Utilization of Chiropractic Services among Older Adults with Spine Pain at a Publicly Funded Canadian Healthcare Facility: A Retrospective Study

Abstract

There are limited data on chiropractic care for older adults, specifically from medically underserved communities. This study describes the characteristics, clinical management, and patient-reported outcomes of older adults with spinal pain who present for chiropractic care at a publicly funded community health centre serving marginalized populations. This retrospective analysis utilized quality assurance data from chiropractic encounters at Mount Carmel Clinic between January 2011 and June 2020 of adults aged 45 and older. Descriptive statistics summarized the study population and their self-reported pain severity scores. Student’s t-tests and repeated-measures ANOVA explored relationships between pain outcomes, age, and clinical characteristics. The sample included 240 middle-aged (45–59 years) and older adults (≥60 years) who recorded baseline and discharge pain scores following chiropractic treatment. Over half of middle-aged participants self-identified as Indigenous or as people with disabilities. Statistically and clinically important improvements in pain were noted across spinal regions and extremities for both cohorts.

Il existe peu de données sur les soins chiropratiques dispensés aux personnes âgées, en particulier dans les communautés médicalement défavorisées. Cette étude décrit les caractéristiques, la prise en charge clinique et les résultats rapportés par les patients âgés souffrant de douleurs vertébrales qui reçoivent des soins chiropratiques dans un centre de santé communautaire public desservant des populations marginalisées. Cette analyse rétrospective a utilisé les données d’assurance qualité issues des consultations chiropratiques à la Mount Carmel Clinic entre janvier 2011 et juin 2020 chez des adultes âgés de 45 ans et plus. Des statistiques descriptives ont permis de cerner le profil de la population étudiée et de compiler les scores de sévérité de la douleur autoévaluée. Des tests-t de Student et des analyses de variance à mesures répétées ont permis d’explorer les relations entre les résultats en matière de douleur, l’âge et les caractéristiques cliniques. L’échantillon comprenait 240 personnes d’âge moyen (45–59 ans) et âgées (≥ 60 ans) qui ont enregistré leurs scores de douleur au début et à la suite du traitement chiropratique. Plus de la moitié des participants d’âge moyen se sont identifiés comme autochtones ou comme personnes handicapées. Des améliorations statistiques et cliniquement significatives de la douleur ont été observées dans toutes les régions et extrémités de la colonne vertébrale pour les deux cohortes.
