 0·054). In summary, when considering the identification of obesity, BAIp shows promise as a screening tool for both boys and girls.
Sjögren's syndrome (SS) is a chronic autoimmune disease caused by immune system disorders. The main clinical manifestations of SS are dry mouth and eyes caused by the destruction of exocrine glands, such as the salivary and lacrimal glands, and systemic manifestations, such as interstitial pneumonia, interstitial nephritis and vasculitis. The pathogenesis of this condition is complex. However, this has not been fully elucidated. Treatment mainly consists of glucocorticoids, disease-modifying antirheumatic drugs and biological agents, which can only control inflammation but not repair the tissue. Therefore, identifying methods to regulate immune disorders and repair damaged tissues is imperative. Cell therapy involves the transplantation of autologous or allogeneic normal or bioengineered cells into the body of a patient to replace damaged cells or achieve a stronger immunomodulatory capacity to cure diseases, mainly including stem cell therapy and immune cell therapy. Cell therapy can reduce inflammation, relieve symptoms and promote tissue repair and regeneration of exocrine glands such as the salivary glands. It has broad application prospects and may become a new treatment strategy for patients with SS. However, there are various challenges in cell preparation, culture, storage and transportation. This article reviews the research status and prospects of cell therapies for SS.
We report a home-built velocity-gradient-tensor-resolved particle image velocimetry (VGTR-PIV) system which spatio-temporally resolves all components of the velocity gradient tensor. This technique is applied to the paradigmatic turbulent Rayleigh–Bénard convection system in a cylindrical cell at three representative positions, i.e. centre, side and bottom regions. The VGTR-PIV system allows us to directly measure, for the first time, the spatio-temporally resolved energy dissipation rate and enstrophy in turbulent thermal convection. In the experiment, the Rayleigh number $Ra$ varied in the range $2 \times 10^8 \leqslant Ra \leqslant 8 \times 10^9$ and the Prandtl number $Pr$ was fixed at $Pr = 4.34$. Compared with the fully resolved energy dissipation rate $\varepsilon$, the pseudo-dissipation provides the best estimate within $3\,\%$, the planar (two-dimensional) surrogate has a larger relative error and the one-dimensional surrogate leads to the largest error. The power-law scalings of the time-averaged energy dissipation rate with the Rayleigh number follow $\langle \varepsilon _c \rangle _t / (\nu ^3 H^{-4}) = 9.86 \times 10^{-6} Ra^{1.54 \pm 0.02}$, $\langle \varepsilon _s \rangle _t / (\nu ^3 H^{-4}) = 9.26 \times 10^{-3} Ra^{1.25 \pm 0.02}$ and $\langle \varepsilon _b \rangle _t / (\nu ^3 H^{-4}) = 2.70 \times 10^{-2} Ra^{1.23 \pm 0.02}$ in the centre, side and bottom regions, respectively where $\nu$ is dynamic viscosity and $H$ is cell height. These scaling relations, along with our earlier measured time-averaged energy dissipation rate at the bottom wall surface $\langle \varepsilon _w \rangle _t / (\nu ^3 H^{-4}) = 9.65 \times 10^{-2} Ra^{1.25 \pm 0.02}$ (J. Fluid Mech., vol. 947, 2022, A15), provide important constraints against which theoretical models may be tested. For the centre and side locations in the convection cell, the probability density functions (p.d.f.s) of the energy dissipation rate and enstrophy both follow a stretched exponential distribution. For the bottom region, the p.d.f.s of dissipation and enstrophy exhibit a stretched exponential distribution outside the viscous boundary layer and an exponential distribution inside the viscous boundary layer. It is also found that extreme events with high dissipation are the most intermittent in the side region, whereas the bottom region is less intermittent than the cell centre.
Increasing evidence shows that maternal hyperglycemia inhibits cardiomyocyte (CM) proliferation and promotes cell apoptosis during fetal heart development, which leads to cardiac dysplasia. Accumulating evidence suggests that the overexpression of miR-21 in CMs has a protective role in cardiac function. Therefore, we investigated whether miR-21 can rescue CM injury caused by high glucose. First, we performed biological function analysis of miR-21-5p overexpression in H9c2 cells treated with high glucose. We found that the proliferation of H9c2 cells treated with high glucose decreased significantly and was rescued after overexpression of miR-21-5p. CCK-8 and EdU incorporation assays were performed to assess cell proliferation. The cell proliferation of the miR-21-5p mimic transfection group was improved compared with that of the NC mimic group (*p < 0.05, miR-21-5p mimics vs. NC mimics) when the proliferation of H9c2 cells was reduced by high glucose (****p < 0.0001, high glucose (HG) vs. normal glucose (NG)). Then, we verified the targeted and negative regulation of miR-21-5p on Rhob using a dual-luciferase activity assay and RT-qPCR, respectively. We further demonstrated that miR-21-5p regulates Rhob to rescue the inhibition of CM proliferation induced by high glucose. The CCK-8 results showed that the cell proliferation of the siRNA-Rhob group was higher than that of the NC mimic group (***p < 0.001) and that of the cotransfection group with Up-Rhob plasmids and miR-21-5p mimics was lower than that of the miR-21-5p mimic group (*p < 0.05). Conclusion: Overexpression of miR-21-5p rescues the inhibition of high glucose-induced CM proliferation through regulation of Rhob.
Convective dissolution is an important mechanism for long-term CO$_2$ sequestration in deep saline aquifers. The presence of an unstable geothermal gradient can affect the process of dissolution. In this paper, we present direct numerical simulations in a three-dimensional porous medium at three different concentration Rayleigh numbers $Ra_S$ with a set of thermal Rayleigh numbers $Ra_T$. Simulations reveal that the flow structures alter when ${\textit {Ra}}_T$ increases for a fixed ${\textit {Ra}}_S$. Strong thermal gradient can yield large-scale convection rolls which change the horizontal distribution and motions of concentration fingers. The time evolution of fluxes also has different responses to different ${\textit {Ra}}_T$. A theoretical model is developed and successfully describes the evolution of concentration flux and volume averaged concentration during the final shutdown stage. We further calculate the dissolved CO$_2$ into the interior over time, which shows non-monotonic variations as ${\textit {Ra}}_T$ increases. At the end of our simulations, the maximum increment of dissolved CO$_2$ occurs when density ratio is around unity for all three concentration Rayleigh numbers we have explored. We apply our results to a typical geological reservoir and discuss their implications.
A single-shot measurement of electron emittance was experimentally accomplished using a focused transfer line with a dipole. The betatron phase of electrons based on laser wakefield acceleration (LWFA) is energy dependent owing to the coupling of the longitudinal acceleration field and the transverse focusing (defocusing) field in the bubble. The phase space presents slice information after phase compensation relative to the center energy. Fitting the transverse size of the electron beam at different energy slices in the energy spectrum measured 0.27 mm mrad in the experiment. The diagnosis of slice emittance facilitates local electron quality manipulation, which is important for the development of LWFA-based free electron lasers. The quasi-3D particle-in-cell simulations matched the experimental results and analysis well.
The COVID-19 pandemic created an unprecedented need for population-level clinical trials focused on the discovery of life-saving therapies and treatments. However, there is limited information on perception of research participation among perinatal populations, a population of particular interest during the pandemic.
Methods:
Eligible respondents were 18 years or older, were currently pregnant or had an infant (≤12 months old), and lived in Florida within 50 miles of sites participating in the OneFlorida Clinical Research Consortium. Respondents were recruited via Qualtrics panels between April and September 2020. Respondents completed survey items about barriers and facilitators to participation and answered sociodemographic questions.
Results:
Of 533 respondents, most were between 25 and 34 years of age (n = 259, 49%) and identified as White (n = 303, 47%) and non-Hispanic (n = 344, 65%). Facebook was the most popular social media platform among our respondents. The most common barriers to research participation included poor explanation of study goals, discomforts to the infant, and time commitment. Recruitment through healthcare providers was perceived as the best way to learn about clinical research studies. When considering research participation, "myself" had the greatest influence, followed by familial ties. Noninvasive biological samples were highly acceptable. Hispanics had higher positive perspectives on willingness to participate in a randomized study (p = 0.009). Education (p = 0.007) had significant effects on willingness to release personal health information.
Conclusion:
When recruiting women during the pregnancy and postpartum periods for perinatal studies, investigators should consider protocols that account for common barriers and preferred study information sources. Social media-based recruitment is worthy of adoption.
We report an experimental study of the viscous boundary layer (BL) properties of turbulent Rayleigh–Bénard convection in a cylindrical cell. The velocity profile with all three components was measured from the centre of the bottom plate by an integrated home-made particle image velocimetry system. The Rayleigh number $Ra$ varied in the range $1.82 \times 10^8 \le Ra \le 5.26 \times 10^9$ and the Prandtl number $Pr$ was fixed at $Pr = 4.34$. The probability density function of the wall-shear stress indicates that using the velocity component in the mean large-scale circulation (LSC) plane alone may not be sufficient to characterise the viscous BL. Based on a dynamic wall-shear frame, we propose a method to reconstruct the measured full velocity profile which eliminates the effects of complex dynamics of the LSC. Various BL properties including the eddy viscosity are then obtained and analysed. It is found that, in the dynamic wall-shear frame, the eddy viscosity profiles along the centre line of the convection cell at different $Ra$ all collapse on a single master curve described by $\nu _t^d / \nu = 0.81 (z / \delta _u^d) ^{3.10 \pm 0.05}$. The Rayleigh number dependencies of several BL quantities are also determined in the dynamic frame, including the BL thickness $\delta _u^d$ (${\sim } Ra^{-0.21}$), the Reynolds number $Re^d$ (${\sim }Ra^{-0.46}$) and the shear Reynolds number $Re_s^d$ (${\sim } Ra^{0.24}$). Within the experimental uncertainty, these scaling exponents are the same as those obtained in the static laboratory frame. Finally, with the measured full velocity profile, we obtain the energy dissipation rate at the centre of the bottom plate $\varepsilon _{w}$, which is found to follow $\langle \varepsilon _{w} \rangle _t \sim Ra^{1.25}$.
The soft actuator is made of superelastic material and embedded flexible material. In this paper, a kind of soft tube was designed and used to assemble two kinds of pneumatic soft actuators. The experiment and finite element analysis are used to comprehensively analyze and describe the bending, elongation, and torsion deformation of the soft actuator. The results show that the two soft actuators have the best actuation performance when the inner diameter of the soft tube is 4 mm. In addition, when the twisting pitch of the torsional actuator is 24 mm, its torsional performance is optimized. Finally, a device that can be used in the production line was assembled by utilizing those soft actuators, and some operation tasks were completed. This experiment provides some insights for the development of soft actuators with more complex motions in the future.
To understand the characteristics and influencing factors related to cluster infections in Jiangsu Province, China, we investigated case reports to explore transmission dynamics and influencing factors of scales of cluster infection. The effectiveness of interventions was assessed by changes in the time-dependent reproductive number (Rt). From 25th January to 29th February, Jiangsu Province reported a total of 134 clusters involving 617 cases. Household clusters accounted for 79.85% of the total. The time interval from onset to report of index cases was 8 days, which was longer than that of secondary cases (4 days) (χ2 = 22.763, P < 0.001) and had a relationship with the number of secondary cases (the correlation coefficient (r) = 0.193, P = 0.040). The average interval from onset to report was different between family cluster cases (4 days) and community cluster cases (7 days) (χ2 = 28.072, P < 0.001). The average time interval from onset to isolation of patients with secondary infection (5 days) was longer than that of patients without secondary infection (3 days) (F = 9.761, P = 0.002). Asymptomatic patients and non-familial clusters had impacts on the size of the clusters. The average reduction in the Rt value in family clusters (26.00%, 0.26 ± 0.22) was lower than that in other clusters (37.00%, 0.37 ± 0.26) (F = 4.400, P = 0.039). Early detection of asymptomatic patients and early reports of non-family clusters can effectively weaken cluster infections.
Previous studies have revealed associations of meteorological factors with tuberculosis (TB) cases. However, few studies have examined their lag effects on TB cases. This study was aimed to analyse nonlinear lag effects of meteorological factors on the number of TB notifications in Hong Kong. Using a 22-year consecutive surveillance data in Hong Kong, we examined the association of monthly average temperature and relative humidity with temporal dynamics of the monthly number of TB notifications using a distributed lag nonlinear models combined with a Poisson regression. The relative risks (RRs) of TB notifications were >1.15 as monthly average temperatures were between 16.3 and 17.3 °C at lagged 13–15 months, reaching the peak risk of 1.18 (95% confidence interval (CI) 1.02–1.35) when it was 16.8 °C at lagged 14 months. The RRs of TB notifications were >1.05 as relative humidities of 60.0–63.6% at lagged 9–11 months expanded to 68.0–71.0% at lagged 12–17 months, reaching the highest risk of 1.06 (95% CI 1.01–1.11) when it was 69.0% at lagged 13 months. The nonlinear and delayed effects of average temperature and relative humidity on TB epidemic were identified, which may provide a practical reference for improving the TB warning system.
The effects of resistant starch on glycaemic control are controversial. In this study, a systematic review and meta-analysis of results from nineteen randomised controlled trials (RCT) was performed to illustrate the effects of resistant starch on glycaemic control. A literature search was conducted on PubMed, Scopus and Cochrane electronic databases for related publications from inception to 6 April 2020. Key inclusion criteria were: RCT; resistant starch as intervention substances and reporting glucose- and insulin-related endpoints. Exclusion criteria were: using type I resistant starch or a mixture of resistant starch and other functional food ingredients as intervention; using substances other than digestible starch as controls. The effect of resistant starch on fasting plasma glucose was significant (effect size (ES) –0·09 (95 % CI –0·13, −0·04) mmol/l, P = 0·001) compared with digestible starch. Subgroup analyses revealed that the ES was larger when the dosage of resistant starch was more than 28 g/d (ES –0·16 (95 % CI –0·24, –0·08) mmol/l, P < 0·001) or the intervention period was more than 8 weeks (ES –0·12 (95 % CI –0·18, –0·06) mmol/l, P < 0·001). The effect on homoeostatic model assessment (HOMA)-insulin resistance (IR) was significant (ES –0·33 (95 % CI –0·51, –0·14), P = 0·001). However, the effects on other insulin-related endpoints were not significant, including fasting plasma insulin, four endpoints from the frequently sampled intravenous glucose tolerance test (insulin sensitivity index, acute insulin response, disposition index and glucose effectiveness) and HOMA-β. The current study indicated moderate effects of resistant starch on improving glycaemic control.
The structural changes recent-onset posttraumatic stress disorder (PTSD) subjects were rarely investigated. This study was to compare temporal and causal relationships of structural changes in recent-onset PTSD with trauma-exposed control (TEC) subjects and non-TEC subjects.
Methods
T1-weighted magnetic resonance images of 27 PTSD, 33 TEC and 30 age- and sex-matched healthy control (HC) subjects were studied. The causal network of structural covariance was used to evaluate the causal relationships of structural changes in PTSD patients.
Results
Volumes of bilateral hippocampal and left lingual gyrus were significantly smaller in PTSD patients and TEC subjects than HC subjects. As symptom scores increase, reduction in gray matter volume began in the hippocampus and progressed to the frontal lobe, then to the temporal and occipital cortices (p < 0.05, false discovery rate corrected). The hippocampus might be the primary hub of the directional network and demonstrated positive causal effects on the frontal, temporal and occipital regions (p < 0.05, false discovery rate corrected). The frontal regions, which were identified to be transitional points, projected causal effects to the occipital lobe and temporal regions and received causal effects from the hippocampus (p < 0.05, false discovery rate corrected).
Conclusions
The results offer evidence of localized abnormalities in the bilateral hippocampus and remote abnormalities in multiple temporal and frontal regions in typhoon-exposed PTSD patients.
The search space of the path planning problem can greatly affect the running time and memory consumption, for example, the concave obstacle in grid-based map usually leads to the invalid search space. In this paper, the filling container algorithm is proposed to alleviate the concave area problem in 2D map space, which is inspired from the scenario of pouring water into a cup. With this method, concave areas can be largely excluded by scanning the map repeatedly. And the effectiveness has been proved in our experiments.
Increased intake of vegetables and fruits has been associated with reduced risk of tuberculosis infection. Vegetables and fruits exert immunoregulatory effects; however, it is not clear whether vegetables and fruits have an adjuvant treatment effect on tuberculosis. Between 2009 and 2013, a hospital-based cohort study was conducted in Linyi, Shandong Province, China. Treatment outcome was ascertained by sputum smear and chest computerised tomography, and dietary intake was assessed by a semi-quantitative FFQ. The dietary questionnaire was conducted at the end of month 2 of treatment initiation. Participants recalled their dietary intake of the previous 2 months. A total of 2309 patients were enrolled in this study. After 6 months of treatment, 2099 patients were successfully treated and 210 were uncured. In multivariate models, higher intake of total vegetables and fruits (OR 0·70; 95 % CI 0·49, 0·99), total vegetables (OR 0·68; 95 % CI 0·48, 0·97), dark-coloured vegetables (OR 0·61; 95 % CI 0·43, 0·86) and light-coloured vegetables (OR 0·67; 95 % CI 0·48, 0·95) were associated with reduced failure rate of tuberculosis treatment. No association was found between total fruit intake and reduced failure rate of tuberculosis treatment (OR 0·98; 95 % CI 0·70, 1·37). High intake of total vegetables and fruits, especially vegetables, is associated with lower risk of failure of tuberculosis treatment in pulmonary tuberculosis patients. The results provide important information for dietary guidelines during tuberculosis treatment.
The present study aimed to investigate the association of early-life exposure to famine with abdominal fat accumulation and function and further evaluate the influence of first-degree family history of diabetes and physical activity on this association. The present work analysed parts of the REACTION study. A total of 3033 women were enrolled. Central obesity was defined as waist circumferences (W) ≥ 85 cm. Chinese visceral adiposity index (CVAI) was used to evaluate visceral adipose distribution and function. Partial correlation analysis showed BMI, W, glycated Hb and CVAI were associated with early-life exposure to famine (both P < 0·05). Logistic regression showed that the risks of overall overweight/obesity and central obesity in fetal, early-childhood, mid-childhood and late-childhood exposed subgroups were increased significantly (all P < 0·05). Compared with the non-exposed group, the BMI, W and CVAI of fetal, early- to late-childhood exposed subgroups were significantly increased both in those with or without first-degree family history of diabetes and in those classified as physically active or inactive, respectively (all P < 0·05). The associations of BMI, W and CVAI with early-life exposure to famine were independent of their associations with first-degree family history of diabetes (all P < 0·01) or physical activity status (all P < 0·001).
