This month’s issue of The Journal of Laryngology & Otology features an Open Access article of the month by Dublin et al.Reference Dublin, Morse, Attlassy, Song, Lackey and Amin¹ This case series describes 12 patients who developed bilateral vocal fold immobility after radiotherapy. No patient recovered mobility, over half required tracheostomy and the majority needed enteral feeding. The authors highlight the need for sustained long-term surveillance in irradiated patients.

The issue includes articles covering a broad range of ENT, but we have a strong otological flavour this month. Mearns et al.Reference Mearns, Lim and Kontorinis² report a retrospective series on the management of drop attacks in Ménière’s disease, documenting a stepwise approach from intratympanic steroids to gentamicin and triple canal occlusion. This article supports the findings of a systematic review on the management of drop attacks published in The Journal in 2024.Reference Selwyn and Mckay-Davies³ Two papers address cochlear implantation in inner-ear malformations. Eroğlu et al.Reference Eroğlu, Külekçi, Kılıç and Sennaroğlu⁴ describe contralateral Cerebrospinal fluid (CSF) fistula as an important differential in post-implantation meningitis in incomplete partition type I, while a further studyReference Wang, Li, Zhao, Sun, Sun and Guan⁵ reports long-term vestibular outcomes in implant recipients with enlarged vestibular aqueduct, showing better saccular preservation than in those without malformation. We also present a semi-randomised trial of endoscopic myringoplasty techniques for large marginal perforationsReference Zhou, Wang and Yang⁶ and a case–control study examining outcomes in endoscopic ear surgery in patients on anticoagulation or antiplatelet therapy.Reference Winchester, Cottrell, Svirsky and Jethanamest⁷
