There are 117.3 million people forcibly displaced because of war, conflict and natural disasters: 40% are children. With growing numbers, many high-income countries have adopted or are considering increasingly restrictive policies of immigration detention. Research on the impact of detention on mental health has focused on adults, although recent studies report on children.
Aims
To synthesise data on the impact of immigration detention on children’s mental health.
Method
Systematic searches were conducted in PsycINFO, MEDLINE and Embase databases and grey literature and studies assessed using PRISMA guidelines (PROSPERO registration CRD42023369680). Included studies were quantitative, assessed children younger than 18 years who had been in immigration detention and reported mental health symptoms or diagnoses. Methodological quality was assessed using the Appraisal Tool for Cross-Sectional Studies. Meta-analyses estimated prevalence for major depression and post-traumatic stress disorder (PTSD).
Results
Twenty-one studies reported data on 9620 children. Most studies were cross-sectional, had small sample sizes and used convenience sampling. A profoundly detrimental impact on children’s mental health across a variety of countries and detention settings was demonstrated. Meta-analysis found pooled prevalence of 42.2% for depression [95% CI 22.9, 64.3] and 32.0% for PTSD [95% CI 19.4, 48.0]. Severity of mental health impact increased with exposure to indefinite or protracted held detention.
Conclusions
Immigration detention harms children. No period of detention can be deemed safe, as all immigration detention is associated with adverse impacts on mental health. Our review highlights the urgency of alternative immigration policies that end the practice of detaining children and families.
Extending the theory of lower bounds to reliability based on splits given by Guttman (in Psychometrika 53, 63–70, 1945), this paper introduces quantile lower bound coefficients λ4(Q) that refer to cumulative proportions of potential locally optimal “split-half” coefficients that are below a particular point Q in the distribution of split-halves based on different partitions of variables into two sets. Interesting quantile values are Q=0.05,0.50,0.95,1.00 with λ4(0.05)≤λ4(0.50)≤λ4(0.95)≤λ4(1.0). Only the global optimum λ4(1.0), Guttman’s maximal λ4, has previously been considered to be interesting, but in small samples it substantially overestimates population reliability ρ. The three coefficients λ4(0.05), λ4(0.50), and λ4(0.95) provide new lower bounds to reliability. The smallest, λ4(0.05), provides the most protection against capitalizing on chance associations, and thus overestimation, λ4(0.50) is the median of these coefficients, while λ4(0.95) tends to overestimate reliability, but also exhibits less bias than previous estimators. Computational theory, algorithm, and publicly available code based in R are provided to compute these coefficients. Simulation studies evaluate the performance of these coefficients and compare them to coefficient alpha and the greatest lower bound under several population reliability structures.
Assembly-speeches and funeral speeches invite comparison. In both, prominent politicians addressed a large and predominantly non-elite audience, and war played a predominant role. Yet, contrasts between them abounded. The funeral oration emphasised the nobility of Athens and more particularly the selflessness and the patriotism of the war dead, whereas assembly-speeches criticised the decadence of Athenian politics and the short-sighted selfishness of Athenian citizens. The speaker of a funeral speech was self-effacing. The speaker in the assembly, by contrast, asserted his insight and knowledge, while he criticised his fellow citizens almost undemocratically. The funeral oration addressed a united Athens and avoided divisive issues, whereas disagreement was the raison d’être of assembly-speeches. In spite of all these differences, similarities lay just below the surface. Insofar as their advice for the future depended on the past, assembly speakers invoked the patriotic and slanted history that was conspicuously promulgated in the funeral oration. Funeral speeches insisted on Athenian exceptionalism in the Greek world. Assembly-speeches did the same, if only to contrast Athens’s current policies with its true role as the leader of the Greek world and the guardian of freedom and justice.
Based on the best-selling Stahl's Prescriber's Guide, this essential guide to psychiatric prescribing has been developed by leading psychiatrists and medical students from the University of Cambridge to support all mental health professionals in achieving optimal care for their patients. Written with the authority of evidence and the guidance of clinical wisdom the formulary covers the psychotropic medications used in daily care including dosing recommendations and drug interactions. With its easy-to-use, full-colour template-driven navigation system, the book combines evidence-based data with clinically informed advice, including guidance on prescribing for children and adolescents and people with addictions. Drugs are presented in the same format to facilitate rapid access to information and are broken down into sections designated by a unique colour background thereby clearly distinguishing information presented on therapeutics, side effects, dosing and use, and the art of psychopharmacology. Popular prescribing 'tips and pearls are included throughout.
