Group-based nutrition interventions to promote healthy eating and mobility in community-dwelling older adults: a systematic review

Objective:
To identify the efficacy of group-based nutrition interventions to increase healthy eating, reduce nutrition risk, improve nutritional status and improve physical mobility among community-dwelling older adults.
Design:Systematic review. Electronic databases MEDLINE, CINAHL, EMBASE, PsycINFO and Sociological Abstracts were searched on July 15, 2020 for studies published in English since January 2010. Study selection, critical appraisal (using the Joanna Briggs Institute’s tools) and data extraction were performed in duplicate by two independent reviewers.
Setting:Nutrition interventions delivered to groups in community-based settings were eligible. Studies delivered in acute or long-term care settings were excluded.
Participants:Community-dwelling older adults aged 55+ years. Studies targeting specific disease populations or promoting weight loss were excluded.
Results:Thirty-one experimental and quasi-experimental studies with generally unclear to high risk of bias were included. Interventions included nutrition education with behaviour change techniques (BCT) (e.g. goal setting, interactive cooking demonstrations) (n 21), didactic nutrition education (n 4), interactive nutrition education (n 2), food access (n 2) and nutrition education with BCT and food access (n 2). Group-based nutrition education with BCT demonstrated the most promise in improving food and fluid intake, nutritional status and healthy eating knowledge compared with baseline or control. The impact on mobility outcomes was unclear.
Conclusions:Group-based nutrition education with BCT demonstrated the most promise for improving healthy eating among community-dwelling older adults. Our findings should be interpreted with caution related to generally low certainty, unclear to high risk of bias and high heterogeneity across interventions and outcomes. Higher quality research in group-based nutrition education for older adults is needed.

Aims
Our aim was to establish a Peer Mentoring Network within Psychiatry Training in Northern Ireland.
Recognising that starting a new job can be a stressful time in any junior doctor's career, we wanted to ensure that new Core Trainees (CT1s) joining our Specialty Programme were well supported through this transition.
Although Clinical and Educational Supervision is well established in providing a support structure for trainees, we believed that a peer mentoring relationship, (with allocation of a Higher Psychiatry Trainee as mentor), would be of additional benefit.
It was hoped that the scheme would prove mutually beneficial to both mentee and mentor.
MethodWe delivered a presentation at CT1 induction and sent out follow-up emails to encourage participation. Higher trainees were also sent information via email and asked to complete a basic application form if interested in becoming a mentor. Prospective mentors then attended a one-day training session.
Two lead mentors, (also higher trainees), were allocated to oversee the scheme, with additional supervision from two lead Consultants. Mentor-Mentee matches were made based on information such as location, sub-specialty affiliations and outside interests.
Matched pairs were advised about the intended frequency and nature of contacts. Check-in emails were sent halfway through the year and feedback evaluations completed at the end.
Result95% of trainees who completed the evaluations said they would recommend the scheme to colleagues.
Mentees reported benefits in terms of personal and professional development, whilst mentors reported improved listening, coaching, and supervisory skills.
A small number of trainees highlighted that 6 monthly rotations impacted on ability to maintain face to face contacts.
Recruitment and engagement have improved annually. We are currently running the third year of the scheme and have achieved 100% uptake amongst CT1s and are over-subscribed with mentors, (19 mentors to 13 mentees).
ConclusionThe majority of feedback received has been positive and interest in the scheme continues to grow.
Potential issues relating to location of postings has been overcome, at least in part, by recent changes to ways of working and the use of alternative forms of contact, such as video calling.
Having exceeded demand in terms of mentor recruitment, we hope to extend the scheme to include trainees of other grades, and particularly those who are new to Northern Ireland.
We are excited to see where the next stage of our journey takes us and hope that others will be inspired to embark on similar schemes within their areas of work.

The first demonstration of laser action in ruby was made in 1960 by T. H. Maiman of Hughes Research Laboratories, USA. Many laboratories worldwide began the search for lasers using different materials, operating at different wavelengths. In the UK, academia, industry and the central laboratories took up the challenge from the earliest days to develop these systems for a broad range of applications. This historical review looks at the contribution the UK has made to the advancement of the technology, the development of systems and components and their exploitation over the last 60 years.

Summary
The rocky shores of the north-east Atlantic have been long studied. Our focus is from Gibraltar to Norway plus the Azores and Iceland. Phylogeographic processes shape biogeographic patterns of biodiversity. Long-term and broadscale studies have shown the responses of biota to past climate fluctuations and more recent anthropogenic climate change. Inter- and intra-specific species interactions along sharp local environmental gradients shape distributions and community structure and hence ecosystem functioning. Shifts in domination by fucoids in shelter to barnacles/mussels in exposure are mediated by grazing by patellid limpets. Further south fucoids become increasingly rare, with species disappearing or restricted to estuarine refuges, caused by greater desiccation and grazing pressure. Mesoscale processes influence bottom-up nutrient forcing and larval supply, hence affecting species abundance and distribution, and can be proximate factors setting range edges (e.g., the English Channel, the Iberian Peninsula). Impacts of invasive non-native species are reviewed. Knowledge gaps such as the work on rockpools and host–parasite dynamics are also outlined.

Persistent tobacco use and excessive alcohol consumption are major public health concerns worldwide. Both alcohol and nicotine dependence (AD, ND) are genetically influenced complex disorders that exhibit a high degree of comorbidity. To identify gene variants contributing to one or both of these addictions, we first conducted a pooling-based genomewide association study (GWAS) in an Australian population, using Illumina Infinium 1M arrays. Allele frequency differences were compared between pooled DNA from case and control groups for: (1) AD, 1224 cases and 1162 controls; (2) ND, 1273 cases and 1113 controls; and (3) comorbid AD and ND, 599 cases and 488 controls. Secondly, we carried out a GWAS in independent samples from the Netherlands for AD and for ND. Thirdly, we performed a meta-analysis of the 10, 000 most significant AD- and ND-related SNPs from the Australian and Dutch samples. In the Australian GWAS, one SNP achieved genomewide significance (p < 5 x 10-8) for ND (rs964170 in ARHGAPlOon chromosome 4, p = 4.43 x 10”8) and three others for comorbid AD/ND (rs7530302 near MARK1 on chromosome 1 (p = 1.90 x 10-9), rs1784300 near DDX6 on chromosome 11 (p = 2.60 x 10-9) and rs12882384 in KIAA1409 on chromosome 14 (p = 4.86 x 10-8)). None of the SNPs achieved genomewide significance in the Australian/Dutch meta-analysis, but a gene network diagram based on the top-results revealed overrepre-sentation of genes coding for ion-channels and cell adhesion molecules. Further studies will be requirec before the detailed causes of comorbidity between AC and ND are understood.
Seamless Pedestrian Positioning and Navigation Using Landmarks

Many navigation services, such as car navigation services, provide users with praxic navigational instructions (such as “turn left after 200 metres, then turn right after 150 metres”), however people usually associate directions with visual cues (e.g. “turn right at the square”) when giving navigational instructions in their daily conversations. Landmarks can play an equally important role in navigation and routing services. Landmarks are unique and easy-to-recognise and remember features; therefore, in order to remember when exploring an unfamiliar environment, they would be assets. In addition, Landmarks can be found both indoors and outdoors and their locations are usually fixed. Any positioning techniques which use landmarks as reference points can potentially provide seamless (indoor and outdoor) positioning solutions. For example, users can be localised with respect to landmarks if they can take a photograph of a registered landmark and use an application for image processing and feature extraction to identify the landmark and its location. Landmarks can also be used in pedestrian-specific path finding services. Landmarks can be considered as an important parameter in a path finding algorithm to calculate a route passing more landmarks (to make the user visit a more tourist-focussed area, pass along an easier-to-follow route, etc.). Landmarks can also be used as a part of the navigational instructions provided to users; a landmark-based navigation service makes users sure that they are on the correct route, as the user is reassured by seeing the landmark whose information/picture has just been provided as a part of navigational instruction. This paper shows how landmarks can help improve positioning and praxic navigational instructions in all these ways.

We aimed to determine the effect of consuming pure isolated micellar casein or pure whey protein isolate on rates of myofibrillar protein synthesis (MPS) at rest and after resistance exercise in elderly men. Healthy elderly men (72 (sem 1) years; BMI 26·4 (sem 0·7) kg/m2) were divided into two groups (n 7 each) who received a primed, constant infusion of l-[ring-13C6]phenylalanine to measure MPS at rest and during 4 h of exercise recovery. Participants performed unilateral leg resistance exercise followed by the consumption of isonitrogenous quantities (20 g) of casein or whey. Blood essential amino acids and leucine concentration peaked 60 min post-drink and were greater in amplitude after whey protein ingestion (both, P < 0·05). MPS in the rested leg was 65 % higher (P = 0·002) after ingestion of whey (0·040 (sem 0·003) %/h) when compared with micellar casein (0·024 (sem 0·002) %/h). Similarly, resistance exercise-stimulated rates of MPS were greater (P < 0·001) after whey ingestion (0·059 (sem 0·005) %/h) v. micellar casein (0·035 (sem 0·002) %/h). We conclude that ingestion of isolated whey protein supports greater rates of MPS than micellar casein both at rest and after resistance exercise in healthy elderly men. This result is probably related to a greater hyperaminoacidaemia or leucinaemia with whey ingestion.

Cystic fibrosis (CF) is the most common genetic disease of Caucasians with an incidence of 1 in 2500 live births and a carriage rate of 1 in 20 individuals. CF patients continue to suåer from recurrent and chronic respiratory tract infections and most of their morbidity and mortality is due to such infections throughout their life (Høiby, 1991). These infections are usually dominated by Gram-negative organisms, especially by the pseudomonads including in particular, Pseudomonas aeruginosa, as well as members of the Burkholderia cepacia complex of organisms.

The following paper reports on the progress of a large collaborative project in the UK to investigate novel strategies for the implementation of GNSS within all phases of marine navigation. The project is funded by the UK Government (Engineering and Physical Sciences Research Council) under the leadership of the IESSG at the University of Nottingham. The project receives the support from a prestigious team of collaborators which include the UK Ministry of Defence Hydrographic Office, Trinity House Lighthouse Service, Northern Lighthouse Board, Commissioners for Irish Lights and the Port of London Authority. This consortium is of strategic importance, as their operations encompass the provision of aids to navigation for all phases of marine transport within the UK and elsewhere.

The task I have set myself in this paper, a task in which I fear I shall hardly succeed, is to endeavour to wipe away from the pages of our national history one of the foulest blots that stains them, viz., the legend, the unproved rumour, of the hideous detail of the murder of king Edward II. in Berkeley Castle.
I suspect I am not the first inquirer on this subject, for I find the wardrobe account of 1 Edward III. which I shall presently refer to has been “galled,” probably for the purpose of making a copy of it.
