Traditional faith healers (TFHs) are often consulted for serious mental illness (SMIs) in low- and middle-income countries (LMICs). Involvement of TFHs in mental healthcare could provide an opportunity for early identification and intervention to reduce the mental health treatment gap in LMICs. The aim of this study was to identify models of collaboration between TFHs and biomedical professionals, determine the outcomes of these collaborative models and identify any mechanisms (i.e., explanatory processes) or contextual moderators (i.e., barriers and facilitators) of these outcomes. A systematic scoping review of five electronic databases was performed from inception to March 2023 guided by consultation with local experts in Nigeria and Bangladesh. Data were extracted using a predefined data charting form and synthesised narratively. Six independent studies (eight articles) satisfied the inclusion criteria. Study locations included Ghana (n = 1), Nigeria (n = 1), Nigeria and Ghana (n = 1), India (n = 1), Hong Kong (n = 1) and South Africa (n = 1). We identified two main intervention typologies: (1) Western-based educational interventions for TFHs and (2) shared collaborative models between TFHs and biomedical professionals. Converging evidence from both typologies indicated that education for TFHs can help reduce harmful practices. Shared collaborative models led to significant improvements in psychiatric symptoms (in comparison to care as usual) and increases in referrals to biomedical care from TFHs. Proposed mechanisms underpinning outcomes included trust building and empowering TFHs by increasing awareness and knowledge of mental illness and human rights. Barriers to implementation were observed at the individual (e.g., suspicions of TFHs), relationship (e.g., reluctance of biomedical practitioners to equalise their status with TFHs) and service (e.g., lack of formal referral systems) levels. Research on collaborative models for mental healthcare is in its infancy. Preliminary findings are encouraging. To ensure effective collaboration, future programmes should incorporate active participation from community stakeholders (e.g., patients, caregivers, faith healers) and target barriers to implementation on multiple levels.
To determine the association between blood markers of white matter injury (e.g., serum neurofilament light and phosphorylated neurofilament heavy) and a novel neuroimaging technique measuring microstructural white matter changes (e.g., diffusion kurtosis imaging) in regions (e.g., anterior thalamic radiation and uncinate fasciculus) known to be impacted in traumatic brain injury (TBI) and associated with symptoms common in those with chronic TBI (e.g., sleep disruption, cognitive and emotional disinhibition) in a heterogeneous sample of Veterans and non-Veterans with a history of remote TBI (i.e., >6 months).
Participants and Methods:
Participants with complete imaging and blood data (N=24) were sampled from a larger multisite study of chronic mild-moderate TBI. Participants ranged in age from young to middle-aged (mean age = 34.17, SD age = 10.96, range = 19-58) and primarily male (66.7%). The number of distinct TBIs ranged from 1-5 and the time since most recent TBI ranged from 0-30 years. Scores on a cognitive screener (MoCA) ranged from 22-30 (mean = 26.75). We performed bivariate correlations with mean kurtosis (MK) in the anterior thalamic radiation (ATR; left, right) uncinate fasciculus (UF; left, right), and serum neurofilament light (NFL), and phosphorylated neurofilament heavy (pNFH). Both were log transformed for non-normality. Significance threshold was set at p<0.05.
Results:
pNFH was significantly and negatively correlated to MK in the right (r=-0.446) and left (r=-0.599) UF and right (r=-0.531) and left (r=-0.469) ATR. NFL showed moderate associations with MK in the right (r=-0.345) and left (r=-0.361) UF and little to small association in the right (r=-0.063) and left (r=-0.215) ATR. In post-hoc analyses, MK in both the left (r=0.434) and right (r=0.514) UF was positively associated with performance on a frontally-mediated list-learning task (California Verbal Learning Test, 2nd Edition; Trials 1-5 total).
Conclusions:
Results suggest that serum pNFH may be a more sensitive blood marker of microstructural complexity in white matter regions frequently impacted by TBI in a chronic mild-moderate TBI sample. Further, it suggests that even years after a mild-moderate TBI, levels of pNFH may be informative regarding white matter integrity in regions related to executive functioning and emotional disinhibition, both of which are common presenting problems when these patients are seen in a clinical setting.
Mathematical models of polyelectrolyte gels are often simplified by assuming the gel is electrically neutral. The rationale behind this assumption is that the thickness of the electric double layer (EDL) at the free surface of the gel is small compared to the size of the gel. Hence, the thin-EDL limit is taken, in which the thickness of the EDL is set to zero. Despite the widespread use of the thin-EDL limit, the solutions in the EDL are rarely computed and shown to match to the solutions for the electrically neutral bulk. The aims of this paper are to study the structure of the EDL and establish the validity of the thin-EDL limit. The model for the gel accounts for phase separation, which gives rise to diffuse interfaces with a thickness described by the Kuhn length. We show that the solutions in the EDL can only be asymptotically matched to the solutions for an electrically neutral bulk, in general, when the Debye length is much smaller than the Kuhn length. If the Debye length is similar to or larger than the Kuhn length, then phase separation can be initiated in the EDL. This phase separation spreads into the bulk of the gel and gives rise to electrically charged layers with different degrees of swelling. Thus, the thin-EDL limit and the assumption of electroneutrality only generally apply when the Debye length is much smaller than the Kuhn length.
Precision Medicine is an emerging approach for disease treatment and prevention that takes into account individual variability in genes, environment, and lifestyle. Autoimmune diseases are those in which the body’s natural defense system loses discriminating power between its own cells and foreign cells, causing the body to mistakenly attack healthy tissues. These conditions are very heterogeneous in their presentation and therefore difficult to diagnose and treat. Achieving precision medicine in autoimmune diseases has been challenging due to the complex etiologies of these conditions, involving an interplay between genetic, epigenetic, and environmental factors. However, recent technological and computational advances in molecular profiling have helped identify patient subtypes and molecular pathways which can be used to improve diagnostics and therapeutics. This review discusses the current understanding of the disease mechanisms, heterogeneity, and pathogenic autoantigens in autoimmune diseases gained from genomic and transcriptomic studies and highlights how these findings can be applied to better understand disease heterogeneity in the context of disease diagnostics and therapeutics.
The Heavenly Kingdom of Great Peace sprang from a Chinese Christian evangelical movement with close affinities, but no institutional ties, to evangelical Protestantism in the United States, Great Britain, and the European continent. It originated with a vision of its later leader Hong Xiuquan in 1837, in which he was taken up to heaven and was ordered by an ‘old man’ to drive out of the heavens the demons who had taken possession of them. After he had accomplished this feat, the ‘old man’, who originally had created the world, ordered him to descend back down to earth and do the same, that is, drive out the demons, who had not only established control there, but had done so with the connivance of many members of the Confucian elite.
Verminotic pneumonia caused by Parafilaroides spp. nematodes is an underreported disease in beached South American fur seals, with scant literature available on the characteristics of parafilaroidiasis, the nematode itself, as well as its occurrence in pinnipeds in Brazil. The present work aims to identify, describe and detail the histological features of the infection and molecular characteristics of verminotic pneumonia in the South American fur seal. Twenty-six specimens of Arctocephalus australis, found dead on the northern coast of Rio Grande do Sul in 2021, were analysed. These animals were identified and submitted to necropsy and histology. For the molecular identification of metastrongylids, lung fragments were subjected to DNA extraction, polymerase chain reaction targeting the Internal transcribed spacer 2 (ITS-2) gene and subsequent sequencing. In total, 12 animals presented with parasites in the lung parenchyma on histological evaluation, and only 1 showed a granulomatous lung lesion at necropsy. Microscopically, the nematodes were found mainly in the alveoli, associated with little or no inflammatory response, and they had morphological characteristics compatible with metastrongylids. Six ITS-2 gene quality sequences were obtained; after comparative analysis via BLAST, they showed similarity with sequences obtained from Parafilaroides sp. Therefore, verminotic pneumonia caused by Parafilaroides represents an important differential diagnosis of lung disease in South American fur seals found on the northern coast of Rio Grande do Sul.
Reproducibility of a deep-learning fully convolutional neural network is evaluated by training several times the same network on identical conditions (database, hyperparameters, and hardware) with nondeterministic graphics processing unit operations. The network is trained to model three typical time–space-evolving physical systems in two dimensions: heat, Burgers’, and wave equations. The behavior of the networks is evaluated on both recursive and nonrecursive tasks. Significant changes in models’ properties (weights and feature fields) are observed. When tested on various benchmarks, these models systematically return estimations with a high level of deviation, especially for the recurrent analysis which strongly amplifies variability due to the nondeterminism. Trainings performed with double floating-point precision provide slightly better estimations and a significant reduction of the variability of both the network parameters and its testing error range.
Richard Jeffrey’s “Conditioning, Kinematics, and Exchangeability” is one of the foundational documents of probability kinematics. However, the section entitled “Successive Updating” contains a subtle error regarding the applicability of updating by so-called relevance quotients in order to ensure the commutativity of successive probability kinematical revisions. Upon becoming aware of this error, Jeffrey formulated the appropriate remedy, but he never discussed the issue in print. To head off any confusion, it seems worthwhile to alert readers of Jeffrey’s “Conditioning, Kinematics, and Exchangeability” to the aforementioned error and to document his remedy, placing it in the context of both earlier and subsequent work on commuting probability kinematical revisions.1
We present a software package for single-dish data processing of spacecraft signals observed with VLBI-equipped radio telescopes. The Spacecraft Doppler tracking (SDtracker) software allows one to obtain topocentric frequency detections with a sub-Hz precision and reconstructed and residual phases of the carrier signal of any spacecraft or landing vehicle at any location in the Solar System. These data products are estimated using the ground-based telescope’s highly stable oscillator as a reference, without requiring an a priori model of the spacecraft dynamics nor the downlink transmission carrier frequency. The software has been extensively validated in multiple observing campaigns of various deep space missions and is compatible with the raw sample data acquired by any standard VLBI radio telescope worldwide. In this paper, we report the numerical methodology of SDtracker, the technical operations for deployment and usage, and a summary of use cases and scientific results produced since its initial release.
Despite a wide range of proposed risk factors and theoretical models, prediction of eating disorder (ED) onset remains poor. This study undertook the first comparison of two machine learning (ML) approaches [penalised logistic regression (LASSO), and prediction rule ensembles (PREs)] to conventional logistic regression (LR) models to enhance prediction of ED onset and differential ED diagnoses from a range of putative risk factors.
Method
Data were part of a European Project and comprised 1402 participants, 642 ED patients [52% with anorexia nervosa (AN) and 40% with bulimia nervosa (BN)] and 760 controls. The Cross-Cultural Risk Factor Questionnaire, which assesses retrospectively a range of sociocultural and psychological ED risk factors occurring before the age of 12 years (46 predictors in total), was used.
Results
All three statistical approaches had satisfactory model accuracy, with an average area under the curve (AUC) of 86% for predicting ED onset and 70% for predicting AN v. BN. Predictive performance was greatest for the two regression methods (LR and LASSO), although the PRE technique relied on fewer predictors with comparable accuracy. The individual risk factors differed depending on the outcome classification (EDs v. non-EDs and AN v. BN).
Conclusions
Even though the conventional LR performed comparably to the ML approaches in terms of predictive accuracy, the ML methods produced more parsimonious predictive models. ML approaches offer a viable way to modify screening practices for ED risk that balance accuracy against participant burden.
The role of interpersonal relationship functioning in trauma recovery is well-established. However, much of this research has been done with cross-sectional samples, often years after trauma exposure, using self-report methodology only, and is focused on intimate relationship adjustment.
Methods
The current study investigated the longitudinal associations between interpersonal (intimate and non-intimate) relationship functioning and clinician- and self-reported posttraumatic stress disorder (PTSD) symptoms in 151 recently (within the past 6 months) traumatized individuals. Participants were assessed at four time points over 1 year.
Results
Approximately 53% of the sample was diagnosed with PTSD at initial assessment, with declining rates of diagnostic status over time to 16%. Latent difference score (LDS) modeling revealed nonlinear declines in both clinician-assessed and self-reported PTSD symptom severity, with faster declines in earlier periods. Likewise, LDS models revealed nonlinear declines in negative (conflict) aspects of interpersonal relationship functioning, but linear declines in positive (support, depth) aspects. The relationship between PTSD and relationship functioning differed for clinician- and self-reported PTSD. Bivariate LDS modeling revealed significant cross-lagged effects from relationship conflict to clinician-assessed PTSD, and significant cross-lagged effects from self-reported PTSD to relationship conflict over time.
Conclusions
These results highlight that the variability in prior results may be related to the method of assessing PTSD symptomatology and different relational constructs. Implications for theory and early intervention are discussed.
How do the media depict the leadership abilities of government leaders, and in what ways are these depictions gendered? Does the focus of leadership evaluations change over time, reflecting the increased presence of women in top leadership roles? To answer these questions, we examined news coverage of 22 subnational government leaders in Australia and Canada, countries in which a significant number of women have achieved the premiership at the state or provincial level since 2007. Analysis demonstrates that newly elected women and men leaders receive approximately the same number of leadership evaluations. Women are assessed based on the same criteria as men. All subnational political leaders are expected to be competent, intelligent, and levelheaded. That journalists prioritize experience and strength while downplaying honesty and compassion indicates a continued emphasis on “masculine” leadership norms in politics. Yet evaluations of new premiers have emphasized the traditionally “feminine” trait of collaboration as key to effective leadership and, over time, have given more attention to likability and emotions when covering male premiers. As our analysis reveals, media conceptualizations of political leadership competencies are slowly expanding in ways that make it easier for women to be seen as effective political leaders.
Purposeful qualitative modelling of embodiment function relations is a challenge in embodiment design. This contribution investigates the applicability and usefulness of the Contact and Channel Approach as a qualitative modelling approach in a survey study. From 23 development and research projects, advantages and challenges regarding applicability and usefulness are identified. A further result is that many different models are used additionally to the Contact and Channel Approach. Based on the findings, research potential for optimization and development of links to other models emerges.
Even though after the October Revolution in 1917 the Bolsheviks enjoyed uninterrupted power and pursued radical secularist objectives, the majority of female monastic communities in Nizhnii Novgorod province were able to survive much longer than their counterparts in the French and Mexican Revolutions. Using the Convent of the Exaltation of the Cross as a case study, this article shows how—despite extremely challenging conditions and the hostility of the Soviet state—female monastic communities proved to be remarkably resilient and managed to exploit openings created by both the Bolsheviks’ strategy for subverting them and conflicts between Soviet authorities. The resiliency of the community at the Convent of the Exaltation of the Cross stemmed from the solidarity, flexibility, and leadership skills it cultivated prior to World War I through the combination of its religious character and practices and its communal organization. By the early 1920s, the community had adapted effectively to post-civil war Soviet urban conditions and was able to survive local attempts to dissolve it. But by the late 1920s, the survival of the community had become intolerable for Soviet authorities, who—like the revolutionary regimes in France and Mexico—ultimately resorted to compulsory means to “liquidate” the community between 1927 and 1935.
Firefighting service is known to involve high rates of exposure to potentially traumatic situations, and research on mental health in firefighting populations is of critical importance in understanding the impact of occupational exposure. To date, the literature concerning prevalence of trauma-related mental disorders such as posttraumatic stress disorder (PTSD) has not distinguished between symptomology associated routine duty-related exposure and exposure to large-scale disaster. The present systematic review synthesizes a heterogeneous cross-national literature on large-scale disaster exposure in firefighters and provides support for the hypothesis that the prevalence of PTSD, major depressive disorder, and anxiety disorders are elevated in firefighters compared with rates observed in the general population. In addition, we conducted narrative synthesis concerning several commonly assessed predictive factors for disorder and found that sociodemographic factors appear to bear a weak relationship to mental disorder, while incident-related factors, such as severity and duration of disaster exposure, bear a stronger and more consistent relationship to the development of PTSD and depression in cross-national samples. Future work should expand on these preliminary findings to better understand the impact of disaster exposure in firefighting personnel.
Bulimia nervosa (BN) is a highly disabling disorder with severe health consequences. In a stepped care approach the NICE (National Institute of Clinical Excellence) guidelines recommend the application of guided self-help as a first step in the treatment of bulimia nervosa before continuing with face-to-face psychotherapy. Most of the self-help supports - available as books, CD-Rom or on an Internet surface - are based on cognitive behavioural concepts and have been proven to be effective. In this workshop we will present advantages of this treatment approach and provide you with a survey of research work implementing new technologies in the treatment of BN. We will give you an insight into the treatment modules involved in SALUT - BN developed by Netunion in cooperation with the University of Geneva and present the latest results and longterm outcome of a RCT comparing Internet-based Self program for BN with bibliotherapy.
To assess the differences in comorbid lifetime substance use (tobacco, alcohol and drug use) between eating disorder (ED) patients and healthy controls.
Method:
Participants were a consecutive series of 779 ED cases, who had been referred to specialised ED units in five European countries. The ED cases were compared to a balanced control group of 785 healthy individuals. Assessment: Participants completed the Substance Use Subscale of the Cross Cultural Questionnaire (CCQ), a measure of lifetime tobacco, alcohol and drug use. In the control group, also the GHQ-28, the SCID-I interview and the EAT-26 were used.
Results:
ED patients had higher lifetime consumption of tobacco and drugs (p <0.01). The only insignificant result was obtained for alcohol (OR= 1.29; δ =0.157; N.S.) and cannabis use (OR= 1.21; δ = 0.037, N.S.). Significant differences across ED sub diagnoses also emerged for all of the assessed variables (p<0.01), with the BN and AN-BP patients generally presenting the highest prevalence rates. The only exception was detected for alcohol consumption where EDNOS patients demonstrated the highest values (p=0.008). Only a few cultural differences between countries emerged (p<0.05).
Conclusions:
Lifetime tobacco and drug use but not alcohol consumption are more prevalent in ED patients than healthy controls. While alcohol appears to be more common in EDNOS, smoking and drug use are more frequent in patients with bulimic symptomatology. The differential risk observed in patients with bulimic features might be related to differences in temperament or might be the result of increased sensitivity to reward.
