Service transitions, interventions and care pathways following remittal to prison from medium secure psychiatric services in England and Wales: national cohort study

Background
Little is known internationally about return to prison from in-patient psychiatric services, including: circumstances leading to return, aftercare services and subsequent patient outcomes.
AimsTo examine and describe: (a) circumstances leading to return to prison from medium secure services; (b) available aftercare and early outcomes of returned persons; and (c) implications for policy development.
MethodProspective cohort design with all patients (n = 96) returned to prisons from 33 National Health Service (NHS) medium secure services over a 6-month period in England and Wales. Follow-up was conducted for 1 year post-remittal, across 60 prisons.
ResultsLess than 20% of patients with legal entitlement to section 117 aftercare under the Mental Health Act 1983 were receiving care managed/delivered via the care programme approach. Subsequent pathways included: inter-prison transfer (30%), use of the Assessment, Care in Custody and Teamwork process (49%), referral to secure services (21%) and community release (30%). Less than half of community releases were referred to a community mental health team.
ConclusionsFindings suggest that persons returned to prison are a vulnerable group of patients, many of whom require intervention (e.g. enhanced monitoring, admission to a healthcare wing, readmission to secure mental health services) on return to prison in the absence of targeted aftercare services. More robust guidance for discharge and aftercare planning procedures for persons remitted to prison should be developed to ensure that the benefits of in-patient admission are maintained and that individuals’ legal rights to ongoing aftercare are upheld.
The effects of pre-sleep consumption of casein protein on next-morning measures of RMR and appetite compared between sedentary pre- and postmenopausal women

The purpose of the present study was to compare next-morning responses of RMR and appetite to pre-sleep consumption of casein protein (CP) in pre- and postmenopausal women. The study was a randomised, crossover, double-blind, placebo-controlled trial. Seven sedentary premenopausal (age: 19·9 (sd 1·2) years; BMI: 23·1 (sd 2·6) kg/m2) and seven sedentary postmenopausal (age: 56·4 (sd 4·9) years; BMI: 26·3 (sd 3·5) kg/m2) women participated. During visit one, anthropometrics and body composition were measured. Following visit one, subjects consumed either CP (25 g) or placebo (PL) ≥2 h after their last meal and ≤30 min prior to sleep on the night before visits two and three. Visits two and three occurred ≥1 week after visit one and were 48 h apart. During visits two and three, RMR (VO2), RER and appetite were measured via indirect calorimetry and visual analogue scale, respectively. Anthropometrics and body composition were analysed by one-way ANOVA. RMR and measures of appetite were analysed using a 2 × 2 (menopause status × CP/PL) repeated-measures ANOVA. Significance was accepted at P ≤ 0·05. RMR was significantly lower in postmenopausal compared with premenopausal women under both conditions (P = 0·003). When consumed pre-sleep CP did not alter RMR, RER or appetite compared with PL when assessed next morning in pre- and postmenopausal women. These data contribute to growing evidence that pre-sleep consumption of protein is not harmful to next-morning metabolism or appetite. In addition, these data demonstrate that menopause may not alter next-morning RMR, RER or appetite after pre-sleep consumption of CP.

This was a metabolic study of bulimia nervosa required to design short-term cognitive-behavioural therapy (CBT) beginning with a brief admission to a psychiatric ward. The treatment produced significant improvements in eating behaviour and results are compared with those of previously published studies. The comparisons do not suggest that brief admission at the onset of therapy might enhance its effectiveness. In other respects, increase in normal meal intake was found to correlate significantly with decrease in hinging. This supports the notion that appropriate food intake at meal times should be an important issue in CBT for bulimia nervosa.
Personalized models of personality disorders: using a temporal network method to understand symptomatology and daily functioning in a clinical sample

Background
An ongoing challenge in understanding and treating personality disorders (PDs) is a significant heterogeneity in disorder expression, stemming from variability in underlying dynamic processes. These processes are commonly discussed in clinical settings, but are rarely empirically studied due to their personalized, temporal nature. The goal of the current study was to combine intensive longitudinal data collection with person-specific temporal network models to produce individualized symptom-level structures of personality pathology. These structures were then linked to traditional PD diagnoses and stress (to index daily functioning).
MethodsUsing about 100 daily assessments of internalizing and externalizing domains underlying PDs (i.e. negative affect, detachment, impulsivity, hostility), a temporal network mapping approach (i.e. group iterative multiple model estimation) was used to create person-specific networks of the temporal relations among domains for 91 individuals (62.6% female) with a PD. Network characteristics were then associated with traditional PD symptomatology (controlling for mean domain levels) and with daily variation in clinically-relevant phenomena (i.e. stress).
ResultsFeatures of the person-specific networks predicted paranoid, borderline, narcissistic, and obsessive-PD symptom counts above average levels of the domains, in ways that align with clinical conceptualizations. They also predicted between-person variation in stress across days.
ConclusionsRelations among behavioral domains thought to underlie heterogeneity in PDs were indeed associated with traditional diagnostic constructs and with daily functioning (i.e. stress) in person-specific networks. Findings highlight the importance of leveraging data and models that capture person-specific, dynamic processes, and suggest that person-specific networks may have implications for precision medicine.

In North America, terrestrial records of biodiversity and climate change that span Marine Oxygen Isotope Stage (MIS) 5 are rare. Where found, they provide insight into how the coupling of the ocean–atmosphere system is manifested in biotic and environmental records and how the biosphere responds to climate change. In 2010–2011, construction at Ziegler Reservoir near Snowmass Village, Colorado (USA) revealed a nearly continuous, lacustrine/wetland sedimentary sequence that preserved evidence of past plant communities between ~140 and 55 ka, including all of MIS 5. At an elevation of 2705 m, the Ziegler Reservoir fossil site also contained thousands of well-preserved bones of late Pleistocene megafauna, including mastodons, mammoths, ground sloths, horses, camels, deer, bison, black bear, coyotes, and bighorn sheep. In addition, the site contained more than 26,000 bones from at least 30 species of small animals including salamanders, otters, muskrats, minks, rabbits, beavers, frogs, lizards, snakes, fish, and birds. The combination of macro- and micro-vertebrates, invertebrates, terrestrial and aquatic plant macrofossils, a detailed pollen record, and a robust, directly dated stratigraphic framework shows that high-elevation ecosystems in the Rocky Mountains of Colorado are climatically sensitive and varied dramatically throughout MIS 5.

Background
Researchers have studied psychological disorders extensively from a common cause perspective, in which symptoms are treated as independent indicators of an underlying disease. In contrast, the causal systems perspective seeks to understand the importance of individual symptoms and symptom-to-symptom relationships. In the current study, we used network analysis to examine the relationships between and among depression and anxiety symptoms from the causal systems perspective.
MethodWe utilized data from a large psychiatric sample at admission and discharge from a partial hospital program (N = 1029, mean treatment duration = 8 days). We investigated features of the depression/anxiety network including topology, network centrality, stability of the network at admission and discharge, as well as change in the network over the course of treatment.
ResultsIndividual symptoms of depression and anxiety were more related to other symptoms within each disorder than to symptoms between disorders. Sad mood and worry were among the most central symptoms in the network. The network structure was stable both at admission and between admission and discharge, although the overall strength of symptom relationships increased as symptom severity decreased over the course of treatment.
ConclusionsExamining depression and anxiety symptoms as dynamic systems may provide novel insights into the maintenance of these mental health problems.

Background
The frequency of full syndromal and subsyndromal delirium is understudied.
AimsWe conducted a point prevalence study in a general hospital.
MethodPossible delirium identified by testing for inattention was evaluated regarding delirium status (full/subsyndromal delirium) using categorical (Confusion Assessment Method (CAM), DSM-IV) and dimensional (Delirium Rating Scale-Revised-98 (DRS-R98) scores) methods.
ResultsIn total 162 of 311 patients (52%) screened positive for inattention. Delirium was diagnosed in 55 patients (17.7%) using DSM-IV, 52 (16.7%) using CAM and 58 (18.6%) using DRS-R98⩾12 with concordance for 38 (12.2%) individuals. Subsyndromal delirium was identified in 24 patients (7.7%) using a DRS-R98 score of 7–11 and 41 (13.2%) using 2/4 CAM criteria. Subsyndromal delirium with inattention (v. without) had greater disturbance of multiple delirium symptoms.
ConclusionsThe point prevalence of delirium and subsyndromal delirium was 25%. There was modest concordance between DRS-R98, DSM-IV and CAM delirium diagnoses. Inattention should be central to subsyndromal delirium definitions.
14C Measurements of Dissolved Inorganic and Organic Carbon in Qinghai Lake and Inflowing Rivers (NE Tibet, Qinghai Plateau), China

There have been a number of studies that have attempted to estimate the past radiocarbon reservoir effects in Qinghai Lake, China. This article reports on measurements on modern samples collected at the lake in October 2003 and October 2009, which allow us to better understand the systematics of the lake and shed new insights on the processes occurring in the lake. The results indicate that atmospheric exchange of 14C is the main process affecting surface dissolved inorganic carbon (DIC) in the lake, but dissolved organic carbon (DOC) can be explained as a combination of sources. We also conclude that sediment carbon can be explained by a model where input from the surrounding rivers and groundwater are important, in agreement with the model of Yu et al. (2007).

We document chemical laboratory procedures and results on international comparison samples at the Hertelendi Laboratory of Environmental Studies, Debrecen, Hungary. We also show results using the new MICADAS system and compare these results to internationally recognized standards and blank materials. The newly developed sample preparation system in HEKAL can handle samples as 1) organic material, 2) cellulose fraction of plant, 3) bones, 4) carbonate and shell, and 5) dissolved inorganic carbon of groundwater. The results of radiocarbon measurements on intercomparison samples confirm the reliability of the sample preparation system at HEKAL Lab and also the good performance of the MICADAS 14C system. The blank levels for each type of sample of 1 mg C carbon content are well reproducible, ∼0.3–0.5 pMC.
Effects of dietary seaweed extract supplementation in sows and post-weaned pigs on performance, intestinal morphology, intestinal microflora and immune status

The present study investigated the effects of dietary supplementation of a seaweed extract (SWE) to sows and weaned pigs on post-weaning growth performance, intestinal morphology, intestinal microflora, volatile fatty acid concentrations and immune status of pigs at days 11 and 117 post-weaning. Gestating sows (n 20) were supplemented with a SWE (0 v. 10·0 g/d) from day 107 of gestation until weaning (day 26). At weaning, pigs (four pigs per sow) were divided into two groups based on sow diet during lactation and supplemented with a SWE (0 v. 2·8 g/kg diet), resulting in four treatment groups: (1) BB (basal sows–basal pigs); (2) BS (basal sows–treated pigs); (3) SB (treated sows–basal pigs); (4) SS (treated sows–treated pigs). Pigs weaned from SWE-supplemented sows had a higher average daily gain (ADG) between days 0 and 21 (P < 0·05) post-weaning compared with pigs weaned from non-SWE-supplemented sows. Pigs offered post-weaning diets (PW) containing SWE had decreased colonic Escherichia coli populations on day 11 (P < 0·01) and decreased colonic Enterobacteriaceae numbers on day 117 (P < 0·05). Pigs offered PW containing SWE had a greater mRNA abundance of MUC2 in the colon at day 11 post-weaning (P < 0·05) compared with pigs offered unsupplemented diets. In conclusion, these results demonstrate that SWE supplementation post-weaning provides a dietary means to improve gut health and to enhance growth performance in starter pigs. Dietary SWE supplementation increased ADG during the grower–finisher (GF) phases. However, there was no growth response to SWE inclusion in GF diets when pigs were weaned from SWE-supplemented sows.

Effect of dietary seaweed extracts and fish oil supplementation in sows on performance, intestinal microflora, intestinal morphology, volatile fatty acid concentrations and immune status of weaned pigs

A 2 × 2 factorial experiment (ten sows per treatment) was conducted to investigate the effect of maternal dietary supplementation with a seaweed extract (SWE; 0 v. 10·0 g/d) and fish oil (FO; 0 v. 100 g/d) inclusion from day 109 of gestation until weaning (day 26) on pig performance post-weaning (PW) and intestinal morphology, selected microflora and immune status of pigs 9 d PW. The SWE contained laminarin (10 %), fucoidan (8 %) and ash (82 %) and the FO contained 40 % EPA and 25 % DHA. Pigs weaned from SWE-supplemented sows had higher daily gain (P = 0·063) between days 0 and 21 PW and pigs weaned from FO-supplemented sows had higher daily gain (P < 0·05) and gain to feed ratio (P < 0·01) between days 7 and 14 PW. There was an interaction between maternal SWE and FO supplementation on caecal Escherichia coli numbers (P < 0·05) and the villous height to crypt depth ratio in the ileum (P < 0·01) and jejunum (P < 0·05) in pigs 9 d PW. Pigs weaned from SWE-supplemented sows had lower caecal E. coli and a higher villous height to crypt depth ratio in the ileum and jejunum compared with non-SWE-supplemented sows (P < 0·05). There was no effect of SWE on E. coli numbers and villous height to crypt depth ratio with FO inclusion. Maternal FO supplementation induced an increase in colonic mRNA abundance of IL-1α and IL-6 (P < 0·05), while SWE supplementation induced an increase in ileal TNF-α (P < 0·01) and colonic TFF3 mRNA expression (P < 0·05). In conclusion, these results demonstrate that SWE and FO supplementation to the maternal diet influenced the gastrointestinal environment and performance of the weaned pig.

Measuring the entorhinal cortex (ERC) is challenging due to lateral border discrimination from the perirhinal cortex. From a sample of 39 nondemented older adults who completed volumetric image scans and verbal memory indices, we examined reliability and validity concerns for three ERC protocols with different lateral boundary guidelines (i.e., Goncharova, Dickerson, Stoub, & deToledo-Morrell, 2001; Honeycutt et al., 1998; Insausti et al., 1998). We used three novice raters to assess inter-rater reliability on a subset of scans (216 total ERCs), with the entire dataset measured by one rater with strong intra-rater reliability on each technique (234 total ERCs). We found moderate to strong inter-rater reliability for two techniques with consistent ERC lateral boundary endpoints (Goncharova, Honeycutt), with negligible to moderate reliability for the technique requiring consideration of collateral sulcal depth (Insausti). Left ERC and story memory associations were moderate and positive for two techniques designed to exclude the perirhinal cortex (Insausti, Goncharova), with the Insausti technique continuing to explain 10% of memory score variance after additionally controlling for depression symptom severity. Right ERC-story memory associations were nonexistent after excluding an outlier. Researchers are encouraged to consider challenges of rater training for ERC techniques and how lateral boundary endpoints may impact structure-function associations. (JINS, 2010, 16, 846–855.)
Radiocarbon Dating of the Last Volcanic Eruptions of Ciomadul Volcano, Southeast Carpathians, Eastern-Central Europe

This paper provides new accelerator mass spectrometry (AMS) radiocarbon age data for the last volcanic events in the Carpathian-Pannonian region of eastern-central Europe. The eruption ages were determined on charcoal fragments collected from pumiceous pyroclastic flow deposits at 2 localities of the Ciomadul Volcano. Two charcoal samples from the southeastern margin of the volcano (Bixad locality) set the date of the last volcanic eruption to 27,200 ± 260 yr BP (29,500 ± 260 cal BC). On the other hand, our data show that the Tusnad pyroclastic flow deposit, previously considered as representing the youngest volcanic rock of the region, erupted at ∼39,000 yr BP (∼41,300 cal BC). Thus, a period of dormancy more than 10,000 yr long might have elapsed between the 2 volcanic events. The different ages of the Tusnad and Bixad pyroclastic flow deposits are confirmed also by the geochemical data. The bulk pumices, groundmass glass, and the composition of the main mineral phases (plagioclase and amphibole) suggest eruption of slightly different magmas. Considering also the assumed long volcanic history (∼600 ka) of the Ciomadul, these data suggest that further detailed studies are necessary on this seemingly inactive volcano in order to evaluate the possible renewal of volcanic activity in the future.

The current status of SiC bulk growth is reviewed, while specific attention is given to the effect of defects in SiC substrates and epitaxial layers on device performance and yield. The progress in SiC wafer quality is reflected in the achievement of micropipe densities as low as 0.92 cm−2 for a 3-inch n-type 4H-SiC wafer, which provides the basis for a high yielding fabrication process of large area SiC power devices. Using a Murphy Probe Yield Analysis for the breakdown characteristics of 10 kV PiN diodes we have extracted an “effective” defect density for 4H-SiC material to be as low as 30 cm−2, providing valuable information to further isolate and address the specific material defects critical for device performance. We address the problematic degradation of the forward characteristics (Vf-drift) of bipolar SiC PiN diodes [CITE]. The underlying mechanism due to stacking fault formation in the epitaxial layers and possible effects of device processing are investigated. An improved device design is demonstrated, which effectively stabilizes this Vf-drift. We show the progression in the development of semi-insulating SiC grown by the sublimation technique from extrinsically doped material to high purity semi-insulating (HPSI) 4H-SiC bulk crystals of up to 100 mm diameter without resorting to the intentional introduction of elemental deep level dopants, such as vanadium. Uniform resistivities in 3-inch HPSI wafers greater than 3 × 1011 Ω-cm have been achieved. Secondary ion mass spectrometry, deep level transient spectroscopy and electron paramagnetic resonance data suggest that the semi-insulating behavior in HPSI material originates from deep levels associated with intrinsic point defects. MESFETs produced on HPSI wafers are free of backgating effects and have resulted in the best combination of power density and efficiency reported to date for SiC MESFETs of 5.2 W/mm and 63% power added efficiency (PAE) at 3.5 GHz.

Summary
Periodic axisymmetric vortex breakdown in a cylinder with a rotating end wall
When the fluid inside a completely filled cylinder is set in motion by the rotation of the bottom end wall, steady and unsteady axisymmetric vortex breakdown is possible. The onset of unsteadiness is via a Hopf bifurcation.
Figure 1 is a perspective view of the flow inside the cylinder where marker particles have been released from an elliptic ring concentric with the axis of symmetry near the top end wall. This periodic flow corresponds to a Reynolds number Re=2765 and cylinder aspect ratio H/R=2.5. Neighboring particles have been grouped to define a sheet of marker fluid and the local transparency of the sheet has been made proportional to its local stretching. The resultant dye sheet takes on an asymmetric shape, even though the flow is axisymmetric, due to the unsteadiness and the asymmetric release of marker particles.When the release is symmetric, as in Fig. 2, the dye sheet is also symmetric. These two figures are snapshots of the dye sheet after three periods of the oscillation (a period is approximately 36.3 rotations of the end wall). Figure 3 is a cross section of the dye sheet in Fig. 2 after 26 periods of the oscillation. Here only the marker particles are shown. They are colored according to their time of release, the oldest being blue, through green and yellow, and the most recently released being red. Comparison with Escudier's experiment shows very close agreement.
