Objectives: To review recent use of psychosurgery in Britain and Ireland; to outline the indications and contraindications; and to summarise the Irish experience of psychosurgery.
Method: The Geoffrey Knight National Unit for Affective Disorders (GKU), now based at the Maudsley Hospital has performed the largest number of psychosurgical operations in the last 30 years. This paper reviews criteria for referral and outcome of stereotactic subcaudate tractotomy at the GKU. The twenty-one referrals from Ireland to the Unit, resulting in eight operations over the fifteen year period 1981-1995, are considered separately.
Results: The efficacy of stereotactic subcaudate tractotomy is of the order of 30-60% and there is a low incidence of long-term adverse effects. The outcome of the eight Irish patients who underwent psychosurgery compare favourably with the overall outcome figures for the Unit.
Conclusions: Stereotactic subcaudate tractotomy remains a treatment of last resort in the rare cases of severely treatment-resistant affective disorders and obsessive compulsive disorders.

Section 48 of the Mental Health Act, 1983 (MHA) permits the Secretary of State to authorise the removal to hospital of an unsentenced prisoner who is ‘suffering from mental illness or severe mental impairment of a nature or degree which makes it appropriate for him to be detained in a hospital for medical treatment and that he is in urgent need of such treatment’. (Mental Health Act, 1983). Its common usage in the past has been in transfers of unsentenced prisoners from prison to hospital.

It was fully anticipated that the Mental Health Act Commission would take special interest in psychosurgery, and Section 57 is clearly intended to supervise this form of therapy. The Chairman of the Commission, Lord Colville, considerately arranged a visit to our Unit shortly after the implementation of the Act, and this proved to be a most useful meeting. Obviously we were ready to give any help or advice needed, based on our experience of over 1,100 operations carried out over about 20 years. Our world-unique unit has eight beds specifically for the assessment and treatment of patients referred for psychosurgery, although intensive antidepressant medication is sometimes used successfully in order to avoid psychosurgery.

I have been a member of the Southern Division Approval Panel since its inception in 1972. Thereafter I visited, with other panel members, two or three hospitals per year. I became Convener in 1978 and in my four years in office I visited over 35 hospitals, although these were not all separate visits. I look after a catchment area in South-East London, with all the in-patient services at Bexley Hospital. It is entirely my personal opinion that if I was not involved with a district service I probably would not have accepted the invitation to be a convener. I do not think I would necessarily have been a worse convener, but I might have been rather less credible.
