The North American Antiepileptic Drug Pregnancy Registry: A Canadian Subgroup Analysis

Background:
The North American AED Pregnancy Registry (NAAPR) provides crucial data for understanding the risks of antiepileptic drug (AED) exposure in pregnancy. This study aims to quantify the Canadian contribution to NAAPR and compare AED usage in pregnancy in Canada and the USA.
Methods:Enrollment rate ratios (ERR) to NAAPR, adjusted for the populations of women of childbearing age, were calculated for the USA, Canada, and for the different Canadian provinces. Methods of enrollment to NAAPR and AED usage were compared between the two countries using chi-squared tests.
Results:Between 1997 and 2019, 10,215 pregnant women enrolled into NAAPR: 4.1% were Canadian (n = 432, ERR = 0.39, CI95% = 0.35–0.43). Within Canada, no patients were enrolled from the three northern territories or from Prince Edward Island. While fewer patients than expected enrolled from Quebec (ERR = 0.35, CI95% = 0.19–0.58), Nova Scotia had the highest enrollment rate (ERR = 1.55; CI95% = 0.66–3.11). Compared with their American peers, Canadians were less likely to have been enrolled by their healthcare provider and more likely to have been enrolled via social media (p < 0.01). Canadian women were more likely to be taking carbamazepine (24% vs. 15%; p < 0.01) or valproic acid (8% vs. 4%; p < 0.01).
Conclusion:The proportion of Canadian enrollees into NAAPR was less than expected based on the relative population size of Canadian women of reproductive age. Greater Canadian enrollment to NAAPR would contribute to ongoing worldwide efforts in assessing the risks of AEDs use in pregnant women and help quantify rates of AED usage, major congenital malformations, and access to subspecialized epilepsy care within Canada.

This article explores the religious response of one neglected writer to the evolutionary philosophy of Herbert Spencer. William Todd Martin was a minister of the Presbyterian Church in Ireland and in 1887 published The Evolution Hypothesis: A Criticism of the New Cosmic Philosophy. The work demonstrates the essentially contested nature of “evolution” and “creation” by showing how a self-confessed creationist could affirm an evolutionary understanding of the natural world and species transformation. Martin's approach reflected a transatlantic Presbyterian worldview that saw the harmony of science and religion on the basis of Calvinism, Baconianism and Scottish Common Sense philosophy. Martin's critique is also relevant to issues that continue to animate philosophers of science and religion, including the connections between mind and matter, morality and consciousness in a Darwinian framework, and the relationship between subjective conscious experience and evolutionary physicalism. Martin was able to anticipate these debates because his critique was essentially philosophical and theological rather than biological and biblicist.

Gravitational waves from coalescing neutron stars encode information about nuclear matter at extreme densities, inaccessible by laboratory experiments. The late inspiral is influenced by the presence of tides, which depend on the neutron star equation of state. Neutron star mergers are expected to often produce rapidly rotating remnant neutron stars that emit gravitational waves. These will provide clues to the extremely hot post-merger environment. This signature of nuclear matter in gravitational waves contains most information in the 2–4 kHz frequency band, which is outside of the most sensitive band of current detectors. We present the design concept and science case for a Neutron Star Extreme Matter Observatory (NEMO): a gravitational-wave interferometer optimised to study nuclear physics with merging neutron stars. The concept uses high-circulating laser power, quantum squeezing, and a detector topology specifically designed to achieve the high-frequency sensitivity necessary to probe nuclear matter using gravitational waves. Above 1 kHz, the proposed strain sensitivity is comparable to full third-generation detectors at a fraction of the cost. Such sensitivity changes expected event rates for detection of post-merger remnants from approximately one per few decades with two A+ detectors to a few per year and potentially allow for the first gravitational-wave observations of supernovae, isolated neutron stars, and other exotica.

Background
Cognitive behavior therapy (CBT) is effective for most patients with a social anxiety disorder (SAD) but a substantial proportion fails to remit. Experimental and clinical research suggests that enhancing CBT using imagery-based techniques could improve outcomes. It was hypothesized that imagery-enhanced CBT (IE-CBT) would be superior to verbally-based CBT (VB-CBT) on pre-registered outcomes.
MethodsA randomized controlled trial of IE-CBT v. VB-CBT for social anxiety was completed in a community mental health clinic setting. Participants were randomized to IE (n = 53) or VB (n = 54) CBT, with 1-month (primary end point) and 6-month follow-up assessments. Participants completed 12, 2-hour, weekly sessions of IE-CBT or VB-CBT plus 1-month follow-up.
ResultsIntention to treat analyses showed very large within-treatment effect sizes on the social interaction anxiety at all time points (ds = 2.09–2.62), with no between-treatment differences on this outcome or clinician-rated severity [1-month OR = 1.45 (0.45, 4.62), p = 0.53; 6-month OR = 1.31 (0.42, 4.08), p = 0.65], SAD remission (1-month: IE = 61.04%, VB = 55.09%, p = 0.59); 6-month: IE = 58.73%, VB = 61.89%, p = 0.77), or secondary outcomes. Three adverse events were noted (substance abuse, n = 1 in IE-CBT; temporary increase in suicide risk, n = 1 in each condition, with one being withdrawn at 1-month follow-up).
ConclusionsGroup IE-CBT and VB-CBT were safe and there were no significant differences in outcomes. Both treatments were associated with very large within-group effect sizes and the majority of patients remitted following treatment.

Introduction: Simulation has assumed an integral role in the Canadian healthcare system with applications in quality improvement, systems development, and medical education. High quality simulation-based research (SBR) is required to ensure the effective and efficient use of this tool. This study sought to establish national SBR priorities and describe the barriers and facilitators of SBR in Emergency Medicine (EM) in Canada. Methods: Simulation leads (SLs) from all fourteen Canadian Departments or Divisions of EM associated with an adult FRCP-EM training program were invited to participate in three surveys and a final consensus meeting. The first survey documented active EM SBR projects. Rounds two and three established and ranked priorities for SBR and identified the perceived barriers and facilitators to SBR at each site. Surveys were completed by SLs at each participating institution, and priority research themes were reviewed by senior faculty for broad input and review. Results: Twenty SLs representing all 14 invited institutions participated in all three rounds of the study. 60 active SBR projects were identified, an average of 4.3 per institution (range 0-17). 49 priorities for SBR in Canada were defined and summarized into seven priority research themes. An additional theme was identified by the senior reviewing faculty. 41 barriers and 34 facilitators of SBR were identified and grouped by theme. Fourteen SLs representing 12 institutions attended the consensus meeting and vetted the final list of eight priority research themes for SBR in Canada: simulation in CBME, simulation for interdisciplinary and inter-professional learning, simulation for summative assessment, simulation for continuing professional development, national curricular development, best practices in simulation-based education, simulation-based education outcomes, and simulation as an investigative methodology. Conclusion: Conclusion: This study has summarized the current SBR activity in EM in Canada, as well as its perceived barriers and facilitators. We also provide a consensus on priority research themes in SBR in EM from the perspective of Canadian simulation leaders. This group of SLs has formed a national simulation-based research group which aims to address these identified priorities with multicenter collaborative studies.
Increased amygdala-visual cortex connectivity in youth with persecutory ideation

Background
Subclinical delusional ideas, including persecutory beliefs, in otherwise healthy individuals are heritable symptoms associated with increased risk for psychotic illness, possibly representing an expression of one end of a continuum of psychosis severity. The identification of variation in brain function associated with these symptoms may provide insights about the neurobiology of delusions in clinical psychosis.
MethodsA resting-state functional magnetic resonance imaging scan was collected from 131 young adults with a wide range of severity of subclinical delusional beliefs, including persecutory ideas. Because of evidence for a key role of the amygdala in fear and paranoia, resting-state functional connectivity of the amygdala was measured.
ResultsConnectivity between the amygdala and early visual cortical areas, including striate cortex (V1), was found to be significantly greater in participants with high (n = 43) v. low (n = 44) numbers of delusional beliefs, particularly in those who showed persistence of those beliefs. Similarly, across the full sample, the number of and distress associated with delusional beliefs were positively correlated with the strength of amygdala-visual cortex connectivity. Moreover, further analyses revealed that these effects were driven by those who endorsed persecutory beliefs.
ConclusionsThese findings are consistent with the hypothesis that aberrant assignments of threat to sensory stimuli may lead to the downstream development of delusional ideas. Taken together with prior findings of disrupted sensory-limbic coupling in psychosis, these results suggest that altered amygdala-visual cortex connectivity could represent a marker of psychosis-related pathophysiology across a continuum of symptom severity.

For this study, we adapted the Montgomery Borgatta Caregiver Burden Scale, used widely in the United States, to the Saudi Arabian context. To produce an Arabic, culturally sensitive version of the scale, we conducted semi-structured interviews with 20 Saudi family caregivers. The Arabic version of the scale was tested, and participants were asked to comment on the appropriateness of items for the construct of “caregiver burden” using the repertory grid technique and laddering procedure – two constructivist methods derived from personal construct theory. From interview findings, we examined the content of the items and the caregiver burden construct itself. Our findings suggest that the use of constructivist methods to refine constructs and quantitative instruments is highly informative. This strategy is feasible even when little is known about the investigated constructs in the target culture and further elucidates our understanding of cross-cultural variations or invariance of different versions of the scale.
Consumption of processed food dietary patterns in four African populations

Objective
To identify predominant dietary patterns in four African populations and examine their association with obesity.
DesignCross-sectional study.
Setting/SubjectsWe used data from the Africa/Harvard School of Public Health Partnership for Cohort Research and Training (PaCT) pilot study established to investigate the feasibility of a multi-country longitudinal study of non-communicable chronic disease in sub-Saharan Africa. We applied principal component analysis to dietary intake data collected from an FFQ developed for PaCT to ascertain dietary patterns in Tanzania, South Africa, and peri-urban and rural Uganda. The sample consisted of 444 women and 294 men.
ResultsWe identified two dietary patterns: the Mixed Diet pattern characterized by high intakes of unprocessed foods such as vegetables and fresh fish, but also cold cuts and refined grains; and the Processed Diet pattern characterized by high intakes of salad dressing, cold cuts and sweets. Women in the highest tertile of the Processed Diet pattern score were 3·00 times more likely to be overweight (95 % CI 1·66, 5·45; prevalence=74 %) and 4·24 times more likely to be obese (95 % CI 2·23, 8·05; prevalence=44 %) than women in this pattern’s lowest tertile (both P<0·0001; prevalence=47 and 14 %, respectively). We found similarly strong associations in men. There was no association between the Mixed Diet pattern and overweight or obesity.
ConclusionsWe identified two major dietary patterns in several African populations, a Mixed Diet pattern and a Processed Diet pattern. The Processed Diet pattern was associated with obesity.
Environmental Effects on the Relative Competitive Ability of Canola and Small-Grain Cereals in a Direct-Seeded System

Growing crops that exhibit a high level of competition with weeds increases opportunities to practice integrated weed management and reduce herbicide inputs. The recent development and market dominance of hybrid canola cultivars provides an opportunity to reassess the relative competitive ability of canola cultivars with small-grain cereals. Direct-seeded (no-till) experiments were conducted at five western Canada locations from 2006 to 2008 to compare the competitive ability of canola cultivars vs. small-grain cereals. The relative competitive ability of the species and cultivars was determined by assessing monocot and dicot weed biomass at different times throughout the growing season as well as oat (simulated weed) seed production. Under most conditions, but especially under warm and relatively dry environments, barley cultivars had the greatest relative competitive ability. Rye and triticale were also highly competitive species under most environmental conditions. Canada Prairie Spring Red wheat and Canada Western Red Spring wheat cultivars usually were the least competitive cereal crops, but there were exceptions in some environments. Canola hybrids were more competitive than open-pollinated canola cultivars. More importantly, under cool, low growing degree day conditions, canola hybrids were as competitive as barley, especially with dicot weeds. Under most conditions, hybrid canola growers on the Canadian Prairies are well advised to avoid the additional selection pressure inherent with a second in-crop herbicide application. Combining competitive cultivars of any species with optimal agronomic practices that facilitate crop health will enhance cropping system sustainability and allow growers to extend the life of their valuable herbicide tools.
Interactions of ALS-Inhibiting Herbicide Residues in Three Prairie Soils

The objective of this study was to determine if the presence of two acetolactate synthase (ALS)-inhibiting herbicide residues in different Saskatchewan soils would result in additive, synergistic, or antagonistic interactions. This was determined through field trials where herbicides were applied sequentially over the course of 2 yr. The herbicides examined in these experiments were imazamethabenz, flucarbazone, sulfosulfuron, and florasulam, each in combination with imazamox and imazethapyr. The phytotoxicity and persistence of the herbicides in soil was assessed using an oriental mustard root inhibition bioassay. The determination of herbicide interaction was made through the comparison of the experimentally observed values to theoretically expected values derived from a mathematical equation. On the basis of the bioassay analysis, it was found that the herbicide residue combinations resulting from sequentially applied ALS-inhibiting herbicides in the three soils produced additive injury effects rather than synergistic or antagonistic interactions.
Determination of Thiencarbazone in Soil by Oriental Mustard Root Length Bioassay

Using an oriental mustard root length bioassay, thiencarbazone bioavailability and dissipation in five Saskatchewan soils was investigated under laboratory conditions. Thiencarbazone bioavailability was assessed at 0 to 3.9 µg ai kg−1. Thiencarbazone concentrations corresponding to 50% inhibition (I50 values) obtained from dose-response curves varied from 0.56 to 1.71 µg kg−1. Multiple regression analysis indicated that organic carbon content (P = 0.018) and soil pH (P = 0.017) predicted thiencarbazone bioavailability. Thiencarbazone dissipation was examined in soils incubated at 23 C and moisture content of 85% field capacity. Thiencarbazone half-lives estimated from dissipation curves were 9 to 50 d, and organic carbon content (P = 0.002) and soil pH (P = 0.008) were significant factors affecting thiencarbazone dissipation. Thiencarbazone bioavailability decreases and dissipation rate is slower in Canadian prairie soils of high organic matter content and low soil pH. Because root length of oriental mustard plants also was reduced by ammonium, therefore ammonium-containing or -producing fertilizers can cause false positive results for thiencarbazone soil residues. Canaryseed roots had the same sensitivity to ammonium as oriental mustard roots but were not inhibited by thiencarbazone. Therefore canaryseed root length bioassay was effective in identifying inhibition caused by ammonium toxicity. Use of oriental mustard root and canaryseed root bioassays together can aid in interpreting bioassay results for detection of thiencarbazone residues.
Relationships between Ligustrum sinense Invasion, Biodiversity, and Development in a Mixed Bottomland Forest

Invasion of closed canopy forests by shade-tolerant alien plants has the potential to modify species composition, stand structure, ecosystem function, and long-term forest development patterns. Ligustrum sinense is a shade-tolerant alien shrub that has invaded bottomland forests throughout the southeastern United States. This species has received comparatively little attention in the literature despite its potential to drastically alter invaded sites. The overarching goal of our study was to document the relationships between Ligustrum sinense invasion and woody plant biodiversity and development patterns in an intact southeastern U.S. bottomland forest. The forest was dominated by Quercus nigra and Liquidambar styraciflua. Ligustrum sinense ranked fifth in basal area contribution, occurred in 97% of our plots, and represented 95% of all understory stems. Spearman's rho for dominance (based on basal area of stems > 5 cm diameter at breast height [dbh]) of L. sinense and woody plant species richness for each plot revealed a significant negative relationship (rs = −0.69, P < 0.01). A similar relationship was revealed between L. sinense density and woody plant species diversity (rs = −0.78, P < 0.01) and evenness (rs = −0.82, P < 0.01). Spearman's rho for L. sinense density and native understory stem density (individuals ≥ 1 m height, < 5 cm dbh) also revealed a significant negative association (rs = −0.48, P < 0.01). Under the current disturbance regime and without active management, we projected the forest would shift to support a stronger component of L. sinense and that structure would transition from tree to shrub dominance for sites within the forest.
Development of a Laboratory Bioassay and Effect of Soil Properties on Sulfentrazone Phytotoxicity in Soil

Sulfentrazone is a phenyl triazolinone herbicide used for control of certain broadleaf and grass weed species. Sulfentrazone persists in soil and has residual activity beyond the season of application. A laboratory bioassay was developed for the detection of sulfentrazone in soil using root and shoot response of several crops. Shoot length inhibition of sugar beet was found to be the most sensitive and reproducible parameter for measurement of soil-incorporated sulfentrazone. The sugar beet bioassay was then used to examine the effect of soil properties on sulfentrazone phytotoxicity using 10 different Canadian prairie soils. Concentrations corresponding to 50% inhibition (I50 values) were obtained from the dose–response curves constructed for the soils. Sulfentrazone phytotoxicity was strongly correlated to the percentage organic carbon (P = 0.01) and also to percentage clay content (P = 0.05), whereas correlation with soil pH was nonsignificant (P = 0.21). Because sulfentrazone phytotoxicity was found to be soil dependent, the efficacy of sulfentrazone for weed control and sulfentrazone potential carryover injury will vary with soil type in the Canadian prairies.

As a weed, wheat has recently gained greater profile. Determining wheat persistence in cropping systems will facilitate the development of effective volunteer wheat management strategies. In October of 2000, glyphosate-resistant (GR) spring wheat seeds were scattered on plots at eight western Canada sites. From 2001 to 2003, the plots were seeded to a canola–barley–field-pea rotation or a fallow–barley–fallow rotation, with five seeding systems involving seeding dates and soil disturbance levels, and monitored for wheat plant density. Herbicides and tillage (in fallow systems) were used to ensure that no wheat plants produced seed. Seeding systems with greater levels of soil disturbance usually had greater wheat densities. Volunteer wheat densities at 2 (2002) and 3 (2003) yr after seed dispersal were close to zero but still detectable at most locations. At the end of 2003, viable wheat seeds were not detected in the soil seed bank at any location. The majority of wheat seedlings were recruited in the year following seed dispersal (2001) at the in-crop, prespray (PRES) interval. At the PRES interval in 2001, across all locations and treatments, wheat density averaged 2.6 plants m−2. At the preplanting interval (PREP), overall wheat density averaged only 0.2 plants m−2. By restricting density data to include only continuous cropping, low-disturbance direct-seeding (LDS) systems, the latter mean dropped below 0.1 plants m−2. Only at one site were preplanting GR wheat densities sufficient (4.2 plants m−2) to justify a preseeding herbicide treatment in addition to glyphosate in LDS systems. Overall volunteer wheat recruitment at all spring and summer intervals in the continuous cropping rotation in 2001 was 1.7% (3.3 plants m−2). Despite the fact that volunteer wheat has become more common in the central and northern Great Plains, there is little evidence from this study to suggest that its persistence will be a major agronomic problem.

Summary
Introduction
The historical evidence suggests that the health professions might never have developed EBP had it not been for the development of sophisticated research tools such as PubMed/MEDLINE and the Cochrane Library for identifying authoritative evidence (Eldredge, 2008a). By working with health professionals in using these tools, health librarians were pivotal figures in the development of Evidence Based Medicine (EBM) and the broader EBP movement. From supporting health professionals in EBP, health librarians have gone on to develop and use evidence within their professional practices – EBLIP. This chapter will provide a context for health librarian's work, describe EBLIP within the health library field and the state of the evidence base, and discuss the types of evidence used by health librarians. Two case studies show how EBLIP has been translated into practice and demonstrate how health librarians continue to push the boundaries of EBLIP. Finally, the future directions for research and EBLIP practice will be considered within a health library context.
The health library context
Health librarians often collaborate with other health professionals in a fast-paced environment that demands high levels of accountability for the accuracy of their work. Any mistakes can result in missed diagnoses, inappropriate treatments, incorrectly trained health professionals (Maggio et al., 2015) or misguided research projects. Many health librarians take years to establish credibility for their expert skills among other health professionals (Hannigan and Eldredge, 2014). With increasing frequency, health librarians work outside of physical libraries in roles as embedded colleagues, liaisons, clinical librarians, informaticists and informaticians; therefore, throughout this chapter the term health librarian will be used to describe all of these roles.
The context in which health librarians work is continuing to change (Funk, 2013). At one time, the majority of health librarians worked in hospital libraries. Now, in the USA many librarians work in centralized academic health-science centre libraries that co-ordinate access to electronic databases for their users, including health professionals and staff in affiliated hospitals. The National Library of Medicine in the USA coordinates outreach and other centralized functions. In the UK, health librarians work in hospitals, academic institutions and, increasingly, throughout other NHS organizations. Collections for NHS staff are centralized and health libraries are monitored and supported by a national Library and Knowledge Service.

Multi-locus variable number tandem repeat analysis (MLVA) is used in clinical and reference laboratories for subtyping verocytotoxin-producing Escherichia coli O157 (VTEC O157). However, as yet there is no common allelic or profile nomenclature to enable laboratories to easily compare data. In this study, we carried out an inter-laboratory comparison of an eight-loci MLVA scheme using a set of 67 isolates of VTEC O157. We found all but two isolates were identical in profile in the two laboratories, and repeat units were homogeneous in size but some were incomplete. A subset of the isolates (n = 17) were sequenced to determine the actual copy number of representative alleles, thereby enabling alleles to be named according to international consensus guidelines. This work has enabled us to realize the potential of MLVA as a portable, highly discriminatory and convenient subtyping method.
