An increasing number of monochorionic (MC) twin pregnancy complications are being detected prior to birth due to major improvements in prenatal ultrasound examinations [1]. For various MC twin complications, fetal therapy is a life-saving option or an alternative to postnatal treatment to prevent permanent organ damage, including damage to the developing fetal brain. The aim of this chapter is threefold: to summarize current knowledge on the neurological and neurodevelopmental outcome after fetal therapy in complicated MC twin pregnancies; to identify risk factors for adverse long-term outcome; and to provide future research and care suggestions.
