In March 2017, the New Jersey Department of Health received reports of 3 patients who developed septic arthritis after receiving intra-articular injections for osteoarthritis knee pain at the same private outpatient facility in New Jersey. The risk of septic arthritis resulting from intra-articular injection is low. However, outbreaks of septic arthritis associated with unsafe injection practices in outpatient settings have been reported.
An infection prevention assessment of the implicated facility’s practices was conducted because of the ongoing risk to public health. The assessment included an environmental inspection of the facility, staff interviews, infection prevention practice observations, and a medical record and office document review. A call for cases was disseminated to healthcare providers in New Jersey to identify patients treated at the facility who developed septic arthritis after receiving intra-articular injections.
We identified 41 patients with septic arthritis associated with intra-articular injections. Cultures of synovial fluid or tissue from 15 of these 41 case patients (37%) recovered bacteria consistent with oral flora. The infection prevention assessment of facility practices identified multiple breaches of recommended infection prevention practices, including inadequate hand hygiene, unsafe injection practices, and poor cleaning and disinfection practices. No additional cases were identified after infection prevention recommendations were implemented by the facility.
Aseptic technique is imperative when handling, preparing, and administering injectable medications to prevent microbial contamination.
This investigation highlights the importance of adhering to infection prevention recommendations. All healthcare personnel who prepare, handle, and administer injectable medications should be trained in infection prevention and safe injection practices.
Executive Summary
Chapter 17 explores possible transformational pathways of the future global energy system with the overarching aim of assessing the technological feasibility as well as the economic implications of meeting a range of sustainability objectives simultaneously. As such, it aims at the integration across objectives, and thus goes beyond earlier assessments of the future energy system that have mostly focused on either specific topics or single objectives. Specifically, the chapter assesses technical measures, policies, and related costs and benefits for meeting the objectives that were identified in Chapters 2 to 6, including:
providing almost universal access to affordable clean cooking and electricity for the poor;
limiting air pollution and health damages from energy use;
improving energy security throughout the world; and
limiting climate change.
The assessment of future energy pathways in this chapter shows that it is technically possible to achieve improved energy access, air quality, and energy security simultaneously while avoiding dangerous climate change. In fact, a number of alternative combinations of resources, technologies, and policies are found capable of attaining these objectives. From a large ensemble of possible transformations, three distinct groups of pathways (GEA-Supply, GEA-Mix, and GEA-Efficiency) have been identified and analyzed. Within each group, one pathway has been selected as “illustrative” in order to represent alternative evolutions of the energy system toward sustainable development. The pathway groups, together with the illustrative cases, depict salient branching points for policy implementation and highlight different degrees of freedom and different routes to the sustainability objectives.
The evolution of stored ferroelectric polarization in PZT thin film capacitors was imaged using synchrotron x-ray microdiffraction with a submicron-diameter focused incident x-ray beam. To form the capacitors, an epitaxial Pb(Zr,Ti)O3 (PZT) thin film was deposited on an epitaxially-grown conductive SrRuO3 (SRO) bottom electrode on a SrTiO3 (STO) (001) substrate. Polycrystalline SRO or Pt top electrodes were prepared by sputter deposition through a shadow mask and subsequent annealing. The intensity of x-ray reflections from the PZT film depended on the local ferroelectric polarization. With 10 keV x-rays, regions of opposite polarization differed in intensity by 26% in our PZT capacitor with an SRO top electrode. Devices with SRO electrodes showed just a 25% decrease in the remnant polarization after 107 switching cycles. In devices with Pt top electrodes, however, the switchable polarization decreased a by 70% after only 5×104 cycles.
We outline a theory of the acceptor-bound magnetic polaron in cubic diluted magnetic semiconductors that exhibits a nonuniform magnetization. Calculations show that the manganese spins have an overall z-alignment, but are canted in an azimuthally symmetric way from that direction. This is new phenomenon we call BMP spin-texture. Spin-texture results from valence band degeneracy, and the important spin-orbit effects associated with it. The possibility of studying BMP spin-texture via neutron scattering is discussed.
