Summary
Clinical cases covering the spectrum of upper limb pathology are presented here. In the hand, these include congenital hand deficiencies, Dupuytren’s disease, rheumatoid disease, nerve lesions and tendon transfers. In the elbow, this includes osteoarthritis and in the shoulder, massive cuff tear, scapula winging and painful shoulder arthroplasty. Clinical examination findings for each of the cases are highlighted.
A new, complex hyaline larger benthic foraminifer, Bigaella orbitoidiformis n. gen. n. sp., from the upper Bartonian-Priabonian of NW Turkey

The upper Bartonian–Priabonian shallow-marine deposits in the Biga Peninsula (NW Turkey) contain some hyaline larger benthic foraminifers (LBF) with a test architecture similar to ‘orbitoidiform’ foraminifers, but displaying some distinctive and complex morphological features that are recorded here for the first time. These coarsely porous specimens are characterized by a flat, disc-shaped, fragile, and smooth test with a layer of equatorial chambers/chamberlets, surrounded by poorly developed lateral chamberlets, never forming a discrete layer on either side of the equatorial layer. The nepionic stage is very distinctive because the bilocular embryonic apparatus is followed by a semi-rounded, notably large auxiliary chamber with a characteristic wavy outline, and consecutive cyclical chambers. The cyclical chamber arrangement is later transformed into annular cycles with numerous, complex arcuate- to cup-shaped chamberlets, as observed in equatorial sections. Bigaella orbitoidiformis Özcan, Mitchell, Pignatti, Simmons, and Yücel, n. gen. n. sp., is established for these specimens, and placed within the family Eoannulariidae Ferràndez-Cañadell and Serra-Kiel, emended herein. The new genus occurs together with Caudriella Haman and Huddleston and Epiannularia Caudri (both originally established from the American bioprovince) and the genus Linderina Schlumberger (found both in the Tethys and the American bioprovinces), together with other typical Western Tethyan LBFs. A comparison of the new genus with the aforementioned taxa is given.

Introduction:
Ensuring equitable access to health care is a widely agreed-upon goal in medicine, yet access to care is a multidimensional concept that is difficult to measure. Although frameworks exist to evaluate access to care generally, the concept of “access to genomic medicine” is largely unexplored and a clear framework for studying and addressing major dimensions is lacking.
Methods:Comprised of seven clinical genomic research projects, the Clinical Sequencing Evidence-Generating Research consortium (CSER) presented opportunities to examine access to genomic medicine across diverse contexts. CSER emphasized engaging historically underrepresented and/or underserved populations. We used descriptive analysis of CSER participant survey data and qualitative case studies to explore anticipated and encountered access barriers and interventions to address them.
Results:CSER’s enrolled population was largely lower income and racially and ethnically diverse, with many Spanish-preferring individuals. In surveys, less than a fifth (18.7%) of participants reported experiencing barriers to care. However, CSER project case studies revealed a more nuanced picture that highlighted the blurred boundary between access to genomic research and clinical care. Drawing on insights from CSER, we build on an existing framework to characterize the concept and dimensions of access to genomic medicine along with associated measures and improvement strategies.
Conclusions:Our findings support adopting a broad conceptualization of access to care encompassing multiple dimensions, using mixed methods to study access issues, and investing in innovative improvement strategies. This conceptualization may inform clinical translation of other cutting-edge technologies and contribute to the promotion of equitable, effective, and efficient access to genomic medicine.

Introduction:
Implementation of genome-scale sequencing in clinical care has significant challenges: the technology is highly dimensional with many kinds of potential results, results interpretation and delivery require expertise and coordination across multiple medical specialties, clinical utility may be uncertain, and there may be broader familial or societal implications beyond the individual participant. Transdisciplinary consortia and collaborative team science are well poised to address these challenges. However, understanding the complex web of organizational, institutional, physical, environmental, technologic, and other political and societal factors that influence the effectiveness of consortia is understudied. We describe our experience working in the Clinical Sequencing Evidence-Generating Research (CSER) consortium, a multi-institutional translational genomics consortium.
Methods:A key aspect of the CSER consortium was the juxtaposition of site-specific measures with the need to identify consensus measures related to clinical utility and to create a core set of harmonized measures. During this harmonization process, we sought to minimize participant burden, accommodate project-specific choices, and use validated measures that allow data sharing.
Results:Identifying platforms to ensure swift communication between teams and management of materials and data were essential to our harmonization efforts. Funding agencies can help consortia by clarifying key study design elements across projects during the proposal preparation phase and by providing a framework for data sharing data across participating projects.
Conclusions:In summary, time and resources must be devoted to developing and implementing collaborative practices as preparatory work at the beginning of project timelines to improve the effectiveness of research consortia.

Objectives:
Prion diseases are rare dementias that most commonly occur sporadically, but can be inherited or acquired, and for which there is no cure. We sought to understand which prion disease symptoms are most problematic for carers, to inform the development of outcome measures.
Design:Self-completed questionnaire with follow-up of a subset of participants by structured interview.
Setting:A nested study in the UK National Prion Monitoring Cohort, a longitudinal observational study.
Participants and measurements:71 carers, of people with different prion diseases with a wide range of disease severity, identified 236 of their four most problematic symptoms by questionnaire which were grouped into ten domains. Structured interviews were then done to qualitatively explore these experiences. Eleven family carers of people with prion disease were selected, including those representative of a range of demographics and disease subtypes and those who cared for people with prion disease, living or recently deceased. Interviews were transcribed and formally studied.
Results:The six most problematic symptom domains were: mobility and coordination; mood and behavior; personal care and continence; eating and swallowing; communication; and cognition and memory. The prevalence of these symptoms varied significantly by disease stage and type. A formal analysis of structured interviews to explore these domains is reported.
Conclusions:We make suggestions about how healthcare professionals can focus their support for people with prion disease. Clinical trials that aim to generate evidence regarding therapies that might confer meaningful benefits to carers should consider including outcome measures that monitor the symptomatic domains we have identified as problematic.

A controversy at the 2016 IUCN World Conservation Congress on the topic of closing domestic ivory markets (the 007, or so-called James Bond, motion) has given rise to a debate on IUCN's value proposition. A cross-section of authors who are engaged in IUCN but not employed by the organization, and with diverse perspectives and opinions, here argue for the importance of safeguarding and strengthening the unique technical and convening roles of IUCN, providing examples of what has and has not worked. Recommendations for protecting and enhancing IUCN's contribution to global conservation debates and policy formulation are given.

Summary
The Anzac Gallipoli Archaeological Database is a unique and detailed record of the information recovered during the JHAS field project. Its value is the documentation of archaeological contexts, thereby enabling features and artefacts to be studied in association. During the project, information was collated and managed in a Geographic Information System (GIS) and research database that includes searchable attributes about each feature and artefact. One outcome of the JHAS has been to make this database available to other researchers and the general public as a web-based, digital archive – the Anzac Gallipoli Archaeological Database (AGAD; <www.arts.unimelb.edu.au/gallipoli-battlefield>) – which is comprehensive in its content. It is an important and complementary resource for this book, as many of the features and artefacts that could not be included or illustrated in the text can be found online in AGAD.
AGAD occupies a unique space among the increasing number of digital archaeological gazetteers, archives and catalogues on web-based platforms that are being constructed as an effective way of making primary data available to a wide audience. With more than 2000 records precisely documented in the field, it aims to assist the study of the First World War through its emphasis on landscape and artefacts. The database is organised around the features and artefacts documented in the GIS, rather than specific sites or locations. The feature entries are of the various types used during the JHAS surveys (see table A.1). Each feature is given a unique Feature ID (and artefacts have an additional catalogue ‘Artefact Number’). Moreover, each feature has a record that displays data attributes, including a description, dimensions, chronological period, location and find-spot information, survey date, associated features, artefact type, material type, preservation rating and between one and three images, such as photographs and maps. Artefacts form a major portion of the archive, as they are the largest group of features recorded, and a separate set of artefact types is identified in the data set. It includes information documented in both Turkish and Anzac-held areas of the battlefield, providing an insight into the battlefield from perspectives on both sides of the conflict.
Feature types have been categorised from observations in the field. Eleven feature types are c. 1915 battlefield features (artefacts, boats, six types of earthworks, graves, roads, structures).

Trypanosomes are blood-borne parasites that can cause severe disease in both humans and animals, yet little is known of the pathogenicity and life-cycles of trypanosomes in native Australian mammals. Trypanosoma copemani is known to be infective to a variety of Australian marsupials and has recently been shown to be potentially zoonotic as it is resistant to normal human serum. In the present study, in vivo and in vitro examination of blood and cultures from Australian marsupials was conducted using light microscopy, immunofluorescence, scanning electron microscopy and fluorescence in situ hybridization. Promastigote, sphaeromastigote and amastigote life-cycle stages were detected in vivo and in vitro. Novel trypanosome-like stages were also detected both in vivo and in vitro representing an oval stage, an extremely thin stage, an adherent stage and a tiny round stage. The tiny round and adherent stages appeared to adhere to erythrocytes causing potential haematological damage with clinical effects similar to haemolytic anaemia. The present study shows for the first time that trypomastigotes are not the only life-cycle stages circulating within the blood stream of trypanosome infected Australian native marsupials and provides insights into possible pathogenic mechanisms of this potentially zoonotic trypanosome species.

Two new species of South African Syllidae of the genus Syllis Lamarck, 1818 are described. Syllis unzima sp. nov. is characterized by having unidentate compound chaetae with long spines on margin, a characteristic colour pattern and its reproduction by vivipary. Vivipary is not common among the polychaetes, but most representatives occur in the family Syllidae Grube, 1850 (in five other Syllis species, two species of Dentatisyllis Perkins, 1981 and two species of Parexogone Mesnil & Caullery, 1818). Syllis unzima sp. nov. differs from the other viviparous species in having large broods (>44 juveniles) which develop synchronously. Development of the juveniles is similar to that of free-spawning Syllis species, but the appearance of the first pair of eyespots and the differentiation of the pharynx and proventricle occur later in S. unzima. Syllis amicarmillaris sp. nov., is characterized by having an elongated body with relatively short, fusiform dorsal cirri and the presence of one or two pseudosimple chaeta on midbody parapodia by loss of blade and enlargement of shaft. Syllis unzima sp. nov. was found in high densities on cultured Holothuria scabra Jaeger, 1833 with single specimens found on a cultured Crassostrea gigas Thunberg, 1793 and on coralline algae, respectively, while S. amicarmillaris was found mainly in sediment outside an abalone farm and less frequently on cultured Haliotis midae Linnaeus, 1758. We discuss the possible benefits of the association with H. scabra to S. unzima sp. nov.

The characteristics of dense gravity currents in coastal regions, where free-surface gravity waves are dominant, have yet to be studied in the laboratory. This paper provides a first insight into the dynamics of dense saline gravity currents moving beneath regular progressive free-surface water waves. The gravity currents were generated by releasing a finite volume of saline into a large wave tank with an established periodic wave field. After the initial collapse, the gravity currents propagated horizontally with two fronts, one propagating in the wave direction and the other against the wave direction. The fronts of the gravity currents oscillated with an amplitude and phase that correlated with the orbital velocities within a region close to the bed. To leading order, the overall length of the gravity current was found to be weakly affected by the wave action and the dynamics of the current could be approximated by simply considering the buoyancy of the released fluid. Other characteristics such as the position of the gravity current centre and the shape of the two leading profiles were found to be significantly affected by the wave action. The centre was displaced at constant speed dependent on the second-order wave-induced mean Lagrangian velocity. For long waves, the centre was advected downstream in the direction of wave propagation owing to the dominance of Stokes drift. For short waves, the gravity current centre moved upstream against the wave direction, as under these wave conditions Stokes drift is negligible at the bed. An asymmetry in the shape of the upstream and downstream current heads was observed, with the gravity current front moving against the waves being much thicker and the front steeper, similar to the case of a current moving in a stream.

Excess body adiposity, especially abdominal obesity and ectopic fat accumulation, are key risk factors in the development of a number of chronic diseases. The advent of in vivo imaging methodologies that allow direct assessment of total body fat and its distribution have been pivotal in this process. They have helped to identify a number of sub-phenotypes in the general population whose metabolic risk factors are not commensurate with their BMI. At least two such sub-phenotypes have been identified: subjects with normal BMI, but excess intra-abdominal (visceral) fat (with or without increased ectopic fat) and subjects with elevated BMI (> 25 kg/m2) but low visceral and ectopic fat. The former sub-phenotype is associated with adverse metabolic profiles, while the latter is associated with a metabolically normal phenotype, despite a high BMI. Here, examples of these phenotypes are presented and the value of carrying out enhanced phenotypical characterisation of subjects in interventional studies discussed.

Background: The Improving Access to Psychological Therapies (IAPT) initiative has depended on the training of a new NHS mental health workforce. At step 3 of the stepped care model, capacity building has required the recruitment of a wide range of mental health professionals into high intensity therapists training posts. This shift naturally entails role transition on the part of trainees into delivering cognitive behavioural psychotherapy (CBP), but no previous research has examined the experience of such transitions. Aim: To describe the lived experience of transition from mental health nurse to IAPT high intensity therapist and to identify possible factors which moderate effective role conversions. Method: Six qualified high intensity therapists were interviewed using a semi-structured interview and the subsequent interviews transcribed. Thematic content analysis (TCA) was used to analyze personal accounts of role transition. All participants had previously been mental health nurses and attended the same IAPT high intensity therapist (HIT) training programme. Results: Six key themes were apparent from the TCA. Three interconnected themes concerning supervision (style, impact of approach and historical context) and three additional themes of the challenge of learning a new clinical approach, high need for support, and forming a new psychotherapist identity. Conclusions: Findings suggest supervision is the most important factor in supporting complex psychotherapy role transitions. Clinical supervisors may need to incorporate dedicated time on role and identity shift during CBP training to ensure effective assimilation and transition. Methodological short-comings are identified and discussed.

The worldwide obesity epidemic over the last 20 years has led to a dramatic increase in the prevalence of non-alcoholic fatty liver disease, the hepatic manifestation of the metabolic syndrome. Estimates of prevalence vary depending on the population studied and the methods used to assess hepatic fat content, but are commonly quoted as between 10 and 30% of the adults in the Western hemisphere. Fatty liver develops when fatty acid uptake and synthesis in the liver exceeds fatty acid oxidation and export as TAG. Studies of pathogenesis point to insulin resistance, lipotoxicity, oxidative stress and chronic inflammation being central to the development and progression of the disease. A proportion of individuals with fatty liver develop progressive disease, though large prospective longitudinal studies are lacking. Nevertheless, fatty liver is associated with increased all-cause and liver-related mortality compared with the general population. Management of fatty liver centres around lifestyle and dietary measures to induce controlled and sustained weight loss. Management of cardiovascular risk factors aims to reduce mortality, while certain dietary interventions have been shown to reduce steatosis and inflammation. Specific pharmacological treatments also show promise, but their use is not widespread. A multi-system and multi-disciplinary approach to the management of this disorder is proposed.
