This chapter discusses (1) the medical complexity and specialized subspecialty care that is part of the tertiary care hospital which may involve “complex” or “second level” units and (2) academic centers and the inclusion of education and research in academic centers.
This chapter deals with management of the complex and extended OU patient by combining protocols for those with increased medical complexity, by allowing additional time (> 24 but < 48 hours), and integrating specialty consultation and follow-up resources. A robust quality assurance program is essential.
Simple observation involves a patient with a single problem with an average stay < 24 hours. Complex observation patients involves significantly more physician or advanced practice provider and nursing time. Another category includes those patients with observation stays > 48 hours who often fail to meet inpatient criteria based on severity of illness alone. An observation unit (OU) or short stay unit, such as a geriatric OU that provides care-coordination and social support service, is a potential solution.
