Sex-dependent differences in vulnerability to early risk factors for posttraumatic stress disorder: results from the AURORA study

Background
Knowledge of sex differences in risk factors for posttraumatic stress disorder (PTSD) can contribute to the development of refined preventive interventions. Therefore, the aim of this study was to examine if women and men differ in their vulnerability to risk factors for PTSD.
MethodsAs part of the longitudinal AURORA study, 2924 patients seeking emergency department (ED) treatment in the acute aftermath of trauma provided self-report assessments of pre- peri- and post-traumatic risk factors, as well as 3-month PTSD severity. We systematically examined sex-dependent effects of 16 risk factors that have previously been hypothesized to show different associations with PTSD severity in women and men.
ResultsWomen reported higher PTSD severity at 3-months post-trauma. Z-score comparisons indicated that for five of the 16 examined risk factors the association with 3-month PTSD severity was stronger in men than in women. In multivariable models, interaction effects with sex were observed for pre-traumatic anxiety symptoms, and acute dissociative symptoms; both showed stronger associations with PTSD in men than in women. Subgroup analyses suggested trauma type-conditional effects.
ConclusionsOur findings indicate mechanisms to which men might be particularly vulnerable, demonstrating that known PTSD risk factors might behave differently in women and men. Analyses did not identify any risk factors to which women were more vulnerable than men, pointing toward further mechanisms to explain women's higher PTSD risk. Our study illustrates the need for a more systematic examination of sex differences in contributors to PTSD severity after trauma, which may inform refined preventive interventions.

Background
Several hypotheses may explain the association between substance use, posttraumatic stress disorder (PTSD), and depression. However, few studies have utilized a large multisite dataset to understand this complex relationship. Our study assessed the relationship between alcohol and cannabis use trajectories and PTSD and depression symptoms across 3 months in recently trauma-exposed civilians.
MethodsIn total, 1618 (1037 female) participants provided self-report data on past 30-day alcohol and cannabis use and PTSD and depression symptoms during their emergency department (baseline) visit. We reassessed participant's substance use and clinical symptoms 2, 8, and 12 weeks posttrauma. Latent class mixture modeling determined alcohol and cannabis use trajectories in the sample. Changes in PTSD and depression symptoms were assessed across alcohol and cannabis use trajectories via a mixed-model repeated-measures analysis of variance.
ResultsThree trajectory classes (low, high, increasing use) provided the best model fit for alcohol and cannabis use. The low alcohol use class exhibited lower PTSD symptoms at baseline than the high use class; the low cannabis use class exhibited lower PTSD and depression symptoms at baseline than the high and increasing use classes; these symptoms greatly increased at week 8 and declined at week 12. Participants who already use alcohol and cannabis exhibited greater PTSD and depression symptoms at baseline that increased at week 8 with a decrease in symptoms at week 12.
ConclusionsOur findings suggest that alcohol and cannabis use trajectories are associated with the intensity of posttrauma psychopathology. These findings could potentially inform the timing of therapeutic strategies.

Background
Posttraumatic stress symptoms (PTSS) are common following traumatic stress exposure (TSE). Identification of individuals with PTSS risk in the early aftermath of TSE is important to enable targeted administration of preventive interventions. In this study, we used baseline survey data from two prospective cohort studies to identify the most influential predictors of substantial PTSS.
MethodsSelf-identifying black and white American women and men (n = 1546) presenting to one of 16 emergency departments (EDs) within 24 h of motor vehicle collision (MVC) TSE were enrolled. Individuals with substantial PTSS (⩾33, Impact of Events Scale – Revised) 6 months after MVC were identified via follow-up questionnaire. Sociodemographic, pain, general health, event, and psychological/cognitive characteristics were collected in the ED and used in prediction modeling. Ensemble learning methods and Monte Carlo cross-validation were used for feature selection and to determine prediction accuracy. External validation was performed on a hold-out sample (30% of total sample).
ResultsTwenty-five percent (n = 394) of individuals reported PTSS 6 months following MVC. Regularized linear regression was the top performing learning method. The top 30 factors together showed good reliability in predicting PTSS in the external sample (Area under the curve = 0.79 ± 0.002). Top predictors included acute pain severity, recovery expectations, socioeconomic status, self-reported race, and psychological symptoms.
ConclusionsThese analyses add to a growing literature indicating that influential predictors of PTSS can be identified and risk for future PTSS estimated from characteristics easily available/assessable at the time of ED presentation following TSE.

Background
This is the first report on the association between trauma exposure and depression from the Advancing Understanding of RecOvery afteR traumA(AURORA) multisite longitudinal study of adverse post-traumatic neuropsychiatric sequelae (APNS) among participants seeking emergency department (ED) treatment in the aftermath of a traumatic life experience.
MethodsWe focus on participants presenting at EDs after a motor vehicle collision (MVC), which characterizes most AURORA participants, and examine associations of participant socio-demographics and MVC characteristics with 8-week depression as mediated through peritraumatic symptoms and 2-week depression.
ResultsEight-week depression prevalence was relatively high (27.8%) and associated with several MVC characteristics (being passenger v. driver; injuries to other people). Peritraumatic distress was associated with 2-week but not 8-week depression. Most of these associations held when controlling for peritraumatic symptoms and, to a lesser degree, depressive symptoms at 2-weeks post-trauma.
ConclusionsThese observations, coupled with substantial variation in the relative strength of the mediating pathways across predictors, raises the possibility of diverse and potentially complex underlying biological and psychological processes that remain to be elucidated in more in-depth analyses of the rich and evolving AURORA database to find new targets for intervention and new tools for risk-based stratification following trauma exposure.

Chemical baits corresponding to natural orchid fragrance chemicals were used to attract and sample male orchid bees in terra firme and flood plain forest sites on the Tambo-pata Reserve in south-eastern Perú. The study was conducted monthly for two years. A total of 38 species of bees was collected at these chemical baits, and although most species were collected from both habitats, 11 species were significantly more common in one habitat than the other. There was considerable variation in the number and types of chemicals to which each of the orchid bee species was attracted.
Two cycles of seasonal abundance occurred annually in the number of orchid bee individuals and species collected at the chemical baits. A major peak occurred a month following onset of the wet season, and a minor peak occurred during the dry season. Species body size was related to seasonal activity, but additional factors such as flowering phenology and nest parasites were also probably involved.
The number of orchid bee species found on the Tambopata Reserve was similar to the number found in forest sites of eastern and central Brazil, Costa Rica, and Panamá. This homogeneity in species richness is in marked contrast to other insect groups such as Odonata, Lepi-doptera, Asilidae (Diptera), Tabanidae (Diptera), and Cicindelidae (Coleoptera), for which the species numbers recorded on the Tambopata Reserve are the highest for any similar sized area in the world.

Summary
Introduction
The study of rates of speciation and extinction, and how these have changed over time, has traditionally mainly been the preserve of paleontology (Simpson 1953; Stanley 1979; Raup 1985). More recently, phylogenies of extant species have been shown to contain information on these rates and how they may have changed, under the assumption that the same rules have applied in all contemporaneous lineages (Harvey et al. 1994; Kubo & Iwasa 1995; see Nee 2006 for a recent review). The first section of this chapter contrasts the strengths and weaknesses of these two approaches – paleontological and phylogenetic – to the study of macroevolution in general.
Moving to a specific macroevolutionary hypothesis, we then outline some tests of the hypothesis that diversification rates have declined in the recent past, either in response to changed abiotic conditions or as a result of density-dependence or diversity-dependence. It has long been appreciated that incomplete species-level sampling can cause a bias in favour of this hypothesis at the expense of the null hypothesis of no change (Pybus & Harvey 2000), but we highlight a further sort of incompleteness that is likely to be very widespread and which is not widely appreciated – products of recent lineage splits are unlikely to be considered as distinct species. We reanalyze the data from a key early paper (Zink & Slowinski 1995) to show how this incompleteness, which is inevitable when taxonomy and phylogeny meet, is sufficiently strong to account for much (though not all) of the apparent tendency for rates to have declined through time.
Anesthesia-Associated Carbon Monoxide Exposures Among Surgical Patients

Objective:
To estimate the extent of, and evaluate risk factors for, elevated carboxyhemoglobin levels among patients undergoing general anesthesia and to identify the source of carbon monoxide.
Design:Matched case-control study to measure carboxyhemoglobin levels.
Setting:Large academic medical center.
Participants:45 surgical patients who underwent general anesthesia.
Results:Case-patients were more likely than controls to undergo surgery on Monday or Tuesday (10/15 vs 7/30; matched odds ratio [mOR], 7.7; 95% confidence interval [CI95], 1.8-34; P=.01), in one particular room (7/15 vs 4/30; mOR, 8.5; CI95, 1.5-48; P=.03) or in a room that was idle for ≥24 hours (11/15 vs 1/30; mOR, 95.5; CI95, 8.0-1,138; P≤.001). In a multivariate model, only rooms, and hence the anesthesia equipment, that were idle for ≥24 hours were independently associated with elevated intraoperative carboxyhemoglobin levels (OR, 22.4; CI95, 1.5-338; P=.025). Moreover, peak carboxyhemoglobin levels were correlated with the length of time that the room was idle (r=0.7; CI95, 0.3-0.9). Carbon monoxide was detected in the anesthesia machine outflow during one case-procedure. No contamination of anesthesia gas supplies or CO2 absorbents was found.
Conclusions:Carbon monoxide may accumulate in anesthesia circuits left idle for ≥24 hours as a result of a chemical interaction between CO2-absorbent granules and anesthetic gases. Patients administered anesthesia through such circuits may be at increased risk for elevated carboxyhemoglobin levels during surgery or the early postoperative period.
A comparative study of ultrasound-triggered behaviour in tiger beetles (Cicindelidae)

This study examines the behavioural responses to ultrasound in 52 species from the family Cicindelidae using both tethered flight and non-flight assays. Tethered flying tiger beetles respond to trains of bat-like ultrasonic pulses with a short-latency, multi-component behaviour. There was no variation in the nature of the behavioural responses regardless of geographical distribution or phylogenetic position. Lowest mean behavioural thresholds lie predominantly between 30 and 45 kHz. Sensitivity, however, varies widely, and several species do not respond at all in these assays. The lowest thresholds for responders are most often between 70 and 80 dB SPL. Almost all subgenera of North American tiger beetles in the genus Cicindela have at least some species with low-threshold acoustic behaviour. The single exception is the large subgenus Cicindela where all species are completely unresponsive or have very high thresholds. There was little relationship between habitat and responsiveness to ultrasound, but there is a strong correlation with seasonal activity – species with adults active in the spring and autumn (all in the subgenus Cicindela) do not show ultrasound-triggered behaviour whereas summer-active species generally do. Superimposing these data on a current phylogeny of the North American tiger beetles suggests that acoustic behaviour (and hearing) is a shared primitive trait among the taxa examined here and that there have been at least five independent losses of this character.
Randomised, double-blind, placebo-controlled treatment trial of fluoxetine and graded exercise for chronic fatigue syndrome†

Background
The Joint Working Group of the Royal Colleges of Physicians, Psychiatrists and General Practitioners (1996) recommended graded exercise and antidepressants for patients with chronic fatigue syndrome. We assessed efficacy and acceptability of these treatments.
MethodSix-month prospective randomised placebo and therapist contact time controlled trial with allocation to one of four treatment cells: exercise and 20 mg fluoxetine, exercise and placebo drug, appointments only and 20 mg fluoxetine, appointments and placebo drug. Drug treatment was double blind and patients were blind to assignment to exercise or appointments.
ResultsNinety-six (71%) of 136 patients completed the trial. Patients were more likely to drop out of exercise than non-exercise treatment (P=0.05). In an intention to treat analysis, exercise resulted in fewer patients with case level fatigue than appointments only at 26 weeks (12 (18%) v. 4 (6%) respectively P=0.025) and improvement in functional work capacity at 12(P=0.005) and 26 weeks (P=0.03). Fluoxetine had a significant effect on depression at week 12 only (P=0.04). Exercise significantly improved health perception (P=0.012) and fatigue (P=0.028) at 28 weeks.
ConclusionsGraded exercise produced improvements in functional work capacity and fatigue, while fluoxetine improved depression only.
