OBJECTIVES/GOALS: Idiopathic pulmonary fibrosis (IPF) is a chronic lung disease characterized by dysregulated collagen accumulation in the lung parenchyma. Our goal is to investigate the role of O-linked N-Acetylglucosamine (O-GlcNAc) transferase (OGT) in pulmonary fibrosis to ultimately discover novel therapies for fibrosis resolution. METHODS/STUDY POPULATION: Lung tissue from IPF and non-IPF donors was subjected to immunohistochemistry (IHC) to assess O-GlcNAc levels. Primary human lung fibroblasts were treated with OGT or O-GlcNAcase (OGA) inhibitors followed by transforming growth factor-beta 1 (TGF-β1) stimulation to assess O-GlcNAc regulation of fibroblast-to-myofibroblast transition (FMT) markers [alpha smooth muscle actin (α-SMA) and type 1 and type 3 collagen (COL1α1, COL3α1)] In Drosophila melanogaster, OGT knockdown (KD)/overexpression (OE) was conditionally induced to assess pericardin, a type IV collagen-like protein, regulation by immunofluorescence. Lastly, a mouse model of bleomycin-induced pulmonary fibrosis was examined following OGT KD and assessed for fibrosis resolution via histology, hydroxyproline assay, and western blotting. RESULTS/ANTICIPATED RESULTS: O-GlcNAc staining was increased in IPF lung tissue compared to non-IPF control lungs. In primary human lung fibroblasts, TGF-α1 administration resulted in increased FMT markers (α-SMA, COL1α1, and COL3α1), which were reduced or increased by OGT or OGA inhibition, respectively. Genetic manipulation in the Drosophila models showed decreased pericardin expression with OGT KD compared to the wild-type, whereas OGT OE increased pericardin compared to control. Additionally, OGT KD in bleomycin treated aged mice resulted in reduced collagen levels at the transcript and protein level and concurrent fibrosis resolution as assessed by Masson’s trichrome staining and total hydroxyproline analysis. Collectively, showing OGT/O-GlcNAc regulating collagen in fibrosis resolution. DISCUSSION/SIGNIFICANCE: These data suggest that the OGT/O-GlcNAc axis regulates collagen deposition in pulmonary fibrosis, and we show that O-GlcNAc is implicated in the pathogenesis of IPF. We identified OGT as a therapeutic target to overcome current drug limitations, opening new horizons for biomedical treatment.
A cyclic perspective on transient gust encounters through the lens of persistent homology

Large-amplitude gust encounters exhibit a range of separated flow phenomena, making them difficult to characterize using the traditional tools of aerodynamics. In this work, we propose a dynamical systems approach to gust encounters, viewing the flow as a cycle (or a closed trajectory) in state space. We posit that the topology of this cycle, or its shape and structure, provides a compact description of the flow, and can be used to identify coordinates in which the dynamics evolve in a simple, intuitive way. To demonstrate this idea, we consider flowfield measurements of a transverse gust encounter. For each case in the dataset, we characterize the full-state dynamics of the flow using persistent homology, a tool that identifies holes in point cloud data, and transform the dynamics to a reduced-order space using a nonlinear autoencoder. Critically, we constrain the autoencoder such that it preserves topologically relevant features of the original dynamics, or those features identified by persistent homology. Using this approach, we are able to transform six separate gust encounters to a three-dimensional latent space, in which each gust encounter reduces to a simple circle, and from which the original flow can be reconstructed. This result shows that topology can guide the creation of low-dimensional state representations for strong transverse gust encounters, a crucial step towards the modelling and control of aerofoil–gust interactions.
Decolonising food regulatory frameworks: importance of recognising traditional culture when assessing dietary safety of traditional foods

As interest in Australian native products continues to grow worldwide, Aboriginal and Torres Strait Islander peoples (First Peoples) are striving to be industry leaders in the production of their traditional foods that are being developed for commercial markets. To successfully gain market approval both within Australia and globally, food regulatory authorities require at least a documented history of safe use to indicate dietary safety. Moreover, many countries also require compositional analysis and safety data to further support their safe human consumption. However, safety data are lacking for many of these traditional food items and the history that surrounds their safe use has rarely been recorded in written form, but rather passed on through cultural practices and language. This review evaluates the suitability of current frameworks for assessing the dietary safety of traditional foods and highlights the food-safety regulatory hurdles currently felt by First Peoples and their businesses attempting to enter the Australian native foods industry. These issues also extend to the requirements of food regulatory authorities around the world, when assessing the market eligibility of traditional food items. Potential solutions to these problems are discussed, including new proposed processes that can be incorporated into the current food regulatory frameworks. Importantly, these proposed processes would allow the dietary risk assessment of traditional foods to be completed in a manner that better accommodates the stories, traditional knowledge and interests of First Peoples, while also meeting the safety data requirements set out by regulatory bodies both within Australia and around the world.

Background
Reward processing has been proposed to underpin the atypical social feature of autism spectrum disorder (ASD). However, previous neuroimaging studies have yielded inconsistent results regarding the specificity of atypicalities for social reward processing in ASD.
AimsUtilising a large sample, we aimed to assess reward processing in response to reward type (social, monetary) and reward phase (anticipation, delivery) in ASD.
MethodFunctional magnetic resonance imaging during social and monetary reward anticipation and delivery was performed in 212 individuals with ASD (7.6–30.6 years of age) and 181 typically developing participants (7.6–30.8 years of age).
ResultsAcross social and monetary reward anticipation, whole-brain analyses showed hypoactivation of the right ventral striatum in participants with ASD compared with typically developing participants. Further, region of interest analysis across both reward types yielded ASD-related hypoactivation in both the left and right ventral striatum. Across delivery of social and monetary reward, hyperactivation of the ventral striatum in individuals with ASD did not survive correction for multiple comparisons. Dimensional analyses of autism and attention-deficit hyperactivity disorder (ADHD) scores were not significant. In categorical analyses, post hoc comparisons showed that ASD effects were most pronounced in participants with ASD without co-occurring ADHD.
ConclusionsOur results do not support current theories linking atypical social interaction in ASD to specific alterations in social reward processing. Instead, they point towards a generalised hypoactivity of ventral striatum in ASD during anticipation of both social and monetary rewards. We suggest this indicates attenuated reward seeking in ASD independent of social content and that elevated ADHD symptoms may attenuate altered reward seeking in ASD.

Summary
Introduction
The ability to perceive the duration of events and distinguish them from other shorter or longer events is a fundamental part of consciousness and the way in which we understand and interact with the world. Despite this, within psychology itself (and even within the study of the senses and perception), the domain of time perception is not widely known about. There are several likely reasons for this: firstly, up until the 1980s, the field was heavily based on animal work, with little or no applied human models or data, apart from some very early experiments in the nineteenth century. Secondly the field is often confused with other types of timing such as circadian timing (regulation of the body's sleep-wake cycle) or reaction time (how quickly one can react to a presented stimulus). Thirdly, although we talk of people having a sense of time, it is not strictly a sense, as we possess no sensory organ for time; unlike the cochlea for hearing, or the retina for vision.
The aim of this chapter is to give the reader who has no prior knowledge of the field of time perception an overview of the important facts, models, distinctions between different types of timing, and the factors that affect them. This chapter does not delve too deeply into the specific implications of these facets to our specific perception of music, as such an undertaking is provided by Phillips in Chapter 1 (this volume), although that is not to say that no reference to music will be made.
I will start by talking about the perception of duration and the models we use to explore it and the factors that can affect or distort it. I will then move on to talk about another type of timing phenomenon termed ‘passage of time judgements’ which are concerned not with the perception of how long an event lasted, but how quickly time seemed to pass while engaged in a particular activity.
The role of telephone consultations in psychiatry

Aims
Telephone consultations have been in clinical use since the early 1960s and are increasing in frequency and importance in many areas of medicine. With the advent of the COVID-19 pandemic in 2020, the use of telemedicine consultations increased dramatically alongside utilization of other digital technologies. Despite promise and potential advantages for clinicians (including remote working, improved time management and safety) there are known drawbacks to telephone consultations for psychiatrists. This includes limitations to assessments of mental state and risk, with loss of non-verbal communication often cited as a point in favour of more sophisticated technologies such as video calling. By adopting telephone consultations to a greater extent during the initial months of the COVID-19 pandemic in the Coventry Crisis Resolution and Home Treatment Team (CRHTT), we aimed to assess the patient experience in telehealth, through a patient survey.
MethodAfter an initial assessment or follow-up consultation with a medical practitioner from the crisis team, patients were invited to take part in a short questionnaire with a member of the administration staff. This consisted of eight questions on a Likert scale and three open questions for comments. Results were collated and analyzed via Microsoft Excel.
ResultMost patients found the telephone consultations satisfactory, with more than 90% returning positive scores in understanding, convenience and overall satisfaction. All patients felt listened to and that their confidentiality was maintained; with all but one respondent willing to engage in further consultations via the telephone. Negative scores were typically returned for practical telephonic problems including poor signal, interference and background noise. In their comments patients expressed largely positive views about their experience with their clinician; analysis revealed key insights into the patient experience, demonstrating the convenience, comfort and flexibility possible with ‘telepsychiatry’.
ConclusionPatient experience of telemedicine in a UK psychiatric crisis team is mostly positive, with clear advantages for both patients and clinicians. Our results show telephone consultations can be expanded to new patient assessments alongside follow-ups, enabling the team to reach a greater number of service users. This includes service users who are housebound due to infirmity, required to shield or have significant anxiety about the pandemic.

In many applications, conventional aerofoils are subject to a number of simultaneous motions that complicate the prediction of flow separation. The purpose of this work is to evaluate the impact of a large-amplitude free-stream oscillation on the timing of vortex formation for a simultaneously surging and pitching wing. Experimental flow field measurements were obtained on a NACA 0012 aerofoil over a wide range of surge amplitudes (
$1.50 \leq \lambda \leq 2.25$) and reduced frequencies (
$0.1 \leq k \leq 0.3$). Particular attention was paid to how various mechanisms of flow separation, specifically the velocity induced by the trailing wake and unsteady effects in the boundary layer, were impacted by a change in the properties of the surge motion. In the regime where
$k \leq 0.3$, a change in the surge kinematics primarily manifested as a change in the relative strength of the trailing wake. Boundary layer unsteadiness was found to have a negligible influence on the timing of vortex formation in the same flow regime. Thus, the timing of leading-edge vortex formation was well predicted by a combination of an unsteady inviscid flow solver and a quasi-steady treatment of the boundary layer, a promising result for low-order predictions of vortex behaviour in unsteady aerofoil flows.
Look duration at the face as a developmental endophenotype: elucidating pathways to autism and ADHD

Identifying developmental endophenotypes on the pathway between genetics and behavior is critical to uncovering the mechanisms underlying neurodevelopmental conditions. In this proof-of-principle study, we explored whether early disruptions in visual attention are a unique or shared candidate endophenotype of autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD). We calculated the duration of the longest look (i.e., peak look) to faces in an array-based eye-tracking task for 335 14-month-old infants with and without first-degree relatives with ASD and/or ADHD. We leveraged parent-report and genotype data available for a proportion of these infants to evaluate the relation of looking behavior to familial (n = 285) and genetic liability (using polygenic scores, n = 185) as well as ASD and ADHD-relevant temperament traits at 2 years of age (shyness and inhibitory control, respectively, n = 272) and ASD and ADHD clinical traits at 6 years of age (n = 94).
Results showed that longer peak looks at the face were associated with elevated polygenic scores for ADHD (β = 0.078, p = .023), but not ASD (β = 0.002, p = .944), and with elevated ADHD traits in mid-childhood (F(1,88) = 6.401, p = .013, $\eta _p^2$
=0.068; ASD: F (1,88) = 3.218, p = .076), but not in toddlerhood (ps > 0.2). This pattern of results did not emerge when considering mean peak look duration across face and nonface stimuli. Thus, alterations in attention to faces during spontaneous visual exploration may be more consistent with a developmental endophenotype of ADHD than ASD. Our work shows that dissecting paths to neurodevelopmental conditions requires longitudinal data incorporating polygenic contribution, early neurocognitive function, and clinical phenotypic variation.
Understanding increasing rates of psychiatric hospital detentions in England: development and preliminary testing of an explanatory model

Background
The steep rise in the rate of psychiatric hospital detentions in England is poorly understood.
AimsTo identify explanations for the rise in detentions in England since 1983; to test their plausibility and support from evidence; to develop an explanatory model for the rise in detentions.
MethodHypotheses to explain the rise in detentions were identified from previous literature and stakeholder consultation. We explored associations between national indicators for potential explanatory variables and detention rates in an ecological study. Relevant research was scoped and the plausibility of each hypothesis was rated. Finally, a logic model was developed to illustrate likely contributory factors and pathways to the increase in detentions.
ResultsSeventeen hypotheses related to social, service, legal and data-quality factors. Hypotheses supported by available evidence were: changes in legal approaches to patients without decision-making capacity but not actively objecting to admission; demographic changes; increasing psychiatric morbidity. Reductions in the availability or quality of community mental health services and changes in police practice may have contributed to the rise in detentions. Hypothesised factors not supported by evidence were: changes in community crisis care, compulsory community treatment and prescribing practice. Evidence was ambiguous or lacking for other explanations, including the impact of austerity measures and reductions in National Health Service in-patient bed numbers.
ConclusionsBetter data are needed about the characteristics and service contexts of those detained. Our logic model highlights likely contributory factors to the rise in detentions in England, priorities for future research and potential policy targets for reducing detentions.
Viscous and inviscid strato-rotational instability

We examine the critical viscous mode of the Taylor–Couette strato-rotational instability, concentrating on cases where the buoyancy frequency
$N$ and the inner cylinder rotation rate
$\unicode[STIX]{x1D6FA}_{in}$ are comparable, giving a detailed account for
$N=\unicode[STIX]{x1D6FA}_{in}$. The ratio of the outer to the inner cylinder rotation rates
$\unicode[STIX]{x1D707}=\unicode[STIX]{x1D6FA}_{out}/\unicode[STIX]{x1D6FA}_{in}$ and the ratio of the inner to the outer cylinder radius
$\unicode[STIX]{x1D702}=r_{in}/r_{out}$ satisfy
$0<\unicode[STIX]{x1D707}<1$ and
$0<\unicode[STIX]{x1D702}<1$. We find considerable variation in the structure of the mode, and the critical Reynolds number
$Re_{c}$ at which the flow becomes unstable. For
$N=\unicode[STIX]{x1D6FA}_{in}$, we classify different regions of the
$\unicode[STIX]{x1D702}\unicode[STIX]{x1D707}$-plane by the critical viscous mode of each region. We find that there is a triple point in the
$\unicode[STIX]{x1D702}\unicode[STIX]{x1D707}$-plane where three different viscous modes all onset at the same Reynolds number. We also find a discontinuous change in
$Re_{c}$ along a curve in the
$\unicode[STIX]{x1D702}\unicode[STIX]{x1D707}$-plane, on one side of which exist closed unstable domains where the flow can restabilise when the Reynolds number is increased. A new form of viscous instability occurring for wide gaps has been detected. We show for the first time that there is a region of the parameter space for which the critical viscous mode at the onset of instability corresponds to the inviscid radiative instability of Le Dizès & Riedinger (J. Fluid Mech., vol. 660, 2010, pp. 147–161). Focusing on small-to-moderate wavenumbers, we demonstrate that the viscous and inviscid systems are not always correlated. We explore which viscous modes relate to inviscid modes and which do not. For asymptotically large vertical wavenumbers, we have extended the inviscid analysis of Park & Billant (J. Fluid Mech., vol. 725, 2013, pp. 262–280) to cover the cases where
$N$ and
$\unicode[STIX]{x1D6FA}_{in}$ are comparable.
Survival, Social Cohesion and Rock Art: The Painted Hands of Western Arnhem Land, Australia

This paper explores the complex story of a particular style of rock art in western Arnhem Land known as ‘Painted Hands’. Using new evidence from recent fieldwork, we present a definition for their style, distribution and place in the stylistic chronologies of this region. We argue these motifs played an important cultural role in Aboriginal society during the period of European settlement in the region. We explore the complex messages embedded in the design features of the Painted Hands, arguing that they are more than simply hand stencils or markers of individuality. We suggest that these figures represent stylized and intensely encoded motifs with the power to communicate a high level of personal, clan and ceremonial identity at a time when all aspects of Aboriginal cultural identity were under threat.

The physics of a rotary wing in forward flight are highly complex, particularly when flow separation is involved. The purpose of this work is to assess the role of three-dimensional (3-D) vortex dynamics, with a focus on Coriolis forces, in the evolution of vortices in the reverse flow region of a rotating wing. High-fidelity numerical simulations were performed to recreate the flow about a representative rotating wing in forward flight. A vorticity transport analysis was performed to quantify and compare the magnitudes of 2-D flow physics, vortex tilting and Coriolis effects in the resulting flow fields. Three-dimensional vortex dynamics was found to have a very small impact on the growth and behaviour of vortices in the reverse flow region; in fact, the rate of vortex growth was successfully modelled using a simple 2-D vortex method. The small role of 3-D physics was attributed to the Coriolis and vortex tilting terms being approximately equal and opposite to one another. This ultimately lead to vortex behaviour that more closely resembled a surging wing as opposed to a conventional rotating wing, a feature unique to the reverse flow region.
Prevalence of low-grade inflammation in depression: a systematic review and meta-analysis of CRP levels

Background
Peripheral low-grade inflammation in depression is increasingly seen as a therapeutic target. We aimed to establish the prevalence of low-grade inflammation in depression, using different C-reactive protein (CRP) levels, through a systematic literature review and meta-analysis.
MethodsWe searched the PubMed database from its inception to July 2018, and selected studies that assessed depression using a validated tool/scale, and allowed the calculation of the proportion of patients with low-grade inflammation (CRP >3 mg/L) or elevated CRP (>1 mg/L).
ResultsAfter quality assessment, 37 studies comprising 13 541 depressed patients and 155 728 controls were included. Based on the meta-analysis of 30 studies, the prevalence of low-grade inflammation (CRP >3 mg/L) in depression was 27% (95% CI 21–34%); this prevalence was not associated with sample source (inpatient, outpatient or population-based), antidepressant treatment, participant age, BMI or ethnicity. Based on the meta-analysis of 17 studies of depression and matched healthy controls, the odds ratio for low-grade inflammation in depression was 1.46 (95% CI 1.22–1.75). The prevalence of elevated CRP (>1 mg/L) in depression was 58% (95% CI 47–69%), and the meta-analytic odds ratio for elevated CRP in depression compared with controls was 1.47 (95% CI 1.18–1.82).
ConclusionsAbout a quarter of patients with depression show evidence of low-grade inflammation, and over half of patients show mildly elevated CRP levels. There are significant differences in the prevalence of low-grade inflammation between patients and matched healthy controls. These findings suggest that inflammation could be relevant to a large number of patients with depression.
Improving the implementation of nutrition guidelines in childcare centres improves child dietary intake: findings of a randomised trial of an implementation intervention

Objective
Evidence suggests that improvements to the childcare nutrition environment can have a positive impact on child dietary intake. The primary aim of the present study was to assess, relative to usual care, the effectiveness of a multi-strategy implementation intervention in improving childcare compliance with nutrition guidelines. As a secondary aim, the impact on child dietary intake was assessed.
DesignParallel-group, randomised controlled trial design. The 6-month intervention was designed to overcome barriers to implementation of the nutrition guidelines that had been identified by applying the theoretical domains framework.

SubjectsForty-five centre-based childcare services.
ResultsThere were no differences between groups in the proportion of services providing food servings (per child) compliant with nutrition guideline recommendations for all five (5/5) food groups at follow-up (i.e. full compliance). Relative to control services, intervention services were more likely to be compliant with guidelines (OR; 95 % CI) in provision of fruit (10·84; 1·19, 551·20; P=0·0024), meat and meat alternatives (8·83; 1·55, –; P=0·023), dairy (8·41; 1·60, 63·62; P=0·006) and discretionary foods (17·83; 2·15, 853·73; P=0·002). Children in intervention services consumed greater servings (adjusted difference; 95 % CI) of fruit (0·41; 0·09, 0·73; P=0·014) and vegetables (0·70; 0·33, 1·08; P<0·001).
ConclusionsFindings indicate that service-level changes to menus in line with dietary guidelines can result in improvements to children’s dietary intake. The study provides evidence to advance implementation research in the setting as a means of enhancing child public health nutrition.
Effects of Crop Residue Management and Tillage on Weed Control and Sugarcane Production

Research was conducted to evaluate the residual effect of burning and mechanical removal of sugarcane crop residue on weed control and sugarcane growth and yield. In one study, crop residue was removed in December or January within 3 wk after sugarcane harvest. Mechanical removal of residue from the row top and placement in the row middle using a Sunco Trash Tiger® was compared with burning and a no removal control. Superimposed on each residue removal treatment was tillage of row sides and middles in March and in May and a no-tillage program. Tillage efficiency in March when sugarcane was emerging from the winter dormant period was not negatively affected when residue was removed mechanically. Crop residue on the soil surface did not completely suppress winter weed emergence and growth. Based on sugarcane and sugar yield averaged across locations and tillage programs, mechanical removal of residue was as effective as burning. Sugar yield was reduced 7.9% when residue was not removed from the row top compared with burning or mechanical removal. In another study, crop residue of sugarcane harvested in early December was burned or removed mechanically from the row top 4 d after harvest and also mechanically in January, February, or March. Crop residue ground cover of 79% in March on the row top where residue was not removed provided some suppression of winter weeds. Sugarcane shoot population in April was reduced 16.1% when residue removal was delayed until March compared with December. Averaged across tillage and no-tillage treatments in March, sugarcane yield did not differ when residue was removed by burning in December compared with mechanical removal in December or January. Delaying mechanical removal of residue until February or March compared with residue removal in December decreased sugar yield an average of 13.1%.

Tillage is used in sugarcane to control weeds, eliminate ruts caused by harvest, destroy residue from the previous crop, and incorporate fertilizer. The effect of weed control and tillage programs on sugarcane growth and yield and on economics was evaluated over two growing seasons. In the first study, weeds were effectively controlled with a March application of hexazinone at 0.59 kg ai/ha plus diuron at 2.10 kg ai/ha either banded or broadcast. When tillage of row shoulders and middles in March was eliminated, soil temperature in the sugarcane drill early in the season was equal to that where March tillage was performed. Sugarcane early and late season stalk population and sugarcane and sugar yield were each equivalent for the full season tillage (tillage of row shoulders and middles in March and in May) and the no-till programs. Elimination of a single tillage operation reduced cost $16.28/ha, and herbicide applied as a band rather than broadcast reduced cost $30.49/ ha. For the no-till program, with herbicide banded in March, net return was increased $32.56/ha. In a subsequent study conducted at five locations, weed control was excellent when either pendimethalin at 2.77 kg ai/ha plus metribuzin at 1.26 kg ai/ha or hexazinone plus diuron at 0.59 kg/ha and 2.10 kg/ha was used. When the March tillage was eliminated, sugar yield was increased 8.6% (620 kg/ ha), and net return was increased $152.68/ha compared with March tillage. When the May tillage was eliminated sugar yield was increased 8% (580 kg/ha), and net return was increased $143.88/ha compared with May tillage. A reduction in tillage cost accounted for only $16.28 of the increase in net return per hectare, with the remainder due to increased yield with the elimination of the tillage operation.

Developing countries are known to exploit their resource frontier to achieve growth objectives and reduce poverty. This can lead to long-term positive outcomes or – if resource exploitation is unsustainable – lose–lose outcomes that leave populations and ecosystems worse off. This paper introduces a dynamic model of resource exploitation to explain how regions may succumb to, avoid or escape this negative outcome. The theoretical model characterizes a frontier community that uses soil as an input into agricultural production. The model shows that there may be a critical point in the soil stock that determines whether agricultural activities lead to sustainable development or a collapse in local income. This suggests that, in the event of a resource collapse, temporary adjustments to the system may permanently rehabilitate the resource base and change a community's development pattern. Calibration of the model to several frontier states of the Brazilian Amazon points toward an overall outcome of steady development.

Can brief mindfulness practice be of benefit? Evidence from an evaluation of group Person-based Cognitive Therapy for depression

Mindfulness-based Cognitive Therapy (MBCT) was not intended for current depression, and lengthy mindfulness practices (≥30 min) can be challenging. Person-based cognitive therapy (PBCT) includes brief mindfulness practices (<10 min). While group PBCT can improve depressive symptoms whether benefits can be attributed to the brief practices is unclear. Twenty-eight participants with chronic major depression were randomly assigned to PBCT (n = 14) or treatment as usual (n = 14). Measures of mindfulness and depression were taken. Six PBCT participants were interviewed. Improvements in mindfulness in mediating the relationship between group and improvements in depressive symptoms just failed to reach statistical significance (95% confidence interval −0.97 to 14.84). Thematic analysis identified four themes: ‘altered relationship to symptoms’, ‘impact on self’, ‘the challenge of letting go’ and ‘effect of the group’. Although bootstrapped indirect effects were in the hypothesized direction they failed to reach statistical significance; this could be due to low power, but further research is needed. Qualitative themes support the potential of brief mindfulness practices and are similar to themes identified of mindfulness-based interventions with lengthier mindfulness practices. Findings suggest that some people experiencing current depression report benefit from the brief mindfulness practices included in PBCT but further research in larger samples is now needed.

ObjectiveDescribe the epidemiology of carbapenem-resistant Enterobacteriaceae (CRE) and examine the effect of lower carbapenem breakpoints on CRE detection.
DesignRetrospective cohort.
SettingInpatient care at community hospitals.
PatientsAll patients with CRE-positive cultures were included.
MethodsCRE isolated from 25 community hospitals were prospectively entered into a centralized database from January 2008 through December 2012. Microbiology laboratory practices were assessed using questionnaires.
ResultsA total of 305 CRE isolates were detected at 16 hospitals (64%). Patients with CRE had symptomatic infection in 180 cases (59%) and asymptomatic colonization in the remainder (125 cases; 41%). Klebsiella pneumoniae (277 isolates; 91%) was the most prevalent species. The majority of cases were healthcare associated (288 cases; 94%). The rate of CRE detection increased more than fivefold from 2008 (0.26 cases per 100,000 patient-days) to 2012 (1.4 cases per 100,000 patient-days; incidence rate ratio (IRR), 5.3 [95% confidence interval (CI), 1.22–22.7]; P = .01). Only 5 hospitals (20%) had adopted the 2010 Clinical and Laboratory Standards Institute (CLSI) carbapenem breakpoints. The 5 hospitals that adopted the lower carbapenem breakpoints were more likely to detect CRE after implementation of breakpoints than before (4.1 vs 0.5 cases per 100,000 patient-days; P < .001; IRR, 8.1 [95% CI, 2.7–24.6]). Hospitals that implemented the lower carbapenem breakpoints were more likely to detect CRE than were hospitals that did not (3.3 vs 1.1 cases per 100,000 patient-days; P = .01).
ConclusionsThe rate of CRE detection increased fivefold in community hospitals in the southeastern United States from 2008 to 2012. Despite this, our estimates are likely underestimates of the true rate of CRE detection, given the low adoption of the carbapenem breakpoints recommended in the 2010 CLSI guidelines.
